3.Renovascular Hypertension Associated with Pheochromocytoma: A case report.

The coexistence of pheochromocytoma and renal artery stenosis has been recognized since 1958, but it is rare. We have experienced an anesthetic management of 43-year-old female patient of renovascular hypertension associated with right adrenal pheochromocytoma. Proper preoperative preparation, sufficient sedation, smooth induction, complete analgesia and good muscle relaxation were very important. Relative cardiovascular stability was achieved with sodium nitroglycerine, sodium nitroprusside and verapamil during tumor manipulation. After the tumor was removed, we could control the hypotension with dopamine, dobutamine, crystalloid and colloid. On the operative field there was no definite anatomic stenosis of renal artery and it was suggested that leakage of catecholamines from pheochromocytoma causes functional, reversible spasm of renal artery and thus renovascular hypertension.

4.Effectiveness of Intramuscular Stimulation Therapy in Patients with Osteoarthritis of the Knee Joint.

BACKGROUND: Osteoarthritis(OA) has become one of the most important medical problem among the old age because it causes severe functional disability, but, up to date, the therapeutic methods for OA have not been so satisfactory to the patients with OA. Authors have investigated the effectiveness of intramuscular stimulation(IMS) therapy as an substitutive therapy for the relief of pain and dysfunction in patients with OA of the knee. METHODS: Eighteen patients who had visited the department of family medicine, Korea university anam hospital, with symptomatic OA of the knee during the period from March 1 to May 31. 2001 were included in this study and treated with IMS therapy on the quadriceps and hamstring muscles four times during two weeks. Patients self-scored Korean Western Ontario and McMaster universities Osteoarthritis Index (KWOMAC) at baseline and after the last therapy. The result was analysed by paired t-test and ANOVA. RESULT: Patients improved on KWOMAC scales for pain, stiffness, function and total scores after IMS therapy(p<0.05). The demographic characteristics such as body mass index (BML), duration of disease, affected site, radiologic finding did not have influence on the result of this study. No adverse effects of IMS therapy were reported. CONCLUSION: In this study, the improvement of pain, stiffness, and function of knee joint after treatment was reported. This study suggest that IMS is an effective and safe substitutive therapy for patients with OA of Knee.

5.Solitary Subdural Osteoma.

Osteomas are benign neoplasms consisting of mature normal osseous tissue. They are common on the long bones of the extremities and found in the sinuses, facial bones, skull and madible in the head and neck region. Much rarer, however, are osteomas arising from subdural space with displacement of the underlying brain. A 42-year-old woman presented with a history of intermittent left frontal headache that was proved to be due to an intracranial lesion. After surgical removal, it was found to be an subdural osteoma. We report this case with pertinent literatual reviews.

6.The Value of Diagnostic Laparoscopy for Assessment of Resectability in Hepatobiliary and Pancreatic Malignancy under the Local Anesthesia.

BACKGROUNDS: Despite the advance of radiologic imaging techniques, not a fewer number of patients with hepatobiliary and pancreatic malignancies are found to be unresectable just at the time of laparotomy. The aim of this study was to evaluate the accuracy of diagnostic laparoscopy for assessing resectability of hepatobiliary and pancreatic malignancies. METHODS: From May 1999 to March 2000, diagnostic laparoscopy was performed in 16 patients with suspected hepatobiliary and pancreatic malignancies after clinical examination, laboratory tests, and radiologic imaging techniques(ultrasonography, computed tomography, ERCP, and PTC). Local anesthesia with intravenous sedation was used. A 10-mm infraumbilical port was used for the telescope and another 5-mm port was used for the working instrument depending upon intra-abdominal findings. During the procedure, we tried to find any evidence of distant metastasis or advanced local invasion such as, peritoneal seeding, metastatic nodule at the liver, major vascular involvement or invasion of adjacent other intraabdominal organ. RESULTS: 4 of 16 patients were found to have unresectable malignancies because of peritoneal seeding, hepatic metastases or ascites by diagnostic laparoscopy. 3 patients had invasion to superior mesenteric vein and adjacent structures overlooked at diagnostic laparoscopy. Sensitivity, specificity, and overall accuracy for assessing resectability were 66.7%, 28.6% and 50.0% for radiologic imaging techniques and 100%, 57.1% and 81.3% for diagnostic laparoscopy. CONCLUSION: Diagnostic laparoscopy under local anesthesia proved to be an effective diagnostic tool for assessing resectability of hepatobiliary and pancreatic malignancies by means of assisting the radiologic imaging technique.

