The majority of ptients with ischemic heart disease such as angina pectoris and myocardial infarction have coronary atherosclerosis as a underlying cause. However, in unusual conditions such as variant angina, aortic valvular heart disease, hypertrophic cardiomyopathy, vasculitis, hypercoagulable states, coronary atherosclerosis is not the cause. Polycythemia vesa is a myeloproliferative disease characterized by infrequent thromboembolic complications such as ischemic heart disease. We report a case of polycythemia vera presenting with angina pectoris without coronary artery stenosis with the review of the literature.

4.The Exposure Status and Biomarkers of Bisphenol A in Shipyard Workers.

OBJECTIVES: Because shipyard workers are involved with various manufacturing process, they are exposed to many kinds of hazardous materials. Welders especially, are exposed to bisphenol-A (BPA) during the welding and flame cutting of coated steel. This study was conducted to assess the exposure status of the endocrine disruptor based on the job-exposure matrix. The effects of the genetic polymorphism of xenobiotic enzyme metabolisms involved in the metabolism of BPA on the levels of urinary metabolite were investigated. METHODS: The study population was recruited from a shipyard company in the K province. A total of 84 shipbuilding workers 47 and 37 in the exposed and control groups, respectively, were recruited for this study. The questionnaire variables included, age, sex, use of personal protective equipment, smoking, drinking and work duration. The urinary metabolite was collected in the afternoon and correction made for the urinary creatinine concentration. The of the CYP1A1, CYP2E1 and UGT1A6 genotypes were investigated using polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) methods with the DNA extracted from venous blood. RESULTS: The urinary BPA level in the welders group was significantly higher than in the control group (p< 0.05). The urinary BPA concentration with the wild type UGT1A6 was higher than the other UGT1A6 genotypes, but with no statistical significant. From themultiple regression analysis of the urinary BPA, the regression coefficient for job grade was statistically significant (p< 0.05). CONCLUSIONS: The grade of exposure to BPA affected the urinary BPA concentration was statistically significant. However, the genetic polymorphisms of xenobiotics enzyme metabolism were not statistically significant. Further investigation of the genetic polymorphisms with a larger sample size is needed.

Primary adenocarcinoma of the duodenum is a rare disease, representing 0.35% of all gastrointestinal neoplasm and 0.042% of all malignant neoplasm. Since Ham-burger described the first duodenal caricinoma in 1746, the incidence has shown an increasing tendency. Compared with upper gastrointestinal contrast studies, the gastroduodenoscopy has gained an increasing popularity as an invesitigative tool in recent years. But the diagnosis is usually made at a very late stage because the symptoms are often non-specific, which contributes to delayed diagonsiss and poorer prognosis. We have experienced two cases of the primary carcinoma of the duodenal bulb and one case of carcinoma of the second portion, which were diagnosed by biopsy with gastrofiberoscopic endoscopy and operative finding. So we report three csses with a brief review of literature.

7.Polyneuropathy in Patients with Chronic Obstructive Pulmonary Disease.

The incidence, type arid distribution of polyneuropathy in patients with chronic obstructive pulmonary disease (COPD) were assessed and also analyzed the causative factors. Forty-four patients, mean age 66.1 years (42 male, 2 female), have been investigated with arterial gas analysis, pulmonary function test, clinical and electrodiagnostic studies. None of them had conditions known to affect the peripheral nervous system such as metabolic disorders or drugs. In a selected group of 44 patients, electrophysiological findings of polyneuropathy were found in 22 patients(50%), clinical polyneurtpathy were diagnosed in 13 patients(9 patients were diagnosed by electrophysiological studies, 4 patients were normal by electrophysiological studies). These findings indicate that subclinical polyneuropathy( 13 patisnts, 30%) more conimorily occurs than clinical polyneuropathy( 9 patients, 20%) in associated with COPD. In the patients with polyneuropathy, the lesions were predominant axonal degeneration, the changes were more involved in leg than arm, more frequently affected sensory fibers. We could not find etiologic factor to cause polyneuropathy in COPD patients.

8.Two Cases of Poikiloderma of Neck and Chest Treated by Intense Pulsed Light.

Poikiloderma is a combination of telangiectasia, irregular mottled hyperpigmentation/depigmentation and superficial atrophy in a reticular pattern. Poikiloderma of Civatte is a rather common, acquired poikiloderma of the face and neck, most often affecting the middle-aged or the elderly. Poikiloderma of Civatte runs a chronic, benign, but irreversible course. The ideal treatment is elimination of both the vascular and pigmented components simultaneously. Most methods to treat this condition are ineffective, inefficient, or inconvenient and may cause significant side effects such as depigmentation and scarring. Recently, intense pulsed light sources have been used in poikiloderma of Civatte with their ability to target vascular and pigment components simultaneously. We experienced two cases of poikiloderma of Civatte treated by intense pulsed light with different parameters than those used in former reports, so we report this case with a review of previously published cases.

9.Inhibitory Effect of Pentose on Biofilm Formation by Oral Bacteria.

A number of bacterial species coexist in oral cavities as a biofilm rather than a planktonic arrangement. By forming an oral biofilm with quorum sensing properties, microorganisms can develop a higher pathogenic potential and stronger resistance to the host immune system and antibiotics. Hence, the inhibition of biofilm formation has become a major research issue for the future prevention and treatment of oral diseases. In this study, we investigated the effects of pentose on biofilm formation and phenotypic changes using wild type oral bacteria obtained from healthy human saliva. D-ribose and D-arabinose were found to inhibit biofilm formation, but have no effects on the growth of each oral bacterium tested. Pentoses may thus be good candidate biofilm inhibitors without growth-inhibition activity and be employed for the future prevention or treatment of oral diseases.

10.Characterization of Actinic Keratosis by Dermoscopy in Koreans.

BACKGROUND: Dermoscopy is a real-time, noninvasive technique that increases the diagnostic accuracy of clinical evaluation by the dermatologist. Characterization of facial actinic keratosis via dermoscopy is valuable in guiding the dermatologist in the differential diagnosis of facial skin lesions. To our knowledge, dermoscopic evaluation of actinic keratosis has not yet been reported in Korean literature. OBJECTIVE: The purpose of this study was to investigate the dermoscopic features of facial actinic keratosis in Koreans. METHODS: We examined 60 patients with facial actinic keratosis using dermoscopy. We determined the frequency of specific features revealed by dermoscopy across the Korean patient population and compared our results with those of other published studies. RESULTS: A total of 67 facial actinic keratoses were observed in 60 patients. The common features of non-pigmented actinic keratoses revealed by dermoscopy were a red pseudo-network (66%), white-to-yellow surface scales (48.9%), linear wavy vessels (38.3%), and targetoid hair follicles (42.6%). The common features of pigmented actinic keratoses revealed by dermoscopy were slate-grey dots (55%) and globules (30%), annular-granular pattern (35%), a brown-to-gray pseudo-network (40%), and targetoid hair follicles (30%). The frequency of specific features revealed by dermoscopy across the analyzed patient population was not significantly different from that reported in other studies, except for rhomboidal structures (5%), which was lower in Koreans than in other populations (35.8%, p<0.05). The frequency of dermoscopic features was different according to histopathologic subtypes (hypertrophic, atrophic, bowenoid, acantholytic actinic keratosis). CONCLUSION: Dermoscopy revealed specific features of facial actinic keratosis that can be utilized to evaluate and diagnose facial skin lesions.
