1.Study on the application of the Chinese version of SF-36 scales and selection of interceptive cents for its grade range.

OBJECTIVETo study the possibility of measuring quality of life by Medical Outcomes Study 36-Item Short-Form Health Survey scale and to subdivide grade range of Medical Outcomes Study 36-Item Short-From Health Survey (SF-36) total cents through a Quality of Life questionnaire among the elderly Chinese.
METHODSThe quality of life of the 167 elderly Chinese in Suzhou was measured simultaneously by SF-36 and the Quality of Life questionnaire developed by Epidemiology Group of Geriatric Medicine Committee of China for the elderly. Validity and reliability were analyzed and subdivided the grade range of SF-36 total scores by Quality of Life questionnaire for the Chinese elderly.
RESULTSEight common factors from factorial analysis were in accordance with their theoretical structure, and the cumulative contribution rates of the Quality of Life questionnaire for the elderly and SF-36 were 74.244% and 72.081%. The split-half reliability of the Quality of Life questionnaire for the elderly was 0.6676. The split-half reliability of SF-36 was 0.9384. In SF-36, the Cronbach's alpha coefficient of internal consistency reliability ranged from 0.81 to 0.89, which was satisfactory for group comparison except 0.63 for the social functioning and 0.42 for mental health scale and 0.69 for vitality scale. The Quality of Life questionnaire for old people seemed to have good validity and reliability but the SF-36 was better. The cent of the furthest truncation between the good quality of life and the medium one in the SF-36 was 117 with a Kappa value of 0.58.
CONCLUSIONThe SF-36 scale could be used for measuring and evaluating the quality of life for the Chinese elderly. The cent of the furthest truncation could provide reference to judge the level of the quality of life of the elderly.

2.Class Ⅲ surgical patients facilitated by accelerated osteogenic orthodontic treatment

Objective To evaluate the treatment time and the anterior and posterior teeth movement pattern as closing extraction space for the Class Ⅲ surgical patients facilitated by accelerated osteogenic orthodontic treatment.Methods There were 10 skeletal Class Ⅲ patients in accelerated osteogenic orthodontic group (AOO) and 10 patients in control group.Upper first premolars were extracted in all patients.After leveling and alignment(T2),corticotomy was performed in the area of maxillary anterior teeth to accelerate space closing.Study models of upper dentition were taken before orthodontic treatment(T1) and after space closing(T3).All the casts were laser scanned,and the distances of the movement of incisors and molars were digitally measured.The distances of tooth movement in two groups were recorded and analyzed.Results The alignment time between two groups was not statistically significant.The treatment time in AOO group from T2 to T3 was less than that in the control group (less than 9.1 ± 4.1 months).The treatment time in AOO group from T1 to T3 was less than that in the control group (less than 6.3 ± 4.8 months),and the differences were significant(P < 0.01).Average distances of upper incisor movement (D1) in AOO group and control group were (2.89 1.48) and (3.10 ±0.95) mm,respectively.Average distances of upper first molar movement(D2) in AOO group and control group were (2.17 ± 1.13) and (2.45 ± 1.04) mm,respectively.No statistically significant difference was found between the two groups (P > 0.05).Conclusions Accelerated osteogenic orthodontic treatment could accelerate space closing in Class Ⅲ surgical patients and shorten preoperative orthodontic time.There were no influence on the movement pattern of anterior aud posterior teeth during pre-surgical orthodontic treatment.
3.Clinical research of improved plastic and reconstruction of anus in situ.

OBJECTIVETo explore the effect of the improved plastic and reconstruction of the anus in situ.
METHODSImproved plastic and reconstruction of anus in situ was performed in 38 cases of low rectal cancers operated while Miles radical operation. Improvement includes: (1) The internal sphincter was rebuilt with 4 layers of muscle layer of the endmost of colon. (2) The last of gracilis was divided into 2 parts to reconstruct the superficial part and deep part of external sphincter muscle. (3) The rectum cape improvement is to firmly stitch the levator ani outside the external sphincter muscle in front of the colon. (4) The rectum valve is improved into three artificial rectum valves.
RESULTSThe form and function and their long term survival rate were good, the rate of superior anus function was 94.73%.
CONCLUSIONIt mains the results of improved plastic and reconstruction of anus in situ is near that of normal persons.

