1.Surgical Outcome of Intraoperative Episcleral Application of 5-Fluorouracil in Primary Trabeculectomy.

We analyzed the outcome of primary trabeculectomy performed with intraoperative 5-fluorouracil(5-FU) sponge application on episclera and under lamella scleral flap in seventeen uncomplicated glaucoma eyes. The mean preoperative intraocular pressure(IOP) was 36.8+/-11.0mmHg. The mean postoperative IOP was 14.2+/-3.5mmHg after mean follow-up of 11.4 months. Two eyes required additional anti-glaucoma medications. Compliations included chronic hypotony(1 eye), shallow anterior chamber(2 eyes), and hyphema (1 eye). No eye showed corneal problems, hypotonic maculopathy, and bleb leakage. This study suggests that a single intraoperative use of 5-FU in primary trabeculectomy is an effective way of improving IOP control and of decreasing complications in eyes with uncomplicated glaucoma.

2.Clinical Characteristics of the Retinoblastoma Diagnosed before One Year Old.

To evaluate the clinical characteristics of retinoblastoma diagnosed before one year old. 19 children with retinoblastoma were observed with clinical records, retrospectively. Thirteen patients had unilateral retinoblastoma. and 6 patients had bilateral retinoblastoma. The mean age at diagnosis was 6.5 months. Patients of bilateral retinoblastoma were younger than those of unilateral retinoblastoma. Sixteen patients had leukocoria. No patients had a known family history. Under Reese-Ellsworth retinoblastoma classification system, 13 eyes(52%) were initially seen with group 4,5 tumors, 16 eyes(64%) were treated with group 4,5 tumors. Eight eyes(32%) had extraocular local metastasis. No eye had distant metastasis. Seventeen eyes(68%) were treated with enucleation or orbital exentration. Four eyes required additional treatment because of 2 eyes for recurrence and 2 eyes for new tumor. For a mean of 31.6 months, 2 children died, but there was no evidence of distant metastasis. These results show the retinoblastoma diagnosed before one year old tend to have bilateral tumor and to be more advanced, therefore the vigorous efforts for diagnosis and treatment should be done.

3.A Case of Free-floating Cyst in the Anterior Chamber Associated with Ciliary Body Cyst.

A 37-year-old patient visited the clinic with driness of eyes and 3 brown colored free-floating cyst was found in his right anterior chamber incidentally. Bilateral ciliary body cyst were also detected by ultrasound biomicroscopy. Stool examination was positive for Metagonimus yokogawai. The cyst was removed through clear cornea incision. Histopathologically, the wall of the cyst. was composed of old red blood cells and fibrotic tissue. It was speculated that a ciliary body cyst. was disloged by blunt trauma and moved into the anterior chamber.

4.A Case of Trichilemmal Carcinoma on the Lower Lip.

We report a case of Trichilemmal carcinoma in a 75-year-old man who was presented with an exophytic nodule on the lower lip. The tumor was composed of glycogen-rich cytoplasmic cells with PAS-positive, diastase-labile and had a characteristic trichilemmal keratinization. The trichilemmal carcinoma is well known to have originated from the outer root sheath of the hair follicle in the hair bearing area. It is interesting that our case had developed on the lip, an exceptional site for a follicular tumor : the hair follicle could not be seen. It might have been developed from an ectopic follicle or at the follicle of vermillion border and grew up into the lip.
