1.Immunohistochemical Expression of Placental Nitric Oxide Synthase in Preeclampsia and Normal Pregnancy.

OBJECTIVE: Our purpose was to compare the expression of endothelial nitric oxide synthase in the placenta and umbilical cord of preeclamptic placenta with that of the normotensive placenta. METHOD: We compared placental endothelial nitric oxide synthase expression in preeclamptic (n=5) with in normal (n=5) pregnancies. Frozen sections of umbilical cords, chorionic plate vessels, and terminal villi were immunostained with a monoclonal endothelial nitric oxide synthase antibody. RESULTS: The age revaled no difference between control (28.1+4.2 years). and study group (26.1+4.7 years). The gestational age was statistically different between control (38.9+1.7 weeks) and study group (34.9+3.5 weeks). The neonatal body weight and placental weight were also statistically different between control (3060+528 g) and study group (2160 417 g). No difference in endothelial nitric oxide synthase immunostaining in the endothelium of the umbilical vessels and stem villous vessels was found between preeclamptic and normotensive pregnancies. In contrast, in the preeclamptic placental endothelial nitric oxide synthase immunostaining was seen in the terminal villous vessels. In the syncytiotrophoblast endothelial niric oxide synthase immunostaining appeared primary basal in location and diffuse in distribution in the preeclamptic placentas but primary apical in the normotensive placentas. CONCLUSION: Differences in endothelial nitric oxide synthase expression in terminal villous vessels and syncytiotrophblast may be a result of vascular alterations or damage that take place in the placenta in preeclampsia.

2.The usefulness of thin-section rapid infusion CT for detection of cerebral aneurysm.

With 2mm-thick CT scanning during the rapid infusion of contrast material(TICT), cerebral aneurysms arising from the circle of Willis and adjacent vessels can be directly visualized. Twenty-five patients who had cerebral aneurysm confirmed by surgery were examined with TIGT and digital subtraction angiography. The authors examined TIGT prospectively to assess the detection rate of the cerebal aneurysms and to evaluate the clinical usefulness of TIGT. The detection rates of aneurysms by TIGT and digital subtraction angiography were 68% and 84%, respectively. TIGT is a rapid, safe and reliable method in the evaluation of patients with suspected cerebral aneurysm, permitting direct visualization of the aneurysm.

3.Pelvic Arteriovenous Malformation: A Case Report.

Arteriovenous mafformation(AVM) of female pelvic organ is a rare disease of unknown cause. The authors' report a case of pelvic AVM which was incidentally found during US examination of the patient with choriocarcinoma after chemotherapy. The real-time sonography revealed several cystic lesions around the uterus with adjacent dilated tortuous vessels. The color Doppler sonography depicted abundant blood flow mixed with red and blue colors within the cystic lesions and rapid turbulent systolic and diastolic flows. CT showed well-enhancing round vascular lesions with elongated vessels in the pelvis, and MRI depicted signal-void cystic lesions on both T1 and T2 weighted images with small portions of high intensity within the lesions on T2 weighted image. The angiography revealed pelvic AVM fed by tortuous uterine and vaginal arteries with a dilated draining vein

5.Electrophysiological Properties of Reinnervated Motor Units by Transplant of Embryonic Spinal Cord in Rat

OBJECTIVE: Recently, we have demonstrated that embryonic ventral spinal cord motor neurons(ESMNs), transplanted into the distal stump of the axotomized tibial nerve, can grow into the denervated gastrocnemius muscle and form neuromuscular junctions. Our interest in study was to see whether these newly formed neuronal connections are physiologically active, and to electrophysiologically characterize the reinnervated motor units. MATERIAL AND METHODS: Three to eight weeks after transplantation rats were prepared for electrophysiological recording. Motor unit (MU) action potential and gross EMG were monitored. In eleven out of sixteen transplanted animals single unit and gross EMG were recorded after electrical stimulation of the transplant site. Total of 63 motor units were analyzed for their stimulus threshold (ST), latency and stimulus intensity, which is required to produce maximum firing. RESULTS: ST intensities activating MUs were significantly higher in transplanted animals than in controls. Maximum firing of MUs occurred at less than 1.2×T in the control but greater variation was observed in the transplanted animals ranging from 1.1×T to 1.6×T. No significant differences were found in the latencies of MU's firing following stimulation. MU firing was reversibly blocked by infusion of succinylcholine. CONCLUSION: we characterized the temporal, morphological and electrophysiological parameters of denervated skeletal muscle reinnervated by dissociated grafts of embryonic ventral spinal cord cells. MUs reinnervated by the embryonic motoneuronal transplant were physiologically active, with some properties similar to the MUs of the normal.

