1.Ventricular premature complexes and associated factors in the early postinfarction period.

To assess the role of multiple factors in influencing occurrence of ventricular premature complexes after acute myocardial infarction twenty-four hour Holter electrocardiographic tape recording were made in 40 survivors of an acute myocardial infarction 10 to 20days after attack. Ventricular premature complexes in the early post infarction period were not correlated with left ventricular function, age, sex, smoking, diabetes mellitus, previous angina, and previous myocardial infarction. The occurrence of ventricular premature complexes showed a positive correlation with the occurrence of ST-T change. The occurrence of ventricular premature complexes during sleep hours was compared to the awake state. In 22 patients, the incidence of ventricular premature complexes are excluded from analysis, the 22 of patients, or in 76 percent, sleep was associated with a lowered occurrence of ventricular extrasystoles.

2.The Effects of Unpreserved Lidocaine on the Corneal Endothelium during Cataract Surgery.

Recently, intracameral injection of 1% unperserved lidocaine was intro-duced as a supplement to the topical anesthesia in cataract operations. We evaluated the effect of intracameral lidocaine on the corneal endothelium. One hundred eyes of 90 patients undergone scleral tunnel incision and phacoemulsification with posterior chamber intraocular lens implantation were divided into 2 group according to the methods of anesthesia. Topical anesthesia with intracameral injection of 0.2 of 1% unpreserved lidocaine was used to the 50 eyes of 45 patients[group 1] and pinpoint anestesia to the rest[group 2]. Specular microscopy using non-contact specular microscope was done preoperatively, 1 week, 1 month and 3 months postoperatively to compare the endothelial morphologic changes between the 2 groups. The reduction of central corneal endothelial cell density at 3 months postoperatively was 9.45% in Group 1 and 8.74% in Group 2. The difference between the 2 groups was not significant statistically[p>0.1]. Postoperative changes in coefficient of variation[polymegathism] and hexagonality[pleomorphism] were not significantly different between the 2 groups either[p>0.1]. We therefore concluded that intracameral injection of 1% unpreserved lidocaine does not adversely affect the corneal endothelial cell function and morphology and may safely be used as a supplement to topical anesthesia in cataract surgeries.

3.Correlation between menton deviation and dental compensation in facial asymmetry using cone-beam CT.

OBJECTIVE: The purpose of this study was to evaluate the correlation between menton deviation and dental compensation in facial asymmetry. METHODS: Tooth axis and distance of first molar and canine to the reference plane were investigated by cone-beam computerized tomography. The subjects consisted of 50 patients with asymmetric mandibles (male 21, female 29, mean age 24.3 years). Control groups were also assessed (male 11, female 9, mean age 25.6 years). Nine measurements (5 linear measurements and 4 angular measurements) were measured in order to evaluate the correlation between menton deviation and the linear and angular difference of first molar and canine in the deviated and none-deviated sides using the defined MPR images. The differences between deviated and non-deviated side, according to menton deviation, were statistically analyzed using stepwise multiple regression analysis. RESULTS: From the result, Menton deviation was negatively correlated with mandibular first molar's angular measurement (Delta Angle LM6-Mn plane (dev.-ndev.)) and positively with maxillary fist molar's angular measurement (Delta Angle UM6-FH plane (dev.-ndev.)) (p<0.01). Two angular measurements (Delta Angle LM6-Mn plane (dev.-ndev.), Delta Angle UM6-FH plane (dev.-ndev.)) explained the variability in menton deviation with a significant r2 value of 0.589. CONCLUSIONS: This study suggests that the tooth axis of upper and lower first molars leans towards the deviated side of Menton when there is mandibular asymmetry with Menton deviation.

4.No Association of CYP2D6*4 and CYP2D6*10 Polymorphisms with Tardive Dyskinesia in Korean Schizophrenics.

P450 CYP2D6 enzyme(debrisoquine hydroxylase) is known to metabolize many neuroleptics and some genetic polymorphisms in the CYP2D6 gene were reported to be associated with tardive dyskinesia(TD). We investigeted the association of two genetic polymorphisms in the CYP2D6 gene, CYP2D6*4 and CYP2D6*10, with TD in Korean schizophrenic subjects. Subjects consisted of 71 Korean schizophrenics and TD was evaluated using the Abnormal Involuntary Movement Scale(AIMS). There were no statistically significant differences in the demographic variables of age, male to female percentage and the current antipsychotic(CPZ equivalent) dose between the grup with TD and the group without TD. But the duration of antipsychotic drug exposure was siginificantly higher in the group without TD(p=0.000, by independent t-test). The mean AIMS score in the group with TD was 11.2+/-6.6(S.D.). Genotypings ofr the presence of CYP2D6*4 and CYP2D6*10 wee done using PCR amplifications and endonuclease digestions. There were no statistically significant genotypic and alleleic associations between TD and CYP2D6*4(by chi-square tests), and between TD and CYP2D6*10(by chi-square tests). These results indicate that the CYP2D6*4 and CYP2D6*10 polymorphisms have no significant roles in the causation of TD.

The frequency distribution of cardiovascular disease are changing recently due to the development of living environment. Unfortunately there are few epidemiological studies of cardiovascular diseases in general population, we tried to estimate the recent trend of cardiovascular diseases studying hospitalized patients in nationwide 13 large hospitals during a year of 1985. The hypertensive disease (24.1%) was the most common cardiovascular disease and the next were cerebrovascular disease (15.8%), arrhythmias (12.2%), ischemic heart disease (9.7%), congenital heart disease (9.1%), and rheumatic heart disease (5.4%) in order. This results showed that hypertensive disease and cerebrovascular disease are still the major cardiovascular disease and ischemic heart disease and arrhythmias are increased. But chronic rheumatic heart disease is declined compared with previous studies in hospitalized patients.
