4.Vesical Endometriosis : A Review and 1 Case Report.

The term endometriosis refers to ectopic endometriume. It most often involves the pelvic organs, including ovaries, broad and round ligaments, fallopian tubes, cervix, vagina and the pouch of Douglas. We report a case of vesical endometriosis in a 4- year old female patient and review literature. Segmental resection of this mass with involved bladder resulted in cure and this lesion was confirmed histologically as endometriosis of the bladder.

5.A Clinical Observation of Epididymal Tuberculosis Mimicking the Symptoms and Signs of Acute Epididymitis.

A clinical observation was undertaken on 7 cases of epididymal tuberculosis mimicking the symptoms and signs of acute epididymitis and 24 cases of all tuberculous epididymitis which were seen during the period from September 1983 to July 1987. The results obtained were as follows. 1. A total of 7 cases of epididymal tuberculosis mimicking the symptoms and signs of acute epididymitis corresponds to 29% of epididymal tuberculosis(24 cases). 2. The highest occurrence was observed in the age of 30 to 39(5 cases). 3. Lateralization showed 5 cases in the right, 2 cases in the left and none in both. 4. Presenting symptoms and signs were painful scrotal swelling in all 7 cases, fever 4 cases, tender epididymal nodule 4 cases and local heating 2 cases but beading of vas and scrotal fistula was not found. 5. In urine examination, pyuria was observed in 5 cases, hematuria in 1 case and no tubercle bacilli was observed. 6. Among 7 cases, we observed that epididymal tuberculosis were associated with pulmonary tuberculosis, active or inactive, in 5 cases and renal tuberculosis in 1 case.

6.Changes in Serum Electrolyte Following Transurethral Resection of the Prostate When 1.5% Glycine Solution and 5% Dextrose-Water Solution are Used as Irrigants.

Transurethral resection syndrome are caused by the sudden intravascular absorption of large volumes of the irrigating solution which are used during transurethral surgery. It relates to many determining conditions of the patient postoperatively. We studied changes in serum electrolyte following transurethral resection of prostate in 61 patients used 5% Dextrose-Water solution as the irrigant and 46 patients used 1.5% glycine as the irrigant of benign prostatic hypertrophy. We divided the patients into group I and II. group I : those were given 5% Dextrose-Water solution as the irrigant. group II : those were given 1.5% glycine as the irrigant. Change in serum ammonia was obtained in 19 patients of group II. Following results were obtained. 1. Operation time, resected weight and amount of irrigant between group I and II showed no difference, statistically. 2. Serum sodium concentration was decreased in 42 cases of group I (range : 1-20mEq/L, mean : 5.0mEq/L). 3. Serum sodium concentration was decreased in 31 cases of group II (range : 1-18mEq/L, mean : 6.0mEq/L). These changes in serum concentration between group I and group II showed no significant difference, statistically. 4. Change in serum sodium concentration in the group I showed no significant correlation with operation time, resected weight and amount of irrigation. 5. Change in serum sodium concentration in the group II showed no significant correlation with operation time, resected weight and amount of irrigation. 6. Serum ammonia concentration was increased in 10 patients of 19 patients in group II (range : 5-100ug/dl, mean : 42ug/dl).

7.Primary malignant Fibrous Histiocytoma (MFH) of Pleura: A case report.

Primary malignant fibrous histocytoma (MFH) of pleura is very rare. Upon microscopic examination, the tumor was characterized by storiform pattern of growth and intense, diffuse immunostaining for alpha l-antichymotripsin. We report a case of primary malignant pleural fibers histiocytoma with extension to lung, which was managed by complete excision of pleural mass and lobectomy with thoracoplasty.

8.Pulmonary Thromboembolism Developed in Recovery Period of upper Abdominal Surgery: Case report.

Pulmonary thromboembolism (PTE) is a serious postoperative complication. Prompt diagnosis of PTE is important but it is difficult because clinical manifestations of PTE are not obvious in most cases. A 59 year-old woman received cholecystectomy and choledocholithotomy under general anesthesia. At the 9th postoperative day, syncope, hypoxemia and hypotension were developed suddenly and the patient was transfered to intensive care unit. PTE was suspected with hemodynamic monitoring from pulmonary arterial catheter and echocardiography and diagnosed with lung perfusion scan and venogram of lower extremities. Proper cardiopulmonary support was done and the patient was recovered and discharged in improved condition. In conclusion, if a patient have syncopal attack with shock in postoperative period, PTE must be thought to be an one of possible causes of syncope and thoroughly investigated.

9.A Case of pulmonary lymphangioleiomyomatosis developed in tuberous sclerosis with renal angiomyolipoma.

Tuberous sclerosis is a rare hereditary anomaly of variable penetrance characterized by mental retardation, epilepsy, and adenoma sebaceum. Pulmonary involvement is uncommon, but, when involve by tuberous sclerosis, it shows characteristic reticulonodular infiltration and cystic changes. Lymphangioleiomyomatosis is a rare disease of unknown cause that affects women of reproductive age. It is characterized by progressive proliferation of smooth muscle in the lung. We experienced one case of pulmonary lymphangioleiomyomatosis associated with tuberous sclerosis in a 26-year-old female patient. This case is reported with a brief of the literature.
