2.Recent developments in diagnosis of periprosthetic joint infection

Periprosthetic joint infection (PJI) is a devastating complication after total joint arthroplasty.Currently,PJI is the main cause for knee revision and the third reason for hip revision.Accurate diagnosis is of quite importance for patients with potential in revision surgery.However,there is no test alone can achieve both perfect sensitivity and specificity in diagnosis of PJI.The following diagnostic methods still have irreplaceable role,including erythrocyte sedimentation rate (ESR),C reactive protein (CRP) test,synovial white blood cell count and polymorphonuclear percentage,bacteria culture and histology analysis.Some novel diagnostic techniques,such as leucocytes esterase strip test,show advantages in diagnosing as fast,convenient and cost-saving.Synovial biomarkers,such as α-defensin,have promising prospect with both high sensitivity and specificity.Among various diagnostic methods,bacteria culture and drug sensitivity test can guide the therapy of PJI.Thus,it is crucial to improve the positive rate of bacteria culture.With the development of technology,sonication and polymerase chain reaction (PCR) will be the future research highlights.Although the imaging technologies,such as labeled leukocyte imaging and 18F-fluorodeoxyglucose positron emission tomography (18F-FDG PET),have been progressed in the past years,most of them are still in controversy.Further researches are still needed in the future.
3.Bibliometric analysis of core periodicals of military medicine based on SCI

Objective To analyze the literature of two core periodicals ,Military Medicine of the USA and Journal of the Royal Army Medical Corps of the UK,in order to highlight some important authors ,research institutes and some hot spots in the field of military medicine .Methods Citespace and Excel were used to analyze the data of authors institutes , keywords and citation frequency of the collected records .Results and Conclusion The core authors and critical research institutes are mainly from the USA and UK .America is absolutely taking the leading position in research on military medicine that focuses on injury risk factors , trauma and care , mental health , veteran affairs , military epidemiology and prevention .The results are of referential value for researchers and research administrators to find out about the main core authors ,research institutes and hot spots in military medicine .
4.Experience of micro-laparoscopic ventriculo-peritoneal shunt

Objective To summarize the experience of ventriculo-peritoneal (V-P) shunt under micro-laparoscope for the treatment of hydrocephalus. Methods Twenty-eight patients with hydrocephalus were treated by V-P shunt under micro-laparoscope from November 2000 to May 2004. An infraumbilical trocar was placed through a 5 mm incision to introduce the operating laparoscope. Under laparoscopic visualization, a 3 mm trocar was introduced into the abdominal cavity along the right side of the ligamentum teres hepatis through an incision just to the right of the xiphoid process. A 1.5 mm subcutaneous incision at the corresponding site of the diaphragmatic surface of the liver on the right anterior axillary line was made and through it a sewing needle with 4-gauge unabsorbable silk suture was delivered into. A grasping forceps was inserted through the xiphoid port into the abdominal cavity to carry the two sides of the suture out. The V-P tubing was fixed in the interspace between the liver and the diaphragm by the ligation from the outside, and then was tunneled to the head. Results All the operations were successfully accomplished. The operation time for iutraabdominal tube placement was 5~10 min.All the patients experienced complete alleviation of symptoms of intracranial hypertension postoperatively. No surgical complications occurred.Follow-up check-ups for 6~36 months(mean,12 months) in 25 patients found no tube obstruction. Conclusions Ventriculo-peritoneal shunt under micro-laparoscope has the advantages of minimal invasion, precise positioning, excellent visualization and rapid recovery, being a safe and feasible method which is worthy of recommendation.
5.Study on Smoking-related Health Knowledge and Attitudes and their Association with Smoking Behaviors among High School Students in Shanghai

Objective To explore smoking -related health knowledge and attitudes and their association with smoking behaviors among high school students in Shanghai. Methods With stratified random sampling, 9239 students aged 11~20 years old from 57 high schools (19 from each of primary, secondary and professional schools) distributing in whole city were investigated with questionnaire which self -completed anonymously by the students. The contents of the questionnaire included smoking behavior, relation between smoking and health, and attitude to tobacco control policies. Results Of those 9 239 students, there were 19.5% (male 28.2%, female 10.2%) of them who ever smoked; the total mean score of their knowledge and attitude to smoking and health-related issues was 54.42?7.274 (the maximum was 65), and the ratio of total mean score and the maximum score was 0.8065, the highest ratio (0.9767) was found in awareness that smoking was harmful and the lowest (0.3825) in awareness of tobacco control rules and regulations. The scores of each smoking and health -related knowledge and attitude were decreased with increase of cigarette number smoked by students. Compared with other high schools, the lowest scores of each indexes and the highest rate of smoking were found in vocational schools. Conclusion It was found that the smoking behavior was related with their knowledge and attitude that smoking was harmful among high school students in Shanghai. And the tobacco control and intervention should be mainly developed in students of vocational schools.
6.In Vitro Release of Lyophilized Aclacinomycin A Solid Lipid Nanoparticle

