1.Analysis of post-operation complications of laparoscopic appendectomy in elderly patients with acute appendicitis
Chinese Journal of Primary Medicine and Pharmacy 2013;20(5):683-684
Objective To investigate the relationship between laparoscopic appendectomy and relative complications post-operation for the elderly patients with acute appendicitis. Methods The clinical data of 64 elderly patients with acute appendicitis and underwent laparoscopic appendectomy were retrospectively analyzed.According to the different operation mode, all cases were divided into two groups, including laparoscopic operation group 38 cases and conventional open operation group 26 cases. The distribution of cases with complications incision infection, ankylenteron, bleeding, abdominal infection, stercoral fistula, appendix stump inflammation post-operation were compared between the two groups. Results Between two groups, incision infection and intestinal cases distribution difference had statistical significance while in the aspects of bleeding, abdominal infection, fistula and appendix stump inflammation, there was no significant difference. Conclusion Compared with conventional open operation, for elderly patients with acute appendicitis, laparoscopic appendectomy appeared to have an advantage to reduce postoperative complications, which could reduce the incision infection and the incidence of intestinal adhesion.

OBJECTIVE To analyze the isolation rate and four consecutive years′(2004-2007) antibiotic-resistance trend of extended-spectrum ?-lactamases(ESBLs) producing Escherichia coli in urinary tract infection and to provide evidence for anti-infection therapy.METHODS The anti-microbial susceptibility test results of E.coli cultured from the midstream urine of outpatients and inpatients of our hospital from 2004 to 2007 were collected.Whonet 5.4 statistical analysis software was used for analysis.RESULTS During four years,no apparent change has been shown about antibiotic resistant rate of ESBLs producing E.coli.The isolation rate of this kind of E.coli increased year by year from 31% to 44% and showed high resistance to third generation cephalosporins and quinolones antibiotics, but was 100% sensitive to imipenem.CONCLUSIONS With the ESBLs-producing E.coli increasing year by year, the antibiotic-resistance rate of E.coli isolated from urinary tract infection is apparently increased and thus screening of antibiotics sensitive to enzyme-producing bacterial is the key point when the empirical therapy especially when the third and fourth generation cephalosporins are ineffective.

Objective To explore the influence of triple anti-tumor therapy which bases on sirolimus combined huaier granule and thymosin α-1 on T lymphocyte of rat model with liver cancer recurrence after transplantation. Methods Seventy-two Sprague-Dawley(SD)rats were randomly divided into triple therapy group, sirolimus group,huaier-granule group, thymosin α-1 group,positive-control group and blank group (n=1 2 in each group).Except the blank group, rats in all the other groups were established the simulation animal model of liver cancer recurrence after liver transplantation by chemical-induced method. After the model was established, rats in the positive control group were executed to appraise whether the model was successful. The proportion of regulatory T cells (Treg)of CD4 + T lymphocytes in peripheral blood (Treg%),the percentage of CD4 + T lymphocyte of total lymphocyte(CD4 +T%)and the percentage of CD8 + T lymphocyte of total lymphocyte (CD8 +T%),were detected by the flow cytometry respectively. The relationship between Treg% and CD4 + T %,CD8 + T %,the ratio of CD4 +/CD8 + T lymphocytes(CD4 +/CD8 +)was analyzed by the method of Spearman rank correlation. Results Pathological section of rat liver tissue suggested that the rat model was established successfully.Treg % of positive control group was higher than that of blank group, the difference had statistical significance(P <0.05).Treg% of triple therapy group was significantly lower than that of the positive control group,huaier-granule group, thymosin α-1 group, and significantly higher than the blank group (all in P <0.05 ).Compared with positive-control group,CD4 +T% and CD8 +T% of triple therapy group, sirolimus group and thymosin α-1 group were significantly higher (all in P <0.05).CD4 +T%and CD8 +T% of triple therapy group were significantly higher than those of thymosin α-1 group, sirolimus group and huaier-granule group(all in P <0.05).The relationship between Treg% and CD4 +T%,CD8 +T%, CD4 +/CD8 + in peripheral blood were negatively correlated for rats in each group. In addition, the triple anti-tumor therapy decreased the negative correlation between Treg% and CD4 +/CD8 +.Conclusions Sirolimus based triple anti-tumor therapy can decrease the peripheral blood Treg level of the liver cancer rat, increase the number of T lymphocyte and CD4 +/CD8 +,and play the role of anti tumor cell growth and proliferation.

Extracellular vesicles (EVs) are phospholipid bilayer vesicles actively secreted by cells, that contain a variety of functional nucleic acids, proteins, and lipids, and are important mediums of intercellular communication. Based on their natural properties, EVs can not only retain the pharmacological effects of their source cells but also serve as natural delivery carriers. Among them, plant-derived nanovesicles (PNVs) are characterized as natural disease therapeutics with many advantages such as simplicity, safety, eco-friendliness, low cost, and low toxicity due to their abundant resources, large yield, and low risk of immunogenicity in vivo. This review systematically introduces the biogenesis, isolation methods, physical characterization, and components of PNVs, and describes their administration and cellular uptake as therapeutic agents. We highlight the therapeutic potential of PNVs as therapeutic agents and drug delivery carriers, including anti-inflammatory, anticancer, wound healing, regeneration, and antiaging properties as well as their potential use in the treatment of liver disease and COVID-19. Finally, the toxicity and immunogenicity, the current clinical application, and the possible challenges in the future development of PNVs were analyzed. We expect the functions of PNVs to be further explored to promote clinical translation, thereby facilitating the development of a new framework for the treatment of human diseases.