7.A Clinical Review of 123 Cases of Pancreaticoduodenectomy.

BACKGROUNDS: Pancreaticoduodenectomy has been applied to diseases of periampullary region since 1935 by Whipple and associates, but this procedure is so complexed that associated with high morbidity and mortality. In the treatment of periampullary cancer, curative surgical resection only promises the possibilities of cure, because other methods of treatment including chemotherapy and radiation therapy have been of little or no benefit to improve the survival rate. METHODS: We performed the clinical analysis on 123 cases of pancreaticoduodenectomies at the department of Surgery, Chonnam University Hospital, during the past 11 years from June 1986 to May 1997 to assess the associated factors with postoperative complications and the survival rate. RESULTS: Among 123 cases, 104 cases(84.6%) were malignant tumor of periampullary region, 16 cases were benign lesion, and 3 cases were type IV pancreatic injury. Pancreaticoduodenectomy was performed in 115 cases, pylorus-preserving pancreaticoduodenectomy in 4 cases, total pancreatectomy in 2 cases and extended pancreaticoduodenectomy in 2 cases. Postoperative complications were developed in 28 cases(22.8%) and the major cause of postoperative complication was the anastomotic leakage, especially in the pancreatico-jejunostomy site. The overall operative and hospital mortality rate was 7.3%. Preoperative percutaneous transhepatic biliary drainage(PTBD) and the serum level of albumin and bilirubin were not related to the development of postoperative complications. Factors affecting survival after resection of malignant tumor were the size of tumor, presence of lymph node metastasis and tumor infiltration to adjacent tissue, but the level of CEA, alphaFP, and CA 19-9 and tumor differentiation were not related to the survival rate statistically. CONCLUSION: Optimal preoperative preparation of the patient and a meticulous and standarized operative technique are mandatory to minimize the operative morbidity and mortality after pancreaticoduodenectomy. In the treatment of periampullary cancer, radical surgical resection is recommended for long-term survival, as well as the development of other treatment modalities to prevent the postoperative recurrence.

8.Spinal Cysticercosis.

Cysticercosis in the human is caused by infestation by larvae of pork tapeworm, taenia solium. Although they may grow anywhere in the body such as the liver, lung, eyes, heart, muscle, ect., they are most commonly found in the brain and its coverings. The cysticercus vesicles have occasionally been found in spinal structures other than the brain. We present a case of spinal cysticercosis in a 63-year-old male. The patient presented with a marked paraparesis and paresthesia below a T10 dermatome level, and moderately decreased deep tendon reflexes. After performing total laminectomies at T11-12 & L3-5, 30 cysticercus vesicles were removed.

9.Humeral Osteomyelitis due to Staphylococcal aureus Infection as the Initial Presentation of Diabetes Mellitus.

Diabetes mellitus is a common disorder and presents in different ways. Shoulder pain is a common musculoskeletal complaint, especially in middle-aged or older-aged men. While it is widely accepted that diabetics have an increased propensity to develop infections, a diagnosis of osteomyelitis of the humerus as the etiology of constant shoulder pain might be delayed, as occurred in this case study, until patients' develop clear signs of infection. Here, we describe a case of subacute osteomyelitis as the first physical symptom of diabetes; the possibility of osteomyelitis should therefore be investigated in all patients with diabetes mellitus who develop joint or bone-related pain.

10.Biomechanical analysis of Korean Ilizarov Fixator.

The purpose of this study is to evaluate biomechanical stability of Korean Ilizarov fixator before clinical application. We measured the ultimate tensile load and stiffness of Ilizarov smooth wire and olive wire, bending stiffness of half-pin, ultimate compressive load and stiffness of Ilizarov ring using Instron 1331. Four types of Ilizarov assembly were tested for measuring stiffness of axial compression, anteroposterior bending, and lateral bending. There were group I fixed with only smooth wires; group II with only olive wires; group III with wires and half-pins; and group IV with only half-pins. The results of Korean Ilizarov apparatus were compared to those of American Ilizarov apparatus using unpaired t-test. The ultimate tensile strength of Korean smooth wire was greater than that of American wire. The ultimate tensile strength of Korean olive wire was lower than that of American wire. There was no significant difference of stiffness of axial compression, anteroposterior bending, and lateral bending between American and Korean apparatus. There was no significant difference of axial compression stiffness and bending stiffness between group III and group IV. In conclusion, Korean Ilizarov apparatus can be used with good stability for treatment of fracture, internal bone transport, and limb lengthening according to this biomechanical study.
