1.Genetic Relationship in Bone Samples Using SNP-Based Human Identification DNA Chip.

DNA profiling with sets of short tandem repeat (STR) markers is the most popular method for identifying human DNA in forensics. Identification by STR typing might fail when DNA is degraded or is present in low amounts, such as in disaster victim identification (DVI) samples. In such cases, more information might be obtained by using additional markers such as single nucleotide polymorphisms (SNPs). Multiplex PCR and microarray are convenient techniques to analyze SNP markers. We used an AccuID(TM) Chip, SNP-based DNA chip manufactured by DNA Link Corporation, to confirm genetic relationship between two human bone samples that had been buried for more than 50 years and blood samples from the alleged descendants of the sources of the bone fragments. The chip combines an Affymetrix resequencing array with a multiplex PCR technology and can genotype hundreds of SNP markers in a single experiment. Genotyping the two bone samples yielded 90.5 and 77 SNP markers. The commonly genotyped markers (61 and 47 SNP loci) in each bone-family pair provided high paternity indices to support the genetic relationships in both cases.

2.The Incidence Rates and Risk Factor of Mild Injury for Two Weeks: Using Korea National Health and Nutrition Examination Survey 2001.

OBJECTIVES: This study was performed to provide the basic data for establishing countermeasures for preventing injury by analyzing the incidence rates, the risk factors, the characteristics of the injury and the utilization of medical care for mild injury that lasted for two weeks. METHODS: We examined the injury survey data among the National Health and Nutrition Survey data. The definition of mild injury that lasted for two weeks was that the injury that caused pain at least once a day for two weeks or the injury for which the usual daily activity of the injured person was severely affected. We used statistical analysis methods such as chi-square test and multiple logistic regression analysis. RESULTS: The incidence rate of injury that lasted for two weeks was 4.7 per 1,000 persons. On the multiple logistic regression, the children and adolescents (OR=3.80, 95%CI=1.63-8.84) had higher rates of injuries than the adults, and the middle and high school (OR=0.51, 95%CI=0.31-0.85) and college(OR=0.34, 95%CI=0.17-0.68) students had lower rates of injuries than the elementary school students. The unemployed (OR=0.39, 95%CI=0.20-0.73) and others (OR=0.38, 95%CI=0.21-0.70) had lower rates of injuries than the blue collar workers. The major causes of injuries were found to be falling and slipping, and the most prevalent place of occurrence was near or at home. CONCLUSIONS: A prevention program needs to be developed and continuous education must be offered to the children, adolescents and blue collar workers.

3.Limitation of Regular Autosomal STR Testing for Paternity within an Isolated Population.

In order to determine paternity by genetic testing, the Paternity Index (PI) and probability of paternity are calculated using likelihood ratio method. However, when it is necessary, additional testing can be performed to validate the genetic relationship. This research demonstrates autosomal short tandem repeat (STR) results of Jeju Island population in order to determine genetic relationship. Two notable cases showed that despite the acceptable PI value obtained from STR testing, average of 12 mismatches were found in total of 169 autosomal single nucleotide polymorphism typing. Such cases imply that cautious statistical approach is necessary when determining genetic relationship, especially within an isolated population group. Moreover, this would suggest that a further research and investigation are needed in order to understand the population structure of Korea.

4.The Relationship Between Childhood Trauma Experience and Complicated Grief: The Importance of Psychological Support for Individuals Coping With Pet Loss in Korea

