Vasectomy has become a popular method for male sterilization in Korea and this, in turn, has been followed by an increase in the number of patients requesting vasectomy reversal. Recently, many authors have reported a high success rate of vasovasostomy using microsurgical techniques. However, a significant discrepancy persist between the patency and pregnancy rate despite improvements in surgical techniques. A total of 200 patients undergoing vasovasostomy from May 1983 to April 1990 were reviewed. Of these 200 patients, only 130 patients (98 patients by two-layer, 32 patients by modified two-layer technique) could be followed up and analyzed as to pregnancy rate related to age, vasal obstructive interval, gross appearance and sperm quality of the vas fluid, operative method, and presence or absence of sperm granuloma. Success rates were 90.3% for patency and 65.4% for pregnancy. With regard to factors influencing to operation, it seemed that the results were better when there was a shorter vasal obstructive interval, watery vas fluid, presence of normal morphologic sperm quality, presence of sperm granuloma, and when the operation was done using the two-layer technique. However, no statistically significant difference was found, except between the bilateral presence and absence of sperm granuloma at the vasectomy site. Thus it is suggested that the most important single factor influencing the success rate of vasovasostomy is a meticulous and skillful operative technique itself, althought other factors may affect the outcome. More effort and research are needed to find the factors affecting the success rate of vasovasostomy.

In order to obsetve the effect of stroke on hypothalamic-pituitary axis, TRH stimulabon test and dexamethasone suppression test(DST) were performed in 52 patients with stroke(cerebral infarction, 16 cases; intracerebral hemorrhage, 15 cases: subarachnoid hemorrhage, 21 cases) and 15 age-sex matched patients control without intracrania disease at Keimyung University Dongsan Hospital from April 1987 to August 1988. The results summarized as follows. 1. The frequency of blunted response on TRH stimulation test is significantly higher in total stroke and patients with cerebral infarction than in control(P<0.06, P<0.05). 2. The frequency of DST non-suppression in patients with cerebral infarction is significantly higher than in control, patients with cerebral hemorrhage and patients with subarachnoid hemorrhage(P<0.001, P<0.01, P<0.01). 3. Patients with left hemispheric stroke tend to show more frequent abnormal neuroendocrine test results than patient with right hemispheric stroke. 4. There are no correlation between abnormal neuroendocrine test results and age, sex, size of stroke and Barthel ADL scale. These results suggest stroke can influence on hypothalamic-pituitary axis, more marked in cerebral infarction and left hemispheric lesion.

A 46 years old male patient had visited to our Ophthalmology department due to abrupt onset of decreased visual acuity in his left eye. He had taken an operation for correction of high myopic astigmatism on both eyes 23 years ago in Japan by Dr. Sato. His visual acuity was: (R)0.3(0.7 x Sph. + 0.75D. = Cyl. - 5.50D.A10 degrees) (L)0.15(0.2 x Sph. - 2.50D. = Cyl. - 2.25D. A180 degrees). On slit lamp examination. hoth cornea had 3mm. (length) radiated linear scars in a number about 50 resulted from myopic operation. In his left cornea, several small vesicles and mild corneal central opacity were observed. We have diagnosed this symptom as artificial corneal endothelial Dystrophy.

The main problems of transurethral prostatectomy were intraoperative blood loss and absorption or irrigating fluid. The reduced serum sodium concentration and osmolality. regarded as cause of TURP syndrome. was caused by dilution by several liters of irrigating fluid. Early recognition. while surgery is being performed is important to prevent development of hypovolemia. especially in the aged group. From January 1990 to June 1990. prospective study for the determination of the amount of irrigating fluid absorbed by use of plasma sorbitol concentration, serum electrolytes change and other related factors was undertaken in 30 patients undergoing TURP with manufactured URIONE solution (sorbitol 2.7 gm+mannitol 0.54 gm/100 ml distilled water) as irrigating fluid. An accurate measurement of plasma sorbitol concentration absorbed was obtained by Beutler`s enzymatic sorbitol analysis. Statistical evaluation of all data obtained was made using the `t-test` with significance at 0.1 percent or less. Mean plasma sorbitol concentration absorbed, blood loss. absorbed irrigating fluid volume were 474.2+/-344.3 mg/1.216 ml and 554.3+/-300.2 ml, respectively. A statistically significant positive interrelationship was found among absorbed plasma sorbitol concentration, serum sodium change and absorbed irrigating fluid volume (p<0.001). Also significant correlations could be obtained between absorbed irrigating fluid volume and serum sodium change (p<0.001).

We performed erotic stimulation that in 63 patients with erectile dysfunction in order to investigate erectile responses to visual sexual stimulation in patients with psychogenic and organic impotence and to elucidate valuable parameters for differential diagnosis between psychogenic and organic impotence. After the test using polygraph system and erotic film, subjective sexual drive in each patient was evaluated as grade I (poor), grade II (moderate), grade III (good). Following parameters were statistically analyzed : degree of maximal rigidity, degree of maximal tumescence, T -up phase (time taking place from the beginning of the erection to maximal level). The T-max phase (time taking place for the plateau phase of maximal erection). The results obtained were summarized as follows. Regardless of sexual drive of each patient, psychogenic impotence group showed better maximal rigidity and shorter T-up phase of rigidity than organic impotence group (p<0.05}. Of patients with psychogenic impotence, good sexual drive group showed better maximal rigidity and maximal tumescence than poor sexual drive group (p<0.06). Psychogenic impotence group showing good sexual drive revealed much better maximal rigidity and shorter T-up phase of rigidity than organic impotence group with same degree of sexual drive (p<0.05). These results indicate erotic stimulation test is highly valuable method for differential diagnosis between psychogenic and organic impotence, especially when the patient shows sexual drive in moderate or good degree. And it is suggested that maximal rigidigity and T-up phase of erection parameters in erotic stimulation test provide high validity for the differention.
