3.Significance of Toxoplasma antibody titers by indirect latex agglutination tests in pregnant women and pelvic tumor patients.

A total of 377 pregnant women, 43 pelvic tumor patients and 80 of multiphysic health center persons as controls were examined by indirect latex agglutination test in order to evaluate Toxoplasma antibody titers at Kang-Nam St. Mary's Hospital in Seoul. Throughout this survey, 1:32 or more titers of diluted sera were regarded as positive. The 337 samples of test sera in pregnant women showed negatives in 319 cases (84.6 percent), 1:2 in 44 cases (11.7 percent), 1:4 in 9 cases (2.4 percent), 1:8 in 2 cases (0.5 percent), 1:16 in 1 case (0.3 percent) and 1:32 in 2 cases (0.5 percent) respectively. The 43 samples of test sera in pelvic tumor patients showed negatives in 29 cases (67.4 percent), 1:2 in 8 cases (18.6 percent), 1:4 in 1 case (2.3 percent), 1:16 in 2 cases (4.7 percent), 1:32 in 1 case (2.3 percent) and 1:128 in 2 cases (4.7 percent). The 80 samples of test sera in multiphysic health center persons as controls negatives in 56 cases (70.0 percent), 1:2 in 19 cases (23.8 percent), 1:4 in 3 cases (3.8 percent), 1:8 in 1 case (1.3 percent) and 1:128 in 1 case (1.3 percent). Among total 420 study cases, 5 cases (1.2 percent) showed positives , and they were 2 cases (0.5 percent) of pregnant women and 3 cases (7.0 percent) of pelvic tumor patients. One case (1.3 percent) out of 80 control sera showed positive result.

4.A comparison of Modified One Layer Vasovasostomy with Optical Loupe and Microscope.

OBJECTIVE: The objective of this study was to compare results of the macroscopic one-layer vasovasostomy with those of microscopic one-layer vasovasostomy and to analyze the change of semen parameters according to the interval of vasal obstruction. METHOD AND MATERIALS: Between March 1987 and December 1997, we performed 121 vasovasostomies using modified one-layer technique with loupe magnification (macroscopic vasovasostomy) or microscope. Among the 68 could be followed post-operatively, 37 patients were treated by macroscopic technique with loupe, and 31 patients by microscopic technique. We compared rates of anatomical patency (sperm count above than 10x106/ml) and pregnancy of macroscopic vasovasotomy with those of microscopic vasovasostomy. Patency and pregnancy fates according to vasal obstructive interval were also examined. RESULTS: The patency rates of macroscopic and microscopic technique were 86.5% and 87.1%, and pregnancy rates of macroscopic and microscopic technique were 64.9% and 67.7%. There was no statistical significance between these two methods (p>0.05). The pregnancy rates and sperm motility were decreased if more than 10 years had elapsed following vasectomy(p<0.05). CONCLUSION: We found little difference in success rates resulting from macroscopic and microscopic vasovasostomy and the former had the advantage of reduced cost and a lower operator skill level. in post-operative semen analysis, the sperm motility was the most probable factor associated with vasal obstructive interval.

5.The Role of the Adenosine Receptor Subtypes and Protein Kinase C in Ischemic Preconditioning in the in Vivo Cat Heart.

BACKGROUND: It is well known that ischemic preconditioning protects the heart against infarction or arrhythmias from a subsequent ischemic injury. Recent laboratory data indicate that the adenosine during the ischemic period may trigger protection via A1 or A3 adenosine receptor and also protein kinase C(PKC) plays a central role. This study was designed to determine the role of adenosine receptor subtypes and PKC in the preconditioning protection. METHODS: All cat heart groups were subjected to 40min ischemia and 30min reperfusion. The preconditioning protocol consists of 4min ischemia and then 10min of reperfusion 4 times. The effects of ischemic preconditioning, nonselective adenosine receptor blocker(SPT), an A1 specific antagonist(DPCPX) and protein kinase C inhibitor(Polymyxin B), on ischemic preconditioning were determined by infarction size. There were 5 groups : (1) control group (Group 1, n=10)(2) Ischemic preconditioned group(Group 2, n=9)(3) DPCPX pretreatment group(Group 3, n=6)(4) SPT preteatment group(Group 3, n=6)(5) Polymyxin B pretreatment group(Group 5, n=6). SPT and DPCPX were given intravenously 5 min before ischemic preconditioning. Polymyxin B was administered to cats for 30min during ischemic preconditioning period. RESULTS: Ischemic preconditioning only or pretreatment with DPCPX prior to preconditioning demonstrated a significant reduction in infarct size(22.6+/-1.5, 25.4+/-0.9% infarction of the risk zone, respectively, p<0.05) with respect to control, SPT-pretreatment, and polymyxin B-pretreatment groups(44.0+/-1.7, 43.0+/-2.0 and 40.3+/-0.4% infarction of the risk zone, respectively). CONCLUSIONS: Ischemic preconditioning protects heart from subsequent ischemia. Protection was blocked by SPT and protein kinase C inhibitor(polymyxin B), but not by A1 antagonist DPCPX. The cardioprotective effects by ischemic preconditioning in the in vivo cat heart appear to be dependent on A3 adenosine receptors and activation of protein kinase C.

