3.Full-mouth rehabilitation of a patient with loss of posterior support and collapsed occlusion utilizing dental CAD-CAM system

Without proper treatment on the multiple tooth missing area, the lack of posterior support and the supra-eruption of the teeth cause many severe complications of occlusion, vertical dimension and masticatory function. This report is a case of full-mouth rehabilitation of a patient with loss of posterior support and collapsed occlusion due to missing teeth area left untreated for a long time. The patient who is 68-year old male patient had some teeth fallen out while removing his old maxillary denture and was complaining about pain in the region of anterior teeth due to traumatic contact. The vertical dimension was corrected by 4 mm from the top cervical point of the canine through various evaluations and the edentulous area was treated with the implant fixed prostheses through computer guided implant surgery based on the diagnosis and treatment plan for definitive prostheses supported by computed tomography (CT) data analysis and CAD-CAM (Computer-aided design/computer-aided manufacturing) technique. After full mouth rehabilitation, the patient was very satisfied with remarkable improvements in mastication, function, and aesthetics.
4.Comparative Proteomic Study of Spinal Cord after Applying Pulsed Radiofrequency to Dorsal Root Ganglion in the Rat.

BACKGROUND: Pulsed radiofrequency treatment (pulsed RF) has recently been described as a non-neurodestructive or minimally neurodestructive alternative to radiofrequency heat lesions. However, there are few data to evaluate the mechanism of this therapy. The aim of this study is to explore the underlying mechanism of pulsed RF on spinal nervous system using comparative proteomic analysis. METHODS: L5 dorsal root ganglion was exposed to pulsed RF and sham treatment. Protein extracts from the spinal cord of both groups were analyzed using comparative proteomics to identify differentially expressed proteins. To validate the differential expression of proteins, we utilized western blotting and immunohistochemistry. RESULTS: The spinal cord taken from pulsed RF treated rats was found to underexpress 4-aminobutyrate aminotransferase, mitochondrial [Precursor]. Immunostaining density of gamma-aminobutyric acid measured from the spinal cord taken from pulsed RF treated rats was significantly increased compared with sham treated rats. CONCLUSIONS: We demonstrated that the decreased expression of 4-aminobutyrate aminotransferase, mitochondrial [Precursor] and increased expression of gamma-aminobutyric acid in the spinal cord after exposure of the lumbar dorsal root ganglion to pulsed RF. That might be the possible underlying mechanism of the analgesic effect of pulsed RF.

Circumscribed palmar or plantar hypokeratosis is a rare dermatosis, and it consists of asymptomatic erythematous, well-circumscribed and depressed patch with scaly borders on the palm or sole. It usually affects middle-aged or elderly women. Histopathologically, it has a sharp stair between the normal and involved skin, and a demarcated area with an abrupt decrease in the horny layer thickness. We report herein a case of circumscribed palmar hypokeratosis that was successfully treated with cryotherapy.

7.Estimate of Radiation Doses in MDCT Using Patient Weight.

The purpose of this study provides measurements of radiation dose from MDCT of head, chest, abdomen and pelvic examinations. A series of dose quantities that are measured of patient weight to compare the dose received during MDCT examinations. Data collected included: weight together with CT dose descriptors, volume CT dose index (CTDIvol) and dose length product (DLP). The effective dose was also estimated and served as collective dose estimation data. Data from 1,774 adult patients attending for a CT examination of the head (n=520) or chest (n=531) or abdomen (n=724) was obtained from spiral CT units using a same CT protocol. Mean values of CTDIvol was a range of 48.6 mGy for head and 6.9, 10.5 mGy for chest, abdomen examinations, respectively. And mean values of DLP was range of 1,604 mGy.cm for head, 250 mGy.cm for chest, 575 mGy.cm for abdomen examinations, respectively. Mean effective dose values for head, chest, abdominal CT were 3.6, 4.2, and 8.6 mSv, respectively. The degree of CTDIvol and DLP was a positive correlation with weight. And there was a positive correlation for weight versus CTDIvol (r2=0.62), DLP (r2=0.694) in chest. And head was also positive correlation with weight versus CTDIvol (r2=0.691), DLP (r2=0.741). We conclude that CTDIvol and DLP is an important determinant of weight within the CT examinations. The results for this study suggest that CT protocol should be tailored according to patient weight.

8.Laryngeal Papillary Cystadenoma: A Case Report

Papillary cystadenoma is a rare, benign salivary gland neoplasm containing cystic cavities with intraluminal papillary projections. In the head and neck area, it occurs mainly in major and intraoral minor salivary glands, but rarely in the larynx. We report a case of a 67-year-old female with a chief complaint of hoarse voice diagnosed as laryngeal papillary cystadenoma. This paper emphasizes the need to consider papillary cystadenoma as one of differential diagnosis when benign looking lesions are observed in the larynx, where it uncommonly occurs.
9.Changes in Incidences of Chronic and Traumatic Diseases before and after Registration as Visually Disabled

Purpose:
To analyze changes in the incidences of chronic and traumatic diseases before and after registration as visually disabled.
Methods:
The incidence and risk of chronic and traumatic diseases were compared between patients registered as visually disabled from 2005 to 2013 and a control group, selected through 1:1 propensity score matching for age, sex, premium quantile, and residential area distribution. Data from the National Health Insurance Service were used and the observation period was set from 2 years before to 5 years after the time of registration as visually disabled. The incidences (%) of chronic and traumatic diseases at the point of interest were determined as the mean values according to year of registration, and the risk was measured by conditional logistic regression analysis.
Results:
131,434 visually disabled patients and the same number of non-visually impaired controls were included. The incidences of chronic disease and fall-related injury were higher in the visually disabled group before registration, but gradually decreased and became similar to the control group after registration. In addition, the risk of developing chronic diseases was higher in the visually disabled group until 2 years after registration, while the risk of fall-related injury remained consistently higher in the visually disabled group during the observation period. The incidence and risk of hip fracture in the visually disabled group increased gradually from approximately 1 year before registration and remained consistently higher than in the control group.
Conclusions
People with visual disability have higher incidences of chronic diseases than do non-visually impaired people and are at greater risk of traumatic injuries, such as hip fractures or falls. There is a need to establish community infrastructure and expand welfare services for the prevention and early treatment of comorbidities, both for patients with registered visual disabilities and for patients who have not yet registered.
10.Estimated Incidence and Projections of Treatment Cost for Vision-threatening Diabetic Retinopathy in Korea

Purpose:
To analyze the incidence and treatment costs associated with vision-threatening diabetic retinopathy (VTDR) in type 2 diabetes and to predict future VTDR populations and treatment expenditures.
Methods:
Using the data from the National Health Insurance Service from 2006 to 2017, we analyzed VTDR treatment costs by year, sex, and age. Based on the results and changes in future population distributions, we estimated the future number and cost of treatments for VTDR.
Results:
The number of treatments increased by 2.5-fold from 37,634 in 2006 to 96,214 in 2017, and treatment costs increased 3.4-fold from $13,528,587 in 2006 to $45,643,561 in 2017. When analyzed by year, age, and sex, all showed an increasing trend. Future number of treatments was estimated to increase from approximately 96,000 in 2017 to 220,000 by 2030 and treatment costs were projected to increase by 129% from about $45,643,561 in 2017 to about $1,043,055,262 by 2030.
Conclusions
As the incidence of VTDR rises due to an aging population and the frequency of intravitreal injections increases due to insurance reimbursement, future treatment expenditures for VTDR are expected to increase as well. Therefore, appropriate policies must be put in place now to secure medical and financial resources to manage VTDR and reduce medical expenditures in the future.
