1.Influence of acupoint-injection on TXB2 and 6-keto-PGF1a in patients with pseudobulbar palsy:a randomized controlled trial

Objective:To prove the feasibility and validity ofXing Nao Jing acupoint-injection (XNJ-AI) at Fengchi (GB 20) for pseudobulbar palsy caused by ischemic stroke (PBP-IS).
Methods:An assessor-blinded, two-parallel-group, randomized controlled trial was conducted, and the patients with PBP-IS were recruited and randomly divided into two groups. Patients in the control group received oral aspirin (100 mg per day for 2 weeks). In addition to oral aspirin; patients in the treatment group received XNJ-AI at Fengchi (GB 20), once a day, for two weeks. The primary outcome was assessed by the water-swallowing test (WST). Thromboxane B2 (TXB2) and 6-keto-prostaglandin F1a (6-keto-PGF1a) in plasma were measured before and after the treatment.
Results:In the treatment group, the percentage of swallowing function no less than grade 3 before and after the treatment was 32% and 88%, respectively; in the control group, it was 28% and 76% before and after the treatment, respectively; the difference after the treatment between the two groups was statistically significant (P<0.05). There were statistical differences between pre- and post-treatment levels of plasma TXB2and 6-keto-PGF1αin the two groups (bothP<0.05). After the treatment, there were significant differences in the levels of plasma TXB2 and 6-keto-PGF1α between the two groups (bothP<0.05).
Conclusion:XNJ-AI at Fengchi (GB 20) can improve the patients’ swallowing function and balance the levels of TXB2 and 6-keto-PGF1α in plasma.
2.Optical Properties and Biomedical Application of Gold Nanorods

Gold nanorods is a capsule-shaped gold nanoparticles. Gold nanorods give rise to two absorption peaks corresponding to their plasmonic modes, transverse mode and longitudinal mode, corresponding to light absorption and scattering along the short and long axis, respectively. The longitudinal surface plasmon resonance can be tuned by adjusting their aspect ratio from the visible to the NIR region and extremely sensitive to changes in the dielectric properties of the surroundings including solvents, adsorbates, and the interparticle distance of the gold nanorods. This unique optical property of gold nanorods opens up fascinating applications in biological and chemical sensors. Optical properties and biomedical application of gold nanorods are introduced, and its future research prospects are discussed.
3.Laparoscopic degradable patch repair of umbilical hernia in adults

Objective To evaluate the methods and outcome of laparoscopic degradable bio-patch repair of umbilical hernia in adults. Methods From January 2003 to October 2008, 21 adult patients underwent elective laparoscopic patch repair of umbilical hernia. There were 15 women and 6 men. The mean age was 56 years old (range of 36-73). The diameter of hernia ring was from 3 to 7.5 cm averaging at 5.2 cm. All patients received general anesthesia. Preoperative bowel preparation routinely started one day before the operation. The patients received prophylaxis systemic antibiotics 30 minutes before the operation. An appropriate size of prosthetic patch (Composix E/X, Bard, USA) was that extended the defect margin for about 3-5 cm. The mesh was then inserted into the peritoneal cavity and spreaded flattening, with the polyplypylene side facing outside and it's center coinciding with that of the defect. The mesh was fixed to the abdominal wall with staple tacks in two rings, which was 3-5 cm along the hernia ring and 1-2 cm along the edge of the mesh. Results There was no conversion to open repair. The operative time was 30 to 96 rain and the average was 52 min. Two patients suffered from a transient postoperative tympanites and which subsided 2 to 3 days after the operation. One patient had a severe pain in the repair area around the umbilical and underwent oral medicine treatment, which disappeared one week after operation. There was no seroma and incision or mesh infection occurred. The postoperative hospital stay was 3 to 8 days and the average was 4. 2 days. The follow-up time was 3 months to 5 years and the average was 32 months. No ileus or hernia recurrence during the follow-up. Conclusion Laparoscopic patch repair of umbilical hernia in adults is a safe and effective procedure.

Genome-wide association studies ( CWAS) use high-throughout genotyping technologies to investigate the relation of hundreds of thousands of gene markers(genotype) with clinical conditions and measurable traits (phenotype). Type 2 diabetes mellitus results from the interaction of environmental factors with genetic variants. Many progresses have been acquired from GWAS. New gene regions have been discovered to be involved in the development and function of islet (3-cells, which provides new strategies for the etiology investigation, prevention, and treatment of type 2 diabetes mellitus.

