3.Prevalence of Tardive Dyskinesia in Inpatients Using Antipsychotics.

OBJECTIVE: This study was to investigate the prevalence of tardive dyskinesia (TD) in Korean inpatients using antipsychotics and the relationship between TD and sociodemographic, clinical variables. METHODS: This study was done from January to Feburary, 2003, at the Seoul National Mental Hospital. A cross-sectional assessment of randomly selected inpatients (n=324;mean age=39.73+/-9.63years) with DSM-IV schizophrenia, schizoaffective, bipolar disorder was performed with standard rating instrument for TD and extrapyramidal symptoms. The relations between the prevalence of TD and sex, age, length of medication and dosage of antipsychotics, clozapine, and other psychotropic medication, EPS were analyzed with Chi-square test or t-test. RESULTS: Using Schooler and Kane's criteria, 20 subjects (6.17%) had TD. TD was significantly less prevalent in patients receiving clozapine. There was no other significant difference between the TD and without TD groups with respect to type of antipsychotics, other psychotropic drug, antiparkinsonian drug, mood stabilizer augmentation. CONCLUSION: The result of this study have confirmed the lower prevalence of TD among inpatients using antipsychotics compared to previous investigations. The study has also replicated the association of TD with older age. Clozapine use was associated with lower prevalence rate of TD.

4.A Case of Ocular Deviation after Excinier Laser Photorefractive Keratectomy.

Patients undergoing monocular excimer laser photorefractive keratectomy (PRK) for myopic correction can develop strabismus secondary to decompensated heterophorias. We experienced a case of ocular esodeviation and diplopia that developed 8 nionths after monocular excimer laser phtorefractive keratectomy. So, we report this case with 8 review of the literature.

5.Expression Patterns of S100A6 Gene in Human Thyroid Diseases.

S100A6 (calcyclin) is a member of the S100 family and has been originally isolated from the cDNA library of Syrian baby hamster kidney cells. The S100A6 gene expression is reported to remain high throughout the cell cycle following induction by serum or growth factors, suggesting that the gene may be required for cell cycle progression. Nevertheless, the role that S100A6 may play in tumor progression remains unknown. In this study, we have explored the expression patterns of S100A6 gene in human thyroid tissues by northern blot analysis. Using the S100A6 monoclonal antibody, we carried out the immunohistochemical staining to determine the distribution/localization of S100A6 protein within tumor or non-tumorous cells of the thyroid. To modulate the regulation of endogenously expressed S100A6 protein in the intracellular level, overexpressed or anti-sense treated transfectant was constructed by using the eukaryotic expression vector. As a result, immunohistochemistry for S100A6 showed a strong positivity in the malignant tumors of thyroid and a high expression level of S100A6 protein affected cell proliferation in the overexpressed transfectant. These findings suggest that S100A6 may be involved in the tumor pathogenesis and provides another parameter for the differentiation of malignant and benign lesions. A well defined monoclonal antibody against S100A6 protein is now available for the immunohistochemical studies of the various thyroid tissues.

6.Intestinal Anisakiasis.

We reported 3 cases of intestinal anisakiasis. The patients had abdominal pain, nausea or vomiting, after eating raw sea-fishes 1 to 5 days before. Intestinal resection was performed under the impression of mechanical obstruction or cancer. The resected intestines showed marked edema, congestion or hemorrhage in the mucosa. Microscopically the larvae were found in the submucosa or inner muscle layer, and surrounded by phlegmonous inflammation with intense eosinophilic infiltration. The larvae were identified as Anisakis spp. by multiple sections.
7.Neurocognitive Function in Patients with Hwa-byung and with Major Depressive Disorder.

OBJECTIVES: Hwa-byung has been studied clinically for several years and introduced as Korean Culture-Bound Syndrome. However, the definition and the diagnostic method are not yet clarified, and there has not been any sufficient comparative study on this disease entity. This study was conducted to determine the clinical symptoms and the profile of the neurocognitive functions in Hwa-byung(HB) and Major Depressive Disorder(MDD), and We wish to identify any critical factors that differentiate the disorders. METHODS: A total of 102 participants were examined, including 34 participants with MDD, 34 with HB, and 34 healthy controls. The MDD and HB patients were recruited from among inpatients and outpatients at the National Medical Center for the period from May to December of 2004. As a major diagnostic tool of MDD, diagnostic reference of DSM-IV-TR was used and as HB's diagnostic tool, We used computerized neurocognitive function test. Psychiatric symptomatology was evaluated by the Beck Depression Inventory(BDI), and Symptom Checkist-90-Revision(SCL-90-R). Oneway ANOVA, Scheffe post-hoc test and Chi-Squre Tests were used for statistical analysis. RESULTS: The participants in three groups did not differ in terms of age, sex, and education. Assessment of BDI indicated that the MDD group had significantly higher total score than the HB group. MDD and HB groups showed significantly higher total scores on the SCL-90-R in comparison to the controls. The MDD group was found to have significantly more symptoms of depression than the HB group, based on the depression subscale of the SCL-90-R. The computerized neurocognitive function test suggest several results 1) Within the memory domain, it was found that one of the two memory tests in MDD and HB groups were significantly impaired in comparison to the control group. 2) Within the attention domain, it was found that only the MDD group was significantly impaired in comparison to the control group. 3) Within the higher cortical function domain, it was found that significant impairment exist in MDD group and HB group compared to the control group; the severity of impairment was found to be more profound in the MDD group than in the HB group. CONCLUSION: These results suggest that both HB group and MDD group have significantly decreased neurocognitive function than the control group, and neurocognitive function of the HB group is better than that of the MDD group.

