2.The Mediating Effect of Depression in the Relationship between Sleep Misperception and Insomnia Severity among Insomnia Patients

Objectives:
Sleep misperception is the underestimation of perceived total sleep time compared to actual total sleep time. It is observed in approximately 50% of patients with insomnia. Insomnia patients with sleep misperception report significantly higher depression than those without sleep misperception. Depression is one of the most consistent risk factors for predicting insomnia. Therefore, this study attempted to confirm the mediating effect of depression in exacerbating insomnia.
Methods:
This study included 77 male and female aged 18–40 years who met diagnostic criteria for insomnia disorder based on the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition. Depression and insomnia severity were measured using self-report questionnaires, and actigraphy data were collected for 1 week. The sleep misperception index was calculated using the sleep diary and actigraphy.
Results:
The Pearson correlation analysis was performed to examine the relationships between sleep misperception, insomnia, and depression. Sleep misperception was positively associated with depression (r=0.399, p<0.01). There was also a significant positive correlation between depression and insomnia severity (r=0.591, p<0.01). However, there was no significant correlation between sleep misperception and insomnia severity (r=0.210, p=0.07). Depression was found to have a full mediating effect on the relationship between sleep disturbance and severity of insomnia (n=77, B=6.1688, 95% confidence interval=2.9960, 10.4562).
Conclusions
This study verified the mediating effect of depression on the relationship between sleep misperception and insomnia severity. The results highlight the importance of considering depression and sleep misperception in insomnia treatment.

4.Solitary Jejunal Tuberculosis with Intestinal Obstruction in an Immunocompetent Patient.

Intestinal tuberculosis is an infection of the gastrointestinal tract by the Mycobacterium tuberculosis complex. To the best of our knowledge, solitary intestinal tuberculosis accompanied by intestinal obstruction, particularly in the middle of the small intestine, is extremely rare. We report a case of solitary jejunal tuberculosis in a 49-year-old man with no underlying disease. He was admitted a few days after the onset of diffuse abdominal discomfort. Upon evaluation, we initially considered a malignancy of the distal jejunum with ileus due to the presence of a mass. Therefore, he underwent laparoscopic resection of the small bowel. Unexpectedly, the histologic specimen showed a chronic caseating granulomatous lesion with acid-fast bacilli. Ultimately, he was diagnosed with solitary jejunal tuberculosis. He was successfully treated with anti-tuberculosis drugs without any complications.

5.Circulating microRNA expression profiling in young obese Korean women

BACKGROUND/OBJECTIVES:
This study investigates correlations between circulating microRNAs (miRNAs) and obesity-related parameters among young women (aged 20–30 years old) in Korea.
SUBJECTS/METHODS:
We analyzed TaqMan low density arrays (TLDAs) of circulating miRNAs in 9 lean (body mass index [BMI] < 25 kg/m2) and 15 obese (BMI > 25 kg/m2) women. We also performed gene ontology (GO) analyses of the biological functions of predicted miRNA target genes, and clustered the results using the database for annotation, visualization and integrated discovery.
RESULTS:
The TLDA cards contain 754 human miRNAs; of these, the levels of 8 circulating miRNAs significantly declined (> 2-fold) in obese subjects compared with those in lean subjects, including miR-1227, miR-144-5p, miR-192, miR-320, miR-320b, miR-484, miR-324- 3p, and miR-378. Among them, miR-484 and miR-378 displayed the most significant inverse correlations with BMI (miR-484, r = −0.5484, P = 0.0056; miR-378, r = −0.5538, P = 0.0050) and visceral fat content (miR-484, r = −0.6141, P = 0.0014; miR-378, r = −0.6090, P = 0.0017). GO analysis indicated that genes targeted by miR-484 and miR-378 had major roles in carbohydrate and lipid metabolism.
CONCLUSION
Our result showed the differentially expressed circulating miRNAs in obese subjects compared to lean subjects. Although the mechanistic study to reveal the causal role of miRNAs remains, these miRNAs may be novel biomarkers for obesity.
6.Factors Affecting the Burden on Employment of Health Care Providers in the Middle Sized Manufacturing Enterprises.

