1.Therapeutic Effect of Ulinastatin Combined with Xingnaojing Injection for Acute Cerebral Hemorrhage

Objective To observe the protective effect to Ulinastatin combined with Xingnaojing Injection (XI) for acute cerebral hemorrhage. Methods One hundred and eight cases of acute cerebral hemorrhage were randomly divided into treatment group ( N=56) and control group ( N=52) . All of the patients were given conventional western medical treatment including relieving cerebral edema, regulating blood pressure, maintaining electrolyte balance, anti-inflammation, preventing epilepsy. Additionally, the control group was given intravenous drip of Ulinstatin, and the treatment group was given intravenous drip of Ulinstatin and XI. The clearance of intracranial hematoma in the two groups was observed on treatment day 3, 7, 14, the absorption of cerebral edema was observed after treatment for one treatment course of 14 days, neurological deficit scores ( NDS) were compared on treatment day 14, 30, and the clinical efficacy was evaluated. Results ( 1) After treatment, hematoma area was reduced in the two groups ( P<0.05 compared with that before treatment); on treatment day 3, the difference of hematoma area was insignificant between the two groups (P>0.05); on treatment day 7 and 14, hematoma area in the treatment group was less than that of the control group, the difference being significant (P<0.05) . (2) On treatment day 14, the total effective rate for absorption of cerebral edema was 89.3% in the treatment group, and was 65.4% in the control group, the difference being significnat ( P<0.05) . ( 3) After treatment, NDS of the treatment group was less than that of the control group, and the total effective rate on NDS was 89.3% in the treatment, higher than 71.2% in the control group, the difference being significant ( P<0.05) . ( 4) In the treatment group, 3 cases had slight increase of aminotransferase. Eight cases of the control group had increased aminotransferase, and then the aminotransferase level recovered to normal after suspension. In the treatment group, 2 cases were dropped out for receiving emergency operation due to rehaemorrhagia during the treatment, and 3 cases were death for illness deterioration. In the control group, 7 cases gave up the treatment for illness deterioration and then were dead during the follow-up. Conclusion Ulinastatin combined with XI shows certain protective effect in treating acute cerebral hemorrhage.
2.Nasal surgery and upper airway radiofrequency ablation in patients with obstructive sleep apnea hypopnea syndrome.

OBJECTIVE:
To investigate the efficacy and related factors of nasal surgery combined with upper air way radiofrequency ablation(RFA) for treatment of obstructive sleep apnea hypopnea syndrome (OSAHS) with chronic nasal blockage.
METHOD:
One hundred and three mild or moderate OSAHS patients with chronic nasal blockage were recruited, all cases had nasal surgery and upper airway RFA. All patients were evaluated by body mass index (BMI), Epworth sleep scale (ESS), snoring scale, and nocturnal polysomnography (PSG). Eighty-nine patients were reevaluated at least 6 months after surgery with the preoperative methods.
RESULT:
After operation, the apnea and hypopnea index (AHI) decreased from (18.67 +/- 9.48)/h to (9.22 +/- 7.18)/h; the lowest artery oxygen saturation (LSaO2) increased from (0.83 +/- 0.08) to (0.92 +/- 0.06); the Epworth sleep scale(ESS) decreased from (8.74 +/- 5.67) to (5.12 +/- 3.74); the snoring scale decreased from (7.16 +/- 2.85) to (3.56 +/- 2.26), the percentage of time with oxyhemoglobin saturation below 0.90 (CT90) decreased from (18.64 +/- 12.98) to (10.73 +/- 8.29). All of the differences were obvious (P<0.01). Success was defined as a postoperative apnea-hypopnea in dex < 10 events per hour and at least 50% less than the preoperative value. The surgical success rate was 75.3% (67/89). No major perioperative complications occurred.
CONCLUSION
Our findings suggest that nasal surgery combined with upper airway RFA can improve snoring and disease-specific quality of life in patients with anatomic na sal obstruction with mild or moderate OSAHS.

