1.Application of low-pressure washing in the minimally invasive percutaneous renal lithotripsy

ObjectiveTo explore the application value of low pressure flushing in minimally invasive percutaneous nephrolithotomy lithotripsy.MethodsClinical data of 38 patients,who adopted the low pressure flushing therapy for curing the renal or upper ureteral calculi in the percutaneous nephrostolithotomy lithotripsy were retrospectively arakyzed.ResultsAming 38 patients after operation,stone-free rate of stage 1 was obtained in 25 cases and the stones were completely removed after secondary operation in 2 cases.5 patients had partial residual stones and two cases were most of the residual stones.The mean operation time was 1 ~ 2.5h.The average duration of hospitalization was 8d.There were no intraoperative and postoperative complications,such as the spread of infection due to the surgery.ConclusionThe low pressure flushing could improve clinical efficacy of percutaneous nephrostolithotomy lithotripsy and it had high security.

3.Determination of the Concentrations of Salicylic Acid in Serum With RP-HPLC and Its Application

OBJECTIVE:To develop a RP-HPLC method for the determination of salicylic acid in serum and to apply this method to the pharmacokinetic and bioavailability study of salycylic acid in compound aspirin preparations.METHODS:Waters2690HPLC instrument was used with Diamonsil C 18 column(5?m,250mm?4.6mm)as stationary phase and methanol-ace?tonitrile-0.2%phosphoric acid(18∶32∶50)as mobile phase at a flow rate of1.0ml/min,and the detective wavelength was237nm.RESULTS:Calibration curve of salicylic acid was linear in the range of0.40~101.00?g/ml(r=0.9999).CONCLUS_ ION:The method is simple,sensitive,rapid and suitable for pharmacokinetic and bioavailability study of salicylic acid.
4.Determination of Two Ingredients in Calamine and Menthol Lotion by GC-MS

OBJECTIVE:To establish a GC-MS assay for determination of the contents of phenol and menthol in calamine and menthol lotion.METHODS:Using DM-5elastic quartz capillary as separation column,I-octanol as internal standard,phenol and menthol were detected separately under the70℃~150℃step-up hyperthermic condition to select the ion fragment peaks with M/Z of94and71.RESULTS:Phenol and menthol were good linear within1.092~21.840?g/ml(r=0.9998)and1.194~9.552?g/ml(r=0.9999).Recoveries were100.2%(RSD=1.34%)and100.4%(RSD=0.74%)respective?ly.CONCLUSION:The method is quick and accurate with highly repeatability and specificity and it is adequate for contents determination and quality control for this preparation.
5.Determination of the Concentration of Valproic Acid in Serum With RP-HPLC

OBJECTIVE:To determine the concentration of valproic acid in serum.METHODS:Determination was performed with RP-HPLC with methanol:water(70∶30) as mobile phase,?-bromoacetophenone as deriving agent and cyclohexanecarboxylic acid as internal standard,and detected at wavelength 248nm.RESULTS:The calibrating curve of valproic acid was linear in the range of 14.47~248.0?g/ml.CONCLUSION:The method was convenient,rapid,accurate and suitable for TDM.
6.Treatment of a elderly patient of myelodysplastic syndrome-ring sideroblastic refratory anemia with decitabine and review of literatures

Objective To study the effect of decitabine treating elderly patient with myelodysplastic syndrome-ring sideroblastic refractory anemia (MDS-RAS) and review the literatures. Methods Decitabine treated a patient with MDS-RAS four courses, at the dose of 25 mg everyday for 5 days per course. Observed the change of symptoms, peripheral blood cell counts, myelogram, T cell polarization, cellular immunity,chromosome. Determined the curative effect combined with efficiency standard of WHO 2008. Results The clinical symptoms got better after two courses. Peripheral blood cell counts began to get better after one course. The number of leukocyte, hemoglobin and platelet got nearly normal after four courses. After two courses, T cell polarization state got normal, the number of iron ring promyelocyticin bone marrow declined from 16 % to 0 and chromosome changed from complex karyotype to normal. Conclusion Decitabine is an effective drug to the old patient with MDS-RAS. But it needs to increase the number of cases and follow up long-term to observe the effective rate and long-term efficacy.
7.Effect of pulsed radiofrequency on phenotypic transformation of lumbar sympathetic ganglion in rats with diabetic neuropathic pain

