1.Leiomyosarcoma of the inferior vena cava: a case report.

A 29 year old woman with unresectable leiomyosarcoma of the inferior vena cave extending into the right atrium of the heart was presented. The upper and middle inferior vena cava involved and there were both intrinsic and extrinsic tumor growth components. The diagnosis was made by a combination of radiological studies including ultrasound, CT, inferior vena cavography and arteriography and the diagnosis was confirmed by ultrasound guided rumor biopsy. The clinical and radiological presenting features were reviewed.

3.A Case of Benign Symmetric Lipomatosis.

Benign symmetric lipomatosis is characterised by diffuse symmetric deposits of adipose tissue, predominantly on the neck, shoulder, back and upper extremities. We report a case of benign symmetric lipomatosis in a 61-year-old-man, which was first noted one year previously. It began growing rapidly about 3 months ago and was associated with alcoholic liver disease. Histopathological findings showed that the reticular dermis had been replaced by normal uncapsulated mature fat cells that were slightly increased in number in the fibrous connective tissue. He was treated with theophylline 300mg/day for two months.

4.Needs Assessment for Functionalities in Electronic Health Record Systems in General Hospitals.

OBJECTIVE: As an electronic health record system is implementing in Korean health care sectors, concerns about key functionalities of electronic health record systems are increasing. The purpose of this study was to identify core functions and set the priority in electronic health record systems under the Korean contexts in order to assure and improve the quality of the systems. METHODS: A survey was conducted using questionnaire developed by the study team based on literature review. The subjects were medical record administrators working at medical record department in general hospitals. RESULTS: The response rate was 59.8%(55/92). The functions which more than ninety percent of subjects responded as necessary right now and/or in near future related to 'drug alert', 'clinical guideline', 'chronic disease management', 'automated real-time surveillance', 'coded data', 'result reporting', 'de-identifying data', 'disease registry', and 'provider-provider communication and connectivity'. CONCLUSION: The results showed the high prioritized functions were decision support and health information/data management.

5.The Inhibitory Effect of Myopic and Astigmatic Progression by Orthokeratology Lens.

PURPOSE: To investigate the clinical effects of orthokeratology lens wear on inhibition of the progression of Korean myopia and astigmatism. METHODS: The authors reviewed out-patient records of 144 eyes of 74 patients wearing orthokeratology lenses. The cycloplegic refraction and keratometry before and after wearing the lens were compared. The 190 eyes wearing spectacles were included into the control group. We evaluated the relationship between orthokeratology lens wear and control group according to age, initial myopia, initial astigmatism, myopic progression, astigmatic progression and duration of orthokeratology lens wearing. RESULTS: The mean cycloplegic refractive error of spherical equivalent was -3.36 +/- 1.96 diopters (D) in the patients wearing orthokeratology lenses, the mean astigmatism was -0.86 +/- 0.72 D, and the mean wearing period was 3.41 +/- 1.5 years. The mean myopic progression was 0.25 +/- 0.31 D in lens wearing, and 0.62 +/- 0.39 D in glasses wearing, respectively. There were statistically significant differences between lens and glasses wearing group (t-test, p < 0.01). The mean astigmatic progression was 0.06 +/- 0.22 D in lens wearing, and 0.15 +/- 0.21 D in glasses wearing control group respectively, and the results also show statistically significant differences (t-test, p < 0.01). There were no relationships between two groups as for age, initial myopia, initial astigmatism and duration of orthokeratology lens wearing (t-test, p > 0.01). CONCLUSIONS: The orthokeratology lens was found to be effective in suppression of the progression of Korean myopia and astigmatism, compared with the glasses.

6.The Business Analysis on the Chronic Disease Management in PublicHealth Centers for the Electronic Health Records System.

