2.Relationship of Serum Uric Acid to Cardiovascular Risk Factors in Korean Male Workers.

OBJECTIVES: To investigate the relationship of serum uric acid to cardiovascular risk factors in Korean male workers. METHODS: We screened 522 male workers at an electrical manufacturing company by a periodic health examination and a questionnaire survey in 2000. We collected data relating to age, smoking status, alcohol drinking status, body mass index (BMI), blood pressure, serum lipid, fasting blood glucose and serum uric acid. The data were analyzed using a variety of methods, including ANOVA, Pearson's correlation, and multivariate regression analyses, to examine the association between uric acid and cardiovascular risk factors with a cross-sectional study design. RESULTS: The concentration of serum uric acid showed positive associations with BMI, total cholesterol, triglyceride, and LDL-cholesterol; it was also significantly correlated to systolic and diastolic blood pressures. Life-style characteristics, such as smoking and alcohol drinking showed no significant association. From the multivariate regression analyses, BMI, total cholesterol and fasting blood glucose levels were found to be independent positive predictors of uric acid; while age, blood pressure and smoking status give no independent contribution explaining the variability of serum uric acid levels. Conclusions : This study demonstrates that serum uric acid level have a significant association with cardiovascular risk factors, such as BMI, total cholesterol and blood glucose in some Korean male workers.

3.The Relationship between Work Posture and Cervical Spine Abnormality among Cervicobrachial Disorder Workers in a Microwave-oven Assembling Factory.

This study conducted to evaluate the association of work posture with cervical spine abnormality toward 106 microwave-oven assemblers. A self-administered questionnaire was used to obtain a general characteristics and a work history of workers. The posture of neck and shoulder joint of workers were measured with video tape recording and the simple X-ray of cervical spine was checked-up to observe the X-ray abnormality including degenerative change, curvature abnormality, disc space narrowing or disc canal narrowing. The result were as follows. 1. The prevalence of degenerative change among cervicobrachial disorder workers was 44.3%(47 persons), curvature abnormality was 43.4%(46 persons), disc space narrowing was 21.7%(23 persons), and disc canal narrowing was 21.7%(23 persons). 2. The prevalence of degenerative change at cervical spine was increased with the degree of neck flexion(flexed versus neutral; odds ratio (OR)=2.7), the total work posture of neck(mild or severe awkward versus neutral; OR=1.2, 3.4). 3. The prevalence of degenerative change at cervical spine was increased with the degree of the right shoulder flexion, the heaviness of the materials carried by the right hand and the degree of awkwardness at the total work posture of right shoulder(p<0.05). 4. There was no evidence of association between curvature abnormality, disc space narrowing, disc canal narrowing and work posture. In conclusion the awkward work posture was related with degenerative change of cervical spine among microwave oven assembling workers and the further study in these field must be made to prove the association objectively.

4.A Clinical Analysis of Intussusception in Infants and Children.

