1.A Case of Xanthogranulomatous Pyelonephritis.

Xanthogranulomatous pyelonephritis is an atypical form of severe chronic parenchymal infection and usually occurs in association with urinary tract infection, obstruction and/or calculi. We experienced a case of xanthogranulomatous pyelonephritis in a 53-year-old male, in which case diagnosis was made preoperatively. 1. Patient revealed a history of genitourinary stone surgery with persisting general weakness. 2. Laboratory study showed anemia, leukocytosis, persistent pyuria and positive urine cultures. 3. Radiologic study, especially abdominal CT, revealed pyonephrosis with multiple renal stones. Above findings made the diagnosis possible as xanthogranulomatous pyelonephritis preoperatively, and which was then confirmed histologically.

2.Functional Analysis of Anti-Refluxing Augmentation Cystoplasty.

Augmentation cystoplasty using bowel is effective method for the functional enlargement of bladder capacity in contracted bladder due to various causes. Prevention of vesicoureteral reflux after augmentation cystoplasty is still controversial but generally accepted that prevention of reflux is essential for good long term result. Therefore, antireflux augmentation cystoplasty including the use of ileocecal valve and hemi-Kock intussuscepted ileal nipple valve has been used recently for this purpose. Herein, we report the result of 3 cases of ileocecal cystoplasty and 6 cases of hemi-Kock ileocystoplasty. Hemi-Kock ileocystoplasty is better than ileocecal cystoplasty in a viewpoint of reflux prevention and continence, but complication is more common and serious in hemi-Kock ileocystoplasty than ileocecal cystoplasty. It is considered that azotemia is not contraindication for these procedures, and renal function deterioration has not been observed postoperatively.

3.Repair of Hypospadias Using Bladder Mucosal Graft.

Since Memmalaar first reported the use of bladder mucosa graft technique in penoscrotal hypospadias in 1947, this technique has been used with reasonable success for the reconstruction of new urethra for the posterior hypospadias and hypospadias cripples. The a bladder mucosa is abundantly available, sterile and has good elasticity. This mucosa does not produce hair, does not excoriate, and does not desquamate from the exposure of the urin. Two new urethrae in patients with posterior hypospadias, aged 11 and 21, were successfully reconstructed with bladder mucosal graft technique. A minor meatal stricture developed in one patient. However, the over-all results of this technique appear to be satisfactory.

4.Large Cell Calcifying Sertoli Cell Tumor of the Testis: A Case Study and Review of the Literature.

A 24-year-old man was admitted due to an incidentally detected mass in his left testis, which showed radiopaque calcification on plain X-ray film. Left orchiectomy was performed, and the resected testis contained a well-demarcated, hard mass measuring 1.1 cm. Histological analysis revealed that the tumor was composed of neoplastic cells, fibrotic stroma, and laminated or irregularly shaped calcific bodies. The individual cells had abundant eosinophilic or clear cytoplasm with round nuclei, each of which contained one or two conspicuous nucleoli. They were arranged in cords, trabeculae, clusters, and diffuse sheets. There were several foci of intra-tubular growth patterns, with thickening of the basal lamina. Immunohistochemically, the neoplastic cells were positive for S-100 protein and vimentin, focally positive for inhibin alpha, and negative for cytokeratin, CD10, and Melan-A. In addition to reporting this rare case, we also review the relevant literature regarding large cell calcifying Sertoli cell tumors.

Holoprosencephaly is a rare malformation complex or development defect including different degrees of incomplete cleavages of the embryonic prosencephalon and varying degrees of the midface defects, resulting from the defect of prechordal mesoderm, migrating forward into the area anterior to the notochord during the third week of fetal development. Early antenatal diagnosis of holoprosencephaly is important to find out its severity, to predict its prognosis, and to determine proper management according to its prognosis and severity. The possibility of early antenatal diagnosis of holoprosencephaly by ultrasound has been suggested, but occasionally missed and rarely confirmed. We present one case of lobar holoprosencephaly, diagnosed postnatally and one case of alobar holoprosencephaly, diagnosed antenatally in our hospitals.

