2.Bicervical Bicornuate Uterus with Unilateral Cervical Atresia and Homolateral Renal Agenesis.

Double uterus with unilateral hematometra and ipsilateral renal agenesis is a rare Mullerian duct malformation. This complex of anomalies is seen in adolescents and young women with progressive dysmenorrhea, abdominal pain, menstrual irregularities and a pelvic mass. Appropriate preoperative diagnosis and treatment will prevent unnecessary procedure and offer relief of symptoms. We report one case of bicornuated uterus with unilateral cervical atresia and homolateral renal agenesis with a brief review of concerned literatures.

3.A Clinical Observation on 24 Hour Holter Monitoring: The Differences between Day and Night Time.

A modern development of ambulatory ECG monitoring gave great advances in the diagnosis and evaluation of various cardiac conditions. Fifteen cases of ischemic heart disease, 11 cases of nonischemic heart disease, 5 cases of noncardiac disease and 9 cases of normal healthy subjects were studied from January 1980 to July 1981 in national Medical Center, with the following results: 1) The heart rate was decreased during night time, with less decreasing tendency in patients with ischemic heart disease. 2) Among 30 cases, including 12 patients with ischemic heart disease, who didn't show arrhythmia on routine 12 lead ECG, 12 cases, including 6 patients with ischemic heart disease, showed arrhythmia on Holter monitoring. 3) Among 5 cases with ischemic heart disease who showed premature ventricular contraction on day time monitoring, 2 cases didn't show premature ventricular contraction on nigh time monitoring. 4) The S-T segment and T wave were changed during night time in 2 cases with ischemic heart disease and in 2 normal subjects. From these results, we could assert that Holter monioring or ambulatory ECG tracing would be a good method for diagnosis and evaluation of ischemic heart disease, and other cardiac conditions. Several another studies with this equipment have to be performed and would give more distinctive outcomes.

5.Factor VIII Related Antigen in Granuloma Pyogenicum.

Using Avidin-Biotin immunoperaxidase method, we investigate the distribution patterns of factor VIII related antigen in various areas of skin lesions of 14 granuloma pyogenicum. and in the skin lesions of 5 capillary hemangioma and 5 normal skin. The results are summarized as follows: 1. Factor VIII related antigen was detected in 9 of 14 cases of granuloma pyogenicum, in 4 of 5 cases of capillary hemangioma, and in 5 of 5 cases of normal skin. 2. In the lesions of granuloma pyogenicum the areas with prominent capillary endothelial cell proliferation had less positivity than the vascular areas. And the areas with relatively large vesels in the lesions of granuloma pyogenicum had similar positivity as capillary hemangioma. 3. The lesions of granuloma pyogenicum and capillary hemangioma had less positivity than the vessels of normal derrnis.

6.A Case of Lichenoid Drug Eruption Caused by Antituberculosis Drug.

Lichenoid drug eruption is lichenoid skin eruptions caused by certain drugs and compounds, and can be identical or similiar to lichen planus. A 75-year-old woman who had taken antituberculosis medication(INH, ethambutol, rifampin) for 4 months developed pruritic generalized erythematous papular eruptions on the trunk and extremities, alopecia and nail dystropy. Histopathologic findings were hyperkeratosis, hypergranulosis, hyc rophic degenaration of basal layer, band like lymphohistiocytic infiltration in the upper dermis and perivascular lymphohistiocytic infiltration in the deep dermis. She was treated with systemic corticosteroid, and then skin lesion were slightly improved. After termination of antituberculosis medication, skin lesions were markedly improved with residual hyperpigmentation. Alopecia and nail dystrophy were also improved.

7.A Clinical and Mycological Study of Tinea Pedis.