4.Study of expression of CD138 and heparinase in hepatocellular carcinoma by tissue microarray.

OBJECTIVETo study the expression of CD138 and heparinase in hepatocellular carcinoma (HCC) and its relationship with tumor development, progression, metastasis and recurrence.
METHODSTissue microarray and immunohistochemical study (EnVision method) for CD138 and heparinase was performed on tissue microarray which consisted of 197 cases of HCC, including adjacent non-neoplastic liver tissues, and 66 cases of HCC metastases.
RESULTSThe rates of CD138 expression in HCC and adjacent non-neoplastic liver tissues were 48.7% (96/197) and 65.0% (128/197, P < 0.05) respectively. In early-stage and late-stage tumors, the expression rates were 61.7% (29/47) and 44.7% (67/150, P < 0.05) respectively. The rate in patients with metastasis was 33.3% (22/66), as compared with 53.6% (45/84, P < 0.05) in patients without metastasis. In patients with tumor recurrence occurring within or after 1 post-operative year, the expression rates were 23.3% (7/30) and 61.1% (11/18, P < 0.05) respectively. On the other hand, the rates of expression of heparinase in HCC and adjacent non-neoplastic liver tissues were 35.5% (70/197) and 12.7% (25/197, P < 0.05) respectively. In early-stage and late-stage tumors, the expression rates were 29.8% (14/47) and 37.3% (56/150, P > 0.05) respectively. The rate in patients with metastasis was 48.5% (32/66), as compared with 28.6% (24/84, P < 0.05) in patients without metastasis. In patients with tumor recurrence occurring within or after 1 post-operative year, the expression rates were 50.0% (15/30) and 44.4% (8/18, P > 0.05) respectively. In the 66 cases of metastatic HCC studied, the expression rate of CD138 was lower in the heparinase-positive subgroup (P < 0.05).
CONCLUSIONSLoss of CD138 expression is related to HCC development, progression, metastasis and recurrence. Overexpression of heparinase, when coupled with loss of CD138 expression, may take part in tumor metastasis of HCC.

OBJECTIVETo observe the effect of needling at Baihui(DU20), Shuigou(DU26) and Shenmen(HT7) on glucose metabolism in different regions of the brain in vascular dementia patients.
METHODSTen patients with vascular dementia were randomized into 2 groups, and the patients in the control group received the treatment with needling in the routine acupoints for hemiplegia (chosen from 6 Yang meridians of the hand and foot), while those in treatment group received needling at 3 additional acupoints, namely Baihui(DU20), Shuigou(DU26) and Shenmen(HT7), which were specific for dementia treatment. All the patients were examined by positron-emission tomography(PET) to detect the glucose metabolism in different brain regions before and after acupuncture treatment by means of semiquantitative analysis of the average values of radioactive count in the brain regions.
RESULTSNeedling at the additional 3 acupoints could obviously enhance glucose metabolism in the bilateral frontal lobes, bilateral thalamus, temporal lobe and lentiform nucleus on the uncompromised side.
CONCLUSIONThe effect of needling at Baihui(DU20), Shuigou(DU26) and Shenmen(HT7) on vascular dementia is closely related to improved cerebral glucose metabolism.

6.Anti-neuroinflammatory Effects of 12-Dehydrogingerdione in LPS-Activated Microglia through Inhibiting Akt/IKK/NF-κB Pathway and Activating Nrf-2/HO-1 Pathway.