7.Anterior Approach Preperitoneal Repair for Inguinal Hernia with Modified Kugel Mesh:A Report of 35 Cases

Objective To evaluate the effects of anterior approach preperitoneal repair for inguinal hernia with modified Kugel mesh.Methods Clinical data of 35 cases receiving inguinal hernia repair with modified Kugel mesh from May 2004 to June 2006 were reviewed.The critical surgical procedures:the hernial sac was dissected at high position,then reversed and reduced into abdominal cavity;a 10 cm?10 cm preperitoneal space was dissociated around the internal ring,into which a modified Kugel mesh was placed.The mesh was fixed to the conjoined tendon by superior tape,and to the shelving edge of inguinal ligament by inferior tape.Results The operation time was(47?10)min.No analgesic requirement in any case.The postoperative hospital stay was(6?4)days.All the wounds healed uneventfully without any seroma or infection.No recurrent case was found in the follow-up of 1-25 months(mean:13.2 months)in the 35 cases.Conclusions Modified Kugel mesh used in inguinal herniorrhaphy causes less pain and swelling response to the wound,therefore it is quite suitable for repairing Ⅲ and Ⅳ type hernia.
8.Effects of the combination of acupuncture and medication on orthostatic hypotension after incomplete cervical spinal cord injury

Objective: To observe the effect of the combination of acupuncture and medication on orthostatic hypotension after incomplete cervical spinal cord injury. Methods: Ninety-two patients with orthostatic hypotension after incomplete cervical spinal cord injury were divided into two groups according to the random number table method, with 46 cases in each group. The control group was treated with oral midodrine hydrochloride on the basis of conventional treatment, and the observation group was treated with acupuncture in addition to the intervention used in the control group. Both groups were treated for 4 weeks. The changes in supine and orthostatic blood pressures, motor and sensory scores, quadriplegic function index score, clinical efficacy, and safety evaluation were observed. Results: During the treatment, 2 cases dropped out in the observation group, and 3 cases dropped out in the control group. After 4 weeks of treatment, the clinical efficacy of the observation group was better than that of the control group (P<0.05). After treatment, the supine systolic blood pressure and diastolic blood pressure in the two groups had no significant changes (P>0.05), while the orthostatic systolic blood pressure and diastolic blood pressure, the motor and sensory scores, and the quadriplegic function index score were significantly higher than those before treatment (P<0.05), and the scores in the observation group were higher than those in the control group (P<0.05). Adverse reactions were mild in both groups. Conclusion: The combination of acupuncture and medication can significantly improve the orthostatic blood pressure, motor and sensory function and daily living ability of patients with orthostatic hypotension after incomplete cervical spinal cord injury, and it is safe and reliable.
9.The expression and significance of β-catenin and p28GANK in residual hepatocellular carcinoma cells after transcatheter arterial chemoembolization

Objective To investigate the expression and significance of β-catenin and p28GANK in residual hepatocellular carcinoma (HCC)cells after transcatheter arterial chemoembolization (TACE).Methods We collected forty-five cases of surgical specimens of hepatocellular carcinoma after TACE ( TACE group ) and thirty cases of surgery without any treatment (pure surgery group).The expression of β-catenin and p28GANK were detected by using immunohistochemical SP method and compared between the two groups .Results The positive expression of β-catenin and p28GANK in TACE group were 77.78%and 75.56%respectively,which were significantly higher than those in pure surgery group (46.67%and 53.33%respectively,P<0.05).In the residual hepatocellular carcinoma cells of TACE group ,the positive expression of β-catenin showed correlation with the positive expression of p28GANK(Φ=0.318,P =0.033).The high expression of β-catenin and p28GANK were closely related to portal vein thrombosis and distant metastasis (P<0.05).Conclusion The ex-pression of β-catenin and p28GANK in the residual hepatocellular carcinoma cells were increased significantly after TACE.The high expression of β-catenin and p28GANK were closely related to portal vein thrombosis and distant metastasis.The high expression of β-catenin and p28GANK may be one of the reasons of hepatocellular carcinoma invasiveness and metastasis .
10.3DMax Mesh tension free inguinal hernia repair by anterior approach under local anesthesia

Objective To assess Bard3DMax Mesh pre-formed Knitted Polypropylene tension free hernia repair under local anesthesia. Methods Bard 3DMax Mesh was used for inguinal hernia repair in 30 cases under local anesthesia at our department. Results Hernia was successfully repaired in all cases using Bard 3DMax Mesh from the myopectineal orifice through anterior approach under local anesthesia.The mean operation time was (63.1 ± 2.6) min.All cases were able to drink and eat immediately after surgery,there was no postoperative urinary retention.Postoperative pain-free or mild pain was reported in 23 cases,moderate pain in 6,severe pain in only one.Wound healed by first intention in all 30 cases.Two cases suffered from subcutaneous ecchymosis,a seroma above the patch was observed in 2 cases.A inner thigh numb was complained in one case,which subsided 4 months later.Three patients complained below groin area foreign body sensation which disappeared after 1 -5 months. All patients recovered well without recurrence at a mean follow-up of ( 11 ± 7) months. Conclusions Bard 3DMax Mesh tension free inguinal hernia repairing the myopectineal orifice through anterior approach under local anesthesia is safe,and less traumatic.It also helps establish intraoperative diagnosis of combined and occult hernia.
