1.Effect of Cluster Needling of Scalp Acupuncture Combined with Treadmill Training on Axonal Regeneration and Expression of S-100 Protein in Rats with Sciatic Nerve Injury

Objective To explore the effect of cluster needling of scalp acupuncture combined with treadmill training on axonal regener-ation and expression of S-100 protein in rats with sciatic nerve injury. Methods Ninety male Sprague-Dawley rats were randomly divided in-to blank group, sham group, model group, treadmill group and cluster needling of scalp point combined with treadmill group (combination group), each group was further divided into 7, 14 and 21 days subgroups, with 6 rats in each subgroup. The sciatic nerve of rats was clamped in the model group, the treadmill group and the combination group. The sham group was subjected to the same surgical procedure with sciat-ic nerve exposure but without crush injury. Three days after modeling, the treadmill group was treated with treadmill training, and the combi-nation group was treated with cluster needling of scalp acupuncture and treadmill training, while the other groups were grabbed regularly, with no other intervention. The sciatic functional index (SFI) was tested, the condition of axon growth was observed with HE staining, and the expression of S-100 protein was examined with immunohistochemistry at each time point. Results At each time point, SFI was higher, the axon grew better, and the S-100 protein was higher expressed in the treadmill group and the combination group than in the model group (P<0.05), especially in the combination group (P<0.05). Conclusion Cluster needling of scalp acupuncture combined with treadmill training can accelerate the regeneration of axons and increase the expression of S-100 protein and improve sciatic function after sciatic nerve injury.
2.Effect of clinical study combined with short term intensive simulation on training of gynecolo-gist hysteroscopy technology

Objective To evaluate effect of clinical study combined with short-term intensive simulation on training of hysteroscopy technology. Methods Trainees receiving training of hystero-scopy technology from the January to December 2012 in Gynecology Minimally Invasive Center, Bei-jing Gynecology and Obstetrics Hospital Affiliated to Capital Medical University were enrolled and di-vided into two groups depending on the different training modes. Twenty-four trainees in group1 par-tic-ipated in three-month hysteroscopic clinical study program. Twenty-two trainees in group2 partici-pated in three-month clinical study program including one-week short term intensive simulation. After the training, the effect was evaluated by the rating scale and the Rank sum test was used for statisti-cal analysis. P<0.05 signifies statistically significant difference. Results The operation time(P=0.03), forward planning (P=0.02), instrument handing (P=0.00) and knowledge of specific procedure (P=0.04) were improved significantly in group 2 compared with those in group 1. Conclusions The training mode of clinical study combined with the short-term intensive simulation can improve hys-teroscopy technology effectively and should be widely applied.
3.Study of ultrastructural features of myocytes in uterine junctional zone

Objective To study the ultrastructural features of myocytes in uterine junctional zone (JZ).Methods From August 2010 to August 2013,there were 16 pre-menopause patients who suffered from cervical neoplasm to be performed hysterectomy.Samples of JZ and outer myometrium (OM) of hysterectomy specimens were collected.There were 8 specimens from the proliferative-phase and 8 specimens from the secretory-phase of endometrium.Ultrastructural features of JZ and OM were examined by using transmission electron microscopy and the related indices of myocytes were compared by using Student's t test.Results At JZ,there were more cytoplasmic process in the myocytes.The myocytes of JZ exhibited significant difference compared with those of OM.Firstly,the contractile structural components,such as the dense patches,dense bodies and the myofilaments were less abundant.In contrast,the perinuclear cell organelles were more distinct.The mitochondria,endoplasmic reticulum and Golgi apparatus were more prominent,denoting active protein synthesis.Secondly,the mean diameter of cell and nuclei demonstrated cyclic change.In proliferative phase of endometrium,the cell diameters of JZ and OM were (4.70_±0.52) and (4.69± 1.20) μm,respectively,which there were no significant difference(P=0.987).While in secretory phase,the cell diameters of JZ and OM were (3.75±0.36) and (4.92±0.51) μm,which there were significant difference (P=0.006).In proliferative phase,the nuclei diameters of JZ and OM were (3.24±0.41) and (2.90±0.62) μm,and in secretory phase,the nuclei diameters of JZ and OM were (2.44±0.27) and (2.92±0.44) μm.There were no significantly different in both phases (P=0.374,P=0.097).The diameters of cell and nuclei had cyclical changes (P < 0.05).However,the cyclical changes were absent in OM (P> 0.05).Thirdly,the myofilaments/cytoplasm ratio of JZ in proliferative and secretory phases were 0.27±0.04 and 0.34±0.03,which were significantly less than those of OM in respective phases (0.49±0.03and 0.54±0.03;P=0.000,P=0.000).The myofilaments/cytoplasm ratio exhibited cyclical changes in JZ (P=0.029),but in OM,the cyclical changes were absent (P=0.083).Conclusions Compared with OM,ultrastructures associated with synthetic organelles are prominent,whereas the contractile organelles are reduced.And there are the cyclical changes in ultrastructural characteristics.The ultrastructural features of JZ are the basis of its physiology.
4.Diagnose and treatment of post-cesarean section scar diverticulum by hysteroscopy combined ultrasonography and laparoscopy

