Pheochromocytoma is one of surgically curable hypertensive syndromes. The tumor is bilateral or extra-adrenal in 5% of cases in adults and in an even greater percentage in children and is most often familial. Herein we report two cases of pheochromocytoma, of which one is bilateral pheochromocytoma in a 20-year-old female and the other is left sided in a 27-year-old female.

4.A Case of Pseudoaneurysm in Neurofibromatosis Type I.

Neurofibromatosis type I(NF-1) is a genetic disorder that usually identified clinically by cafeau-lait spots, and cutaneous neurofibromas. Vascular abnormalities are also associated with von Rechlinghausen neurofibromatosis. Whereas renal and gastrointestinal system lesions are common, involvement of the craniocerebral vessels is relatively rare. More than 85% of the reported lesions are of a purely occlusive or stenotic nature, including progressive arterial occlusive disease. Aneurysms are the second most frequently reported vascular abnormality in NF-1. Recent reported cases of extracranial vertebral artery aneurysms and carotid artery aneurysms have been treated by endovascular detachable balloon technique, and yielded good result. The authors experienced a case of pseudoaneurysm in NF-1 patient who complained of sudden-onset neck swelling. That was identified by doppler sonography, and then angiographic embolization was done.

5.Autologous Transfusion in Pregnant Women with Significant Risk for Hemorrhage.

Autologous Transfusion, storage of one's own blood for subsequent infusion if needed, is safe and effective in a variety of scheduled operative procedures. Obstetric involvement in such programs in very limited, however, because of concern over the possibility of inducing premature labor or causing fetal distress by blood volume change or vasovagal reactions. We describe our experience with pregnant women in this program. The incidence of vagovagal reactions of autologous donation was 9.5% (2.21). After entry into this program, 17pastients received a total 37pints, which consist of 19 Autologous and 18 Homologous. Homologous transfusion was avoided in 30% of patients receiving blood. The values of the mean hematocrits before and after hpebotomy were 34.1% and 31.8% respectively. It was stastically significant (p<0.01). We recommended that autologous blood donation by pregnant women in third trimester is safe for mothers or infants and it should be strongly encouraged for patient with placenta previa and repeated cesarean section.

6.MR study of normal nasal cycle.

Magnetic resonance images frequently demonstrate asymmetrical mucosal volume in the nasal area of asymptomatic patients. To further evaluate this phenomenon, sequential MR examinations of the nasal cavity and paranasal sinuses were performed three to four times during a 3 to 8 hours period in ten normal volunteers. The study demonstrated that the mucosal volume changes alternated from side to side during this period in eight volunteers. Changes were also observed within the ethmoid sinuses, nasal septum and nasolacrimal ducts. Awareness of MR findings of nasal cycle may reduce the likehood of inflammatory disease being confused with normal physiologic changes, and also may provide another method in the study of this physiologic phenomenon.

7.Analysis of Plasma Long-Chain Fatty Acids in Hypertensive Patients.

BACKGROUND: Numerous studies have suggested that polyunsaturated fatty acids (PUFAs)-supplemented diets may decrease cardiovascular morbidity and mortality. Especially, omega-3 PUFAs may exert beneficial effects to the treatment and prevention of dyslipidemias, arrhythmias, atherosclerosis, and hypertension. METHODS: This study investigated plasma lipid profiles including total cholesterol, triglycerides (TG), LDL, HDL, and antioxidant status indicative of vitamin A, vitamin E, malondialdehyde (MDA) and distribution of plasma long-chain fatty acids (C12-C24) in 28 normal subjects and 24 hypertensive patients. Also, the correlation among PUFAs, levels of antioxidant status, and lipid profiles of the subjects were estimated. RESULTS: The distribution of omega-3 PUFAs, saturated fatty acids, and monounsaturated fatty acids showed significant differences (P<0.05), but that of omega-6 PUFAs did not exhibit significant differences. The omega-6/omega-3 ratio exhibited 36.96 in normal group and 14.29 in the hypertensive patient group. The levels of vitamin A, vitamin E, and MDA were increased significantly in the hypertensive patient group. CONCLUSION: PUFA levels were estimated in the hypertensive patients and normal group. The results suggest that dietary intake of proper omega-6/omega-3 ratio is needed for prevention and treatment of hypertension.

9.The Effect of Atrial Repolarization Waves on the Maximal Exercise Loading Test.

BACKGROUND: The atrial repolarization waves might contribute to ST segment depression during the exercise loading test. As atrial repolarization waves are opposite in direction to P waves, they could shift the ST segment in the absence of myocardial ischemia. METHODS: The exercise test in 20 patients was seemed false positive because of normal coronary angiogram. Twenty-six patients with at least one coronary stenosis > or =50% served as a true positive group. The P waves, PR segments and ST segment were studied in leads II, III, aVF and V4 to V6 in those 46 patients whose exercise ECG suggested ischemia. RESULTS: The finding of steeply downsloping PR segment, particularly in the inferior ECG leads, in patients with longer exercise time and higher exercise heart rate might predict a false positive exercise test in those with a normal rest ECG and no apparent reason for a false positive test. The combination of downsloping PR segments in two of three inferior leads plus either exercise duration > or =5 min or peak heart rate > or =130 bests/min idenified false positive tests with a sesitivity of 75% and a specificity of 81% to 89%. CONCLUSIONS: Atrial repolarization waves might play a role in exercise induced ST segment depression.

10.Transitional cell carcinoma of the renal pelvis and ureter: the prognostic value of deoxyribonucleic acid ploidy studied by flow cytometry.

Prognostic factors in transitional cell carcinoma of the upper urinary tract were assessed with histopathological examination and flow cytometric analysis in a series of 33 patients treated between Jan. 1980 to Dec. 1990. During follow-up from 6 to 120 months, 14 patients died of transitional cell carcinoma. The rate of DNA aneuploidy pattern was 54.5 % and DNA aneuploidy pat tern showed significantly a poorer prognosis than diploidy pattern. DNA ploidy pattern was relatively correlated with grade, stage and clinical outcome. For stage TS (superficial tumor) and low grade, the occasional detection of DNA aneuploidy pattern identified an important subgroup which showed significantly a poorer survival. Only the stage and, to a lesser extent, tumor shape, grade and DNA ploidy pattern were determinants for prognosis in multivariate analysis. Based on these 4 variables 3 different subgroups of patients can be identified. In low risk group including stage TS (superficial tumor), low grade, DNA diploidy pattern and papillary tumor, 3 and 5 year survival rates were 88.9 % and 69.1 %. In high risk group including stage TE (extensive invaded tumor) and DNA aneuploidy pattern, 2 and 3 year survival rates were 15.3 %, 0.0 %. In intermediate risk group including stage TS (superficial tumor), DNA aneuploidy pattern, 3 and 6 year survival rates were 55.9 % and 43.5 %. In summary, DNA ploidy may be a useful parameter to identify risk groups, especially stage TS and low grade and to plan appropriate management of patients with primary transitional cell carcinoma of the upper urinary tract.
