Open brochus with diffuse narrowing, stretching, and leafless tree appearance of the bronchi is the well know bronchographic criterior of bronchiloalveolar carcinoma. Though similar findings are expected in CT, authors couldfind no report concerninng the open bronchus sign of bronchioloalveolar carcinoma demonstrated by CT., Authorspresents CT of bronchioloalveolar carcinoma showing lobar or segmental distribution and patent bronchus within thetumor mass.

2.A Case of Squamous Cell Carcinoma and Sebaceous Carcinoma Arising from Bowen' s Disease.

Bowens disease is generally regarded as a premalignant dermatosis. If untreated, 3% to 5% of patients may develop squamous cell carcinoma. However, sebaciou carcinoma arising from Bowens disease is very rare. We presented a case of quarnous cell carcinoma and sebaceous circ s disease in a 68 year-old male. He had multiple bowenoid skin lesi nsties. A bean-sized nodule as developed on the bowenoid lesion of the he had a large yellow crust.ed exudative tumor on the Rt. thigh. We took a biopsy specimen of these three discrete lesions. The nu lipid stain of frozen section revealed Bowens disease, squarnous concllnoma arising from Bowen on the trunk and extremi Rt. lower abdomen. Almost of routine histology and carcinoma, and sebaceous carcinoma, respectively.

3.Scanning Electron Microscopic Findings of Hair Anomalies.

Scanning electron microscopic(SEM) examination of hair is an efficient and definite method for the diagnosis of hair anomaly. The Hair specimen is placed on a stub, gold coated, and directly viewed without prior fixation or treatment for identification. We have analyzed 25 cases of scalp hair and 2 cases of eyebrow that were sampled with the suspicion of hair anomalies at Seoul National University Hospital during the last 7 year period from January 1988 to June 1992. All 27 cases had certain abnormalities of the hair. They consisted of monilethrix(1 case), hair casts(4 cases), pili torti(1 case), uncombable hair syndrome(1 case), trichorrhexis nodosa(5 cases), woolly hair(5 cases) and other anomaly(1 case). Some cases of them were mixed with two or three anomalies in hairs. Their combinations were; hair casts and woolly hair(1 case), hair casts and pili torti and uncombabla hair syndrome(1 case), hair casts and uncombable hair syndrome and trichorrhexis nodosa(1 case), trichorrhexis nodosa and uncombable hair syndrome(3 cases). Most of these defects included brittleness of scales and distortion of the hair shaft.

5.Cineangiographic analysis of T.O.F.

The total 223 cases of tetralogy of Fallot is analysed angiographically, in which selective biplanecinecardioangiography with axial projection has been done at the Departement of Radiology, Seoul National University Hopital, in recent 2 year and 4 months, with specific refrence to the location of pulmonic stenosis,and associated cardiac anomalies. The summary of the analysis is as follow; 1. Left ventriculogram (long axialoblique view) was necessary to detect the ventricular septal defects including anterior marginal VSD, and washelpful to identify the coronary artery anatomy and presence of PDA and evaluate the size of left ventricle. 2.Right ventriculogram(elongated RAO view) was the best projection for the demonstration of pulmonic infundibulumand was essential to differenciate and localize the ventricular septal defects. 3.In the right ventriculogram(4chamber view), well demonstrated the right ventricular outflow tract, pulmonary valve, and pulmonaryarteries(main, right, left and peripheral), and also perimitted the detection of ASD in levophase. 4. The positionof ventricular septal defects was perimembranous in 199 cases(89.2%), subarterial in 15 cases(6.8%) andinfundibular (intracristal) in 9 cases(4%). Multiple ventricular septal defects (perimembranous& marginal ) werefound in 11 cases(4.5%). 5. In 35 cases(15.5%), pulmonary stenosis is found only in pulmonary infundibulum. Thecombination of its components was most commonly anterior wall and conus septum hypertrophy in 24 cases(68.6%) 6.Combined pulmonary stenosis was identified in 188 cases(84.3%). The most common combination was infundibular andpulmonary valvular stenosis in 56 cases(29.8%) 7. Associated cardiac anomalies were right aortic arch in 53 cases(23.8%), persistent left SVC in 17 cases (7.7%), ASD in 13 cases (5.9%), PDA in 9 cases(4.1%), aberrant subclavianartery in 5 cases(2.2%), IVC interruption with azygos continuation in 2 cases, single coronary artery in 2 cases,and anterior descending artery from right coronary artery in 1 case, etc. 8. Selective biplanecinecardioangiography with axial projection is essential to evaluate the size of left ventricle and to localizethe ventricular septal defects and pulmonary stenosis, and to detect associated cardiac anomalies in T.O.F. Sometimes aortography is necessary when anomaly of coronary arteries is suspected.