Objective To investigate the application of hysteroscopy combined ultrasonography and laparoscopy in the diagnose and treatment of post-cesarean section scar diverticulum (PCSD). Methods From March 2011 to February 2013, 27 patients with PCSD were diagnosed and treated in Beijing Obstetrics and Gynecology Hospital. All patients were diagnosed by hysteroscopy and ultrasonography. Treatment protocols were decided by the thickness of the diverticulum. The clinical data of all patients were analysed fully. Results (1)All patients were diagnosed by hysteroscopy combined ultrasound, of which only 17 cases were diagnosed by preoperative ultrasound, the coincidence rate was 63%(17/27). (2)The thickness of diverticulum was measured by hysteroscopy combined ultrasound. Thickness of less than 3 mm in 19 cases, were treated by hysteroscopy combined with laparoscopy diverticulectomy repair;no less than 3 mm in 8 cases, were used hysteroscopy diverticulum incision. (3)The effective rate was 7/8 after hysteroscopy, and which was 16/19 after laparoscopy combined with hysteroscopy surgery. There was no difference in the treatment effect (P=0.663). The ineffective rate was 1/8 after hysteroscopy, and which was 3/19 after laparoscopy combined with hysteroscopy surgery. The size of the PCSD was smaller and the thickness of diverticulum was thicker than preoperation in the later one, which has no difference in the former one. Conclusions (1)Hysteroscopy combined ultrasound is an accurate method for the diagnose of PCSD. (2)According to the thickness of the diverticulum operation method is choosed, which is beneficial to reduce the trauma, and relieve symptoms. (3)Diverticulum poor positioning and incomplete resection are the main causes of postoperative recurrence of the diverticulum.
5.Psychosocial intervention for improving health in patients with bariatric surgery:a Meta-analysis

Objective To systematically evaluate the intervention effect of social psychological intervention on the health status of patients with bariatric surgery(BS).Methods 8 databases,including PubMed,PsycInfo,and Embase and Clinic Trials,were retrieved to recruit randomized controlled trials with computer from database establishing time to Sep.2022.Independent quality evaluation was conducted by 2 researchers,and Meta-analysis was performed by the RevMan5.3 software.Results Totally 23 RCTs were included.The meta-analysis showed that psychosocial interventions in BS patients improve emotional eating and binge eating behavior[SMD=-0.44,95%CI(-0.78,-0.09),P=0.010;MD=-5.88,95%CI(-8.65,-3.11),P<0.001],promote better quality of life[SMD=0.30,95%CI(0.02,0.59),P=0.040]and physical mobility,alleviates anxiety and depression[SMD=-0.37,95%CI(-0.67,-0.08),P=0.010;SMD=-0.59,95%CI(-0.84,-0.33),P<0.001].However,the effect on improving eating disorders[MD=-0.01,95%CI(-0.19,0.18),P=0.950]is not significant,and subgroup analysis results of different intervention measures and follow-up times showed that there was no statistically significant difference in weight changes between the social psychological intervention group and the control group.Conclusion Psychosocial intervention can effectively improve the mental state and eating behavior of the bariatric surgery patients,improve the quality of life and increase physical activity of patients.However,the effect of intervention on eating disorder and weight change is still unclear.More high-quality clinical studies need to be carried out for further verification.
6.Impacts of andrographolide on sciatic nerve function damage in diabetic peripheral neuropathy rats by regulating HMGB1/RAGE signaling pathway

