1.Practice and Experience of the Development of Pharmaceutical Care by Clinical Pharmacists in Our Hospital Based on Micro Letter Public Platform

OBJECTIVE:To discuss the feasibility of the development of pharmaceutical care by clinical pharmacist based on micro letter public platform. METHODS:The content and method of pharmaceutical care based on micro letter public platform were introduced by registering micro letter public platform public number,building reasonable medication guidance team,releasing reasonable medication information regularly. RESULTS:40 pieces of reasonable medication information had been pushed by micro letter public platform of our department during Aug.-Nov. 2014,and 350 users added our public platform. The pushed information was mainly about reasonable drug use(57.5%),read by 18 585 persons,and read by 465 person per time in average. Reposted or collected 2 190 times in total,reposted or collected 55 times in average,each piece of information was read by 4 219 person per time at the most. CONCLUSIONS:The pharmaceutical care by pharmacists depending on micro letter public platform contribute to put pharmacist team together,build professional image and provide pharmaceutical care for the public in the information age.
2.Relationship among depression associated type 2 diabetes mellitus and personality and quality of life

Objective To study the relationship among depression associated type 2 deabetes mellitus (T2DM) and personality and quality of life.Methods A total of 246 cases of hospitalized T2DM patients were investigated by schedule tables:self-rating depression scale (SDS),Eysenck personality questionaire (EPQ) and adjusted diabetes-specific quality of life scale (A-DQOL).According to the results of SDS,patients were divided into depression group (171 cases) and non-depression group (75 cases).Correlation analysis and regression analysis were carried out for the collected data.Results There were differences in satisfaction,influence,anxiety Ⅰ,anxiety Ⅱ,neuroticism and extraversion between the two groups(P < 0.05 or P < 0.01).Depression extents was related to the quality of life,neuroticism,extraversion and course(r =-0.243-0.392,P < 0.05 or P < 0.01).Anxiety Ⅱ,satisfaction and course enter the regression equation of depression (β =-0.205-0.322,P < 0.05 or P < 0.01).Conclsion Anxiety Ⅱ,satisfaction and course are key factors inducing depression in T2DM.Neuroticism and extroversion may be the predisposing personality characteristics in T2DM patients with depression.In clinical work,it is helpful for treatment by changing personality and mood of patients.
3.Special Review of 2596 Prescriptions Including Potassium Chloride Injection in Neurosurgery Department of a Hospital

Objective:To discuss the management norms for the rational use of potassium chloride injection in neurosurgery department of a hospital.Methods:A special review of the medical advice including potassium chloride injection from January to June in 2016 in neurosurgery department was carried out.Results:In the 2 596 prescriptions including potassium chloride injection,the irrational drug use rate was 17.87%,and among them,92.67% were irrational compatibility,which showed serious risk of drug use.Conclusion:Some defects still exist in the application management of potassium chloride injection in neurosurgery department of this hospital.In order to avoid the risk of clinical medication,it is necessary to establish standardized management norms for the use of potassium chloride injection in this hospital to further promote the management of high-risk drugs with intravenous administration commonly used in clinics.
4.The application of nursing intervention in the prevention of psychiatric disorder in patients after coronary artery bypass grafting

Objective To discuss the effeet of nursing intervention in the prevention of psychiatric disorder in patients after coronary artery bypass grafting (CABG). Methods Patients with CABG(202 cases) were divided into the control group (100 cases) and the intervention group (102 cases).The control group received routine treatment and nursing while the intervention group was given systemic nursing intervention.The mental status was evaluated by mini-mental status examination (MMSE). Results The incidence of psychiatric disorder in the intervention group was 4 cases (3.92%),which was statistically lower than those in the control group(16 cases,16.00%). Conclusions Effective nursing intervention combined with effective treatment could prevent and control the psychiatric disorder of patients after CABG.
5.Compatible Stability of Carbohydrate-Electrolyte Injection and Commonly Used Vitamin-Electrolyte Injections

