Transfollicular extrusion of sebaceous gland lobules was observed in a case of actinic keratosis and 2 cases of sebaceous hyperplasia. The patient of act.inic keratosis was a 44-year-old white man and the patients of sebaceous hvperplasia were a i3-year-old Korean girl and a 28-year-old Korean man. The sebaceous gland lobules located within the dilated infundibular area and partly between separated granular layers were intact and partly surrounded by keratin materjal. No histopathologic overlapping of sebaceous gland lobules was observed. We suppose that transfollicular ext,rusion of sebaceous gland lobules may not be artifact. but one of natural phenomena. But further study will be necessary to evaluate the significance of this peculiar histopathologic findings.

PURPOSE: The purpose of the study was to investigate five cases with chronic Achilles tendon rupture that occurred after steroid injections. MATERIALS AND METHODS: In our hospital, we experienced five cases of chronic Achilles tendon rupture from September 2010 to March 2012. All patients had got steroid injection for Achilles tendinitis at the other hospitals, and their heel pain was aggravated when they visited our outpatient department. After treatment, signs and symptoms of Achilles tendon rupture were developed and the diagnosis was confirmed by ultrasonography or magnetic resonance imaging (MRI). Surgical treatment was done for Achilles tendon rupture. RESULTS: There was difference between intra-operative findings of Achilles tendon rupture and usual chronic Achilles tendon rupture. Unlike usual findings of chronic Achilles tendon rupture whose scar tissue or tissue attenuation are found around the defect area of Achilles tendon, there were partial necrosis of tendon severe adhesion with surrounding tissue, extensive defect and longitudinal rupture on ruptured area. Also, severe inflammation of paratenon, granulation and fibrinoid deposit were found on biopsy findings in four cases. CONCLUSION: Based on review of data about relative risk and benefit of local corticosteroid injection to inflammatory lesion in Achilles tendon, it requires more attention to Achilles tendon rupture following local corticosteroid injection.

Parakeratosis pustulrisa occurs most often on the upper cxtiemities of girls and that is considered to be a variant of either eczema or psoriasis. It presents with scaling of the tip of the finger, subungual hyperkeratosis, and irregular t.hickening and rumblirg of the distal nail plate. We report a 4-year-old female child patient, who showed spon.aneous nail extraction with erythematous scaly path with pustule formation on the left thumb, ring finger and right middle finger nails. The microbiologic and mycologic examinaiors revealed negative results. Histopathologic examination showed focal parakeratosis of the nail bed and the lower part of the nail plate. We treated her with etretinate(Tigason, 0.6mg/kg/day).

OBJECTIVE: We studied to determine the effect of blood or meconium contamination on the TDx-FLM assay for the assessment of fetal lung maturity. We also studied to evaluate the degree of diluted contaminants that affect the results. METHODS: Nineteen samples of amnotic fluid-14 cases < GA 37weeks and 5 cases > GA 37weeks-were collected and assayed for assessment of fetal lung maturity using tbe TDx-FLM assay. Among the above 19 samples, we used 12 samples-7 cases < GA 37weeks and 5 cases > GA37 weeks-to contaminate with blood or meconium. Maternal blood was added to the amniotic fluid at increasing concentrations fro 1:10 to 1:1280. Diluted meconium (0.5g meconium/10ml amniotic fluid) was added at increasing concentration fiom 1:1 to 1:128. Each samples were assessed by TDx assay. RESULTS: TDx values in the cases of gestational age 37 weeks or more were matured level or borderline level(TDx value > 50mg/g), but below 37 weeks, TDx values wae immature level(TDx value < 50mg/g) except one case. In preterm cases, blood or meconium contamination did not affect the TDx values significantly, although the thick meamium contamination (diluted meconium: amniotic fluid 1:1 - 1:4) increased the TDx values. In term cases, they did not affect the TDx values. CONCLUSION: TDx test was suitable for the evaluation of fetal lung maturity regardless of blood or meconium contamination.

The corpus callosum consists of white fibers connecting the cerebral hemispheres. Agenesis of the corpus callosum is an uncommon congenital anomaly which is easily diagnosed in the postnatal period by ultrasound and computed tomographic scan or MRI, but its prenatal sonographic diagnosis is difficult because of fetal head positioning and limiting trans-axial scans. We experienced a case of agenesis of the corpus callusum with chromosomal anomaly. The prenatal sonographic findings are ventricular abnormalities that demonstrated dilatation of lateral ventricles and disproportionate enlargement of the occipital horns, which were suggestive findings for the corpus callosal agenesis. We could confim the diagnosis of the corpus callosal agenesis with chromosome anomaly by postnatal MRI and chromosome analysis.

Congenital onychodysplasia of the index fingers(COIF) is characterized by hypoplasia or dysplasia of the nails peculiarly restricted to the index fingers. The cause is unknown, but clinical study suggested that ischemia of the finger at a certain period of fetal life might play an important role in its pathogenesis. Major characteristics of this condition are anonychia, micronychia, and polyonychia restricted to the index fingers. We report two cases of COIF, one of which was presented with anonychia of the left index finger, micronychia of the right index finger, and ventricular septal defect of the heart in a 2-week-old male baby, and the other which was presented with polyonychia of the right index finger in a 1-month-old male baby.

Generalized perforating granuloma annulare is a rare variety of granuloma annulare presenting a distinct clinical pattern. We, herein, report a case of generalized perforating granuloma annulare with diabetes mellitus, which developed on a 73 year-old-female who had pruritic generalized small papular lesions with central umbilication, especially on the back and lower extremities for 1 month. Histopathologic examination showed an epidermal perforation and transepidermal elimination of necrobiotic material. The epidermal perforation was communicated with areas of necrobiotic collagen surrounded by palisading granuloma in the dermis.

In order to find out the distribution of fibronectin and fibrinogen in placenta among pregnancy induced hypertensive (PIH) patients, 6 normotensive pregnancies and 17 PIH patients were chosen. The placentas were obtained right after delivery and soaked in the 10% formalin solution. The score was measured in terms of the positiveness of the stain in immunohistochemical stain by using I antibody with the rabbit antihuman fibronectin and the rabbit anti-human fibrinogen. Comparison between two groups are as following: 1.The placenta of PIH patients showed significantly reduced positiveness of fibronectin in their fetal villous vessels and villous stroma. 2. In the PIH patients, the positiveness was reduced in the group giving the birth to intrauterine fetal growth retardation compared to giving birth to normal baby. 3. In both normotensive and PIH patients, the villous basement membrane did not show the staining response to fibronectin, while showing heavy staining response to fibrinogen. 4. The positiveness of fibrinogen in fetal vessels, villous basement membrane, intervillous space and stroma were almost same in both normotensive and PIH patients. From above results, we can conclude that there is abnormality in the distribution of fibronectin especially in the fetal vessels and the villous stroma of placenta among PIH patients. The cause of this result needs further study.
