1.A Clinical Study on Childhood Leukemia.

Clinical analysis of 170 patients with childhood leukemia was made, who were admitted to the department of Pediatrics, Pusan Paik Hospital, Inje University, College of Medicine during 10 years from 1981 to 1990. The results were as follows: 1) The annual number of the patients with childhood leukemia was in the range of 6~25 cases. And the annual rate of the number of the patients in the department of the Pediatrics was 0.37~1.60% with the average of 1.06%. 2) Of the 170 patients, 72 cases(42.3%) were in the range of 0~5 years of age, which was the largest age group. In male, of the 97 patients, the largest age group was the age group of 0~5 years, which included 45 cases (46.4%), while in female, the largest was that of 6~10 years, which included 29 cases of 73 patients (39.8%). 3) Of the 170 patients, sex ratio was 1.33:1 with male dominance in all age groups except 1:1.6 with female dominance in the age group of 6~10 years. 4) Of the 170 patients, 113 cases were ALL (66.5%), 50 cases AML (29.4%), and 7 cases CML (4.1%). 5) In ALL, 51 cases of the 113 patients (45.1%), were in the age group of 0~5 years, 35 cases (31%) in the age group of 6~10 years, and 27 cases (23.9%) in the age group of 11~15 years. In AML, 19 cases of the 50 patients (38.0%) were in the age group of 0~5 years, 17 cases (34%) in the age group of 6~10 years, and 14 cases (28%) in the age group of 11~15 years. 6) In ALL, the sex ratio of male: female was 1.21:1, 1,50:1 in AML, and 2.5:1 in CML. 7) According to the FAB classification in ALL, of the 113 patients, 72 cases (63,7%) were in L1 type, 34 cases (30.0%) in L2 type, and only 3 cases in L3 type. In AML, of the 50 patients, 19 cases (38%) were in M2 type, 16 cases (32%) in M1 type, and 6 cases (12%) in M4 type, in order. 8) In CML, of the 7 patients, 4 cases were juvenile type, who were all under 10 years old, while 3 cases were adult type, who were all male above 11 years old. 9) Of the 170 patients, clinical symptoms on admission were pallor in 112 cases (65.9%), fever in 84 cases (49.4%), hemorrhage in 78 cases (45.9%), weakness in 40 cases (25.3%), bone pain in 32 cases (18.8%) in order. 10) Of the 170 patients, physical findings on admission were hepatomegaly in 113 cases (66.5%), lymphnode swelling in 84 cases (49.4%), splenomegaly in 78 cases (45,9%)in order. 11) Of the 170 patients, the hemoglobin level on admission were 4~7gm/dl in 95 cases (55.9%), 8~10gm/dl in 42 cases (24.7%), and 0~3gm/dl in 22 cases (12.9%) in order. 12) Of the 170 patients, the WBC count on admission were 10,000~50,000/mm3 in 58 cases (34.1%), below, 5,000/mm3 in 49 cases (28.8%), and 5,5000~100,000/mm3 in 24 cases (14.1%) in order. 13) Of the 170 patients, the platelet count on admission were below 50,000/mm3 in 93 cases (54,7%), above 100,000/mm3 in 48 cases (28.2%), 20,000~50,000/mm3 in 43 (25,3%), 43 cases (25,3%), and 50,000~100,000/mm3 in 29 cases (17.1%) in order.

4.Early Development of the Nose in Human Embryo: A Stereomicroscopic and Histologic Analysis.

BACKGROUND AND OBJECTIVES: We aimed to analyze the morphologic features of the nose in the human embryo from 4th to 8th developmental weeks according to the Carnegie stage. MATERIALS AND METHOD: A total of 27 cases of embryos, ranging from Carnegie stage of 13 to 23, were analyzed. The external morphology was observed with a stereomicroscope, photographed and analyzed. The histologic features were observed with a light microscope in the horizontally-transected specimens stained with hematoxylin-eosin staining. RESULTS: The nasal placode was observed at the stage 13, and it became flat or even concave at the stage 14. At the stage 15, the nasal pit was formed. At the stage 16, the nasal sac and nasal fin were observed. At the stage 17, the oronasal membrane was formed by thinning of the nasal fin. At the stage 18, the primitive choana was established by rupture of the oronasal membrane. At the stage 19, the lateral palatine process projected vertically below the level of the tongue. The cartilaginous nasal capsule was formed at the stage 20. At the stage 21, the olfactory area was localized to the upper portion of the lateral nasal wall and the nasal septum. At the stage 22, the lateral palatine process developed in a somewhat horizontal orientation. At the stage 23, the premaxilla and primitive choana were formed. CONCLUSION: The development of the nose is most active from the Carnegie stage of 13 to 19, which corresponds to the end of the 4th embryonic week to the end of the 7th week. Thus, this period is considered to be the most important period in human nasal embryonic development.