Objective To evaluate the effect of pulsed radiofrequency (PRF) on the phenotypic transformation of the lumbar sympathetic ganglion (LSG) in the rats with diabetic neuropathic pain (PDN).Methods Twenty-four clean-grade healthy adult male Sprague-Dawley rats,aged 2 months,weighing 180-220 g,were divided into 4 groups (n =6 each) according to the method of random number table:control group (group C),group PDN,group PRF,and PRF control group (group PC).The PDN model was established by intraperitoneal injection of streptozotocin 60 mg/kg in anesthetized rats.Citrate-sodium citrate buffer 6 ml/kg was intraperitoneally injected in group C.Group PC only received radiofrequency needle puncture.PRF was performed on the right L3 LSG in group PRF.The mechanical paw withdrawal threshold (MWT) to yon Frey filament stimulation was measured before intraperitoneal injection (baseline,T0),before PRF and at 1,3,5,7 and 14 days after PRF.The rats were then sacrificed,and ipsilateral L3 LSGs were removed for determination of the expression of tyrosine hydroxylase (TH) and vesicle glutamate transporter2 (VGLUT2) in LSGs (by double immunofluorescent staining) and for examination of pathological changes (with a light microscope).The number of neurons expressing VGLUT2 was counted.Results Compared with group C,the MWT was significantly decreased at T1-6,and the number of neurons expressing VGLUT2 was increased at T6 in PDN,PC and PRF groups (P<0.05).Compared with PDN and PC groups,the MWT was significantly increased at T2-6,and the number of neurons expressing VGLUT2 was decreased at T6 in group PRF (P<0.05).TH expression in LSGs was found,and no VGLUT2 expression in LSGs was observed in group C,the expression of TH and VGLUT2 in LSGs was found in the other three groups,especially in PDN and PC groups,and most of the neurons expressing VGLUT2 expressed TH simultaneously.Conclusion The mechanism by which PRF mitigates PDN is related to inhibiting the phenotypic transformation of LSGs in the rats.

Objective:To analyse the biological function of anti-IL-6Rβ(gp130) monoclonal antibody and its regulatory effect on IL-6 signaling.Methods:Biological characteristics of anti-IL-6Rβ(gp130) mAb were assessed by Western blot analysis, capture ELISA and peptide ELISA .The phosphorylation of STAT 3 was tested by Western blot analysis in IL-6-stimulated U266/RA-FLS/RA-PBMC with or without anti-IL-6Rβ(gp130) mAb treatment.Results:3 strains of mouse anti-human gp130 mAb were with high affini-ty and different binding epitopes , the kaff of 10A1 was 2.62E-10.In U266, RA-PBMC and RA-SFMC, IL-6 signaling highly activated STAT3 which could be inhibited by anti-gp130 mAb.Conclusion: Anti-IL-6Rβ( gp130 ) mAb might have different binding epitopes and could affect IL-6 stimulated phosphorylation of STAT3, which provides a preliminary experiment for analyse the correlation of IL-6 signaling and RA .
9.Determination of artemisinin in rat plasma with HPLC-mS and its application.

OBJECTIVETo develop a method for determining artemisinin in rat plasma in vivo.
METHODHPLC-MS was adopted. Estazolam was selected as an internal standard (I.S.). The sample and I.S. were extracted using methyl tertbutyl ether and measured at m/z of 305 and 296, respectively.
RESULTWithin the linear range of 5-500 microg x L(-1), the ratio of artemisinin's peak area and I.S. peak area and the concentration showed good linearity, thus the minimum concentration was set to be 5 mictrog x L(-1).
CONCLUSIONThe methodology proved that the method is so suitable for determining the drug concentration in rat blood that it can be used for studying pharmacokinetics in animals.

10.Application of CBL and PBL based on SMART principle in nursing practice teaching in radiotherapy

Objective:To apply CBL combined with PBL based on SMART (specific, measurable, attainable, relevant and time-based) principle in nursing practice teaching in radiotherapy.Methods:A total of 100 nurses who performed nursing practice in the Department of Radiotherapy in Guangdong Provincial Hospital of Traditional Chinese Medicine from May 2016 to May 2020 were selected as the research objects. They were divided into a control group and a study group according to their admissions, with 50 people in each group. The study group used CBL combined with PBL teaching based on SMART principle, and the control group used traditional practice teaching. After the clinical practice, the two groups were assessed on theoretical and clinical practice skills, and the two groups' teaching satisfaction and teaching effects were evaluated through seminars and questionnaire surveys. SPSS 22.0 was used for t test and chi-square test. Results:The theoretical and clinical practice performance assessment scores of the practical nurses in the study group were higher than those in the control group, with statistically significant differences ( P<0.001). The teaching satisfaction rate of the practice nurses in the control group was 62.00% (31/50), and that of the practice nurses in the study group was 96.00% (48/50), with significant differences ( P<0.001). In terms of improving independent learning ability, information acquisition and problem analysis ability, improving clinical thinking ability, mobilizing learning enthusiasm, enhancing teamwork ability, nurse-patient communication ability, and recognition of innovation ability, the teaching satisfaction of the research group was higher than that of the control group. Conclusion:The application of SMART principle in teaching has the advantages of clear goals and quantifiable evaluation. The combination of CBL and PBL based on SMART principle can help to improve the mastery of theoretical and practical skills of radiotherapy practice nurses, and achieve satisfactory teaching results.
