1.Urinary Retention after Perianal Operation.

BACKGROUND: Urinary retention is the most common complication after perianal surgery. The authors tried to evaluate the influence of the types and duration of the operation and the types of anesthesia on the incidence of urinary retention. METHODS: The medical and anesthetic records of 106 patients were reviewed retrospectively. They got perianal surgerys after recieving one of the regional blocks; 0.5% hyperbaric bupivacaine 5~8 mg intrathecally, 0.5% hyperbaric tetracaine 5~8 mg intrathecally, or 2% lidocaine 300 mg with epinephrine 5 g/ml caudally. The incidences of urinary retention were compared with each other by Chi-square test and Student t-test, according to the above mentioned points. RESULTS: Fifty percent of these patients underwent urinary catheterization. The incidence of urinary retention after hemorrhoidectomy (56.6%) was higher than that of other anorectal procedures (p<0.05, Chi-square test). The longer operating time was associated with urinary retention (p<0.05, Student t-test). The influence of local anesthetics (bupivacaine, tetracaine, and lidocaine) was absent (p>0.05, Chi-square test), but the difference between spinal and caudal anesthesia was slightly significant (p<0.05, Chi-square test), i.e. the incidence of urinary retention after caudal anesthesia was low. CONCLUSIONS: Short duration of operation, less traumatized perianal surgery, and caudal anesthesia are thought to lead to the lower incidence of urinary retention.

2.A Case of Allergic Contact Dermatitis to Nitrofurazone Ointment.

Nitrofurazone is a well known sensitizer and is widely used particularly in surgical department for the treatment of secondary infection, but the reports of all ergic contact dermatitis to nitrofurazone are rare in Korean literature. We experienced a case of allergic eontact dermatitis to 0.2% nitrofurazone in 31-year-old female after dressing the grafted wound on the left index finger with the gauze embedded with 0.2% nitrofurazone olintmnt. The patch test results revealed a strong positve reaction to 0.2% nitrofurazone ointment after 48 hours.

3.Immunohistochemical Expression of p53, E-cadherin, and nm23 Proteins in Metastatic Carcinoma of Neck Lymph Node and Corresponding Primary Carcinoma.

This study was carried out to evaluate the immunohistochemical expressions of p53, E-cadherin, and nm23 proteins in 114 cases of metastatic carcinoma of the neck lymph node (MTLNCA) and corresponding primary carcinoma (PRCA). The positive expressions of p53, E-cadherin, and nm23 proteins were 62.3%, 58.8% and 64.0%, respectively in PRCA, and 40.4%, 38.6%, and 43.9%, respectively in MTLNCA with significant down-regulation from PRCA to MTLNCA (p<0.05). The down-regulation was correlated with female gender, moderate and poor differentiation, and adenocarcinoma in p53 protein, female gender, respiratory and gastrointestinal carcinoma in E-cadherin protein, and female gender, respiratory carcinoma, moderate differentiation, and squamous cell carcinoma in nm23 protein (p<0.05). There was no significant relationship among expressions of p53, E-cadherin, and nm23 proteins (p<0.05). In conclusion, these results suggest that the expressions of p53, E-cadherin, and nm23 proteins seem to be down-regulated from PRCA to MTLNCA and this down-regulation may play a role in invasion and metastasis.

4.A Case of Eccrine Spiradenoma.

We present a case of eccrine spiradenoma occurring on the trunk in a 33 year old woman. She developed a pea sized, pinkish tender nodule on the anterior chest 8 years ago. On histologic examination, there were several lobules surrounded by thin connective tissue septa in dermis. The lobules themselves consisted of small dark staining cells and large pale-staining cells, forming solid cords or ducts.

5.A Case of Recurrent Melanocytic Nevus.

Recurrent melanocytic nevus forms after incomplete removal of a melanocytic nevus and is chatacterized by somewhat proliferation of atypical melanocytes at the dermoepidermal junction and epidermis or recurrent junctional activity. A 73-yeat-old man had a pigmented lesion on the volar aspect of the right heel, which was excised 20 years ago. Five years thereafter, the pigmented lesion recurred in the previous operation scar and had been excised again. But pigmented lesion reappeared in the scar as an irregular patch of variegated pigmentation of browm, gray and black. Histopathologic findings showed fibrosis in the dermis and proliferation of normal and atypical rnelanocytes in the overlying epiderrnis.

6.Histopathological Observation of Fixed Drug Eruption.

Thirty-nine patients with fixed drug eruption who visited the Department of Dermatology in National Medical Center from January 1g7g to December 1982 were selected. Among them, twentyseven patients performed skin biopsy and were evaluated histopathologically on the basis of duration. The results were as follows; ].In the early stage less than 1 week of duration, hydropic degeneration of basal cells, necrotic keratinocytes with eosinophilic cytoplasm and pknotic nucleus in the epidermis, and mixed cell infiltration with eosinophlis in the upper dermis were prominently. In the late stage more than 20 days of duration, acanthosis, an increased amount of melanin in the basal cell layer and presence of melanophages were found more prominent, however, hydropic degeneration of basal cells, papillary dermal ederna, inflammatory infiltration of mononuclear cells were found less prounced or absent.

9.Present status of Enterobius vermicularis infestation among children in kindergartens.

BACKGROUND: An Epidemiological study was undertaken to evaluate the prevalence of E. vermicularis infestation among children in kindergartens. METHODS: A total of 247 cellotape anal swab specimens from kindergarten children in Inchon were examined for E. vermicularis eggs. Each specimen was collected in the morning. RESULTS: Out of 247 specimens from two kindergartens in Inchon, E. vermicularis eggs were detected in 168 specimens, the egg detection rate being 68.0%. The egg detection rates in boys and girls were 63.9% out of 133 children and 72.8% out of 114 children, respectively. The egg detection rate in girls was higher than that of boys. The egg detection rates by age group was 65.5%(age 3), S7.4%(age 4), 72.0%(age 5) and 75.4%(age 6), respectively. CONCLUSIONS: Enterobiasis is a cosmopolitan infection. Surveys for children of kindergartens indicate that E.vermicularis is probably the most widely distributed human helminth, its incidence varing from 59.2% to 73.8% in groups studied since pinworm infection is frequently a familial problem that infestation of the household frequently results in reinfection. Careful washing of hands and cleansing of fingernails before meals and after use of the toilet are the most practical measures, all member of family should be treated simultaneously when practicable.