8.The significance of the spleen-liver ratio in liver scanning

Increased splenic uptake of radiocolloid is a helpful sign in the scintigraphic diagnosis of diffuse hepatocellular diseases, but any attempt has been made to quantify this phsiologic phenomenon. The purpose of thestudy is to evaluate a simple computer quantitation of S/L ratio and to determine normal range and S/L ratios forvarious hepatic diseases. Authors analized S/L ratios of 194 cases of liver scintigraphy from July 1984 to May1985. The results are as follows; 1. The age distribution of normal and various heptic disases was most frequentin 30 to 40 decade. 2. The classification of studied groups were normal subjects (80 cases), hepatitis(30 cases),liver cirrhosis (59 cases), hepatoma(8 cases), metastasis(10 cases), and miscellaneous diseases(7 cases). 3. Thesimple computer quantitation method exhibits small interobserver variation.(r=0.92, p<0.001) 4. The mean S/L ratioin normal group was determined 0.34 (S.D=0.12) with a its range from 0.10 to 0.58 (0.34±2 S.D). The mean S/Lratios in various hepatic diseases were as follows; 0.52 (S.D=0.18) in hepatitis, 1.10 (S.D=0.43) in cirhhosis,0.77 (S.D=0.38) in hepatoma, 0.47 (S.D=0.21) in metastasis, and 0.43(S.D=0.17) in miscellaneous diseases. 5. Theelevated S/L ratios rather than normal values were found in hepatitis (30%), cirrhosis(51%), hepatoma(63%), and metastasis(20%). 6. The sensitivity of single scintigraphic diagnosis of liver cirrhosis was 63%, but thesensitivity was improved to 90% when combined with S/L ratio. 7. The simple computer quantitation of the S/L ratiois a valid and useful method in the interpreation of liver scintigraphy and also may increase the sensitivity inthe diagnosis of liver cirrhosis and hepatoma combined with cirrhosis.

9.Dobutamine Echocardiography in Chronic Coronary Artery Disease with Left Ventricular Dysfunction.

BACKGROUND: Dobutamine echocardiography has been shown to be a valuable tool for determining myocardial viability in both acute and chronic coronary artery disease with left ventricular dysfunction. The purpose of the our study was to identify the role of dobutamine echocardiography in the prediction of improvement of regional left ventricular (LV) dysfunction after revascularization of chronic coronary artery disease. METHODS: Twenty-three patients (mean age 61.2+/-9.0 years;20 men) with chronic LV dysfunction underwent dobuta-mine echocardiography (dobutamine:baseline, 5, 10, 20 microgram/Kg/min) before coronary revascularization (coronary artery bypass graft surgery 16, percutaneous coronary angioplasty 7). The mean LV ejection fraction was 42.9+/-8.8% with ranging from 26% to 58%. Follow-up echocardiography was performed at 2 to 21 months (mean 9.0+/-6.2 months) after revascularization. RESULTS: During dobutamine echocardiography, there was no major complication. Improvement of the dysfunctional myocardium was observed in 12 of 23 patients in dobutamine echocardiography. Among them, 10 patients showed functonal recovery after revascularization. Another 11 patients did not show improvement of dysfunctional myocardium in dobutamine echocardiography, however 3 of them showed functional recovery after revascularization. One hundred fifteen dysfunctional segments were found in 368 segments of 23 patients, and improvement of wall motion abnormality was observed in 46 of 115 segments in dobutamine echocardiography. Among them, 31 segments showed functional recovery after revascularization. Another 69 segments did not show wall motion improvement in dobutamine echocardiography. But among them, 13 segments showed functional recovery after revas-cularization. The sensitivity and specificity of dobutamine echocardiography for the prediction of postoperative improvement of segmental wall motion were 70% and 79%, respectively. The positive and negative predictive value of dobutamine echocardiography were 67% and 81%, respectively. CONCLUSION: In patients with chronic LV dysfunction, dobutamine echocardiography can be used as a predictor of the improvement of dysfunctional segments after revascularization.

10.A Case of Type 1 Ushers Syndrome with Bilateral Cataract.

Ushers syndrome is an autosomal recessively inherited entity which is characterized by a retinitis pigmentosa and congenital sensorineural hearing loss. This syndrome represents different clinical features according to its subtypes. We experienced one case of type 1 Ushers syndrome, who had congenital hearing loss, visual field loss, visual loss of early childhood onset. We performed bilateral cataract extraction. Fundus examination after operation showed characteristic findings of retinitis pigmentosa. ERG was non-recordable ERG. Her audiogram confirmed the bilateral sensorineural hearing loss.
