4.A Case of Primary Cutaneous Marginal Zone B-cell Lymphoma.

We herein present a rare case of a 42-year-old man with primary cutaneous marginal zone lymphoma(MZL) of the B-cell type on his shin. MZL is known to be the cutaneous counterpart of MALT(mucosa-associated lymphatic tissue) lymphoma. Histopathologically, MZL is characterized by multi-nodular infiltrates of centrocyte-like and centroblast-like marginal cells with bottom-heavy patterns, lymphoplasmacytoid differentiation, typical distribution of tumor cells in the marginal zone and follicular colonization by tumor cells. This B-cell lymphoma of a benign grade should be differentiated from mantle cell lymphoma and follicular center cell lymphoma.

5.A Electrodiagnostic Evalustion of Motor Neuron Involvement in Patients with Herpes Zoster.

BACKGROUND: Traditionally, herpes zoster (HZ) has been known to be a recrudescence of a latent viral infection of sensory ganglia, with the spread of the virus along sensory nerves and a dermatomal cutaneous eruption. However, clinically detectable muscular wenkness often occurs in patients with HZ. Accordingly, segmental or non-segmental motor neuron involvement by HZ may occur in addition to the involvement of sensory ganglion. OBJECTIVE: This study was conducted to estimate the frequency of motor neuron involvement in patients with HZ in Korea. METHODS: Fifty six subjects with HZ were enrolled in this study which used a conventional needle EMG (electromyography) and motor nerve conduction velocity test. Studies were conducted a minimum of 2 weeks after the onset of the rash. EMG was performed on the ipsilateral paravertebral muscles at the dermatomial levels of the eruption and controlateral sides. RESULTS: 1. One or more abnormal EMG findings(fibrillation potential, positive waves, bursts of high frequency discharge, polyphasia, fasciculation) were found in 51.8% of HZ patients. 2. Among the subjects with motor involvement, thoracic dermatome was most frequently involved (58.6%). 3. Ipsilateral motor involvement was more common(72.5%) than controlateral involvement(27.5%) among the patients with abnormal EMG findings. 55.2% of patients showed non-segmental motor involvement and that the altered EMG findings were not confined to the segment invaded by the eruption. 4, There was no association between the presence of postherpetic neuralgia or severity of pain or initial allodynia and EMG changes. CONCLUSION: Segmental or non-segmental motor neuron involvement by HZ may not be uncommon in Korea. HZ seems to cause widespread motor nerve changes, albeit usually subclinical alterations in the motor system, easily identifiable on EMG.

6.Risk Factors of Various Cerebrovascular Diseases and Sonographic Findings of Carotid Artery in Cerebral Infarction.

BACKGROUND: In Korea, cerebrovascular accident (CVA) is the most significant cause of death among older people, and the incidence of cerebral hemorrhage is much higher than that of developed countries. There have been many investigations about the risk factors for CVA in both Korea as well as developed countries. A few papers reported various risk factors for cerebral hemorrhage in developed countries:however, well-designed studies of risk factors for the various causes of CVA were rare in Korea. Therefore, the purpose of this study was to compare the risk factors for the various causes of CVA and to evaluate the risk factors compared with age- and sex-matched control groups. In addition, duplex sonographic findings of the carotid artery were evaluated in patients with cerebral infarction. METHODS: One hundred and sixty-four patients admitted to the hospital in 1996 were enrolled. The four groups were divided based on the following states: cerebral infarction (n-63), cerebral hemorrhage (n-64), cerebral infarction with atrial fibrillation (n-19), and lacunar infarction (n-18). Major risk factors were compared with age- and sex-matched control groups and among CVA groups. Duplex sonography of the carotid artery was done in 14 patients with cerebral infarction. RESULTS: In multiple logistic regression analysis, patients with cerebral infarction had higher prevalence of diabetes mellitus and lower high density lipoprotein-cholesterol level than the control group, and hypertension showed borderline significance. Patients with cerebral hemorrhage had higher prevalence of hypertension, higher high density lipoprotein-cholesterol level, and more frequent prevalence of smoking compared with the control group. Patients with cerebral infarction showed lower high density lipoprotein-cholesterol, higher low density lipoprotein-cholesterol levels, more frequent diabetes mellitus, lower prevalence of hypertension and older age than patients with cerebral hemorrhage. Patients with cerebral infarction and atrial fibrillation showed only older age than patients with cerebral infarction only. There were no differences in risk factors between patients with cerebral infarction and lacuna infarction. Atheromatous plaque was found in 71% of patients with cerebral infarction. CONCLUSION: Metabolic abnormalities played more important role in the development of cerebral infarction and hemodynamic abnormalities in cerebral infarction. Sonographic examination of the carotid artery may be useful for predicting the occurrence of cerebrovascular accident in high risk patients.

