1.BONE REGENERATION IN COMPOSITE GRAFT OF FREEZE-DRIEDDEMINERALIZED BONE AND HYDROXYLAPATITE IN RABBIT CRANIAL DEFECTS.

Recent development of oral and maxillofacial surgery has taken an interest in the regeneration of facial bone defects. For the reconstruction of bony defects, autogenousbone grafts have been widely used. However, there are some clinical problems ; the morbidity of donor sites, resorption of autogenous bone graft and the availability of the proper form and size. The purpose of this study is to find the proper filling materials for the bone defects. The present study was designed to evaluate the roles and regenerating capacity of maikture of freeze-dried domineralized allogenic bone and hydroxylapatite. Fifteen adult New Zealand white rabbits were used as the experimental animal, Four trephine defects were made by drilling on the parietal bone of each rabbit. The size of each defects was 8x8mm. In first group, the defect was filled with freeze-dried demineralized allogenic bone and hydroxylapatite. The defect in second group was filled with autogenous bone chip only. The third group was filled with autogenous bone chip and hydroxylapatite. The fourth group was filled with freeze-dried dimineralized allogenic bone only. The results were carefully examined grossly and micorsocopically from the 2 weeks to 12 weeks postoperatively. The following results were obtained ; 1. In the case of the graft with autogenous bone chip only, new bone formation was more active than any other groups. 2. In the case of the graft with combination of freeze-dried demineralized allogenic bone and hydroxylapatite, new bone formation was similar to the case of autogenous bone and hydroxylapatite mixture. 3. In the case of the graft with freeze-dried demineralized bone, new bone formation was observed, but less than the other groups. And the time of new bone appeared was later than the other groups. A combination of freeze-dried deminearalized allogenic bone and hydroxylapatite plays an effective role in forming new bone. The results of this study may suggest the possible usage of the mixture of freeze-dried allogenic bone and hydroxylapatite as an alternative to autogenous bone in maxillofacial orthopedic surgery.

3.A Psychiatric Study on Hahn.

OBJECTIVE: This study aimed at identifying the etiological background, symptomatic expressions, defense styles and coping strategies related to hahn. METHOD: The subjects were given a questionnaire for demographic data, life experiences and psychological and physical symptoms, Bond's questionnaire of defense style, and Weisman's scale for coping strategies and scales for severity of hahn and hwabyung. The subjects were 110 normal healthy adults and 151 patients with neurotic disorders, diagnosed with ICD-9. RESULTS: More vivid hahn was found in the married women, the less educated, and those from a low socio-economic state. In the area of etiological life experiences, hahn is significantly related with an unhappy and frustrated marital life, unsatisfactory filial Piety, Poor family background, poverty, little education, a hard life since childhood, betrayal, failure in business, personality problems, injustice in society and resignation to fate. Hahn, unresolved and accumulated, was believed to cause various psychological and physical illnesses. In many cases hahn was corhad been dealt with by simple suppression and being dependent on religious faith or sharing and communicating with people. More people considered hahn as one of the etiologies of hwabyung, a chronic anger syndrome. Hahn of patients with hwabyung was more serious than that of those without hwabyung. Symptoms of hahn were - in addition to various symptoms of depression, anxiety and other neurotic conditions or somatization - a mass in the epigastrium, sighing, something pushing-up in the chest, paranoid ideas and hyprochondria sis nor hygochondriacal ideas. On the other hand, hypersensitivity and irritability were less significant. Defense styles and coping strategies related with hahn were somatization, splitting-projection, passive-aggressiveness, incorporation, primitive idealization, stimulus reduction, self-pity and shared-concerns and dependency. CONCLUSION: These results suggest that hahn is an unique emotional condition which is characterized by culture-related somatization symptoms, paranoid ideas and hypochondriasis, in addition to general neurotic symptoms such as depression and anxiety and related somatic symptoms.

4.The epidemiological studies on the filariasis in Korea II. Distribution and prevalence of malayian filariasis in southern Korea.

During 1964 to 1967, a survey of 30,534 persons for filariasis was made on all over the country with the following results: Among 24,816 draftees from all over the country in the army recruitment camp 155(0.63 %) were found infected with Brugia malayi. Cheju Do showed the highest microfilaria rate(3.5 %), North Kyongsang Do(1.4 %) the next and South Cholla Do(1.2 %) the third. Blood films from 2,308 inhabitants were examined and 407(17.6 %) showed microfilaria in Cheju Do. 30(3.1 %) out of 974 inhabitants in North Kyongsang Do were found to be infected. The microfilaria rates were 2.0 per cent for 400 inhabitants of Chindo island in South Cholla Do. However, no positive case of microfilaria was found in the inhabitants of Kokumdo among 1,820 persons examined in South Cholla Do and of Namhae island(among 165 persons examined) in South Kyongsang Do. The mean microfilarial density per 20 cu. mm of blood was 52.6 in the inhabitants of Cheju Do, 12.2 in North Kyongsang Do and 27.3 in Chindo island(South Cholla Do). Any distinct relationship between the incidence of positive cases of microfilariae and age or sex of the cases was not observed in Cheju Do. However, in Norh Kyongsang Do and South Cholla Do the microfilaria rate of the male group is higher than the one of the female group of the inhabitants. From the above survey results it turned out that filariasis was found throughout Southern Korea except Kyonggi Do and South Kyongsang Do. A total 30,534 persons examined, 601(2.0 %) were found to be infected by Brugia malayi. Therefore, it seems that there are some endemic foci of malayian filariasis in three main areas such as North Kyongsang Do, South Cholla Do and Cheju Do.

