1.Research progress of dexmedetomidine for elderly patients during anesthesia

Degenerative changes in autonomic nervous system happen in elderly. The cardiac function reduces, the ability of regulating the target organ decreases, and the stability of hemodynamics is not easy to maintain in old people. How to maintain the hemodynamic stability of elderly patients during anesthesia has become the focus of clinical research. Dexmedetomidine, as a new type of 2- alpha adrenergic agonist, has the function of anti-anxiety, sedation, analgesia and anti-sympathy, and which is wildly used in clinic. This article describes the research progress of dexmedetomidine used in elderly patients during anesthesia.
2.Preparation of rhubarb lactobacillin dispersible tablets

Objective: To obtain the optimum prescription of the rhubarb lactobacillin dispersible tablets.Methods: The kind and amount of the excipient were investigated by using the disintegrating time to establish the optimum prescription and preparation process.Results: The tablets were prepared by compressing moist granulation with the PVPP and CMS-Na as the disintegrant,MCC as diluent,sodium alginate as sweller,magnesium stearate as lubricant and 80% alcohol as adhesive agent.The preparation complied with the requirement of Pharmacopeia of the People's Republic of China(2005edition).Conclusion: This prescription of the tablet is reasonable and the preparation process is feasible.
3.In vitro study of type Ⅰ intron-mediated dual reporter gene imaging for carcinoembryonic antigen

Objective To develop a specific trans-splicing intron ribozyme type Ⅰ-mediated dual reporter gene system (Rib53-Fluc-tk) for targeting CEA.Methods The novel CEA-targeting trans-splicing ribozyme with the downstream reporter system (Rib53-Fluc-tk) was constructed by genetic engineering technology.The trans-splicing reaction product was evaluated using the 131I-5-iodo-2'-fluro-l-beta-D-arabinofuranosy-luracil (FIAU) cellular uptake rates and the bioluminescence.Two-sample t test,the analysis of variance and the least significant difference (LSD) t test was performed for data analysis.Results The sequence of Rib53-Fluc-tk was proved by gene-sequencing test.Human MCF-7 breast cancer cells showed a high ratio of firefly luciferase/renilla luciferase (0.64±0.10,n =4).A 520 bp band of product existed,which matched with the predicted size using RNA from cells transfected with Rib53-Fluc-tk in MCF-7.Signals were detected by bioluminescence in human embryonic kidney 293T cells co-transfected with Rib53-Fluc-tk and pCDNA3.1-CEA.The labelling rate of 131I-FIAU was (64.02±4.79)% (n =3).The radiochemical purity was (95.96± 1.07)% (n=3),and the stability of the radiocompound remained high in human serum at least for 24 h.The uptake of 131I-FIAU in 293T cells transfected with Rib53-Fluc-tk was (0.31±0.01)% (n=4),while it increased with the incubation time in 293T cells co-transfected with pCDNA3.1-CEA and Rib53-Fluc-tk and reached (1.40±0.06)% at 4.5 h (F=1 007.29,t=136.34,both P<0.01).Conelusions A novel and specific reporter gene in the cellular level was established.Taking advantage of trans-splicing reaction of the ribozyme,it could improve the specificity of the reporter gene imaging.
4.Accuracy of ultrasound-measured inferior vena cava diameter in assessment of preoperative blood volume in elderly patients

Objective To evaluate the accuracy of ultrasound-measured inferior vena cava (IVC) diameter in assessment of the preoperative blood volume in elderly patients.Methods Sixty patients,aged 60-75 yr,with body mass index 20-25 kg/m2,of ASA physical status Ⅰ-Ⅲ,scheduled for elective transurethral resection of prostate,were randomly divided into 3 groups (n =20 each):control group (group C),lactated Ringer's solution group (group RL),and hydroxyethyl starch group (group H).Lactated Ringer's solution 8 ml/kg was infused intravenously in group RL.Hydroxyethyl starch 130/0.4 8 ml/kg was infused intravenously in group H.SpO2,mean arterial pressure (MAP),HR and central venous pressure (CVP) were monitored before and after fluid therapy.The IVC diameters,both during expiration (IVCe) and inspiration (IVCi),were measured using ultrasound.IVC collapsibility index (IVC-CI) was calculated.Results The IVCeand IVCi were significantly increased,and IVC-CI was decreased after fluid therapy as compared with those before fluid therapy in RL and H groups.Compared with group C,the IVCe and IVCi were significantly increased,and IVC-CI was decreased after fluid therapy in RL and H groups.IVCe and IVCi were positively correlatedwith CVP (r=0.746 and 0.697,respectively).IVC-CI was negatively correlated with CVP (r =-0.547).Conclusion Ultrasoundmeasured IVC diameter provides better accuracy in assessing the preoperative blood volume in elderly patients.
5.Correlation of CT features and pathological characteristics with EGFR gene mutations in invasive peripheral pulmonary adenocarcinoma