5.Echocardiographic Diagnosis of Left Ventricular Hypertrophy.

Echocardiography was done on 51 cases with various cardiovascular disease and on 23 cases of normal control subjects from April, 1981 to March, 1982 in the Department of internal medicine, Eul Ji General Hospital, Seoul, Korea. The results obtained were as follows: 1. The left ventricular mass estimated by the measurements made with standard convention was 261+/-8gm in patient group and 126+/-6gm in control group. With penn convention, the left ventricular mass of the patients group was 297+/-11gm and that of control group was 127+/-6gm. The differences between both conventions were significant statistically. 2. In measurements made with standard convention, those who showed increased left ventricular posterior wall thickness were 37 cases(73%) and those who showed increased left ventricular internal dimension were 15 cases(29%). On the other hand, number of cases who showed increased left ventricular mass were 45 cases(88%) and this results suggested estimation of left ventricular mass seems to be more useful method in the diagnosis of left ventricular hypertrophy than simple measurements of left ventricular posterior wall thickness of left ventricular internal dimension. 3. The correlation coefficient between left ventricular mass and maximum voltage of electrocardiography, left ventricular posterior wall thickness left ventricularinternal dimension was 0.70, 0.74 0.51 respectively in standard method and 0.82, 0.76, 0.44 respectively in penn convention. Left ventricular mass was correlated fairly closely with the maximum voltage of electrocardiography and left ventricular posterior wall thickeness, less closely with the left ventricular internal dimension.

6.Invention of the Guide Catheter Irrigation Monitoring Device for Neuroendovascular Therapy.

OBJECTIVE: The thromboembolic events during neuroendovascular therapy (NET) are the major complications of concern that can be occasionally fatal. The thrombotic occlusion of the guide catheter for NET is thought to be the risk of the thromboembolic events. We have developed an idea for inventing the monitoring system of the continuous irrigation through the guide catheter. We herein present a unique invention of the guide catheter irrigation monitoring device. METHODS: We have developed ideas for preventing the thrombotic occlusion of the guide catheter. In order to design a convenient device working in the practical use, we have consulted and shared the ideas with the electrical engineers about putting the invention. RESULTS: The guide catheter irrigation monitoring device (GCIMD) consisted of three parts of optical sensor, main body and electric adapter. In brief, the basic principles of working of the GCIMD are as follows. The optical sensor is attached to the dripping chamber of the line to irrigation solution. The main body had the small light and speaker to make an alarm sounds. The sensor monitors the dripping of flush solution. If the dripping stops more than three seconds, a warning alarm has been activated. So, the operating physicians can concentrate and check the guide catheter irrigation. After the use of the GCIMD, there was no major thromboembolic complication in conjunction with the thrombotic occlusion of the guide catheter in our institute. CONCLUSION: We have developed a brilliant invention of the GCIMD for NET.

7.Shade comparative analysis of natural tooth measured by visual and spectrophotometric methods.

STATEMENT OF PROBLEM: A clinically successful color match is one of the important factor to get an esthetic dental restoration. Dental shade guides are commonly used to evaluate tooth color in restorative procedure. But numerous reports have indicated that common shade guides do not provide sufficient spectral coverage of the natural tooth colors. To address issues associated with the shade guide, distinct avenues have been pursued objective spectrophotometric / colorimetric assessment. PURPOSE: This study compared the accuracy of tooth color selection of spectrophotometer with that of human visual determination. Three main factors were investigated, namely, the effect of light, the individual variation and the experience of the observer. MATERIAL AND METHODS: At the first experiment, on ten patients, one operator independently selected the best matching shade to the unrestored maxillary central incisor, using a Vita Classical Shade Guide in the morning, at noon and in the afternoon. The same teeth were measured by means of a reflectance spectrophotometer. At the second experiment, on ten patients, ten operators (5 experts, 5 novices) selected and measured by the same method above at noon. At the third experiment, the results of the second experiment were divided into two groups, expert and novice, and analyzed. RESULTS: 1. There was significant difference between visual and spectrophotometric assessment (mean delta E values) in experiment 1, 2, 3 (p < .05). 2. There was no significant difference between experts and novices group, when comparing with each visual and spectrophotometric assessment (mean delta E values). CONCLUSION: Spectrophotometer could be used to analyze the shade of natural tooth objectively. Thereby, this method offers the potential tominimize considerably the need for corrections or even remakesafter intraoral try-in of restoration. Furthermore, to achieve its advantage, both the shade-matching environment and communication between dentist and technician should be optimized with use of visual and instrumental shade-matching systems.

