3.A Case of Tuberculosis-related Retinal Vasculitis.

PURPOSE: Tuberculosis is a chronic infection caused by mycobacteria species. M. tuberculosis causes disease predominantly in the lung, although every organ in the body may be involved. Retinal tuberculosis is very rare and usually unilateral. Possible hypotheses on the pathogenesis of tuberculosis-related retinal vasculitis are an infectious process and a hypersensitivity mechanism. In Korea, no case has been reported. METHOD: We experienced a case of bilateral retinal vasculitis who was suffering from tuberculous meningitis. RESULT: His vasculitis was successfully treated with anti-tuberculosis medications. We report a case of tuberculosis-related retinal vasculitis with the review of literature.

4.A Case of Childhood Dermatofibrosarcoma Protuberans.

Dermatofibrosarcoma protuberans(DFSP) is a rare, distinctive cutsneous tumor originating in the dermis that appears as a raised uninodular or multinodular lesion on the trunk or the proximal extremities. In about 10% of the cases, DFSP arises in childhood and it is rarely evident already at birth. Histopathologically, DFSP shows the chsracteristic storiform and cartwheel patterns, with the fibroblasts arranged radially about a small central hub of fibrous tissue. We report herein a case of childhood DFSP in a 7-year-old boy.

5.Anal Endosonographic Findings of Internal Anal Sphincter in Normal Adult Korean.

PURPOSE: To describe the appearance and average thickness of the internal anal sphincter with anal endosonography in healthy Korean adults. MATERIAL AND METHODS: 184 subjects(male: 96, female: 88) with no history of anorectal disease or surgery were studied with anal endosonography. The average thickness of internal sphincter was meas ured at the mid-anal canal. For the internal sphincter, which is often asymmetric, the thickness of each 4 part(12, 3, 6 and 9 o'clock direction) were measured in left lateral decubitus position. RESULTS: The anal wall was well visualized in 5 layers(mucosa, submucosa, internal anal sphincter, intersphincteric plane, external anal sphincter) with anal endosonography. The anal endosonogram showed the internal anal sphincter as a homogenous, well-defined, hypoechoic, circular band, and slightly asymmetric. The average thickness of the internal anal sphincter in the area of mid-anal portion was 2.0 0.3 mm(range: 1~3 mm). There was no sexual difference; however, a significant positive correlation with age was found in average thickness of the internal anal sphincter. The correlation with lean body mass was not found. CONCLUSION: The internal anal sphincter is well-visualized, best defined structure by anal endosonography. Average thickeness of the sphincter in Korean appeared to be the same as in the Western.

6.Choroidal Metastasis of Adenocarcinoma of the Lung: A case report.

Choroidal metastatic carcinoma is very rare. We recently experienced a case of lung adenocarcinoma which presented to the clinic with ocular symptoms. This 57-year-old Korean male patient visited the department of ophthalmology due to decreased visual acuity and pain of the left eye. On MRI scan, a nodule was attached to the retina of the left eyeball. On simple chest radiograph, a large amount of pleural effusion was noted in the left pleural cavity. Emergency enucleation of the left eyeball was done with an impression of malignant melanoma causing an intractable ocular pain. Grossly, the lesion in the eyeball was rising from the choroid. On histologic examination, tumor cells formed many irregular, small gland-like structures. The tumor cells showed alcian blue-positive mucin in the cytoplasm and glandular lumens and were positive for CEA. Chest CT scan was performed postoperatively and showed a huge mass in the left lower lobe and multiple nodular opacities in both lung fields. Bronchoscopic biopsy revealed moderately differentiated adenocarcinoma similar to that of the eyeball.

8.Diganostic Values of Bone Scan Followed by CT Scan in Undetected Pelvic bone Fracture.

Pelvic bone is complex in its shape, so we often miss to find the fractures in cases with unclear fracture line. We studied 25 patients who were suspected to have occult fractures in pelvic area. We were able to find lots of occult fractures with bone scan followed by CT scan. The results were as follows; 1. There were 31 unexpected hot spots in 25 patients in bone scan. 2. Six cases (19.3%j of new injuries were found on re-evaluation of plain film or additionally rechecked plain films. 3. Twenty-one new fractures (67.7%) were found on CT film study. 4. Total 27 additional fractures (89.1%) were found after bone scan followed CT scan. In conclusion Bone Scan followed by CT is a effective method of detecting occult fracture in pelvic area.

9.Further Increase in the Angulation after Clinical Union of the Conservatively Treated Tibial Shaft Fractures

Fracture of the tibial shaft is one of the most common fractures of the long bones and frequently followed by malunion, delayed union and nonunion. We experienced the cases of further increase in the angulation after clinical union of the tibial shaft fractures treated by conservative method. So we analyzed 39 cases of tibial shaft fractures which were followed up from 12 weeks to 37 weeks after clinical union in adults. The results were as follows; l. 7 of the 39 cases(18%) showed the increase in the angulation, and all of them were male. 2. Increase in the angulation occurred more frequenlty in the comminuted fractures than simple fractures. 3. The increment of angulation was more marked in group of patients with higher angulation at the time of clinical union. 4. Increase in the angulation occurred more commonly in the cases not associated with fibula fracture than in the cases associated with fibula fracture. 5. Further increase in the angulation was less common in the cases in whom early weight bearing with PTB cast was started.

10.Platynosomum fastosum Infestation in a Domestic Cat in Korea.

Cat liver fluke (Platynosomum fastosum) was identified upon necropsy of a Felis catus (domestic cat). This trematode has not been reported in domestic cats previously in Korea. Diagnosis of this fluke was made by the presence of eggs in the feces of the cat through the fecal sedimentation method. Flukes in the gallbladder, bile duct, and liver parenchyma were revealed by the necropsy findings. This report describes as the first case of a domestic cat with Platynosomum fastosum in Korea.