3.Observation of proper nutritional therapy in critically ill patients guided by indirect caloimetry

Objective To evaluate the effect of proper nutritional therapy guided by indirect caloimetry . Methods According to the digitaltable ,70 critically ill patients whose APACHE II >5 were randomly divided into oberserving ( nutritional therapy guided by indirect caloimetry ) and control groups ( nutritional therapy guided by harris-benedict formula),each group 35 cases.The results of nutrition support were analysed .Results TP,MAMC,ALB were higher than that before treatment in oberserving group (all P<0.05),the MAMC was also higher than that in control groups(P<0.05).There was no significant difference in Hb and TSF after treatment IN oberserving group (all P>0.05).Conclusion The nutritional therapy guided by indirect caloimetry by harris-benedict formula have the some nutritional effect in critically ill patients .
4.Influence of rhBNP on cardiac function,levels of hsCRP and MMP-9 in patients with acute heart fail-ure

Objective:To evaluate the clinical therapeutic effect and mechanism of recombinant human brain natri-uretic peptide (rhBNP,Xinhuosu)in patients with acute heart failure (AHF).Methods:A total of 90 AHF patients were randomly and equally divided into rhBNP group (received intravenous injection of rhBNP based on routine treatment for 3d)and routine treatment group.Left ventricular ejection fraction (LVEF),levels of serum high sen-sitive C reactive protein (hsCRP)and matrix metalloproteinase-9 (MMP-9)were measured and compared between two groups before and after treatment.Results:Compared with before treatment,after treatment,LVEF signifi-cantly rose,levels of hsCRP and MMP-9 significantly reduced in both groups,P <0.05 or <0.01;compared with routine treatment group after treatment,there was significant rise in LVEF [(41.4±12.8)% vs.(51.3±13.9)%], and significant reductions in levels of hsCRP [(8.63±3.57)mg/L vs.(6.67±3.97)mg/L]and MMP-9 [(17.89 ±4.75)mg/L vs.(14.64±4.89)mg/L]in rhBNP group,P <0.05 or <0.01. Conclusion:The rhBNP possesses significant therapeutic effect on acute heart failure,and its mechanism may be related with reducing hsCRP and MMP-9 levels.
5.Effects of scalp acupuncture plus acupuncture exercise therapy on walking ability in children with spastic cerebral palsy

Objective: To compare the effect of scalp acupuncture and scalp acupuncture plus acupuncture exercise therapy (AET) on walking ability in children with spastic cerebral palsy (CP). Methods: A total of 60 spastic CP children with gross motor function classification system (GMFCS) grades Ⅰ-Ⅲ were divided into a control group and an observation group by the random number table method, with 30 cases in each group. Both groups were treated with the same conventional rehabilitation and scalp acupuncture therapy for CP. The control group received conventional rehabilitation first and then scalp acupuncture. The observation group received AET, which was to receive the conventional rehabilitation and scalp acupuncture simultaneously. Before and after treatment, the clinical efficacy was evaluated by the modified Ashworth scale (MAS) score, scores of dimensions D and E of the gross motor function measure (GMFM) scale, walking speed, and walking distance. Results: During treatment, there were 2 dropouts in the observation group. After 3 courses of treatment, the MAS scores in both the control group and observation group decreased compared with the same group before treatment (P<0.05), and the scores of dimensions D and E of the GMFM, walking speed, and walking distance were increased (P<0.05); the between-group comparison showed that the MAS score in the observation group was lower than that in the control group (P<0.05), and the scores of dimensions D and E of the GMFM, walking speed, and walking distance in the observation group were higher or longer than those in the control group (P<0.05). Conclusion: W ith the same treatments, scalp acupuncture combined with AET is superior to the conventional scalp acupuncture method in reducing lower-limb muscle tone, improving standing balance ability, and walking stability in children with spastic CP.
6.Mutual prediction of peer relationship and internalizing problem of preschool children

Objective:
To investigate the mutual prediction relationship between peer relationship and internalizing problem.
Methods:
One-year follow up survey was conducted with a sample of 220 preschool children from 4 kindergartens in Shandong province. The quality of peer relationship and degree of internalizing problem were evaluated with the Peer Relationship Scale and the Child Behavior Checklist Cross-lagged panel analysis was used for mutual prediction among variables.
Results:
The quality of preschool children’s peer relationship showed an increasing trend with grade(F=6.40, 4.81, P<0.01), while the degree of internalizing problem showed a downward trend(F=7.65, 5.46, P<0.01). The predictive effect of pre-test peer relationship and internalizing problem on post-test corresponding behaviors were all statistically significant (β=0.56, 0.49, P<0.01). The predictive effect of pre-test peer relationship on post-test internalizing problem was statistically significant(β=-0.19, P<0.05).
Conclusion
Both peer relationship and internalizing problem has a certain stability across time, and early peer relationship and internalizing problem could predict later corresponding behaviors. Early peer relationship can predict later internalizing problem, while early internalizing problem cannot predict later peer relationship.
7.Laparoscopic cystogastrostomy with posterior approach for pancreatic pseudocyst drainage.