PURPOSE: The purpose of this study was to determine factors affecting the burden on employment of occupational health care providers as well as to develop strategies to reduce burden on employment of them in the middle size manufacturing enterprises. METHODS: The target population of this study was 123 managers working in the middle size manufacturing enterprises. The study questionnaires were selected, as theories and literature suggested, for explaining employment burden, general characteristics of participants and occupational health providers, general characteristics of companies, evaluation of occupational health provider's role and burden score. RESULTS: The mean of employment burden score of manager was 2.2. There was a significant difference in the employment burden scores, manager's age and education, and occupational health provider's age, type of work, certification, and employment status. Also there was a negative relationship between employment burden scores and occupational health provider's role scores (need, role, satisfaction, and benefit). In the results of the standard multiple regression analysis, manager's need scores on occupational health providers were significant predictors of the employment burden scores. CONCLUSION: It is necessary to change the manager's perception to promote employment of occupational health providers.

7.Analysis on the Affective Factors and Preventive Measures of Alcohol-involved Accident in the Construction Industry.

PURPOSE: This study was to analyze the current preventive measures on alcohol- involved accident and the factors that affect such preventive measures in the construction industry. METHOD: The survey was administered to examine how the preventive measures on alcohol-involved accident were executed using the data of '2005 Occupational Safety and Health Survey' conducted in 2005 by the Occupational Safety and Health Research Institute. For this study, we analyzed 944 work places in the construction industry. RESULT: The preventive measures on alcohol- involved accident were being executed in 62.1% of construction companies. As for the number of actual preventive measures on alcohol-involved accident, work places in Jeolla-do executed 2.63 times more frequently than those in Chungcheong-do, and work places with the Occupational Safety and Health Conference executed 2.22 times more frequently than those without such a measure. CONCLUSION: Joining the Occupational Safety and Health Conference was to be one of the most influential ways of preventive measures on alcohol-involved accident in the construction industry. Accordingly, if workers and employers look for active measures and administer them through the Occupational Safety and Health Conference, it will reduce alcohol-involved accident in the construction industry and contribute to the better preventive measures on alcohol-involved accident in the construction and other industries.

9.Effects of a Health Education Program based on Social Cognitive Theory on the Health Promotion of University Students with Metabolic Syndrome.

PURPOSE: The purpose of this study is to evaluate effects of a health education program based on social cognitive theory on university students with risk factors for metabolic syndrome. METHODS: A nonequivalent control group pretest-posttest design was used. Participants were 88 students who had at least 2 risk factors for Metabolic Syndrome (47 students for the experimental and 41 for the control group). The health education program consisted of thirteen sessions. Knowledge regarding smoking and alcohol drinking, self-efficacy and self-esteem were evaluated. t-test, chi2-test, Fisher's exact test, and paired t-test were conducted to analyze the data. SPSS/WIN 19.0 Statistics program was used. RESULTS: There was a statistically significant increase in self-efficacy in the experimental group compared to the control group. No significant changes in knowledge and self-esteem were found. CONCLUSION: The results suggest that the health education program for university students was partially effective. Further work is required to develop more effective health education programs.

10.Information System Success Model for Customer Relationship Management System in Health Promotion Centers.

OBJECTIVES: Intensified competitiveness in the healthcare industry has increased the number of healthcare centers and propelled the introduction of customer relationship management (CRM) systems to meet diverse customer demands. This study aimed to develop the information system success model of the CRM system by investigating previously proposed indicators within the model. METHODS: The evaluation areas of the CRM system includes three areas: the system characteristics area (system quality, information quality, and service quality), the user area (perceived usefulness and user satisfaction), and the performance area (personal performance and organizational performance). Detailed evaluation criteria of the three areas were developed, and its validity was verified by a survey administered to CRM system users in 13 nationwide health promotion centers. The survey data were analyzed by the structural equation modeling method, and the results confirmed that the model is feasible. RESULTS: Information quality and service quality showed a statistically significant relationship with perceived usefulness and user satisfaction. Consequently, the perceived usefulness and user satisfaction had significant influence on individual performance as well as an indirect influence on organizational performance. CONCLUSIONS: This study extends the research area on information success from general information systems to CRM systems in health promotion centers applying a previous information success model. This lays a foundation for evaluating health promotion center systems and provides a useful guide for successful implementation of hospital CRM systems.
