1.Glomus Tumor: 4 cases Report

Glomus tumor is well recognized and documented lesion which usually presents as painful dermal nodules in fingers. The tumor results from hypertrophy of a glomus, which is a normal structure of the skin, a coiled arterioyenous shunt regulating body temperature. Glomus tumor may occur at any age but is most frequent in the middle age of life. Pain, tenderness and cold sensitivity are the classic triad of symptoms. Complete surgical exision is the treatment of choice for glomus tumors. Successful removal of tumor leads to complete relief of pain and return to normal function. We report 4 cases of subungual glomus tumor successfully treated by complete exision with review of literatures.

2.Diganostic Values of Bone Scan Followed by CT Scan in Undetected Pelvic bone Fracture.

Pelvic bone is complex in its shape, so we often miss to find the fractures in cases with unclear fracture line. We studied 25 patients who were suspected to have occult fractures in pelvic area. We were able to find lots of occult fractures with bone scan followed by CT scan. The results were as follows; 1. There were 31 unexpected hot spots in 25 patients in bone scan. 2. Six cases (19.3%j of new injuries were found on re-evaluation of plain film or additionally rechecked plain films. 3. Twenty-one new fractures (67.7%) were found on CT film study. 4. Total 27 additional fractures (89.1%) were found after bone scan followed CT scan. In conclusion Bone Scan followed by CT is a effective method of detecting occult fracture in pelvic area.

4.A Case of Herpes Zoster Oticus Involving Unilateral 9th and 10th Cranial Nerves without Facial Palsy.

Herpes zoster oticus is a viral disease associated with otalgia, vesicular eruptions, facial paralysis, hearing disturbance, tinnitus, vertigo and other symptoms. Among cranial nerves, trigeminal and facial nerves are the most commonly affected in patients with herpes zoster oticus, but on rare occasions 8th, 9th, 10th, 11th, 12th cranial nerves are influenced as well. Authors ex-perienced a case of herpes zoster oticus involving glossopharyngeal nerve and vagal nerve without facial palsy in a 58-year-old male. We report this case with a review of literatures.

5.Double Pylorus, A Case Report.

Double pylorus is a rare condition consisting of a double communication between gastric antrum and duodenal bulb; in most cases it is a complication of penetrating ulcer, and sometimes it is a congenital abnormality. The prevalence of this rare anomaly ranges from 0.02% to 0.13%; the male: female ratio is about 2: 1. A case of double pylorus, in 41 year-old male, was diagnosed by gastrofiberscopy and UGIS. Two ovoid and crescent pits of the pyloric canal divided by smooth thickened septum were observed endoscopically. The upper GI series showed gastroduodenal fistula connecting the leeser curve of distal antrum with the duodenal bulb. The relevant literatures on the subject were reviewed.

6.The Effect of the Synthetic Bone Glass on the Fibrovascularization into Porous Polyethylene Orbital Implant.

PURPOSE: To evaluate the effect of the synthetic bone glass particulate (BG) on the fibrovascular ingrowth into porous polyethylene orbital implant (PP). METHODS: Forty eight rabbits were divided into 4 groups according to the surgical techniques and implanted materials. One eye was enucleated one eye and PP was implanted in group 1, was eviscerated and PP was implanted in group 2, was enucleated and BG, containing 30% by weight synthetic bone glass particulate, was implanted in group 3, and was eviscerated and BG was implanted in group 4. All implants of each group were assessed by histologic study at the first, second, fourth, and eighth weeks postoperatively. The area of fibrovascular ingrowth was calculated by histologic examination. RESULTS: There were no statistically significant differences in the fibrovascular ingrowth among these groups. A greater number of vessels per unit area and matured fibrous tissue was found in the more outer zone at longer time after implantation, but there was not statistically significant difference among the four groups. CONCLUSIONS: On the basis of this study, the synthetic bone glass particulate did not significantly increase the rate of fibrovascular ingrowth into porous polyethylene orbital implant in rabbits.

7.Tuberous Sclerosis Manifested by Spontaneous Rupture of Renal Angiomyolipoma in a Patient with Polycystic Kidney Disease.

Tuberous sclerosis is an autosomal dominant disorder characterized by seizure, mental retardation and harmatomatous lesions in multiple organs. The renal lesions of tuberous sclerosis are multiple angiomyolipomas often associated with cysts of various sizes. A 47-year-old man who had been on hemodialysis for 12 years was admitted to our hospital because of sudden onset of right flank pain. He had polycystic kidney disease and adenoma sebaceum. Abdominal computed tomography showed an enlarged right kidney with massive hemorrhage, and renal arteriography showed massive bleeding. Immediate transarterial embolization and radical nephrectomy on the right kidney was done. Pathologic examination revealed ruptured renal angiomyolipoma, confirming that he had contiguous gene syndrome. We experienced a case of tuberous sclerosis with spontaneous rupture of renal angiomyolipoma in a hemodialysis patient with autosomal dominant polycystic kidney disease.

8.A Case of Obstructive Esophageal Hematoma after Endoscopic Variceal Ligation.

Endoscopic variceal ligation(EVL) is an accepted alternatives to endoscopic injection sclero- therapy(EIS) in many patients with bleeding esophageal varices. Esophageal hematoma is rare and an unusual complication after EVL or EIS. We present a patient with liver cirrhosis who developed an obstructive esophageal hematoma after EVL.

9.Incidence and Complications of Obesity in Pubescent School Children.

PURPOSE: In recent years, obesity in children has become one of the major nutritional problems, and the trend is that number of obese children is increasing. The purpose of this research is to examine the incidence of obesity in pubescent school children in Seoul and Cheju, and to analyze body fat component, complications and eating habits of those obese children in order to obtain basic information for prevention of obesity in children in the future. METHODS: The total number of 4,555 junior high school students from 4 schools consisting of 1,234 boys and 3,321 girls were examined based on their height and weight. For those children, obesity index was calculated based on the 50th percentile obtained from standard weight for height of children proposed by the Korean Pediatric Association in 1985. The body fat component was measured and compared between 275 children of 530 obese children(11.6%) and 275 children from the control group by Futrex-5000 A body fat and fitness computer. Of those children, oral glucose tolerance test, blood pressure, and biochemical studies were performed on 31 children whose follow-up was possible, and their exercise load and eating habits were examined. RESULTS: It was found that 11.6% of the subject children were obese, which consisted of 12.4% in boys and 11.4% in girls. The study showed 11.4% obesity ratio in Seoul and 13.2% obesity ratio in Cheju, but the difference was insignificant. Percents of bodyfat was 25.3+/-5.7% (mean+/-SD) for control group, 32.9+/-4.8% for mild obesity group, 35.6% for moderate obesity group, and 37.2+/-4.0% for severe obesity group. The morbidity of complications of those 31 children whose follow-up was possible showed the highest percentage of 29.0% in hyperlipidemia, and lower percentage in hypertension, elevated AST and/or ALT, glucose intolerance, diabetes, respectively. The eating habit analysis showed the total calorie intake/total energy expenditure ratio of 1.10 which is higher in calories than energy consumption, and the food intake evaluation showed that 23.3% of fat was contained in total calorie intake. CONCLUSIONS: The childhood obesity becomes the primary cause of adult obesity, hypertension and diabetes, and therefore, eating habits and exercise load should be carefully observed in order to prevent childhood obesity. In addition to the health care provided currently by schools, regular physical check-up should undertaken for early diagnosis to conduct appropriate tests and treatment.