OBJECTIVE:
To determine the clinical value of laparoscopic cystogastrostomy in the treatment of pancreatic pseudocyst.
METHODS:
Twenty-one patients with pancreatic pseudocyst received total laparoscopic cystogastrostomy. The data on intra-operative bleeding, operative time, post-operative time to get out of bed, time of first flatus/bowel motion, complication and duration of hospital stay were observed and analyzed retrospective1y.
RESULTS:
Twenty-one patients were successfully carried out the laparoscopic surgery. The average operation time was 90(62-120) min. The blood loss was less than 100 mL in all patients. The average time of hospital stay was 8 d. After 12-18 month follow-up, all patients recovered smoothly without any complication.
CONCLUSION
Total laparoscopic cystogastrostomy with the posterior approach is a feasible, safe and minimal invasive procedure for pancreatic pseudocyst, which can be recommended to the clinical application.

8.Meta-analysis of total thyroidectomy for multinodular goiter.

OBJECTIVE:
To systematically evaluate the efficiency and safety of total thyroidetomy (including near-total tyhroidectomy) versus subtotal thyroidectomy for multinodular goiter.
METHODS:
The literatures were searched from Cochrane Library, PubMed, Embase, Chinese Biological Medical Datebase, Chinese National Knowledge Infrastructure, and Chinese Science and Technology Journal Full-text Database as of November 2013. We included all randomizad controlled trials on total (including near-total) versus subtotal thyroidectomy in the treatment of multinodular goiter. The collecting of data and quality assessment were respectively completed by 2 researchers. RevMan5.1 software was used for Meta-analysis.
RESULTS:
We collected 7 literatures conforming to the standard, incuding 2 192 patients. The Metaanalysis outcomes showed that total thyroidectomy was associated with lower nodule recurrence rate (OR=0.13, 95% CI: 0.07-0.22, P<0.001) and higher in transient hypoparathyroidism rate (OR=2.33, 95% CI: 1.72-3.17, P<0.001). However, no statistical difference was seen comparing total and subtotal thyroidectomy in permanent recurrent laryngeal nerve paralysis rate (OR= 0.81, 95% CI: 0.24-2.74, P=0.74) and permanent hypoparathyroidism rate (OR=2.94, 95% CI: 0.48- 18.11, P=0.24).
CONCLUSION
Nodule recurrence rate of total thyroidectomy for multinodular goiter is lower than subtotal thyroidectomy and does not increase permanent complications.

9.Establishment and application of networked full-coverage multi-disciplinary team management mode under hospital administrative guidance

Multi-disciplinary team(MDT)mode is regarded as the key to standardized diagnosis and treatment of malignant tumors. The model, however, encounters such roadblocks in the current form of MDT organization, as costly clinical resources and time consumption, low efficiency, poor management of participating experts in MDT, and lack of enforceability of the therapeutic decisions made. This paper summarized the practical MDT experiences of the Affiliated Cancer Hospital of Shandong First Medical University. It introduced the construction of an intranet-based MDT system covering a large proportion of newly diagnosed malignant tumor patients, and the practices and achievements of such MDT management system under hospital administrative guidance. The authors proposed to use reporting ratio as the main assessment indicator in promoting MDT, and that to define the performance, responsibilities and rights in MDT practice. These measures aim at to upgrading individual behaviors of doctors to organizational behaviors of hospitals, and providing cancer patients with more standardized, comprehensive and personalized diagnosis and treatment decisions.
10.The physiology of plant seed aging: a review.

Seed quality plays an important role in the agricultural and animal husbandry production, the effective utilization of genetic resources, the conservation of biodiversity and the restoration and reconstruction of plant communities. Seed aging is a common physiological phenomenon during storage. It is a natural irreversible process that occurs and develops along with the extension of seed storage time. It is not only related to the growth, yield and quality of seed and seedling establishment, but also has an important effect on the conservation, utilization and development of plant germplasm resources. The physiological mechanisms of seed aging are complex and diverse. Most studies focus on conventional physiological characterization, while systematic and comprehensive in-depth studies are lacking. Here we review the recent advances in understanding the physiology of seed aging process, including the methods of seed aging, the effect of aging on seed germination, and the physiological and molecular mechanisms of seed aging. The change of multiple physiological parameters, including seed vigor, electrical conductivity, malondialdehyde content and storage material in the seed, antioxidant enzyme activity and mitochondrial structure, were summarized. Moreover, insights into the mechanism of seed aging from the aspects of transcriptome, proteome and aging related gene function were summarized. This study may facilitate the research of seed biology and the conservation and utilization of germplasm resources.