6.Skin Hydration Status and Skin Surface pH According to the Body Parts of the Aged with Immobilization.

PURPOSE: This study was performed to identify skin hydration status and skin surface pH according to the body parts of the aged with immobilization. METHODS: The subjects were 101 aged patients in a hospital and a institution for the elderly in Seoul. Data for skin hydration and skin surface pH were collected using corneo-meter and skin pH-meter. The body parts of measurement were 10 parts; face, forearm, back of the hand, flank, upper abdomen, leg, back of the foot, heel, scapular, and sacrum. The data were analyzed by SPSS/WIN 12.0 program. RESULTS: Skin hydration status were relatively low especially on lower extremity and back of body, and skin surface pH of scapular and sacrum were high. There was significant negative correlation between skin hydration status and skin surface pH on the 6 body parts. CONCLUSION: Skin hydration status and skin surface pH, especially on scapular, sacrum, and lower extremity need to be considered as important control factors for increasing skin health status of the aged with immobilization.

8.Metabolism of C(14)-acetate by cestodes.

The adult worm and plerocercoid larva(sparganum) of Diphyllobothrium mansoni and Moniezia expansa employed in this experiment. The adult worms were divided into three portions, i.e. immature, mature and gravid proglottids, and each proglottids were incubated in 50 cc or 250 cc volume of special incubation flasks with incubation medium consisting of 10 cc of 25 cc of Krebs-Ringer phosphate buffer (pH 7.4). The incubation medium was added C(14)-acetate and non-radioactive carrier Na-acetate so as to contain acetate concentration of 50 mg per cent. The worms were allowed to incubate for 5 hours in the Dubnoff metabolic shaking incubator at 38 C. After incubation period, the lactate and pyruvate appearance rate, total CO(2) production tate, the turnover rates were employed as pervious report(Seo et al., 1965). The quantitative analysis of C(14)-acetate utilized by the adult worm and plerocercoid larva of D. mansoni and M. expansa were compared and discussed in this report. According to these data of the experiment, it is impressed that the fatty acid such as acetate may play a role of major part of their metabolism in the adult worm and plerocercoid larva of D. mansoni , whereas minor part of acetate participated in the metabolism by M. expansa.

9.The association of the percentage change of bone mineral density and bone markers after one year of hormone replacement therapy in postmenopausal women.

BACKGROUND: To predict the therapeutic efficacy of osteoporosis, one or two years is needed to evaluate the therapeutic effect by the measurement of bone mineral density(BMD), whereas three to six months is sufficient with bone markers. Using this information, we can change therapeutic plan or modulate drug dosage if necessary. This approach would provide appropriate therapy for osteoporosis. The purpose of this study is to evaluate the association between the percentage change of BMD which was measured by peripheral quantitative computed tomography(pQCT), and bone markers after 1 year of hormone replacement therapy(HRT) in healthy postmenopausal women. METHODS: Bone mineral density of nondominant distal forearm in 89 postmenopausal women was measured by pQCT. We measured serum alkaline phosphatase(ALP) and intact osteocalcin(iOC, Novocalcin) as bone formation markers, urinary deoxypyridinoline(dPyr, PyriLinks-D(TM)) as bone resorption marker by using enzyme immunoassay. After 1 year of HRT, 54 subjects dropped out and 33 subjects were reevaluated. RESULTS: After 1 year of HRT, the drop-out rate was 61%. There was no significant difference in age, age of menopause, years since menopause, initial BMD, initial bone markers between remained and drop out groups. But osteocalcin level was significantly high in remained group(p=0.02). ALP(-27.6 %), iOC(-29.9%), dPyr(-25.2%) were significantly decreased after 1 year of HRT(p<0.001). Trabecular BMD was increased by 2.4%(p=0.003), but the percentage change of total and cortical BMD was not significant(p>0.05). The levels of BMD and bone markers between before and after was significantly correlated, demonstrating the homogeneity of response to HRT. The percentage change of trabecular BMD was negatively correhted with the percentage change of dPyr after HRT(r=-0.45, p=0.01). The variance of the percentage change of dPyr contributed to the percentage change of trabecular BMD by 20%. There was no correlation between the percentage change of total BMD or cortical BMD and the change of ALP, iOC, or dPyr after HRT. CONCLUSIONS: After 1 year of HRT in postmenopausal women, all biochemical bone markers were decreased significantly, whereas only trabecular BMD measured by pQCT was increased significantly. This result suggests that bone markers was more sensitive than BMD to monitor the therapeutic efficacy of HRT. The percentage change of trabecular BMD was correlated with the change of dPyr after HRT only. dPyr might be the most sensitive marker among bone markers tested. Therefore, we can predict the change of BMD after HRT through monitoring the levels of dPyr.