OBJECTIVE To investigate the impacts of andrographolide on sciatic nerve function injury in diabetic peripheral neuropathy (DPN) rats by regulating high-mobility group protein box 1 (HMGB1)/receptor for advanced glycation end products (RAGE) signal pathway. METHODS A total of 84 rats were randomly divided into the control group (normal saline), DPN group (normal saline), low-dose andrographolide group (0.833 mg/kg), high-dose andrographolide group (3.332 mg/kg), lipoic acid group (positive control, 0.1 g/kg), recombinant rat HMGB1 protein (rHMGB1) group (8 μg/kg), and high-dose andrographolide+ rHMGB1 group, with 12 rats in each group. All rats except those in the control group were fed with high glucose and high fat diet combined with intraperitoneal injections of streptozotocin to establish the DPN rat model. After 24 hours of successful modeling, medication was administered daily for 8 weeks. The changes in fasting blood glucose, mechanical pain threshold, heat pain threshold and sciatic nerve conduction velocity were detected. Pathological changes in the sciatic nerve of rats and the activity of superoxide dismutase (SOD) and the content of malondialdehyde (MDA) in the sciatic nerve of rats were also detected. Besides, the expressions of HMGB1, RAGE proteins and phosphorylation level of nuclear factor κB p65(NF-κB p65) protein in rat sciatic nerves were found. RESULTS Compared with the control group, the pathological damage of the sciatic nerve of rats in the DPN group was strengthened, the fasting blood glucose, heat pain threshold, MDA content and the 诊治。E-mail:dqiaur@163.com expressions of HMGB1, RAGE proteins and phosphorylation level of NF-κB p65 protein were increased (P<0.05), while the mechanical pain threshold, sensory nerve conduction velocity, motor nerve conduction velocity, and SOD activity were decreased/slowed down (P<0.05). Compared with the DPN group, the above indexes were significantly potentiated in the andrographolide low- and high-dose groups and lipoic acid group (P<0.05), and the corresponding trends in the rHMGB1 group were opposite to those in the above three administration groups (P<0.05). Moreover, rHMGB1 attenuated the hypoglycemic effect of high-dose andrographolide on blood glucose and the improvement of oxidative stress injury in the sciatic nerve of DPN rats (P<0.05). CONCLUSIONS Andrographolide may reduce blood glucose by inhibiting the HMGB1/RAGE pathway and oxidative stress, thus ameliorating sciatic nerve injury in DPN rats.
7.Study on the Antioxidation and Anti-hypoxia Effects of Yuanshen Granule in Normal Mice

OBJECTIVE:To study the anti-oxidation and an ti-hypoxia effect of Yuanshen granule on normal mice. METHODS : Totally 800 mice were randomly divided into blank control group ,Yuanshen granule low-dose ,middle-dose and high-dose groups (10,20,40 g/kg)and Hongjingtian oral liquid group (positive control ,2 mL/kg),with 160 mice in each group. Administration groups were given relevant medicine intragastrically ,once a day ,for consecutive 30 d. Blank control group was given constant volume of normal saline by the same method. At 1st,3rd,7th,14th and 30th day after administration ,8 mice in each group at each time point were randomly selected for anoxia tolerance test under normobaric pressure and decompression ;the contents of MDA and SOD in myocardium and cerebral tissue were measured ;the hemolysis effect of red blood cell induced by H 2O2 was investigated. RESULTS :Compared with blank control group ,the survival time of anoxia tolerance under normobaric pressure and under decompression (after 7 days of administration )were prolonged significantly in Yuanshen granule groups (P<0.05). The contents of MDA in myocardium and cerebral tissue (after 3 or 7 days of administration ;after 3,7 or 14 days of administration ) were significantly decreased (P<0.05),and the contents of SOD (after 7,14 or 30 days of administration ;after 7 days of administration)were increased significantly (P<0.05);the erythrocyte hemolysis rate (after 3 or 7 days of administration )was significantly decreased (P<0.05). At some time points ,the improvement effect of the above indexes in the Yuanshen granule middle-dose and high-dose groups was significantly better than that of Hongjingtian oral liquid group (P<0.05). CONCLUSIONS : Yuanshen granule can effectively prolong the hypoxia tolerance survival time of normal mice ,it has anti-oxidant and anti-hypoxia effects on normal mice ,and the effect of Yuanshen granule middle-dose and high-dose groups is better than that of Hongjingtian oral liquid group.
8.Content changes of chemical constituents and comparison of the anti-ulcerative colitis effect of processed products of Terminalia chebula at different temperatures