Objective To investigate the correlation of CT features and pathological characteristics with epidermal growth factor receptor(EGFR)gene mutations in invasive peripheral pulmonary adenocarcinoma.Methods The amplification refractory mutation system was used to determine EGFR mutations in 1 9 3 surgically resected invasive peripheral pulmonary adenocarcinomas.CT features and pathological characteristics were analyzed retrospectively.Results The total EGFR mutation rate was 62.2% (120/193).Among the features on CT,the maximum tumor diameter (Dmax)in axial plane CT images was significantly smaller in patients with EGFR mutations than that with wild-type EGFR patients [(2.52 ± 1.01)cm vs (3.11 ± 1.34)cm,P<0.05].Receiver operating characteristic (ROC)results indicated that Dmax=2.01 cm was the diagnosis threshold in forecasting EGFR gene mutations,with the sensitivity and specificity of 79% and 64%,respectively.The frequency of EGFR mutations was significantly greater in tumors with ground-glass opacity(GGO)than that without GGO (78.0% vs 56.6%,P<0.05),and in tumors without cystic airspaces than that with cystic airspaces (65.5% vs 40.0%,P<0.05).No correlations were observed between EGFR mutations and other CT features,including GGO/tumor ratio(G/T),lobulation,spiculation,pleural retraction,vascular convergence,air bronchograms,and vacuole signs (P>0.05).Among pathological characteristics,compared with other subtypes,EGFR mutations occurred most frequently in lepidic predominant adeno-carcinomas (77.5% vs 58.2% in other subtypes,P<0.05),and the least frequently in solid predominant adenocarcinomas(26.3%vs 66.1% in other subtypes,P<0.05).The EGFR mutation rate was significantly higher in tumors without lymph node metastases than that with lymph node metastases (66.9%vs 50.9%,P<0.05).Conclusion The CT features and pathological characteristics may be useful indicators to predict EGFR mutation status in invasive peripheral pulmonary adenocarcinoma.
6.Correlation between MSCT findings and pathological classification of thymic neuroendocrine tumors

Objective To study the MSCT appearances of thymic neuroendocrine tumors (NETs)and its correlation with the WHO histological grade.Methods MSCT features of 16 patients with thymic NETs confirmed by pathology were analyzed retro-spectively.The patients were divided into 2 groups according to tumor's grade,i.e.low and intermediate grade,high grade.Results There were 8 patients in low and intermediate grade,8 in high grade.No difference was found among tumor location,size,tumor morphology,calcification,pericardiac thickening,pericardial effusion,pleural thickening,pleural effusion,disappearance of fat line around tumor and mass-pulmonary interface with irregular shape,but significant difference was detected in lymph node metastasis. On enhanced MSCT scanning,statistical difference was detected in linear enhancement of the blood vessels in the tumors,but no difference was found between necrosis and peripheral vessel invasion.Conclusion MSCT findings of different grades in thymic NETs have some characteristics and can be helpful in the grades of predictability.
7.Relationship between thrombospondin-1 gene GI678A polymorphism and acute coronary syndrome

