Throughout medical science today there is increasing emphasis upon understanding the familial and associated genetic interplay in various disease processes. We observed a case of Legg-Calve-Perthes disease developed in monozygotic male twins. They have some mirror image of each other, for example hair whorl and involved hip. This report of the disease which is present in mirror fashion in idenitical twins illustrates enen more vividly the genetic interaction in the disease. Therefore, those who are aware that Legg-Calve- Perthes disease has appeared in a family should keep in mind the probability of the disease developing in another member.

2.Effects of Fresh and Degenerated Autogenous Nerve Graft in Segmental Defect of Sciatic Nerve of Rabbit.

The potential for nerve regeneration and recovery of its function exists in the presence of a suitable pathway for regenerating axon and endoneurial tube can serve as nerve conduit for regenerating axon in fresh nerve graft. But value of degenerated nerve as donor nerve has not been established till now. This experiment assessed the chronologic influence of fresh and degenerated nerve graft on axonal growth for 10mm gap of sciatic nerve in rabbit and how long degenerated nerve was ahle to serve as a nerve conduit microscopically. Electromicroscopically, the regenerating axons which were ohserved in the degenerated nerve graft had more abundant unmyelinated fibers and revealed abundant collagen fibers in the endoneurium. And these regenerating axons became gradually surrounded with newly developed basal lamina and decreased the collagen fibers at l2 week of degenerated nerve graft. Histologically, myelinated axons which were observed in central area of the degenerated nerve graft at 8 week of degenerated nerve graft were relatively thinner, but a definite structural difference of regenerating axons was not found except reduction of number compared with those of tresh nerve graft. The numher of myelinated axons was 6,072+/-l42 in normal sciatic nerve, 4,479+/- 157 in fresh nerve graft group, and 2,968+/-168 in the degenerated nerve graft group. Difference of the number of myelinated axons between fresh and degenerated nerve graft group was significant stati stically(P<0.05). These results showed that the ability of a degenerated nerve graft as a passage for the regenerating axons and it can be employed as one of the favorable nerve conduits.

3.Femoral Varus Derotation Osteotomy in the Treatment of Acetabular Cysplasia in Deveolpinetal Dysplasia of the Hip

We have attempted to define the acetabular remodelling after varus derotation osteotomy and limitations of pelvic osteotomy in the treatment of DDH. We studied the case of thirty eight patients (42 hips) who were divided into three groups according to age at operation and into two groups, subluxation with acetabular dyaplasia and complete dislocation. The cases were evaluated the influence of femoral remodeling, age, acetabular response and instability and requirement of other additional surgery. The length of follow up ranged from two to fourteen years. A return to a valgus femoral neck shaft angle by remodeling was observed in case without avascular necrosis of femoral head. Acetabular correction by remodeling occurred in case of well reduced the femoral head in the socket. Acetabular remodeling was observed even in age of 8. Twenty eight of 42 hips which were complete dislocation required no pelvic osteotomy. A varus derotation osteotomy is a necessary procedure for hip stability and open reduction in treatment of DDH. Good results should be anticipated in patient whose femoral head was well seated in the acetablum.

4.Acute Hydrogen Sulfide Poisoning: TWO CASES REPORT.

Hydrogen sulfide poisoning is frequently encountered in the workplace. Two workers lost their consciousness in an underground tank at a factory producing paper. The tank contained liquid mixture of used paper, sodium oxygenate chloride(NaOC1), and sodium thiosulfate pentahydrate(NaSO3 5H90). A worker(worker A; 36-year-old man) entered tank to remove sludge. When worker A lost his consciousness, worker B entered the tank to rescue worker A, however he lost consciousness inside the tank. We discuss in detail the clinical features of this condition. Hydrogen sulfide poisonings have occurred in industries involving petroleum refining, the manufacture of heavy water, tanning of hides, vulcanization of rubber, and the manufacture of rayon. And it is necessary to stress the health education for workers and managers in these industries.

