1.An analysis of 445 lawsuits of medical damage liability disputes in Beijing

Objective To identify departments involved, and the appraisal and compensation of such disputes in Beijing, for preventive recommendations. Methods A retrospective descriptive statistical analysis was carried out to retrieve and extract keywords from the first-instance judgments totaling 445 cases of medical damage liability disputes from 2014 to 2016 in Beijing. This analysis covered the age, disease, departments involved, liability judgment among other information. These cases were published at the website of China judgment documents. Results Patients with musculoskeletal connective tissue diseases, circulatory system disease and tumor accounted for the most lawsuits. The departments involved with more disputes were the department of gynecology and obstetrics (80/445, 18. 0%), orthopedics (69/445, 15. 5%), and emergency department(45/445, 10. 1%). 83. 8 percent of the disputes were found with medical quality problems;and 84. 3% were lost with compensations. Conclusions It is imperative to elevate the diagnosis and treatment level on musculoskeletal connective tissue diseases, circulatory system disease and tumors, and take precautions against disputes at such key departments as gynecology and obstetrics department, orthopedics department, and emergency department. Other precautions include normalized medical record writing, medical risk disclosure, higher medical quality and patient safety.
2.Study on the influence of Beijing Medical Reform on the hospitalization expenses of medical and surgical patients in a hospital

Objective:To analyze the impact of Beijing Medical Reform on inpatient costs of medical and surgical patients in a hospital, and to provide reference for the hospital to explore the path of patient cost control.Methods:Based on the data of medical and surgical inpatients′ expenses from June 2016 to December 2019 in a hospital in Beijing, breakpoint regression method was used to analyze the impact of the two reforms(namely the comprehensive reform of separation of medicine and pharmacy and the comprehensive reform of medical consumption linkage)on the level of medical and surgical inpatients′ expenses and the cost structure of drugs and consumables.Results:The two reforms controlled the rapid rise of hospitalization expenses and reduced the proportion of drugs and medical consumption. The effect of the reforms on the total cost control of internal medical patients was better than that of surgical patients.For internal medical patients, the reform of separation of medicine and pharmacy played a greater role in the proportion of medicine; For surgical patients, the two reforms had effectively reduced the proportion of consumables, and the proportion of drugs had gradually decreased.Conclusions:The two reforms have effectively controlled the growth trend of hospitalization expenses, and changed the cost structure of drugs and consumables.
3.Study on the perceived differences of medical staff in the hierarchical diagnosis and treatment effectiveness of telemedicine from the supply and demand perspective

Objective:To explore the perceived differences of medical staff in the hierarchical diagnosis and treatment effectiveness of telemedicine from the perspective of supply and demand, as well as the moderating effects in their respective regions, in order to provide reference for the integrated development of telemedicine services and hierarchical diagnosis and treatment.Methods:From June to September 2023, a survey was conducted on the current status and effectiveness of telemedicine services among medical staff from 245 hospitals in 27 provinces. Using non participants as the reference group, an ordered probit model was used to explore the differences in perception of the hierarchical diagnosis and treatment effectiveness of telemedicine services among medical staff who only served as suppliers, only as demanders, and simultaneously as suppliers/demanders; Using the region as a moderating variable, explore the perceived differences among medical staff in different regions.Results:A total of 997 valid questionnaires were obtained. Medical staff who only served as the demanders ( β=0.197, P<0.05) and simultaneously served as suppliers/demanders ( β=0.189, P<0.01) thought that the implementation of telemedicine could significantly improve the effectiveness of hierarchical diagnosis and treatment, while those who only served as the suppliers thought that the implementation of telemedicine did not significantly improve the effectiveness of hierarchical diagnosis and treatment ( β=-0.028, P>0.05). With the eastern region, medical staff in the central and western regions believed that providing telemedicine services simultaneously as suppliers/demanders could better improve the effectiveness of hierarchical diagnosis and treatment ( β=-0.324, P<0.05). Conclusions:The role and location of medical staff involved in telemedicine services could lead to differences in their perception of hierarchical diagnosis and treatment effectiveness.