OBJECTIVE:To study the in vitro release of lyophilized aclacinimycin A solid lipid nanoparticles(ACM-SLN). METHODS:The release was studied by dynamic dialysis method.Different equations were selected to fit the release law.RESU_ LTS&CONCLUSION:It is good to fit the release law by1 st order equation and Weibull equation.
7.Effect of the PGE_2 Combined with TNF-? to the IP3-Ca~(2+) Pathway Mediated by Membrane TNF-?R of Fibroblasts

Objective To study the effect of different prostaglandin E2 (PGE2) combined with tumor necrosis factor-?(TNF-?) to the IP3-Ca2+ pathway mediated by membrane TNF-?R of fibroblasts. Methods The lung fibroblasts of breed mouse were primarily cultured. 10 ?g/L TNF-? and 10 ?g/L TNF-? combined with PGE2 of different doses were added to culture medium of fibroblasts. At some observation time,the expression of TNF-?R on the cellular membrane of fibroblasts was detected by the method of immunohistochemistry,the level of IP3 in cells was detected by the method of radioimmunoassay and the contents of Ca2+ in fibroblasts by the method of flow cytometry. Results With doses of PGE2 increasing at 30 s time point and 60 s time point,the cpm value of IP3 of fibroblast decreased gradually. The cpm value of IP3 significantly increased when the dose of PGE2 was 1.0 ?g/L,but obviously lower when the dose of PGE2 was 2.0 ?g/L at 120 s time point. The expression of TNF-?R and the contents of Ca2+ decreased in a dose-dependent way with the dose of PGE2 increasing. Conclusion Certain dose of PGE2 could suppress the effect that TNF-? enhance the cell proliferation of fibroblast by the IP3-Ca2+ pathway mediated by membrane TNF-?R of fibroblasts.
8.Combined external beam radiotherapy and intraluminal brachytherapy for tracheal and bronchogenic carcinoma

Objective To discuss the role of combined external beam radiotherapy and intraluminal brachytherapy for tracheal cancer and bronchogenic carcinoma. Methods From February 1987 to August 1996, 4 patients with primary tracheal cancer, 22 with primary lung cancer and 14 (18 sites) with postoperative recurrent lung cancer were irradiated. External beam radiotherapy (EBR ) was delivered with linac X-ray to a total dose of 30-77?Gy (median 52?Gy). Intraluminal irradiation (IR ) was delivered with low dose rate 192 Ir (1.48?BGq) to a total dose of 10-53?Gy (median 28?Gy), 4-6?Gy per fraction on the bronchial mucosa. Results Complete response (CR) was obtained in 37 patients, partial response (PR) in 2 and minor response (MR) in 1. The 3-year and 5-year local control rates by Kaplan-Meier method were 75% and 65%. Twenty-three patients have survived for 3 years or longer, with a 3-year survival rate of 57.5%. Complications were rare. Conclusions Combined external beam radiotherapy and intraluminal irradiation is effective for primary lung cancer and localized tracheal cancer, possibly giving long-term survivals.
9.Military medical literature collection:construction and analysis

Objective To explore the way to compile a global military medical literature collection and perspectives for its research.Methods The names of military medical research institutions and their trends of development were summa-rized via semantic analysis.The collection of retrieval words for military medical research institutions( then the collection) was constructed based on expert consultation.The names of military medical research institutions were collect-ed with manual screening after retrieval withthe collection.The literature collection of military medical research institu-tions was completed with coordinated retrieved of their papers and other publications.Results According to different needs of information analysis, the literature collection of military medical research institutions could be analyzed in terms of their size, types of development, and academic authority.Conclusion Based onthe collection, the military medical research institutions collected in this article included institutions that used to be neglected during the course of information tracking of military medicine.Three kinds of institutions should be paid more attention to.The institutions were the ones with a large number of papers and citations, the ones whose papers increased or decreased dramatically, as well as the ones whose research directions were the priority fields of Chinese PLA.
10.A proteomic approach to study cerebrospinal fluid from migraine patients

Objective To identify the biomarkers associated closely with migraine by proteomics analysis of cerebrospinal fluid (CSF).MethodsForty-five patients were involved in the present study and they were divided into the following two groups:25 with migraine (5 with aura and 20 without aura) were assigned to migraine group,and 20 with giddiness or peripheral neuropathy were assigned to control group.The patients' CSF was collected,and the protein of CSF was extracted by acetone precipitation.The two-dimensional electrophoresis (2D-PAGE) with immobilized pH gradient (IPG) was performed to display the differently expressed protein spots.These spots were then identified by two-dimensional liquid chromatography and tandem mass spectrometry (2D-LC/MS-MS),and semi-quantitatively analyzed by Western blotting.ResultsDifferences were found in the results of 2D-PAGE between migraine group and control group.Ten spots of 9 kinds of proteins were successfully identified,of which 7 spots were down regulated,including transthyretin (TTR),CBX6 53 kDa protein (CBX6),agrin precursor (AGRN),FAM3C precursor (FAM3C),neuronal pentraxin receptor (NPR),dermcidin-isform2 (DCD) and albumin (ALB);whereas junction plakoglobin (JUP) and H2A histone family,member J (H2AFJ) were up-regulated.The results of Western blotting revealed that the net protein retention (NPR) in migraineurs group (0.3351?0.0275) was obviously decreased compared with that of control group (0.8854?0.0957,P