Objective To investigate the possible association between the thrombospandin-1(TSP-1) gene GI678A (Ala523Thr)polymorphism and acute coronary syndrome (ACS) in a Chinese Han population.Method he ease cohort studied was compsed of 412 hospitalized patients with ACS recruited from four participating hospitals between November 2003 and May 2006.The diagnosis of ACS was based on the criteria of AHA/ACC set in 2002.The eontrul group was consisted of 319 age- and sex-matched subjects from partiei pating hospitals,and they were free from coronary artery disease judged by history,clinical examination,electrocardiography,exercise test and angiography.The TSP-1 GI678A polymorphism was determined by polymerase ehain reaction and restriction fragment length polymurphism analysis(PCR-RFLP).Results The prevalence OfAA genotype of the G1678A polymorphismin patients with ACS was significantly higher than that in the control subjects (49.5%% vs.40.4%,P=0.015).The frequencies of GA and GG genotypes were not significantly different between patients with ACS and controls (CA:39.3% vs.46.1%,P=0.070;CA;11.2% vs.13.5%,P=0.340).The frequencies of A allele in the ACS group and control group were 69.2% and 63.5%,respectively (P=0.022).Furthermore,multiple logistic regression analysis showed that the AA genotype was a significant risk factor for ACS (OR=1.52;95% CI:1.11~2.08;P=0.010).Conclusions The present findings suggest that the AA genotype in TSP-1 gene GI678A polymorphism may be associated with a risk factor for ACS in the Hart nationality of China.The AA genotype may be a genetic marker of the liability to the inheritance of AC,S.
8.Relationships between the levels of serum bone-specific alkaline phosphatase, cross-linked telopeptide of type Ⅰ collage and bone metastasis in patients with breast cancer

Objective To analysis the relationships between bone markers, bone-specific alkaline phosphatase (BAP) and cross-linked telopeptide of type Ⅰ collage (ICTP), and bone metastasis of breast cancer.Methods A total of 217 patients' serum were collected.The 217 cases were divided into two groups:109 cases with bone metastasis, 108 cases without bone metastasis. Serum BAP and ICTP was measured by ELISA. The relationships between factors of bone metastasis and serum levels of BAP, ICTP were analyzed.Results The levels of serum BAP and ICTP in bone metastases group were significantly higher than those in non-bone metastasis group[BAP:24.8 μg/L(7.60-213.70 μg/L) vs 21.2 μg/L(7.3~68.8 μg/L),ICTP:7.0μg/L(1.4~32.4 μg/L) vs 4.1 μg/L(0.0~15.8 μg/L) (P=0.003,P=0.000)].The level of serum BAP and ICTP in patients with multiple bone metastasis was significantly higher than that in patients with single bone metastasis[BAP:32.3 μg/L(9.A~213.7 μg/L) vs 18.1 μg/L(7.6~60.0 μg/L),ICTP:7.6 μg/L(1.4~32.4 μg/L) vs 4.9 μg/L(1.8~10.5 μg/L),(P=0.001,P=0.010)].The sensibility of BAP and ICTP was 45.0 % (49/109)and 46.8 % (51/109),respectively.The specificity of ICTP and BAP was 83.3 % (90/108)and 84.3 % (91/108),respectively.Joint detection of BAP and ICTP had improved sensibility in the diagnosis of bone metastasis in breast cancer patients. Conclusion Joint detection of serum bone biochemical markers ICTP and BAP have a little values for diagnosing bone metastasis in breast cancer patients.

Objective: To observe the role of Qudu medicinal granules (祛毒冲剂) on enterogenous endotoxemia . Methods: Sixty-one cases with enterogenous endotoxemia were randomly divided into two groups:Qudu medicinal granule group (n=30) that was treated with Qudu medicinal granules combined with western medicine, and smecta group which was treated with smecta and western medicine (n=31). Changes of symptoms and signs were observed before treatment and 1, 3, 7 days after treatment. Blood samples were collected in the morning to measure the white blood cell (WBC), plasma lipopolysaccharide (LPS) and tumor necrosis factor (TNF) levels. Objective UPLC-MS/MS bio-analysis method was developed for the simultaneous determination ofberberine,naringin,hesperidin,and neohesperidin in plasma of rats.Methods UPLC Acquity BEH C18 (50 rmm × 2.1 mm,1.7 μm) column was used,mobile phases were containing 0.05% formic acid and 2 mmol/L ammonium formate in water (A)-containing 0.05% formic acid in acetonitrile (B) as the mobile phase gradient elution;SD rats were randomly divided into oral administration berberine group,Citrus aurantium extract group,and berberine and C.aurantium extract compatibility group.Results UPLC-MS/MS method could be applied to determination of berberine,naringin,hesperidin,and neohesperidin,method validation meets the requirements of biological sample analysis.When rats were administered with berberine and C.aurantium extract compatibility,the plasma concentration of berberine was much more than single dose of berberine group and the bioavailability of berberine was increased.Meanwhile,naringin and neohesperidin can be detected in rat's plasma.Conclusion The bioavailability of flavonoids is significantly improved as well compared to the single dose of C.aurantium extract.This suggests that berberine and C.aurantium extract compatibility has significant drug-drug interaction.
