1.Effect of Extracorporeal Shock Wave Lithotripsy of Caliceal Stone according to the Location of the Stone .

PURPOSE: Lower caliceal stones treated with ESWL do fail to pass more frequently than middle or upper caliceal stones. We analyzed the results of ESWL of renal caliceal stones according to the location of calix. MATERIALS AND METHODS: We reviewed the stone-free rate in 119 caliceal stone patients treated with 3rd generation lithotriptor, EDAP LT-02 according to the caliceal location of the stone. Location of the stones were upper calix in 30, middle calix in 30, and lower calix in 59 patients. RESULTS: Stone-free rate was 90.0%(27/30 patients) in middle caliceal stones, 74.6%(44/59 patients) in lower caliceal stones, and 66.7%(20/30 patients) in upper caliceal stones. CONCLUSIONS: Stone-free rate of lower caliceal stones is higher than upper caliceal stones.

2.A Case of perifolliculitis Capitis Abscedens et Suffodiens.

We presented a case of perifolliculitis capitis abscedens et suffodiens associated with acne conglobata in 40-year-old rnale. He had a sausage-shaped plaque containing many fistular tracts on right temnporal area and cicatrizing alopecia on occiput. On the middle portion of upper back he had a adult fist-sized plaque containing many fistular tracts. Histopathologically epidermis showed mild byperkeratosis and keratotic plugging and in the dermis most hair follicles were destroyed and replacel by fibrous tissue but there were moderxte cellular infiltration composed of neutrophils, lymphocytes a.nd hist.iocytes around the remained hair follicles. On serum electrophoresis alpha,-globulin was elevated and gamna globulin was the upper liviit of nornal variation. Skin tests with murnps vaccine, dinitrochloro benzene (DNCB) and old tuberculin disclosed the evidence of de reased c "ll-mediated immunity(CMI). All th se findings suggest thzt immune rriechanisni r,-ay be concerned ivith the pathogenesis in this case. He wa.s treated with th internal administration of antibiotic and corticosteroid and with surgical opening and curettage of fistular tracts

3.Arthroscopic Surgery of the Mediopatellar Plica Syndrome

The role played by synovial plica in internal derangement of the knee has been increased recently. A clinical application of arthroscopy of the one hundred eighty patients were performed, and eighteen patients with a symptomatic plica were successfully managed and followed by arthroscopic surgery at the department of orthopaedic surgery, Soon Chun Hyang University Hospital from May 1983 to May 1984. The results were summerized as follows; 1. Arthroscopy is the most accurate diagnostic and operative methods for the investigation of the incidence, pathology, clinical significance and treatment of the synovial plica. 2. The incidence of the mediopatellar plica in our view was 41% that is, 74 of 180 symptomatic knee. 3. Arthroscopy is a practical and valuable procedure with minimal associated morbidity or complication. 4. The increased accuracy of diagnosis help to avoid unnecessary operations and to be basic methods of treatment on a better knowledge of the pathology.

5.MRI of Vertebral Compression Fracture: Benign versus Metastasis.

PURPOSE: This study was performed to evaluate differentiating features of spinal compression fractures between benign and metastatic lesions. MATERIALS AND METHODS: We reviewed MR imaging in 52 patients (benign 38, metastasis 14) with vertebral compression fracture. Signal intensity of fracture and uninvolved areas, presence of contrast enhancement, fragmentation, and paravertebral mass were analyzed retrospectively. RESULTS: Signal intensity of fracture site was variable in benign lesions, but low signal intensity on T1-weighted image and high on T2*-weighted image were seen in all cases of metastasis. Signal intensity of uninvolved area was high on T1-weighted image and low on T2*-weighted image in 84% of benign lesions. On the contrary, normal marrow signal intensity was not seen in the uninvolved areas of all metastatic fractures. Contrast enhancement were observed in all cases of benign & metastatic compression fractures. Fragmentation were seen in 1 case of metastasis(7%) and in 11 cases of benign lesions(29%). Paravertebral mass were seen in 5 cases of metastasis(36%) and in 7 cases of benign lesions(18%). CONCLUSION: Presence of normal marrow signal intensity in the uninvolved area of fracture site could be the most useful sign for differentiating benign causes from metastasis.

6.Laparoscopy for nonpalpable testes.

Laparoscopy was performed successfully at operation in 18 consecutive boys who had 19 nonpalpable testes Of the testes. 2 were in the inguinal region. 6 were in the just proximal to the internal inguinal ring. 2 were high intra-abominal position. and 9 were vanished testes (intracana1icular). In every case of vanished inguinal testis and intracanalicular testis. vas and gonadal vessels were observed laparoscopically to exit the internal inguinal ring. In these vanished testes. hernia or patent processus vaginalis was not observed and contralateral testes were located normally in the scrotum. OF 8 intraabdominal testes. 6 were detected laparoscopically. Laparoscopy provided localization or either testis or the course of spermatic vessels in all patients and these information facilitated accurate planning of operative repair.

8.MR Imaging Finding of Intracranial Metastatic Melanoma: Report of Two Cases.

MR finding of melanoma shows peculiar signal intensity based on paramagnetic effect of melanin or intratumoral hemorrhage, which displays decreased T1 and T2 relaxation times in many cases. Authors report MR findings in two cases of intracranial metastatic melanoma, which is pathologically confirmed by operation.

Cystic teratoma on the floor of the mouth is infrequent disease and known as congenital origin. Teratoma has no sex distribution. Treatrnent of the cystic teratoma is completely surgical excision and recurrence is rare. The authors have experienced a case of cystic teratoma on the floor of the mouth in neonate. We reported this case with review of literatures.