Objective To study the compatible stability of the carbohydrate-electrolyte injection and commonly used vitamin-electrolyte injections. Methods By simulating clinical use of medicines,the carbohydrate-electrolyte injection and various vitamin-electrolyte injections were mixed respectively.The content of sodium acetate was measured by HPLC,and changes in appearance,pH value and insoluble particles of the injections were observed. Results At room temperature,the compatibility solutions showed no significant changes in appearance,pH value,the number of insoluble particles and the content of sodium acetate within 8 h. Conclusion The carbohydrate-electrolyte injection is compatible with commonly used vitamin-electrolyte injections,and the admixtures are stable within 8 h at room temperature.
6.Analysis of Social Support Conditionand Influencing Factors of Rural Elderly in Shandong Province

Obiecrive:To investigate the social support condition and its influencing factors of the rural elderly in Shandong province.Methods:With multi-stage stratified random sampling method,a questionnaire survey was conducted among 2200 elderly people aged over 60 years using self-developed questionnaire and Social Support Rating Scale (SSRS).Results:Total score of social support in 1115 rural elderly people was (38.01 ±8.39),higher than the national norm.Multiple factor analysis showed that marital status (P < 0.01) and self-care ability (P <0.01) had significant effect on the social support of the rural elderly in Shandong Province.Conclusion:The social support condition of the rural elderly in Shandong province is good.We should pay more attention to the spir-itual demands of the rural elderly and further improve the social support of the rural elderly,and thusto promote the development of the rural elderly physical and mental health.
7.Mental Health Status of Old People in Endowment Institutions and the Influencing Factors in Shandong Province

Objective:To study the mental health of old people who live in endowment institutions in Shandong Province and its influencing factors. Methods: A sample of 2200 old people over 60 years old was selected with the stratified random sampling method. They were investigated using the self-designed questionnaires, symptom checklist (SCL-90), and Social Support Rating Scale. A total of 2013 questionnaires were recovered effectively (91. 5%), and 271 questionnaires were analyzed in this study. Results:41. 7% of old people in endowment insti-tutions in Shandong Province is positive in SCL -90 . Multivariate analysis showed that self -care ability ( P =0 . 045 ) and type of chronic disease ( P=0 . 025 ) associated with mental health problems of old people in Shandong Province. Conclusion:The mental health status of old people in endowment institutions in Shandong Province was not optimistic and its influencing factors included self-care ability and the type of chronic disease. It should take measures to promote successful aging in Shandong Province.
8.Analysis of the Influencing Factors and Countermeasures on the Subjective Well-being of Female Elderly in Urban Community

Objective:To explore the subjective well-being and its influencing factors of female elderly in ur-ban community in Shandong province. Methods: Used the multi-stage stratified sampling method to extract 449 elderly women in 6 communities of 18 cities in Shandong Province. The basic situation and Subjective well-being of their were investigated by using the self-designed questionnaire and Memorial University of Newfoundland Scale of Happiness ( MUNSH) . Results:The economic situation, the number of children, living conditions, self-care ability, chronic disease severity, and other variables have a significant effect on the subjective well-being of fe-male elderly. Conclusions:To improve the economic situation of the urban community elderly women, pay atten-tion to their physical and mental health level, improve the pension and health insurance system, intensify the con-struction of old-age care institutions, playing the roles of family, children, society in the network to support the community elderly women society, can significantly improve the community elderly women′s subjective well-be-ing.
9.Effects of silibinin in improving liver function of rats with alcoholic fatty liver