PURPOSE: Intussusception is a very common pediatric problem and needs early management. This study evaluated the risk factors of intussusception in infants and children for early diagnosis and treatment. METHODS: 216 cases of pediatric intussusception occurring between 1993 and 1999 in Mokpo Catholic Hospital were investigated retrospectively. 164 patients treated with barium reduction comprised the barium reduction (BR) group and 52 patients treated with manual reduction or bowel resection was operation (OP) group. RESULTS: The age incidence under 1 year old was 108 patients (65.9%) in BR group, 45 patients (86.5%) in OP group. Male to female ratio was 1.8:1 in BR and 1.7:1 in OP, respectively. In the seasonal distribution spring was more common in both group (34.1%, 34.6%). 60.4% in BR group, 76.9% in OP group were revealed leukocytosis above 10.000/mm3 in serum. Previous combined diseases were upper respiratory infection (31.7%, 42.3%), acute gastroenteritis (1.2%, 1.9%) and multiple familial polyposis (0.5% in BR group). The frequent symptoms and signs were bloody stool (86.0%,92.3%), abdominal pain and irritability (86.6%, 82.7%), vomiting (76.9%, 67.3%), and abdominal mass (56.7%, 76.9%). Tachycardia was 12.2% in BR and 44.2% in OP. In plain abdominal radiography, intestinal obstructive pattern was present in 5.5% of BR group and 53.8% of OP group. Success rate of barium reduction within 24 hours after symptom appearance was 82.3%. Within 24-48 hours was 61.1%, above 48 hours was 25%. There was a statistically significant difference between BR group and OP group of patients under 1 year old, with tachycardia, symptoms for longer than 48 hours and obstructive pattern on plain abdominal radiography (p<0.05). Of surgical cases, 84.6% were idiopathic. The common anatomical type was ileo-colic type (53.9%). The methods of operation were manual reduction (94.2%) and bowel resection (5.8%). Postoperative complications occurred in 4 cases; 3 cases of wound infection and 1 case of pulmonary complication. Recurrence rate was 12.8% in BR group. CONCLUSION: Risk factors of high incidence such as under 1 year old, severe obstructive pattern on the plain abdominal radiograph, symptoms for longer than 48 hours, and tachycardia, were related with a low success rate of barium reduction. Therefore, a gentle barium enema is recommended in high risk patients for diagnosis and prompt surgical intervention.

5.A Study on the low back pain and its associated factors of the staffs in a university.

This study was performed to find the prevalence and the associated risk factors with low back pain(LBP) of the staffs in a university at Kwang-ju city. The data were collected by a questionnaire including SDS(Self-rating depression Scale) battery from September 1st to October 15th, 1996. The results were as follows; 1. The prevalence rate of LBP during lifetime was 73.1%, 53.1% for the last one year, and 23.6% for the last one week. 2. Statistically significant factors related to LBP during lifetime were sex and satisfaction with job. Sex, smoking, alcohol drinking, occupation, work posture, forward bending of trunk and satisfaction with job were selected as significant factors related to LBP during the last one year. The significant determinants related with LBP during the last one week were sex, smoking, alcohol drinking, occupation, forward bending of trunk and satisfaction with job. 3. Through the analysis of self-rating depression scores according to LBP during lifetime were selected to be female(OR=2.63, 95% CI: 1.08~6.40), dissatisfaction with job(OR=2.16, 95% CI: 1.16~4.15), those for the last one year were chosen to be female(OR=2.30, 95% CI: 1.03~5.12), forward bending of trunk(OR=2.18, 95% CI: 1.26~3.79), dissatisfaction with job(OR=1.84, 95% CI: 1.06~3.21), and those for the last one week were female(OR=3.00, 95% CI: 1.17~7.69), forward bending of trunk(OR=2.85, 95% CI: 1.38~5.88). In conclusion, for effective prevention of the LBP at work site, appropriate improvement of work posture and various psychological factors should be considered.

6.A Clinical Analysis of Thyroid Tumor.

BACKGROUND: Malignant thyroid tumors may mimic the clinical symptoms and signs of a non-malignant tumor in the early course of the disease. The choice of the treatment modality for a thyroid tumor can be based on exact interpretation of FNAC (fine needle aspiration cytology) and frozen section. METHODS: We have experienced 237 cases of thyroid tumors during the past ten years at St. Columban hospital. From September 1988 to August 1998, subjects were selected and a retrospective study was done. RESULTS: (1) Of the 237 cases in this study, 194 had non-malignant lesions and 43 had malignant lesions, a ratio of 4.5:1. (2) The female to male ratio was 9.2:1 with non-malignant lesions (female: 195/male: 19) and 9.8:1 with malignant lesions (female: 39/male: 4). (3) The age distribution was the 4th to the 6th decades, regardless of the nature of the lesions. (4) The duration of disease was within 3 months with 42.2% of subjects, and 71.8% of subjects came within first year of disease, regardless of the nature of lesions. (5) Thyroid function tests showed no abnormality with most of the patients, regardless of the nature of lesions. (6) Fine needle aspiration cytology biopsy showed a sensitivity of 32.7%, a specificity of 100%, and an accuracy of 90.8%, and revealed preoperative diagnostic validity. (7) Ultrasonography of the neck, showed a sensitivity of 32.7%, a specificity of 88.1%, and an accuracy of 77.4%, and had no diagnostic value. (8) With benign lesions, adenomatous goiters were most common (96 cases), and with malignant lesions papillary carcinomas were predominant (36 cases). CONCLUSIONS: FNAC and frozen section are valuable preoperative and intraoperative diagnostic tool for thyroid tumors.