6.Tratment of Pathological Fractures in Metastatic Bone Tumors

Better palliative management of patients with disseminated cancer has prolonged their lives but has raised the incidence of pathologic fractures in their variable lives. The essence of treatment is to prolong life, maintain comfort, and stabilize the patient with metastatic disease. The treatment of pathologic fractures with internal fixation has long been accepted by orthopedic surgeons and the benefits afforded to the patient are well known, so adequate fixation is essential. A combination of methylmethacrylate and internal fixation devices can greatly enhance the stability of fixation, because diseased bone is a poor base on which to insert an internal fixation device. The authors have treated 19 cases of the pathologic fracture of spine, humerus and femur by use of bone cement with and without internal fixation devices. The results are as follwed. 1. All cases utilized methylmethacrylate and there was no complication in methylmethacrylate in itself. 2. All cases had received various type of ancillary treatment: radiation in 9 cases, chemotherapy in 4 cases and in 8 cases, conservative pain control. 3. In spinal pathologic fracture with paraplegia or neurologic deficiency, the patients could walk with walker at an average of two weeks after operation. 4. Among the 19 cases, the pain was relieved markedly and ambulation was possible easily and early after operation in 16 cases. 5. Among the 19 cases, two survived less than 2 months, 7 surrived from 2 month to 3 month, five survived from 3 months to 6 months, four survived from 6 months to a year and one survived more than a year.

7.Clinical Observation in the Primary Malignant Bone Tumors

A total 73 cases of primary malignant bone tumors was reviewed and analysed clinically at the department of orthopaedic surgery, Kosin medicsl center, Pusan, Kores for 11 years from January, 1975 to December, 1985. The results were obtained as follows ; l. In the 73 cases of primsry malignant bone tumors, osteogenic sarcoma was the most common primary malignant bone tumor (57%) and followed by chondrossrcoma (10%), multiple myeloma (8%). 2. Average survival times according to each primary malignant bone tumors was more than 3 years in chondrosarcoma, reticulum cell sarcoma, and synovial sarcoma, 28 months in osteogenic sarcoma, and 7 months in Ewings sarcoma. Ewings sarcoma had the worse prognosis and the slowly progressing tumors-chondrosarcoma, reticulum cell sarcoma and synovial sarcoma are needed long term follow up. 3. In osteogenic sarcoma, the prognosis was better when developed in their 3rd decsde than when developed in their 2nd decade. 4. There is a slight difference in average survival time on the location of the site, for example when tumor is located in the distal femur, the prognosis was worst. 5. There is no difference in the prognosis. The mode of treatment did not effect to their prognosis. 6. It was clear that the tumors which had not been responded to chemotherapy or radiation therapy had poorer prognosis. Chondrosarcoma, fibrosarcoma and synovisl sarcoma were considered as slowly progressed tumors, and so it may be benefit to the patients that chemotherapy and/or radiotherapy were prescribed.

9.Lobar Agenesis of the Liver'Imaging Findings.

PURPOSE: Congenital Iobar agenesis of the liver is a rare anomaly. We report five cases (three cases of right Iobar agenesis and two cases of left Iobar agenesis) and discuss the radiologic findings of this congenital anomaly. MATERIALS AND METHODS: Between July, 1992 and February, 1993, three cases of right Iobar agenesis and two cases of left Iobar agenesis of the liver were diagnosed by means of computed tomography(CT) and/or sonography. MR imging was performed in two patients, cholangiography in two, and digital subtraction angiography in one. RESULTS: The main findings of right Iobar agenesis of the liver were nonvisualization of the right portal vein and absence of liver tissue to the right of gallbladder. The findings of left Iobar agenesis were nonvisualization of left portal vein, absence of liver tissue to the left of the gallbladder, and absence of ligamentum teres. The ancillary finding of the Iobar agenesis was visualization of less than three hepatic veins. CONCLUSION: It is important to consider Iobar agenesis of the liver in differential diagnosis when imaging studies reveal abnormal portal vein branches, unusual position of gallblader, absence of ligamentum teres, and visualization of less than three hepatic veins.
