5.A Case of Granulomatous Reaction to Tattoo Pigment.

A 48-year-old woman, who had had tattoo on her eyebrows, was seen with pruitic erythematous indurated plaques on the tattooing sites. The skin react ion was begun to observe about 4 weeks after getting tattoo. Skin biopsy specimens from the indurated lesions showed granulomatous reactions. Intradermal tests by means of multiple pricks and patch test were done on her low back with the same tattoo dye and also 4 others available in Korea. Serial punch biopsy specimens were obtained from the sites of intradermal test at 2nd, 7th, 28th, and 56th day after intradermal test. Granulomatous reactions were found in the spiecimens taken at the 28th and 56th day.

6.A Case of Xeroderma Pigmentosum.

Xeroderma pigmentosum is characterized by sunlight sensitivity, autosomal recessive inheritance, and multiple cutaneous malignant melanoma, angiosarcoma and fibrosarcoma. Some patient also demonstrate neurological manifestations, ie, mental deficiency, microcephaly, ataxia, choreoathetosis, and deafness. A 22 year old Korean soldier was seen at the Capital Armed Forces General Hospital in May of 1974 because of pigmentation and ulceration on the exposed area. Since the age of 3, the patient has suffered from freckles on exposed area and the lesions were gradually increased in number. About 3 years ago, he observed nodule on the left side of face, which underwent ulceration eventually. There were no history of unconsciousness, convulsion or other neurological symptoms. The histopathological picture in the margin of ulcerated lesion revealed hyperkeratosis and acanthosis in epidermis, and masses of various shaped and sized composed of basalioma cells in dermis. The peripheral cell layer of the tumor masses showed pallisade arrangement of the nuclei.

7.A Case of Becker's Nevus.

The Becker's nevus (pigmentary hairy edidermal nevus) is a variety of epidermal nevus in which epidermal thickening may be minimal, and hairiness and pigmentation obvious. Characteristically it is a unilateral lesion of the shoulder in males but it may affect other sites, be multiple and bilateral, and be found in women and negroes. Histologically nevus cells are not seen in the dermis. A man aged 25 had the nevus covering extensive area on his right shoulder since 2 years ago. The pigmentation was dark brownish and hairy. The pigmented area was noninfiltrated and in place showed perifollicular darkening. Coarse hairs were noticed since a year. ago. The histopathological picture showed hyperkeratosis, acanthosis, papillomatosis and increased melanin pigments in the basal cell layer. Nevus cells were not seen in the dermis.

8.Ultrasonic Assessment of Gastric Emptying According to Feeding Types and Postprandial Postures.

PURPOSE: Regurgitation, vomiting and feeding intolerance are frequent in the neonates. Esophageal function and gastric peristalsis are not fully developed in the neonates, so we should give attention to reduce the incidence of regurgitation and vomiting after feeding. It is necessary to shorten the gastric emptying by change of feeding types and postprandial postures. Gastric emptying time was measured by ultrasound in the neonates to evaluate the effect of feeding types and postprandial postures. METHOD: We measured gastric antral cross sectional area along the abdominal aorta at the level of the superior mesenteric artery in longitudinal section at NPO state (4 hours after feeding), 0 and every 30 min. after feeding until the value goes below or back to the NPO state. Fifteen neonates were examined in each breast-fed and formula-fed group in supine position. Eighteen and 15 neonates were examined in supine and prone posture after formula feeding, respectively. We used 5 MHz convex prove with Aloka Echo Camera SSD-650. RESULT: 1) Gastric emptying time of breast-fed infants was 76.0+/-20.02 min. which was significantly shorter than 96.0+/-20.28 min. of formula-fed infants. 2) Gastric emptying time on postprandial prone posture was 85.0+/-22.43 min. which was not significantly different from 96.0+/-20.28 min. on postprandial supine posture. CONCLUSION: Breast feeding is strongly recommended to the neonates to shorten gastric emptying time. So we can expect to reduce the incidence of regurgitation, vomiting and feeding intolerance. The postprandial posture depends on the traditional trend which is safe and comfortable to the mothers.

10.A Case of Stestocystoma Myltiplex.

Steatocystoma multiplex is a relatively rare condition characterized by multiple, small, yellowish, cystic nodules 2 to 4mm in diameter locatcd princioally on the upper arms and the thighs. This has a high familial tendency and probably has an autosomal dominant mode of transmission. These tumors of steatocystoma multiplex characteristically contain hair follicles and sebaceous glands, and are to be the dermoid cyst. There was a familial and congenital inferiority in the ectoderm in both patient and family, ie, ichthyosis, hypohidrosis, koilonychia, milia and comedones. A 26 year-old Korean soldier was seen in February of 1970. His condition started 10 years previously. The widespread cystic nodules were 0. 2 to 1 cm in size and yellowish in color, and located on chest, back, scrotum and behind both ear lobes. There was thickening and daky brownish discolorations of all finger and toe nails. His father and two brothers had similar disorders, HistoIogieally clusters of hairs, mainly of lanugo size but some of intermediate character, were found in the lumen.