5.An Analysis of Industrial Accidents in an Automobile Factory.

OBJECTIVES: To know the present conditions of industrial accidents in automobile factories in Korea and make the basic materials essential to prevention of industrial accidents METHODS: We reviewed and analyzed the records of industrial accidents occurred in an automobile factory in 1997 by time of occurrence, job categories(fixed production, skilled trades, mechanical material handling, and clerical 8r technical), type of injury, kind of accident, and location of injury. RESULTS: 1. By age groups, the accident rate was the highest in age 45-49(1.6 %o) followed by age 35-39(1.3 %), 40-44(0.9 %), 30-34(0.9 %), 25-29(0.6 %), and 20-24(0.4 %) and by working duration, it was highest in group of experience over 25 years, and so we could see that the more age or working duration increases the more accident rate increases. 2. By work time, the accident rate was the highest in 10-11 a.m. (21.4 %) followed by 2-3 p.m. (17.9%). By the day of the week, it was as follows, Friday(20.5%) Wednesday (17. 9%), Monday (16. 2 %), Thesday (15. 4 %), Saturday (12. 0 %), Thursday(11.1 %), Sunday(6.8 %) in order. By the month, it was high in May(17.9 %), March(11.1 %), and April(11.1 %) followed by August(10.3 %) and January(9.4 %), so we could see it was high in spring. 3. The most frequent injury was in the back by 41.0 % followed by 17 cases in fingers(14.5 %), legs(9.4 %), hands(7.7 %), head(6.0 %) in order. And by the type of injuries, the sprain was most frequent by 38. 5% followed by fracture(22. 2 %, lumbar HNP(11.1 %), and contusion(9.4 %) in order. The causes of accident were handling heavy materials(34.2 %), upset(17.1 %), stenosis(16.2 %), and falling(6.8 %) in order. CONCLUSIONS: By the above results, to prevent the industrial accidents in automobile factories in Korea, preventive activities for handling heavy materials and back injury, and continuous industrial education are necessary.

6.A Case of Crossed Aphasia in a Dextral.

Crossed aphasia refers to disturbances of language following a purely right-hemisphere lesion in right-handed individuals. In right-handed individuals, aphasia is almost invariably related to a left cerebral lesion and crossed aphasia occurs in only 1 percent who developed a sudden left hemiparesis with global aphasia. He has neither family history of left-handedness or ambidexterity. Brain MRI showed an acute large infarct of the middle cerebral artery territory on the right side and brain SPECT disclosed extensive areas of hypoperfusion in the right hemisphere and no abnormal finding in the left hemisphere. The sensory component of language function was almost improved, but the motor component has been changed. Further studies of crossed aphasia may supply more data on the functional organization of the brain for speech and language.

Traumatic atlanto-occipital dislocation is usually fatal. To date, few cases have been reported in the literature because survival after traumatic atlanto-occipital dislocation is extremely rare. We present the case of a 47-year-old man with traumatic atlanto-occipital rotatory posterior dislocation combined with atlanto-axial rotatory subluxation and treated by occipito-cervical fusion using Bohlman's triple wiring technique.

10.Intraosseous Calcifying Pseudotumor of Axis: Case Report.

A case of fibrocalcifying pseudotumor occuring at a very unusual site, the intraosseous region of axis, is presented. Previous reports of similar lesions in skull base, intracranial parenchyme, soft tissue around spine, mediastinum, and pleura have been described under the designation 'fibro-osseouslesions' and 'calcifying pseudoneoplasm' The etiology, pathogenesis and natural course of the lesion are still unknown. Bvt the lesion is probably benign nature and reactive lesion rather than neo-plastic. Authors performed resection of lamina, spinous process, and a portion of pedicles and occipitocervical fusion to prevent spinal cord compression due to cortical expansion or fracture. Microscopically, amorphous, basophilic, hyaline, and chondroid calcifying masses were rimmed by palisading histiocytes and foreign body-type giant cells. No evidence of malignancy was found.