Ginger, one of worldwide consumed dietary spice, is not only famous as food supplements, but also believed to exert a variety of remarkable pharmacological activity as herbal remedies. In this study, a ginger constituent, 12-dehydrogingerdione (DHGD) was proven that has comparable anti-inflammatory activity with positive control 6-shogaol in inhibiting LPS-induced interleukin (IL)-6, tumor necrosis factor (TNF)-α, prostaglandin (PG) E₂, nitric oxide (NO), inducible NO synthase (iNOS) and cyclooxygenase (COX)-2, without interfering with COX-1 in cultured microglial cells. Subsequent mechanistic studies indicate that 12-DHGD may inhibit neuro-inflammation through suppressing the LPS-activated Akt/IKK/NF-κB pathway. Furthermore, 12-DHGD markedly promoted the activation of NF-E2-related factor (Nrf)-2 and heme oxygenase (HO)-1, and we demonstrated that the involvement of HO-1 on the production of pro-inflammatory mediators such as NO and TNF-α by using a HO-1 inhibitor, Zinc protoporphyrin (Znpp). These results indicate that 12-DHGD may protect against neuro-inflammation by inhibiting Akt/IKK/IκB/NF-κB pathway and promoting Nrf-2/HO-1 pathway.

7.Characteristics of circulatory disturbance and the treatment of severe hand-foot-and-mouth disease.

OBJECTIVETo investigate the characteristics of circulatory disturbance and treatment of severe hand-foot-and-mouth disease (HFMD).
METHODThe clinical characteristics, laboratory findings, therapy and outcome of 22 severe HFMD patients were retrospectively analyzed.
RESULTAll the 22 severe HFMD patients came from the countryside. All these patients had encephalitis. Fifteen cases had myocardial injury. All had symptoms of sympathetic excitation and 17 cases had hypertension [(128 ± 16)/(81 ± 14) mm Hg (1 mm Hg = 0.133 kPa)]. Fourteen cases had exacerbation with rapid decline of blood pressure [(61 ± 12)/(33 ± 12) mm Hg]. In cardiorespiratory failure stage, 13 patients had neurogenic pulmonary edema accompanied by circulatory failure and 12 cases had a lower glasgow scores (less than 7). Myocardial injury and ECG change were found in some cases. Inotropic and pressor drugs were given in patients with circulatory collapse. Five cases received fluid resuscitation due to refractoriness to inotropic drugs. Nine patients received blood purification. Seventeen survived and 5 cases died due to circulatory failure.
CONCLUSIONCirculation failure of severe HFMD is the main cause of death. Early and appropriate circulation support is very important to reduce mortality.

objecfive To know and compare the intelligence level of children born in different time periods in regions with iodine deficiency disorders(IDD)in Liaoning province.Methods All 7-14 year-old children from ten schools were chosen as the subjects respectively from six villages in each of the six counties and in regions with iodine deficiency,who were respectively born at the initialization of iodinated salt supplying period(1978-1980);non-iodinated salt supplying period(1981-1990);recovery of supplied iodized salt period(1991-1995);universal iodized salt period(1996-2000),respectively.Intelligence quotient(IQ)was measured by Combined Ravens Test in China(CRT-C)and Combined Ravens Test-the Rural,in China,2nd edition(CRT-RC2).Results IQ of children during the non-iodized salt period(91.9±14.3)was significantly lower than the initial supply of iodized salt period(95.8±14.6,q=8.60,P<0.01),recovery of supplied iodized salt period(99.7±14.7)was significantly higher than the initial supply of iodized salt period, non-iodized salt sales period(q = 9.53, 18.13, all P < 0.01 ),universal salt iodization( 104.3 ± 14.9) was significantly higher than the initial supply of iodized salt period, non-iodized salt sales period, recovery of supplied salt iodization(q = 20.00,28.00,10.46, all P < 0.01). Children's rate of mental retardation (IQ≤69) was higher in non-iodinated salt supplying period (6.7%, 88/1314 ) than the initial supply of iodized salt (4.4%, 21/471, χ2 = 3.85, P < 0.05), recovery of supplied iodized salt period(3.3%,48/1470) was significantly lower than non-iodinzed salt supplying period (χ2 = 15.37, P < 0.01), universal salt iodization period(2.7%, 36/1344) was lower than the initial supply of iodized salt period(χ2 = 4.41, P < 0.05) and non-iodinzed salt supplying period(χ2 = 26.34, P < 0.01 ). The IQ and intelligent retarded rates in children born during the initial years of iodinated salt supplying period were not different. The IQ of the children during ten years of non-iodized salt supplying period fluctuated in a "∪" curve, while the intelligent retardation rates in a "∩" curve.The children born during the period of recovery supplied iodized salt increased their IQ and lowered the retardation rates year after year. The IQ of the children in universal iodized salt period kept on increasing while intelligent retarded rates reduced to the lowest level. Conclusions The intelligence level of children born in regions with IDD during non-iodized salt supplying period is remarkably lower than that of the beginning years of iodinated salt supplying period. The intelligence level of children born after universal iodized salt period is remarkably higher than that of the initial iodinated salt supplying period and recovery of supplied iodized salt period, respectively.
9.Quantitative assessment of late lumen loss after biodegradable polymer and permanent polymer sirolimus-eluting stents implantation.

