4.Fibrous Pseudotumor of Paratesticular Region: A case report.

Fibrous pseudotumor of paratesticular region is rare, but one of the most common neoplasm of that region. It has also been called as nodular fibrous proliferation, pseudofibromatous periorchitis, benign fibrous paratesticular tumor, and fibrous mesothelioma(pseudofibroma). We herein report a case of fibrous pseudotumor with characteristic histological findings. The patient is a 59 year-old male who had incidentally found scrotal mass and undergone radical orchiectomy. There was two separate nodules at tunica vaginalis and proximal spermatic cord which had bulging whitish-gray cut surface with focal myxoid change. Histologically, the mass was composed of dense collagenous tissue with scattered lymphoid follicles and numerous chronic inflammatory cells. There was a proliferation of spindle or stellate shaped cells, some of which featured enlarged hyperchromatic nuclei with prominent nucleoli, and abundant basophilic cytoplasm. These cells stained positive for vimentin and actin immunohistochemically, suggesting that this lesion might derive from proliferation of myofibroblasts.

Choriocarcinoma associated with a normal pregnancy is rare. Futhermore, choriocarcinoma coexistent with a viable pregnancy is even rarer and associated with a greater risk of hepatic and cerebral involvement. So timely diagnosis of the disease is important for successful treatment and aggressive diagnostic procedures may therefore warranted. The patient should be treated with primary intensive combination chemotherapy(EMA-CO) and the selective use of irradiation and surgical therapy. We experienced a case of metastatic choriocarcinoma of lung and kidney following preterm delivery at 34th week of pregnancy and so present it with brief review of literature.

7.Therapeutic effects of praziquantel (Embay 8440) against Taenia solium infection.

A total of 53 patients infected with Taenia solium were treated with praziquantel in two different dose levels. Twenty-six cases were treated with praziquantel in a single oral dose of 5 mg/kg body weight. All cases except one were completely cured. The cure rate was 96.2 per cent. The other 27 cases were treated with a single dose of 10 mg/kg body weight. All cases were completely cured and revealed a cure rate of 100 per cent. Side effects were mild and transient. About half of the cases complained of a mild abdominal pain. Soft stool or diarrhea and dizziness were observed in some cases. Tests on clinical hematology, serum biochemistry and urinalysis were performed immediately before and one day after treatment in all cases. There were no significant abnormalities detected in these tests.

8.Mini-open Treatment Using Plate of Clavicle Mid-shaft Fractures.

BACKGROUND: Increased frequency of comminuted clavicle mid-shaft fractures and importance of functional satisfaction through early joint exercise has resulted in higher emphasis on surgical treatments. This study aimed to evaluate the clinical radiological results of treatment of clavicle mid-shaft fractures by open reduction and internal fixation using a plate with a small incision. METHODS: The subjects of this study were 80 clavicle mid-shaft fracture cases treated with internal fixation using a plate from October 2010 to July 2014. Clavicle mid-shaft fractures were internally fixated using anatomical plates or locking compression plates. Achievement of bone union, union period, and clavicle length shortening were evaluated radiologically, and clinical assessment was done by using Constant and University of California at Los Angeles (UCLA) scores. RESULTS: All 80 cases were confirmed to have achieved bone union through radiographs with an average union period of 10.9 weeks (range: 7–18 weeks). The average clavicle length of shortening in the affected side was 1.8 mm (range: 0–17 mm). The average UCLA score and Constant score were 33.6 (range: 25–35) and 92.5 (range: 65–100), respectively. Regarding complications, four cases reported skin irritation by metal plates, and one case reported a screw insertion site fracture due to minor trauma history. CONCLUSIONS: We were able to induce successful bone union and obtain clinically satisfactory results in displaced mid-shaft fractures of the clavicle without major complications such as nonunion through treatment of internal fixation using a plate.

10.Pattern of Extraocular Muscle Involvements in Miller Fisher Syndrome

BACKGROUND AND PURPOSE: The most-common initial manifestation of Miller Fisher syndrome (MFS) is diplopia due to acute ophthalmoplegia. However, few studies have focused on ocular motility findings in MFS. This study aimed to determine the pattern of extraocular muscle (EOM) paresis in MFS patients. METHODS: We consecutively recruited MFS patients who presented with ophthalmoplegia between 2010 and 2015. The involved EOMs and the strabismus pattern in the primary position were analyzed. Antecedent infections, other involved cranial nerves, and laboratory findings were also reviewed. We compared the characteristics of the patients according to the severity of ophthalmoplegia between complete ophthalmoplegia (CO) and incomplete ophthalmoplegia (IO). RESULTS: Twenty-five patients (15 males and 10 females) with bilateral ophthalmoplegia were included in the study. The most-involved and last-to-recover EOM was the lateral rectus muscle. CO and IO were observed in 11 and 14 patients, respectively. The patients were aged 59.0±18.4 years (mean±SD) in the CO group and 24.9±7.4 years in the IO group (p<0.01), and comprised 63.6% and 21.4% females, respectively (p=0.049). Elevated cerebrospinal fluid protein was identified in 60.0% of patients with CO and 7.7% of patients with IO (p=0.019) for a mean follow-up time from the initial symptom onset of 3.7 days. CONCLUSIONS: The lateral rectus muscle is the most-involved and last-to-recover EOM in ophthalmoplegia. The CO patients were much older and were more likely to be female and have an elevation of cerebrospinal fluid protein than the IO patients.