BACKGROUND: Silibinin has broad pharmaceutical effects, such as anti-free radicals, anti-lipid peroxidation, anti-lipoid oxidase, anti-glutathione (GSH) depletion, anti-neoplastic and serum lipid-lowering effects. Clinically, silibinin is often used in treating alcoholic liver disease. OBJECTIVE: To investigate the pharmacological mechanism of silibinin for alcoholic fatty liver in rats. DESIGN: Randomized and controlled study.SETTING: Guangzhou Hospital of Traditional Chinese Medicine.MATERIALS: The experiment was conducted at the Animal Experimental Laboratory of Guangdong Pharmaceutical Institute from August to October 2003. Totally 57 SD rats, without unusual bacteria, weighting (150±10)g and of either gender, were selected. Yiganling tablets containing 38.5 mg silibinin were produced by Zhuzhou No.3 Pharmaceutical Factory (Batch No. 20020808).METHODS: Among the 57 SD rats, 18 rats were regarded as normal control group. Rats in normal control group were administered with normal saline by gavage, and fed with normal food and distilled water in place of alcohol for 10 weeks. Rats in model group and silibinin group were fed with high-calorie food and 100 mL/L alcohol for 6 weeks to establish model of rat alcoholic fatty liver. The other rats were divided into model control group (n=18) and silibinin group (n=21). Rats in model control group were treated with distilled water while those in silibinin group were treated with 100 mg/kg silibinin. Meanwhile, 100 mL/L ethanol and hyperalimentation feed were given for 4 weeks. After animals were killed, TG, SOD, GSH and MDA levels were measured with liver suspension.MAIN OUTCOME MEASURES: Contents of serum alanine aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase (AKP), total cholesterol (TC), triacylglycerol (TG), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), tumor necrosis factor (TNF)-α , and transforming growth factor (TGF)-β1.RESULTS: All the 57 rats entered the final analysis. Silibinin could inhibit the activities of serum AST, ALT and AKP [(2 550.5±400.1), (533.4±100.0), (2 217.1±750.2)nkat/L], and the differences were significant as compared with those in model control group [(3 600.7±666.8), (800.2±100.0), (2 900.6±1 333.6) nkat/L, P < 0.05-0.01]. Contents of TG, LDL-C, TNF-α and TGF-β1 in silibinin group [(1.8±0.8), (0.17±0.04), (6.66±1.38), (24.1±4.1) mmol/L] were lower than those in model group [(2.8±1.4), (0.20±0.05), (7.81±1.06), (28.8±6.3) mmol/L] with significant differences (P < 0.05-0.01). Silibinin could increase the content of HDL-C but decrease the contents of TG and MDA (P < 0.05-0.01), and improve SOD activity as well as hepatocyte and fatty degeneration (P < 0.01).However, it had no obvious effect on the content of reduced estathion (P > 0.05).CONCLUSION: Silibinin can inhibitthe formation of alcoholic fatty liver in rats. The pharmacological mechanism of silibinin may involve anti-oxidation, removing free radicals, inhibiting lipid peroxidation, regulating blood lipid component, reducing fatty sediment in liver, and anti-immunoinflammation and anti-hyperplasia effects.
10.Application of enhanced recovery after surgery in preventing post-traumatic stress disorder after adult cardiac surgery

Objective To explore the effect of enhanced recovery after surgery (ERAS) in reducing post-traumatic stress disorder (PTSD).Methods Totally 95 patients who received cardiopulmonary bypass surgery in the Department of Cardio-Thoracic Surgery, the Second People's Hospital of Liaocheng between January 2013 and July 2014 were selected as a control group, while another 95 patients who were hospitalized between August 2014 and December 2015 were selected as a treatment group. The patients in the control group received conventional nursing in the Department of Cardio-Thoracic Surgery, while the patients in the treatment group received ERAS on the basis of conventional nursing. The positive rate and level of PTSD symptoms of the patients in the two groups were then evaluated with PCL-C Scale.Results The positive rates of the patients in the treatment group and the control group were 3.16% and 10.53%, respectively (χ2=4.046,P<0.05). The PCL-C scores at different time were statistically different between the patients in the two groups(F=135.15,P<0.05),the PCL-C scores were statistically different between the patients in the two groups(F=33.84,P<0.05),and there were interaction between time and intervention (F=8.47, P<0.01). The positive rates of higher awareness, emphasis on experience and evasion/numbness symptoms of the patients in the treatment group were 21.89%, 12.42% and 10.83%, lower than those of the patients in the control group 28.42%, 15.16% and 14.73% (χ2=16.118, 4.489, 13.678;P<0.01).Conclusions The concept of ERAS used during the perioperative period of cardiac surgery can effectively reduce the incidence and level of PTSD symptoms.