BACKGROUNDSirolimus-eluting stents (SES) are reported to be associated with reduced late lumen loss (LLL), resulting in less frequent restenosis when compared to bare-metal stent. The current study aimed to assess the difference in LLL between SES with biodegradable and with permanent polymer.
METHODSFrom March 2010 to June 2011, 300 consecutive patients having only biodegradable polymers or permanent polymer SES for all diseased vessels were included. Serial quantitative coronary analysis was performed on both the "in-stent" and "segment" area, including the stented segment, as well as both five mm margins proximal and distal to the stent. The primary endpoint was the LLL defined as the minimal lumen diameter (MLD) post-stenting minus the MLD at nine-month after the indexed procedure.
RESULTSLLL was comparable between the two stents. Importantly, LLL for the distal segment (median 0.05 mm, interquartile 0 to 0.09 mm) was less severe compared with in-stent (median 0.13 mm, interquartile 0.08 to 0.18 mm) and proximal segment LLL (median 0.12 mm, interquartile 0.06 to 0.14 mm, all P < 0.001). In general, the LLL was associated with the post-procedure MLD (b = 0.28, P = 0.002), hyperlipidemia (b = 0.14, P = 0.021), and calcified lesions (b = 0.58, P = 0.001). The R(2) and Radj of the multiple regression model were 0.651 and 0.625, respectively.
CONCLUSIONSSES with either biodegradable or permanent polymer had lower value of LLL. The small amount of LLL at the distal segment possibly contributed to the less distal edge stenosis.

10.Application of combined cone-beam CT and ExacTrac X-ray image-guided system in high-grade glioma radiotherapy

Objective To explore the optimal image-guided verification mode by using combined cone-beam CT(CBCT)and ExacTrac X-ray(ETX)image-guided system for the position verification during the first and remaining fractionated radiotherapy of high-grade glioma patients.Methods Twenty high-grade glioma patients undergoing intensity-modulated radiotherapy at some hospital from January 2018 to June 2020 were analyzed retrospectively.CBCT image-guided system was used for the patients treated for the first time to determine the corresponding position of the treatment center on the body surface and to reset and mark the treatment center,then on-line auto registration of the CBCT images with CT positioning images was carried out,and the residual setup errors were verified with ETX image-guided system;position verification of the setup errors was performed with ETX image-guided sysem during the remaining fractionated treatment.The setup errors and their interval distributionswere calculated for the patients in six directions including left-right direction(Lat),head-foot direction(Lng),anterior-posterior direction(Vrt),rotation around left-right(Pitch),rotation around head-foot(Roll)and rotation around anterior-posterior(Yaw).SPSS 22.0 statistical software was used for data analysis.Results There were 75%patients treated for the first time and 78.62%ones undergoing the remaining fractionated radiotherapy only needed one time of setup error corre-ction.Combined CBCT and ETX image-guided resetting during the first radiotherapy met clinical requirements;during the remaining fractionated radiotherapy there were significant differences between the setup errors in the six directions before and after calibration(P<0.05).The interval distribution of setup errors showed the error values in the six directions were all restricted within 0 to 1 mm and within 0° to l °during the first and remaining fractionated radiotherapy.Conclusion Involve-ment of combined CBCT and ETX image-guided system in the first and remaining fractionated radiotherapy of high-grade glioma patients after operation contributes to determining and resetting the treatment center rapidly and accurately,decreasing setup errors and enhancing the accuracy and repeatability of radiotherapy.[Chinese Medical Equipment Journal,2024,45(8):57-62].