Heterotopic pregnancy is the coexistence of intrauterine and extrauterine pregnancy. It is a rare obstetrical phenomenon with an incidence rate about 0.003%. The risk factors of heterotopic pregnancy are IUD, PID, endometriosis, tubal surgery, etc resulting in functional and anatomical injury to tubes. Recently, the wide use of ovulation induction and in vitro fertilization has increased the incidence rate. The clinical manifestation are low abdominal pain, adnexal mass, peritoneal irritation signs, vaginal bleeding, etc. Early detecion of heterotopic pregnancy is very difficult. However, recently, due to development of pelviscopy, ultrasonogram and other diagnostic procedures, the diagnostic rate has increased.Because of the high maternal morbidity and mortality resulted from intraabdominal bleeding after rupture of ectopic pregnacy, immediate termination of ectopic pregnancy is required usually by operative methods. We experienced a case of heterotopic pregnancy after tubal reanastomosis and report this case with a brief review of literatures.

Congenital anomalies occur in 2-3% of neonates and have unknown and variable causes. It's occurance rate is higher in twin gestations than in singleton gestations, especially in monozygotic twins. In most cases of twin anomalies, one fetus is normal and the other fetus is not. When an anomaly is found in one fetus, various tests, such as chorionic villus sampling, amniocentesis, and umbilical cord aspiration are strongly recommended in high risk groups of chromosmal anomaly for accurate diagnosis and proper treatments. A case of congenital anomalies in both twins diagnosed in a 35 year old multiparous woman is presented with brief review of literatures.

9.A Case of Langerhans Cell Histiocytosis in an Adult.

Langerhans sell bistiocytosis(LCH) is a reactive disease in which abnomal Langerhans cells accurnulate in various body sites and cause damage to affected organs. LCH usually occurs in childrea bur can also affect adults although rarely. LCH in a case of 65-year-old man initially involwed the lymph nodes (left supraclacicular and right inguinal area) and pelvis bone, and eight months later curancous involvement was noted. The skin lesions were waxy from the waxy papules showed that the dense infiltrate, ptedominantly of histiocytes, was present in the dermis, Immunohistochemistry for S-100 protein showed positive staining snd electron miscoroscopy disclosed Birbeck granules, which is characteristic findings of Langerhans cells.

10.The Effects of Leukocyte Depleted Priming Solution on the Gastric Mucosal CO2 Partial Pressure and Serum IL-8 Level during Cardiopulmonary Bypass in Korean Mongrel Dogs.

BACKGROUND: Cardiopulmonary bypass is an essential process to maintain circulation for saving life during the cardiac surgery. But it is a process in which systemic inflammation was evoked inevitably because of the exposure of blood to foreign surface. The injuries to distal organs during the cardiopulmonary bypass were resulted from systemic inflammation and the disturbances of micro-circulations in the organs. We designed this study to research the effects of leukocyte depletion from pump-oxygenator priming solution on the systemic inflammation, and the micro-circulation of gastric mucosa that is suggested by the gastric mucosal CO2 partial pressure and acidity. MATERIAL AND METHOD: The dogs were divided into three groups according to the different pump-oxygenator priming solutions; non-hemic crystalloid solution; leukocyte-depleted homologous blood; and non leukocyte-depleted homologous blood. Each priming solution group contained five dogs. In all three groups, 2 hours of cardiopulmonary bypass, and 4 consecutive hours of general anesthesia was maintained on the mechanical ventilation. Each dog was evaluated for the gastric mucosal pH, CO2 partial pressure, arterial pH, CO2 partial pressure, the exhaled air CO2 partial pressure and the level of IL-8 on before the cardiopulmonary bypass, 1 hour after the cardiopulmonary bypass, 2 hours after the cardiopulmonary bypass, 2 hours after the restoration of normal circulation, and 4 hours after the restoration of normal circulation after the cardiopulmonary bypass. The levels of IL-8 were measured with ELISA (enzyme linked immunosorbent assay) technique. RESULT: 1. There were significant differences of gastric mucosal CO2 partial pressure between the leukocyte-depleted homologous blood group and other two groups(vs non leukocyte-depleted homologous blood group; p=0.02, vs non-hemic crystalloid solution group; p=0.01). 2. The gastric mucosal pH of leukocyte-depleted homologous blood group was significantly different from non leukocyte-depleted homologous blood group (p=0.01). 3. The levels of IL-8, which examine the systemic inflammation, showed significantly better results in leukocyte-depleted homologous blood group and non-hemic crystalloid solution group than non leukocyte-depleted homologous blood group (p=0.01, 0.01). CONCLUSION: Based upon these results, we concluded that the leukocyte depletion from the pump-oxygenator priming solution has a beneficial effects in reducing systemic inflammation and the preserving of gastric mucosal micro-circulation.