OBJECTIVE To study the content changes of ch emical constituents of processed products of Terminalia chebula at different temperatures ,and to compare its anti-ulcerative colitis effect. METHODS Processed products of T. chebula at different temperatures(160,180,200,220,240,260,280,300 ℃)were prepared by sand scalding technology. HPLC method was adopted to determine the contents of gallic acid ,chebulagic acid ,chebulinic acid and ellagic acid in crude drug and processed products of T. chebula at different temperatures. The mice were divided into blank group ,model group ,Mesalazin enteric-coated tablets group (positive control ,0.4 g/kg),crude drug and processed products groups of T. chebula at different temperatures (1.3 g/kg),with 10 mice in each group. Except for blank group ,other groups were given 6% acetic acid 0.1 mL via anus to induce ulcerative colitis model. After modeling ,blank group and model group were given water intragastrically ,and other groups were given relevant drug intragastrically ,20 mL/kg,once a day ,for consecutive 7 days. The general physical signs of mice in each group were observed and the body weight was recorded. The colorectal length and index ,serum levels of related inflammation indexes [superoxide dismutase (SOD),malondialdehyde (MDA),interleukin-10 (IL-10),IL-1 β ,tumor necrosis factor α (TNF-α)] were detected. The pathomorphological changes of colon and rectum were observed ,and the comprehensive score of pharmacodynamics was performed. RESULTS With the increase of processing temperature ,the contents of chebulagic acid and chebulinic acid decreased gradually ,the content of gallic acid increased first and then decreased ,and the content of ellagic acid increased. Compared with model group ,the general physical signs ,body weight ,colorectal length ,colorectal index and related inflammation indexes were all improved significantly in crude drug and processed products groups of T. chebula at different temperatures(P<0.05 or P<0.01). The glandular recess structure of colorectal tissue was repaired ,the infiltration of inflammatory cells was reduced ,and the comprehensive score of efficacy of processed products prepared at 260 ℃ was the highest. CONCLUSIONS The contents of chemical components in T. chebula processed at different temperatures change significantly and their anti-ulcerative colitis effects are different. The processed products of T. chebula prepared at 260 ℃ show the best anti-ulcerative colitis effect.
9.Optimization of the extraction and separation process of chebulagic acid and chebulinic acid

OBJECTIVE To optimize the extraction and separation process of chebulagic acid and chebulinic acid from Terminalia chebula. METHODS Based on single factor experiment, with particle size, liquid-solid ratio, extraction time and extraction times as factors, using the contents of chebulagic acid and chebulinic acid as indexes, orthogonal experiment was designed to optimize the extraction process of chebulagic acid and chebulinic acid. Taking sample concentration, elution solvent and the ratio of eighteen-group bonded silicone reverse phase (ODS) to the amount of raw medicine as factors, the separation processes of chebulagic acid and chebulinic acid were optimized. RESULTS The optimal extraction process of chebulagic acid and chebulinic acid included ethanol volume fraction of 70%, ultrasonic extraction, particle size of 120 mesh, liquid-solid ratio of 25∶1 (mL/g), extraction time of 20 min, and extracting for 2 times. After 3 experiments, the average comprehensive score was 99.33 (RSD= 0.68%, n=3), and the average contents of chebulagic acid and chebulinic acid were 107.05 and 58.32 mg/g, respectively. The optimal separation process of the two components included the concentration of sample loading solution was 0.5 g/mL (1 mL was equivalent to 0.5 g of medicinal materials), the ratio of ODS to the amount of raw medicine was 10∶1.5 (g/g), methanol-water (1∶4, V/V) eluted chebulagic acid, methanol-water (3∶7, V/V) eluted chebulinic acid. After 3 experiments, the average total yields of the two components were 53.33%, 39.23%. After recrystallization, the purity of both components was 100%. CONCLUSIONS Established extraction and separation process of chebulagic acid and chebulinic acid is simple and feasible.