OBJECTIVE: This study analyzes the chronic disease management, which is a core business of the public health centers, and grasps the functions of the information system that users need. METHODS: It is conducted by analyzing work flows in two public health centers that using two different health information systems, Posdata system and Daeryun systems. We also performed detailed interview and discussion to formulate the functionality for the information system in the future. RESULTS: Both two health information systems currently used at public health centers emphasized hypertension, diabetes mellitus and hyperlipidemia as major target disease. They used information systems as a tool of case registration, assessment, reporting, consultation and education and patient care. But functionality of current systems supports only a part of the whole system and manual data input using excel program was still applied. The current information system is just inputting the data of manual works in computer and many of the functions are not used and still there are many duplicate and manual works. Two major problems of information systems in Korean public health centers are lack of standardization in the business process and reporting, and the current system does not support current functionalities. CONCLUSION: We found that management of the chronic disease is to be more important work for the public health centers in the future. The future direction of the work process of the management of chronic disease would be upgrading the current functionality as well as new functionalities. Better linkage with direct care system and interoperability with national electronic health records system are new demands.

9.Diagnostic Performance of a Combination of Shear Wave Elastography and B-Mode Ultrasonography in Differentiating Benign From Malignant Thyroid Nodules

Objectives:
. This study was conducted to compare clinicopathologic and radiologic factors between benign and malignant thyroid nodules and to evaluate the diagnostic performance of shear wave elastography (SWE) combined with B-mode ultrasonography (US) in differentiating malignant from benign thyroid nodules.
Methods:
. This retrospective study included 92 consecutive patients with 95 thyroid nodules examined on B-mode US and SWE before US-guided fine-needle aspiration biopsy or surgical excision. B-mode US findings (composition, echogenicity, margin, shape, and calcification) and SWE elasticity parameters (maximum [Emax], mean, minimum, and nodule-to-normal parenchymal ratio of elasticity) were reviewed and compared between benign and malignant thyroid nodules. The diagnostic performance of B-mode US and SWE for predicting malignant thyroid nodules was analyzed. The optimal cutoff values of elasticity parameters for identifying malignancy were determined. Diagnostic performance was compared between B-mode US only, SWE only, and the combination of B-mode US with SWE.
Results:
. On multivariate logistic regression analysis, age (odds ratio [OR], 0.90; P=0.028), a taller-than-wide shape (OR, 11.3; P=0.040), the presence of calcifications (OR, 15.0; P=0.021), and Emax (OR, 1.22; P=0.021) were independent predictors of malignancy in thyroid nodules. The combined use of B-mode US findings and SWE yielded improvements in sensitivity, the positive predictive value, the negative predictive value, and accuracy compared with the use of B-mode US findings only, but with no statistical significance.
Conclusion
. When SWE was combined with B-mode US, the diagnostic performance was better than when only B-mode US was used, although the difference was not statistically significant.
10.Comparison of Blinking Patterns When Watching Ultra-high Definition Television: Normal versus Dry Eyes.

PURPOSE: To analyze blinking patterns when watching an ultra-high definition (UHD) television and to compare the results between normal eyes and dry eyes. METHODS: A total of 59 participants aged from 13 to 69 years were instructed to watch a colorful and dynamic video on a UHD television for 10 minutes. Before and after watching the UHD television, we measured the best corrected visual acuities, autorefraction, tear-break-up-time, degree of corneal erosion and conjunctival hyperemia via slit lamp biomicroscopy. In addition, questionnaires for the evaluation of eye fatigue and symptoms of a dry eye were completed. The definition of dry eye syndrome was that the tear-break-up-time of one of the eyes was less than 5 seconds, conjunctival injection, or marked corneal erosion. The number of blinks and the duration of blinking were both measured and analyzed at the early and late phases of video-watching. RESULTS: After watching the UHD television in the normal eye group, the tear-break-up-time was significantly decreased (p < 0.001) and the degree of corneal erosion was significantly increased (p = 0.023). However, the subjective symptoms of participants were not aggravated (p = 0.080). There were no significant differences in blinking patterns in the dry eye group. On the other hand, in the normal eye group, the mean blinking time was significantly increased (p = 0.030). CONCLUSIONS: Watching an UHD television changes the tear-break-up-time, degree of corneal erosion, and blinking pattern in normal eyes, which may increase the risk of dry eye syndrome.