BACKGROUND: In tinea pedis, the response of treatment and prognosis are different according to clinical types. Positivity in KOH mount and causative agent in culture are also different. OBJECTIVE: Our purpose was to evaluate the clinical characteristics and mycologic findings of tinea pedis according to the clinical type. METHODS: A clinical and mycological study was conducted with 97 cases of tinea pedis among out patients examined for 7 months from June 1994 to December 1994 at Yeungnam University Hospital and Catholic Skin Clizic, Taegu, Korea. RESULTS: 1. Age distribution showed patients in their fourth decade to be most common. The ratio of male to female was 1.2: 1. The distribution of patients by clinical type was interdigital type, interdigital combined with hyperkeratotic type, interdigital combined with vesicular type, hyperkeratotic type, and hyperkeratotic combined with vesicular type, in descending order. One to five years was the most comrrion duration of tinea pedis. Duration of tinea pedis was the shortest in the vesicular type, otherwis was longer in hyperkeratotic type. Rate of family history of tinea pedis was 54.6%. The larger the size of family was, the higher the positivity in family history. The rate of coexistent dermatiophytosis with tinea pedis was 39.1%, and tinea unguium was the most common one. 2. The isolated dermatophytis were T. rubrum, 90.7%, T. mentagrophytes, 7.2%, and T. rubrum rnixed with T. mentagrophytes, 2.1%. T. rubrum showed an even distribution in all clinical types of tinea pedis whereas T. mentanophytes was isolated only in the interdigital type, vesicular type, and interdigital combined with vesicular type. T. rubrum mixed with T. mentagrophytes was isolated in the interdigital combine with vesicular type. Distribution of dermatophytes was relatively even arnong the age groups. T. rubrum showed a relatively even distribution in duration of tinea pedis, but T. mentagrophytes was isolated in tinea pedis with shorter duration.

8.A Case of Angiosarcoma.

Angiosarcoma is a rare malignant vascular tumor of endothelial cell origin, Cutaneous angiosarcoma usually occurs on the scalp and face of the elderly and most frequently in the sixth and seventh decade. We presented a case of angicisarcoma in a 70 year old woman. The patient had a well-demarcated, 3 x 3cm sized, dark brownish-colored, ulcerative nodule on the vertex with hemorrhagic bulla on the right. temporal scalp. Histopathologic examination of the nodule showed a well differentiated tumor with irregular anastomosing scular channels lined by atypical endothelial cells in the dermis and subcutaneous fat. Immunohistochenlical study for factorVlll-related antigen was partially positive in tumor channels. She was treated by wide surgical excision but she expired 5 months after discharge from the hospital.

9.Two Cases of Lichen Planus.

We have experienced 2 cases of lichen planus. Case 1 was a. 76-year-old male who has suffered from discrete, scaly, round to polygonal papules on the trunk and the e:xtremiities for 3 months. The VDRL test for syphilis was reactive at a titer of 1:2 and the TPHA test was positive. The skin lesions were responded to 3 weekly injections of benzathine penicillin 2.4 million units, Case 2 was a 17-year-old male who has suffered from discrete, erythematous to violaceous colored, flat papules on the right shin for several months. He has treated with oral medication of griseofulvin. After 2 weeks of the medication, the skin ]esions showed improvement with gradual reduction of pruritus and flattening of the papules. About one month of the medication, all skin lesions cliappeared with dark brownish hue and hypopigmentation. And recurreci lesions after stopping medication are disappeared after 2 months of retreatment with griseofulvin.

10.Refraction in School Children.

611 children (297 boys and 314 girls) of a primary school were determined under 2% cyclogyl. (1) Among 611 children, refractive error was 70.46 % and emmetropia. 29.54 %. (2) As to the distribution of refractive errors, 72.47 % were hyperopic and 27.53 % myopic Hyperopia was more common than myopia. (3) Hyperopic refractive error was increased until 7 years of age. Since then it has been decreased 8.74 % yearly. Myopic refractive error has been increased 8.14 % yearly since 8 years of age. (4) The refractive errors were more frequent in women (37.81) than men (32.65 %) (5) Refractive errors were numerous around the age of 11. (24.77 %). (6) The degree of refractive errors varied, but in the great majority was less than 3 diopters. (7) As to the type of astigmatism; 59.42 % were with the rule and 39.94 % were against the rule.