7.EFFECTS OF HYPOXIA IN HEAD INJURY.

To evaluate the effects of hypoxia to the outcome of patients with head injury, we analyzed 223 patients prospectively. We divided patients into two groups; without hypoxia and with hypoxia. Hypoxia was defined as PaO2<60mmHg at room air. And clinical parameters such as patient's age, causes of injury, time interval from injury until arrival at the emergency medical center, Glasgow Coma Scale(GCS) on admission, types of brain lesion, and presence of hypotension were compared. Hypoxia was seen in 37 patients with head injury(16.6%). Hypoxia was noted in 32 patients among 162 traffic accidents(19.8%), in 28 patients among 150 cases with mass lesions(18.6%), and in 18 patients among 56 cases with obliterated basal cisterns in CT scan(32.1%). Hypoxia was more commonly seen in patients with severe head injury(GCS< or =8)(26.3%) compared to patients with mild to moderate head injury(GCS>9)(8.9%). Hypoxic insult to the already-injured brain was closely associated with increased mortality and morbidity. Of the 37 patients with hypoxic insult, 23(62.2%) showed poor outcome (vegetative state & dead); only 48 patients(25.8%) without hypoxia showed poor outcome(p<0.001). Among hypoxic group, the incidence of poor outcome was especially high in patients injured in vehicular accidents(55.6%), in low consciousness level(GCS<8) on admission(80.7%), and in intracranial mass lesions(67.9%). Care for the patient with head injuries should start at the scene of injury. 100% oxygen should be administered via a suitable airway during transportation to the hospital, and early positive-pressure ventilation may be necessary.

10.Studies on anticancer effects of extracts caesalpinia sappan on oral carcinoma and osteosarcoma cells.

Anticancer effect of methanol extract of Caesalpinia sappan L. on oral carcinoma (KB) and osteosarcoma (HOS) cells were investigated in this study. In order to elucidate the anticancer mechanism of Caesalpinia sappan L, we analyzed telomerase inhibitory effect of the methanol extract of Caesalpinia sappan L. In addition we prepared 5 fraction samples according to its polarity differences and analyzed anticancer effects on oral carcinoma and osteosarcoma cells. Following results are obtained in this study. 1. 50% cell proliferation inhibitory value (IC50) of the methanol extract of Caesalpinia sappan L. against oral carcinoma (KB) cells and osteosarcoma (HOS) cells were 9.0 microgram/ml and 10.9 microgram/ml, respectively. 2. The methanol extract of Caesalpinia sappan L. showed inhibitory effect of telomerase which is required for cancer cell immortality. Therefore, it seems that the anticancer effect of methanol extract of Caesalpinia sappan is at least partially due to telomerase inhibitory effect. 3. Five fraction samples were prepared according to its polarity and 88.7% of ingredient of total methanol extract was transferred to ethylacetate fraction. Thin layer chromatography analysis showed that dichloromethane fraction contained ingredient with relatively high polarity and ethylacetate fraction contained similar ingredient found in total methanol extract. 4. Anticancer effect was observed in n-hexane, dichloromethane, and ethylacetate fractions. The highest anticancer effect was found in dichloromethane fraction which had IC50 value of 4.4 and>4.0 microgram/ml against oral carcinoma (KB) cells and osteosarcoma (HOS) cells, respectively.