8.Refractory pseudomembranous colitis that was treated successfully with colonoscopic fecal microbial transplantation.

Pseudomembranous colitis (PMC) is a nosocomial and opportunistic infection caused by Clostridium difficile. PMC is related to the use of antibiotics leading to intestinal dysbiosis and an overgrowth of C. difficile. Metronidazole or vancomycin is considered to be the standard therapy for the management of PMC. However, PMC has a 15%-30% recurrence rate and can be refractory to standard treatments, resulting in morbidity and mortality. Here we describe a patient who experienced refractory PMC who was treated with fecal microbiota transplantation. A 69-year-old woman was admitted to the hospital with consistent abdominal pain and diarrhea, which had been present for 5 months. She was diagnosed with PMC by colonoscopy and tested positive for C. difficile toxin. Even though she took metronidazole for 10 days, followed by vancomycin for 4 weeks, her symptoms did not improve. Because of her recurrent and refractory symptoms, we decided to perform fecal microbiota transplantation. Fifty grams of fresh feces from a donor were obtained on the day of the procedure, mixed with 500 mL of normal saline, and then filtered. The filtered solution was administered to the patient's colon using a colonoscope. After the procedure, her symptoms rapidly improved and a follow-up colonoscopy showed that the PMC had resolved without recurrence.

9.Early Electrodiagnostic Features of Upper Extremity Sensory Nerves Can Differentiate Axonal Guillain-Barré Syndrome from Acute Inflammatory Demyelinating Polyneuropathy.

BACKGROUND AND PURPOSE: Serial nerve conduction studies (NCSs) are recommended for differentiating axonal and demyelinating Guillain-Barré syndrome (GBS), but this approach is not suitable for early diagnoses. This study was designed to identify possible NCS parameters for differentiating GBS subtypes. METHODS: We retrospectively reviewed the medical records of 70 patients with GBS who underwent NCS within 10 days of symptom onset. Patients with axonal GBS and acute inflammatory demyelinating polyneuropathy (AIDP) were selected based on clinical characteristics and serial NCSs. An antiganglioside antibody study was used to increase the diagnostic certainty. RESULTS: The amplitudes of median and ulnar nerve sensory nerve action potentials (SNAPs) were significantly smaller in the AIDP group than in the axonal-GBS group. Classification and regression-tree analysis revealed that the distal ulnar sensory nerve SNAP amplitude was the best predictor of axonal GBS. CONCLUSIONS: Early upper extremity sensory NCS findings are helpful in differentiating axonal-GBS patients with antiganglioside antibodies from AIDP patients.

10.Emphysematous Prostatic Abscess Due to Klebsiella pneumoniae: Report of a Case And Review of the Literature.

Emphysematous prostatic abscess is a very rare form of prostatitis. Emphysematous prostatic abscess due to Klebsiella pneumoniae may have a poor prognosis according to a few previous reports. We report a rare case of successfully treated emphysematous prostatic abscess with cystitis due to Klebsiella pneumoniae in a 50-yr-old man with 15-yr history of diabetes mellitus. The patient was referred to the emergency room of our hospital. The KUB film revealed gas shadows in the lower pelvic area suggestive of emphysematous cystitis or emphysematous prostatic abscess. The gas was mainly occupying the prostate and was also seen in the bladder on pelvic CT. The patient was successfully treated with long-term antibiotic use and additional percutaneous drainage of the abscess. Emphysematous prostatic abscess may be misdiagnosed as emphysematous cystitis due to the similar location of gas shadows on radiography. Computerized tomography and transrectal ultrasonography are helpful in making the diagnosis of emphysematous prostatic abscess. Appropriate use of effective antibiotics with drainage of pus is the best treatment. This case emphasizes the importance of timely and accurate diagnosis followed by appropriate treatment in emphysematous prostatic abscess in diabetic patients.