6.Effect of Transforming Growth Factor-beta1 on Expressions of Epidermal Growth Factor and Transforming Growth Factor-alpha in DU145 Androgen-Independent Prostate Cancer Cells.

PURPOSE: This study was designed to identify the possible mechanism of insensitivity of DU145 prostate cancer cells to the transforming growth factor (TGF)-beta1-mediated growth inhibition. MATERIALS AND METHODS: DU145 cells were treated with 4, 40, 100 pM TGF-beta1 for 3, 6, 9 days. Initially we performed the growth assay. After that, we analysed the change of cell cycle using fluorescence flow cytometry. At each time point, Western blot analysis with cell pellets was performed to investigate the change of expressions of epidermal growth factor(EGF), TGF-alpha, EGF receptor(EGFR) and TGF receptorII(TbetaR-II) proteins. RESULTS: The growth rate of TGF-beta1-treated cells was initially suppressed, but over time returned to control level. Flow cytometric analysis revealed that TGF-beta1-treated cells showed an increase in apoptotic/G1 phases, and concurrent decrease in S, G2/M phases until 6days. On day 9, however, TGF-beta1-treated cells showed a persistent increase of apoptotic unclei in spite of recovery of apoptotic/G1, S and G2/M phases. Western blot analysis showed that the intensity of EGF or TGF-alpha band decreased in dose-sependent manner on day 6. However, the intensity of each band increased up to the control level on day 9. the expression of EGFR or TbetaR-II protein did not change after treatment of TGF-beta1. CONCLUSIONS: these results suggest that EGF and TGF-alpha sould mediate in part the escape fron the inhibitory effect of TGF-beta1 in DU145 cells.

7.Two Cases of Graves Disease Associated The Empty Sella Syndrome

The empty sella syndrome is characterized by obesity, frequent pregnancy, headache and high blood pressure, but its exact cause remains unknown. Usually the incomplete diaphragmatic sella has been considered as the cause of the empty sella syndrome, but some authors recently have suggested that the antipituitary antibody way be related to development of pituitary atrophy and the pituitary empty sella syndrome, and thus it may be clinically useful as screening test for the empty sella syndrome. We experienced two empty sella syndromes associated Graves disease and applied the antipituitary antibody as the diagnostic tool of the empty sella syndrome. But none of this two patients had antipituitary antibody and we report these cases with reviews of literatures.

8.Biochemical properties of full-length hepatitis C virus RNA-dependent RNA polymerase expressed in insect cells.

The hepatitis C virus (HCV) RNA-dependent RNA polymerase, NS5B protein, is the key viral enzyme responsible for replication of the HCV viral RNA genome. Although several full-length and truncated forms of the HCV NS5B proteins have been expressed previously in insect cells, contamination of host terminal transferase (TNTase) has hampered analysis of the RNA synthesis initiation mechanism using natural HCV RNA templates. We have expressed the HCV NS5B protein in insect cells using a recombinant baculovirus and purified it to near homogeneity without contaminated TNTase. The highly purified recombinant HCV NS5B was capable of copying 9.6-kb full-length HCV RNA template, and mini-HCV RNA carrying both 5'- and 3'-untranslated regions (UTRs) of the HCV genome. In the absence of a primer, and other cellular and viral factors, the NS5B could elongate over HCV RNA templates, but the synthesized products were primarily in the double stranded form, indicating that no cyclic replication occurred with NS5B alone. RNA synthesis using RNA templates representing the 3'-end region of HCV minus-strand RNA and the X-RNA at the 3'-end of HCV RNA genome was also initiated de novo. No formation of dimersize self-primed RNA products resulting from extension of the 3'-end hydroxyl group was observed. Despite the internal de novo initiation from the X-RNA, the NS5B could not initiate RNA synthesis from the internal region of oligouridylic acid (U)20, suggesting that HCV RNA polymerase initiates RNA synthesis from the selected region in the 3'-UTR of HCV genome.

9.The Effect of VDT Work on Vision and Eye Symptoms among Workers in a TV Manufacturing Plant.

