1.Composite Graft for Eyelid Reconstruction.

PURPOSE: The purpose of this article is to describe successful reconstruction of upper eyelid defect using a composite graft from the contralateral eyelid. CASE SUMMARY: A 42-year-old-female patient presented with pigmented mass of the left upper tarsoconjunctiva. She underwent tumor resection and eyelid reconstruction with periosteal flap due to recurred malignant melanoma on her left lower conjunctiva 3 years ago. The left upper eyelid was reconstructed using a composite graft from the contralateral upper eyelid. CONCLUSIONS: A satisfactory result, both functionally and cosmetically, was obtained during the follow-up period of 10 months. Composite graft isan ideal option for the reconstruction of eyelid defect which is difficult to correct successfully with other conventional methods. A case of eyelid reconstruction using a composite graft from the contralateral eyelid is herein reported with good functional and cosmetic results.

3.A Case of Primary Malignant Lymphoma of the Conjunctiva.

Involvement of the conjunctiva with malignant lympoma is very rare. Primary malignant lymphoma of the conjunctiva equally affects men and women over age of 40 years and has a predilection for inferior conjunctival fornices. Most lesions occur unilaterUy, involving right and left sides equally frequently. Periorbital swelling is the most common presenting symptom followed by proptosis, ptosis, pain and poor or blurred vision. Radiation therapy appears to be treatment of choice for malignant lymphoma involving the conjunctiva Surgical treatment alone is less likely to remove all the tumor and should be conjoined with a wide field of radiotherapy. The prognosis of the case depends upon whether or not the disease is generalized. Although in Korea a few cases of secondary involvement of the conjunctiva with generalized malignant lymphoma have been reported, no primary case has yet been reported. The authors reported here a case of primary malignant lymphoma of the conjunctiva in a 48 year-old female patient with a review of the literatures.

6.A Case of Pigmented Contact Dermatitis Caused by Cinnamic Aldehydes.

Pigmented contact dermatitis denotes a kind of secondary hyperpigmentation resulting from recurrent contact dermatitis of low degree. Cinnamic aldehyde is a component of cinnamon,which is widely used in foods and fragrances. A 21 year-old girl presented with a well-defined dark brownish patch on right side of chest for 4 years. Histopathologic examination revealed epidermal spongiosis, hypermelanosis of basal layer, scattered melanophages and mild perivascular inflammatory cell infiltration in the upper dermis Patch test findings were positive to fragrance mix, cinnamic aldehyde and body shampoo which was used by the patient. Peroral challenge with cinnamon tea resulted in flare-up of the positive patch-test sites and the skin lesion.

Radiofrequency ablation (RFA) has been used as an alternative method of surgical treatment to treat neoplasms of variable body organs. In considerable proportion of RFA cases, anesthesiologists are asked to conduct general anesthesia. RFA has been known to be a safe and effective treatment, however injury to adjacent normal tissue during RFA develops serious complications. In particular, unintended injury to normal adrenal tissue of adrenal tumors can cause severe complications such as hypertensive crisis due to excessive secretion of catecholamine. However, serious complications of primary or metastatic adrenal tumors have been rarely reported due to RFA. We report a case of hypertensive crisis with associated tachycardia and ventricular arrhythmia during RFA of hepatocellular carcinoma metastatic to the adrenal gland.

8.Multiple hepatic adenomas in a patient with a history of oral steroid: A case report.

Multiple hepatic adenoma is a rare entity in which characterized by the presence of numerous adenomas within an otherwise normal liver. Although the disease is rare, its important lies in its tendency to produce symptoms such as abdominal pain and its potential for abdominal hemorrhages. However, the prognosis and the evolution of this entity is generally uncertain. We report a case multiple hepatic adenomas, which was proved by pathology, in 24 years old man who has been prescribed long-term oral steroid due to rheumatoid arthritis. He underwent incisional biopsy and mass enucleation of hepatic tumors with 95% alcohol injection. In the case, dissection of the liver showed many other diffuse and smaller nodules. Histologically, tumor was liver cell adenoma without pleomorphism, or mitosis. During the follow-up period of two years, new nodules were found by ultrasonography and magnetic resonance image without developed of serious complications.

Nonsurgical drainage of malignant obstructive jaundice is an interesting alternative to surgical drainage in the palliative treatment of pancreaticobiliary neoplasms. Biliary drainage by endoprosthesis is as effective and better supported than percutaneous external drainage, but more difficult to control. Endoscopic retograde biliary drainage (ERBD) is a safe and effective biliary drainage procedure, and is indicated with malignant obstructive jaundice in patients on whom endoscopic retrograde cholangiopancreatography (ERCP) and endoscopic sphincterotomy (EST) can be performed. A nonoperative method of palliation was used in patients with malignant obstructive jaundice, in whom a biliary endoprosthesis could not be placed endoscopically due to complete obstruction of the bile duct. A guide wire was manipulated through the lesion by a percutaneous transhepatic route, after puncturing the tumor by a fine needle, and retrieved from the duodenum through an endoscope. A stent was then passed through the endoscope over the guide wire across the stricture.

10.Effectiveness of Walking-exercise on Bowel Preparation for Colonoscopy.

BACKGROUND/AIMS: This prospective study was conducted to assess the effect of walking-exercise on the bowel cleansing before colonoscopy and to define subgroup of patients to whom walking-exercise was beneficial. METHODS: Before taking 2.5 L of polyethylene glycol, 393 out-patients were randomized into two groups (G1; walking-exercise, G2; non-exercise) and the amount of walking-exercise was estimated by the step counter. A single colonoscopist performed the procedure with a single-blinded manner and estimated the bowel cleansing. RESULTS: A total of 366 patients were included by per protocol analysis. The number of step count was significantly different between two groups (p<0.000). However, the groups were similar in other data collected. The degree of bowel cleansing between two groups was significantly different (p<0.01). Age, history of abdominal surgery, constipation, obesity and walking- exercise were related to the degree of bowel cleansing. Interestingly, walking-exercise was especially beneficial to patients with age less than 65 years, without history of abdominal surgery, and non-obesity. Multivariate analysis demonstrated that absence of constipation (OR=4.74), walking- exercise (OR=2.04), and younger age (OR=1.97) were independent factors associated with better bowel cleansing. Most of walking patients (97.4%) answered that walking-exercise is more comfortable than taking the solution. CONCLUSIONS: The walking-exercise was of an additional benefit to improve colonoscopic bowel cleansing without significant patients discomfort.