6.Effect of Nurse’s Character for Care and Sense of Coherence on Professional Quality of Life Among Oncology Nurses

Purpose:
The purpose of this study was to identify the influence of the nurse’s character on care and the sense of coherence (SOC) on the professional quality of life among oncology nurses.
Methods:
Data were collected from June 2 to July 1, 2020. The participants were 185 oncology nurses caring for cancer patients. Data were analyzed using independent t-test, one-way ANOVA, Scheffé test, Pearson correlation coefficient, and hierarchical multiple regression with the SPSS/WIN 22.0 program. Nurse’s Character Scale for Care in Clinical Settings (NCS_C) and SOC, Professional Quality Of Life (ProQOL), which is conceptualized with three sub-dimensions (compassion satisfaction, compassion fatigue, burnout, and secondary traumatic stress), along with demographic and work-related variables were measured.
Results:
The compassion satisfaction and burnout results had a significant correlation with the nurse’ s character for care and SOC. In multiple regression analysis, the meaningfulness of sub-dimension SOC (β=.58) accounted for 67.8% of compassion satisfaction and the meaningfulness of sub-dimension SOC (β=-.37) accounted for 55.4% of burnout.
Conclusion
Meaningfulness of sub-dimension SOC was an important factor that could be used to improve the professional quality of life among oncology nurses. Therefore, it is necessary to develop and make available programs for oncology nurses to improve compassion satisfaction, and methods to decrease burnout needs to be developed.

8.Azathioprine-associated Acute Pancreatitis in Rheumatoid Arthritis.

Drug-induced pancreatitis is now recognized as a distinct, although uncommon, entity. Azathioprine has been incriminated as a causative agent of acute pancreatitis. We report a 36-year-old female who developed acute pancreatitis in the course of treatment with azathioprine for rheumatoid arthritis. She was admitted due to epigastric pain, radiating to the back, for four days. Serum amylase and lipase levels were 226U/L (normal : 30-110U/L) and 1300U/L(normal : 23-300U/L), respectively. The abdominal ultrasonogram and CT scan revealed diffuse enlargement of the pancreas. We diagnosed it as acute pancreatitis. There was no history of alcohol use or gallbladder disease. We considered drugs, especially azathioprine, as the cause of acute pancreatitis. The patient was treated conservatively with intravenous fluid and analgesics. The symptoms subsided within three days. Thirteen days later, azathioprine was resumed. After six hours, the patient complained of epigastric pain. The elevated level of serum lipase(542U/L) confirmed the recurrence of pancreatitis. Her symptoms subsided when azathioprine intake stopped, and the serum lipase level returned to normal within four days.

There are diverse ring-like narrowings in the lower esophagus: mucosal ring, muscular ring, ring-like annular peptic stricture. Esophageal muscular ring is extremely rare and generally asymptomatic. It occurs at the proximal border of the esophageal vestibule that corresponds to the upper end of the lower esophageal sphincter. We experienced a case of lower esophageal muscular ring with a symptom of intermittent mild dysphagia for 40 years and report the findings with a review of the literature.

10.A Case of Cyclic Vomiting Syndrome Responding to Gonadotropin-Releasing Hormone Analogue.

Cyclic vomiting syndrome (CVS) is a disorder characterized by recurrent episodes of incapacitating nausea and vomiting interspersed with symptom free periods. Common triggers of cyclic vomiting include noxious stress, excitement, fatigue and menstrual period. Here, we report a case of cyclic vomiting syndrome in adult patient characterized by stereotypical vomiting attack, occurring in every menstruation period. Recurrent vomiting episodes began 6 years ago and we treated this patient with subcutaneous injection of goserelin, a gonadotropin-releasing hormone analogue (GnRHa) and oral estrogen. After 4 months of therapy, she was symptom free for the following 5 years, even with the resumed normal menstruation. Recurrence of vom - iting attack with same pattern occurred 1 month before readmission. Treatment with intravenous lorazepam aborted vomiting, but could not prevent recurrences of vomiting and epigastric pain. We treated the patient with GnRHa and oral estradiol again which effectively prevented recurrence of the symptoms.