This study was conducted to evaluate the effect of VDT work on eyes and vision among workers in a TV manufacturing plant. The study subjects consisted of 264 screen workers and 74 non-screen workers who were less than 40 years old male and had no history of opthalmic diseases such as corneal opacities, trauma, keratitis, etc and whose visual acuity on pre-employment health examination by Han's test chart was 1.0 or above. The screen workers were divided into two groups by actual time for screen work in a day; Group 1, 60 workers, lesser than 4 hours a day and group 11, 204 workers, more than 4 hours a day. From July to October 1992 a questionnaire was administered to all the study subjects for the general charateristics and subjective eye symptoms after which the opthalmologic tests such as visual acuity, spherical equivalent, lacrimal function, ocular pressure, slit lamp test, fundoscopy were conducted by one opthalmologist. The proportion of workers whose present visual acuity was decreased more than 0. 15 in comparison with that on the pre-employment health examination by Han's test chart was 20.6% in Group Ii, 15.0% in Group I and 14.9% in non-screen workers. However, the differences in proportion were not statistically significant. The proportion of workers with decreased visual acuity was not associated with the age, working duration, use of magnifying glass and type of shift work (independent variables) in all of the three groups. However, screen workers working under poor illumination had a higher proportion of persons with decreased visual acuity than those working under adequate illumination (P<0.05). The proportion of workers whose near vision was decreased was 27.5% in Group II, 18.3% in Group I, and 28.4% in non-screen workers and these differences in proportion were not statistically significant. Changes of near vision were not associated with 4 independent variables in all of the three groups. Six out of sever-subjective eye symptoms except tearing were more common in Group I than in non-screen workers and more common in Group II than in Group I (P<0.01). Mean of the total scores for seven subjective symptoms of each worker (2 points for always, 1 point for sometimes, 0 point for never) was not significantly different between workers with decreased visual acuity and workers with no vision change. However, mean of the total scores for Group II was higher than those for the Group I and non-screen workers (P < 0. 01). Total eye symptom scores were significantly correlated with the grade of screen work, use of magnifying glass, and type of shift work. There was no independent variable which was correlated with the difference in visual acuity between the pre-employment health examination and the present state, the difference between far and near visions, lacrimal function, ocular pressure, and spherical equivalent. Multiple linear regression analysis for the subjective eye symptom scores revealed a positive linear relationship with actual time for screen work and shift work(P<0.01). In this study it was not observed that the VDT work decreased visual acuity but it induces subjective eye symptoms such as eye fatigue, blurred vision, ocular discomfort, etc. Maintenance of adequate illumination in the, work place and control of excessive VDT work are recommended to prevent such eye symptoms.

10.Comparison of the Effect of Transurethral Resection of Prostate and Prostatic Laserthermia for Benign Prostatic Hyperplasia.

Benign prostatic hyperplasia is one of the most common afflictions in an aged man, but the surgery has been the only therapeutic option in the treatment of benign prostatic hyperplasia. Recently, other alternative therapeutic modalities such as pharmacological, hormonal, less invasive treatment(laser, stents, thermotherapy, etc.) were developed. The prostatic laserthermia is one of the new therapeutic tool being evaluated for the treatment of BPH. The aim of this study is that the prostatic laserthermia will be an alternative therapeutic modality for BPH as transurethral resection of prostate(TURP). A total of 92 patients(ages 49 to 90 years) who were diagnosed as BPH were included in the study, underwent uroflowmetry, transrectal ultrasound and AUA symptom scoring. 34 of 92 patients were treated with TURP. And 58 of the 92 patients were treated with prostatic laserthermia. After the treatments, each patients were followed by uroflowmetry and AUA symptom score at post operative 1, 3 and 6 months. In TURP group, the preoperative peak flow rate was changed from 8.33+/-4.5ml/sec, to 14.65+/-4.74 ml/sec, 14.67+/-4.08 ml/sec and 14.60+/-4.63 ml/sec, at postoperative lst, 3rd and 6th month respectively. The amount of average residual urine was decreased from 158.l+/-121.6 ml, to 41.9+/-22.3 ml, 43.3+/-27.1 ml and 58.2+/-27.5 ml, at 1st, 3rd and 6 month respectively. Also the AUA symptom score was decreased from 22.5+/-7.0, to 9+/-4.7, 9.3+/-4.3 and 9.5+/-5.2, at postoperative 1st, 3rd and 6th month. In prostatic laserthermia group, the preoperative average peak flow was 9.33+/-6.30 ml/sec, changed to 11.46+/-5.20 ml/sec, 11.67+/-5.40 ml/sec and 11.72+/-5.27 ml/sec at postoperative lst. 3rd and 6th month. The preoperative average mean flow was 4.75+/-2.87 ml/sec, increased to 5.47+/-2.69 ml/sec, 5.l8+/-2.69 ml/sec and 5.28+/-3.06 ml/ sec at 1st, 3rd and 6th month. The preoperative average residual urine was 130.9+/-154.7 ml/sec, decreased to 59.9+/-66.6 ml/sec, 50.6+/-57.0 ml/sec and 31.3+/-45.7 ml/sec at postoperative lst, 3rd and 6th month. The preoperative average AUA symptom score was 27.0+/-5.9, decreased 19.1+/-8.1, 18.4+/-8.7 and 17.5+/-9.1 at 1st, 3rd and 6th month. When comparing the parameters between two treatment groups, improvement of peak flow rate was greater in TURP than in Prostatic laserthermia group(p<0.01). Decrease of AUA symptom score and amount of residual urine also were greater in TURP than in prostatic laserthermia group. As the result TURP is more effective than prostatic laserthermia in peak flow rate at 1st month and AUA symptom score at lst, 3rd and 6th month t < 0.05). But laserthermia also appears to be as effective as TURP in the decrease of postvoid residuals. It could be suggested that prostatic laserthermia is an alternative therapeutic modality of BPH, especially old man who has some risk factors for operation.