6.Analysis of percutaneous nephrostomy in 72 patients

Percutaneous nephrostomy has been effectively performed 107 times in 72 patients for relief of obstruction,drainage of urinary fistula and renal or extrarenal fluid collection, and functional preservation of urinarysystem, and follow-up check has been made for recent 3 years at department of Radiology in Seoul NatinalUniversity Hospital. All procedures were successful and bilateral nephrostomy was done in 1 case of bilateral congenital UPJ obstruction. Percutaneous nephrostomy was done in left kidney in 38 cases(52.8%), permanent drainagewas done in 55 cases(76.4%) and external drainage was done in 65 cases (90.3%), and the duration of follow-up wasup to 8 months. Underlying disease were supravesical obstruction in 63 cases(87.5%), urinary fistula in 3 casses,inflammatory disease in 5 cases and infravesical obstruction in 1 case. Satisfactory outcomes were made in 68patients, whose BUN and serum creatinine levels have decreased near to upper normal range in at least 3 weeks. Major complications have occurred in 7 cases; symptomatic urinary infection in 3 cases, fracture of the catheterin 2 cases, pneumothorax in 1 case and sepsis in 1 case. Minor complications have occurred in 76 cases; catheterdislodgement or obstruction in 42 cases, fever in 10 cases, perirenal extravasation in 2 cases and periureteralleakage in 2 cases, etc. These complications can be prevented by adequate method duing Seldinger technique undnerultrasonography, CT or fluoroscopy, prophylactic and post-nephrostomy antibiotics, use of disposable catheterkits, frequent irrigation of the catheter and dressing of catheter fixation site when contaminated, prevention ofextraction of the catheter due to patient's carelessness, adequate fixation of the catheter to skin, and continuous follow-up of patients, It is expected that the territory of this iterventional procedure will beextended by instrument insertion and drug instillation through the nephrostomy tract.

8.Livedo Reticularis Idiopathica Associated with Mononeuropathy Multiplex Syndrome and Bilateral Ulnar-median Nerve Anastomosis.

A 31-year-old woman had a livedo reticularis of reticulated, bluish discoloration on both arms and legs for 18 months with a burning pain in the right hand and numbness in both lower legs. The findings of the electromyography were consistent with mononeuropathy multiplex and bilateral ulnar-median nerve anastomosis. A biopsy specimen from the right sural nerve showed perivascular lymphocytic infiltration and occasional myelin digestion chambers, which were consistent with vasculitic neuropathy. We present a patient with livedo reticularis idiopathica associated with mononeuropathy multiplex syndrome who also has bilateral ulnar-median nerve anastomosis.

9.The Expression of IL-4 and Interferon-gamma Genes in Peripheral Blood Mononuclear Cells of Patients with Severe Atopic Dermatitis:Evaluation in Proportion to Serum IgE Levels.

BACKGROUND: It is not yet clear whether the abnormal cytokine production in relation to serum IgE levels in atopic dermatitis (AD) is associated with the amount of mRNA of cytokine gene. OBJECTIVE: Our purpose was to delineate the effect of reciprocal correlation in the level of mRNA between interleukin-4 (I") and interferon-gamma (IFN-γ) in severe AD. METHODS: We examined 15 cases including 5 AD patients with high serum IgE (>2,000 kU/liter), 5 AD patients with low serum IgE (<100 kU/liter), and 5 healthy controls. Using semi quantitative reverse transcription-polymerase chain reaction, IL-4 and IFN-γ gene expressions in peripheral mononuclear cells (PBMC) were examined. RESULTS: 1) IL-4 gene expression in spontaneous PBMC was higher in AD patient groups than in control group, significantly higher only in AD patient group with high serum IgE level (p < 0.05). 2) IFN-γ gene expression in spontaneous PBMC showed increased tendency in AD patient groups than in control group without statistical significance. 3) IL-4 and IFN-γ gene expressions in stimulated PBMC were not different among all three groups. CONCLUSION: In light of our results, high and low IgE subgroups in AD can exist and AD may not be R characterized by the shift in the reciprocal relationship between IL-4 and IFN-γ when T cells are stimulated under antigen presenting cell-independent conditions.

10.A Case of Cutaneous Metastasis of Male Breast Cancer.

Male breast cancer is a rare disease, representing approximately 1% of breast cancer and less than 1.5% of malignant tumors occurring in men. We report a case of cuaneous metastasis of male breast cances. a 64-year-old male who presented with multiple nodules and erythematous plaque on the right anterior chest and enlargement of the right axillary lymplinodes. The histopathologic findings of the nodule showed atypical tumor cells with a somew eat gland-like pattern. The cells showed potive reactions in PAS, DPAS and alcian blue stains, and also positive for estrogen and roiesterone receptors. Biopsy specimens taken from the plaque showed cellular arrangement in show line, referred to as Indian filing, between thiclenecin collagen bundles. He was treated not, tamoxifen, 5-fluorouracil, adriamycin, cyclophosphamide and megestrol acetate. 5 months after nitiation of the treatment, the skin lesions were improved vvith a decreased number and size or dules and decreased intensity of erythema.
