2.Lung Cancer in patients with Idiopathic Pulmonary Fibrosis: Frequency and CT Findings.

PURPOSE: The incidence of lung cancer in patients with idiopathic pulmonary fibrosis(IPF) is higher than that of general population. To evaluate the frequency and CT findings of lung cancer associated with idiopathic pulmonary fibrosis, we analyzed 19 patients with lung cancer associated with idiopathic pulmonary fibrosis. MATERIALS AND METHODS: We analyzed retrospectively 19 patients with histologically confirmed lung cancer out of 208 patients diagnosed as IPF either by CT and clinical findings(n=188) or histologically(n=20). All 19 patients were male, aged 40--85 years (mean 66 years). Scanning techniques were conventional CT in 12 patients, HRCT in 1 patient and both conventional CT and HRCT in 6 patients. We analyzed the CT patterns of lung cancer and IPF, locations of the tumor and histologic types of lung cancer. RESULTS: The incidence of lung cancer in patients with idiopathic pulmonary fibrosis was 9.1%(19/208). In 11 of 19 patients, CT findings of lung cancer were ill-defined consolidation-like mass. Lung cancer was located mainly in lower lobes(right lower Iobe;10/19, left lower Iobe;5/19) and at the periphery(12/19). Histologically, squamous cell carcinoma was the most common cell type (11/19). CONCLUSION: The incidence of lung cancer in patients with idiopathic pulmonary fibrosis was much higher than that of general population. Typical CT findings of lung cancer were predominantly ill-defined consolidation like mass at the peripheral lung portion which is the [ocatiaon where the most advanced fibrosis occur.

3.Fatal Inhalation Injury by Sulfuric Acid Fumes: Case Report.

Sulfuric acid can cause local or systemic effects after exposure by inhalation, ingestion, or topical application. Direct ingestion is the main exposure route for fatal sulfuric acid injury. Fatal accidents involving the inhalation of toxic sulfuric acid vapors are rare. Inhalation of sulfuric acid fumes causes severe irritation or corrosive damage to the upper respiratory tract. Consequently, severe congestion, edema, and inflammation of the mucous membranes of the upper respiratory passages hinder the entry of air into the lungs, possibly leading to fatalities in victims. A 35-year-old man died at his home after complaining of a severe sore throat. One day earlier, he had worked in a sulfuric acid tank in a copper-smelting plant while wearing an ordinary gas mask that offered no protection against sulfurous acid vapors. Upon autopsy, the larynx and epiglottis showed pronounced edema, congestion, and inflammation that histologically mimicked an acute bacterial suppurative inflammation, accompanied by severe pulmonary edema. A field analysis of the air inside the sulfuric acid tank revealed SO4(2-) positivity and a 40% sulfuric acid concentration.

6.The Effect on the Dryness of Eye during VDT Work.

The ocular symptoms of visual display terminal (VDT) syndrome are caused by asthenopia, electromagnetic waves, and eye fatigue. To find out what cause make the fatigue of eye during VDT work, we measured frequency of blinking and height of palpebral fissure at near, reading a book, and during VDT work. Tear film break-up time(BUT) was measured immediately after the VDT work and during reading a book. The frequency of blinking during VDT work and book reading decreased significantly comparing with the one at rest (p<0.01). The average frequency of blinking is 20.52 +/- 5.89/min at rest, 8.29 +/- 4.15/min at VDT work, and 10.31 +/- 5.48/min at reading. The BUT measured immediately after VDT work decreased much more than in the resting state (p<0.01). The average BUT was 11.52 +/- 3.41sec at rest and 6.18 +/- 2.65sec immediately after VDT work. The height of palpebral fissure during VDT work increased significantly comparing with the value at rest (p<0.01). The average height of palpebral fissure was 7.39 +/- 0.78mm at rest and 8.83 +/- 0.75mm during VDT work. This study reveals the fatigue of eye during VDT work was induced by the dryness of eye resulted from the increased evaporation due to the decrement in the frequency of blinking and the widening of palpebral fissure. To prevent the fatigue of eye caused by dryness during VDT work, it is important for VDT workers to intend to blink frequently, place VDT lower than eye level to reduce the height of palpebral fissure, humidify the room adequately, put artificial eye tear in eye frequently, and take a rest after 20 mimutes work at least.

The importance of tick-borne diseases is increasing because of climate change, with a lack of long-term studies on tick-borne pathogens in South Korea. To understand the epidemiological characteristics of tick-borne diseases, the monthly distribution of field ticks throughout the year was studied in South Korea between May 2014 and April 2018 in a cross sectional study. The presence of various tick-borne pathogens ( During the study period there were 11,717 ticks collected and 4 species identified. In conclusion, due attention should be paid to preventing tick-borne infections in humans whilst engaged in outdoor activities in Spring and Autumn, particularly in places where there is a high prevalence of ticks.
8.Non-Hodgkin's lymphoma with necrotic low density in CT

Malignant lymphomas with cental and/or peripheral low densities suggesting internal infarcted necrosis arerare, probably due to the same reason as that of normal lymph nodes in which spontaneous infarction seldom occuresbecause of dual vascular connection and low oxygen consumption of activated lymphocytes. We report eight cases ofhistologically proven non-Hodgkin's lymphoma, that showed necrotic low density within tumor mass in CT scan, priorto any teatment. The locations of necrotic lymphomas were mediastinum in 3 cases, abdomen in 4 cases, and neck in1 case. The histologic types of the cases wre diffuse histiocytic (Rappaport) in 4 cases, porly differentiatedlymphocytic types in 2 cases, lymphoblastic type in 1 case, and mixed type(predominantly poorly differentiatedlymphocytic) in 1 case. It seems that malignant lymphoma can be included in differential diagnosis oflymphadenopathy with cental and/or peripheral necrotic low densities on CT.

9.Reconstruction Of Achilles Tendon Using Fascia Using Fascia Lata Allogrft: A Case Report

We had a female patient with soft tissue tumor involving large portion of Achilles tendon. After resection of tumor, Achilles tendon was reconstructed using an allogenic human fascia lata manufactured by Pfrimmer-Viggo. EMG findings on postoperative 8 months show marked improvement of function of medial and lateral gastrocnemius muscles. Biopsy specimen of fascia lata allograft, which was acquired during TAL 8 months postoperatively, shows myxoid degeneration, a few lymphocytic infiltration and some foreign body giant cells. Functional, histologic and EMG findings suggest that human fascia lata allograft could be used to bridge defect of Achilles tendon being resected during tumor surgery.

10.FDG-PET/CT Complements Bone Scan with Respect to the Detection of Skip Metastasis of Osteosarcoma: A Case Report.

Skip lesion is not uncommon feature in osteosarcoma and considered to be importantly associated with poor prognosis factor, and thus, should be excised with the main mass. The accurate pre-operative evaluation of the intramedullary extent of osteosarcoma is essential, because it determines the level of bone resection. Among the reliable detection methods, bone scan has a drawback of high rate of false negative results and regional MRI has a difficulty to cover the whole involved lesions without clinical suspicion. The authors report a case of osteosarcoma of the distal femur with a proximal skip lesion that was not detected by either regional MR imaging or by bone scan, but which was visualized by FDG-PET/CT.