Background:
Loss of companion animals can result in various psychological reactions, including complicated grief, depression, anxiety, and insomnia. However, research on the prevalence of patients requiring clinical intervention is limited. Moreover, research examining the relationship between childhood trauma and psychological symptoms after pet loss is lacking. Therefore, this study aimed to investigate the rates of clinically significant psychological distress after pet loss and the impact of childhood trauma on adults who have experienced pet loss.
Methods:
An online survey was conducted with non-clinical adult participants who had experienced the loss of a companion animal. Psychiatric characteristics were evaluated using standardized assessments, including the Inventory of Complicated Grief (ICG), the Patient Health Questionnaire-9 (PHQ-9), the Generalized Anxiety Disorder-7 (GAD-7), and the Insomnia Severity Index (ISI). Childhood trauma was evaluated using the Childhood Trauma Questionnaire (CTQ).
Results:
In a sample of 137 individuals who had experienced pet loss, the percentages of those who exceeded the cutoff points were 55% for the ICG, 52% for the PHQ-9, 40% for the GAD-7, and 32% for the ISI. The group that reported childhood trauma exhibited significantly higher scores on the ICG (t = 2.16, P = 0.032), PHQ-9 (t = 3.05, P = 0.003), GAD-7 (t = 2.61, P = 0.010), and ISI (t = 2.11, P = 0.037) than in the group without childhood trauma.For participants who experienced pet loss for less than one year, there was no significant difference in the ICG between the trauma and non-trauma groups, as both had extremely high scores. However, the trauma group had a significantly higher PHQ-9 (t = 2.58,P = 0.012) than the non-trauma group. In contrast, for participants who experienced pet loss for more than one year, the trauma group had a significantly higher ICG (t = 2.22, P = 0.03) than the non-trauma group, while there was no significant difference in the PHQ-9. Additionally, the emotional abuse scores on the CTQ were most significantly correlated with the ICG scores even after controlling for depressive symptoms.
Conclusion
People who experienced pet loss had significant psychological symptoms, ranging from 32% to 55%. Childhood trauma experiences affect complicated grief, depressive symptoms, anxiety symptoms, insomnia and prolonged grief disorder after pet loss. After pet loss, people with childhood trauma may require more psychological help than those without trauma.

FI has a higher prevalence with age and has an equivalent prevalence to urinary incontinence in patients with genitourinary disease, but is often not confirmed in these cases. A thorough investigation is needed to diagnose FI, with the common etiology of this condition in mind, and several questionnaires can be used to identify symptoms. The physical examination contains digital rectal examination carries out to identify the patient’s condition. Ultrasound, colonoscopy, and rectum pressure test can be performed. Patients educated in diet-related issues, bowel movements, and defecation mechanism. Nonoperative options such as diet control and Kegel exercise should be performed at first. Surgical treatment of FI is considered when conservative management and oral medications produce no improvement. Surgical options include less invasive procedures like bulking agent injections, and more involved approaches from sacral nerve stimulation to invasive direct sphincter repair and artificial bowel sphincter insertion. Good outcomes in FI cases have also recently been reported for barrier devices.

7.Parallel-excision infrabrow blepharoplasty with extensive excision of the orbicularis oculi muscle in an Asian population

Background:
Infrabrow blepharoplasty has become a common surgical method used to rejuvenate aged upper eyelids in Asians. In this paper, we describe the parallel excision method for infrabrow blepharoplasty as a useful alternative to the conventional elliptical excision method. The authors’ experience over a 3-year period is presented and reviewed.
Methods:
A retrospective review of parallel excision infrabrow blepharoplasty cases at our hospital between 2014 and 2017 was performed. Three oculoplastic surgeons compared preoperative and postoperative photographs using the Strasser grading system.
Results:
From the medical records of 123 patients, a total of 93 patients with moderate-to-severe bilateral dermatochalasis were selected as subjects. The exclusion criterion was levator function less than 8 mm. The total mean follow-up period was 2 years (range, 0.5–3.5 years). The mean skin excision height and width were 9.75 mm (range, 5–16 mm) and 58.51 mm (range, 42–75 mm), respectively. All patients who underwent surgery recovered without major complications, and all patients had high levels of satisfaction and improvements in their visual field. In the Strasser evaluation performed by the oculoplastic surgeons, most patients were found to have excellent results.
Conclusions
The parallel excision method for infrabrow blepharoplasty is a safe and effective technique that yields more natural- and youthful-looking eyelids than the conventional elliptical excision method. In our method, more effective manipulation of the orbicularis oculi muscle led to a reduction in frontalis compensation, resolution of sunken eyelids, and correction of lateral hooding.
8.Effect of Tamsulosin 0.2 mg on the Short-Term Treatment of Urinary Stones: Multicenter, Prospective, Randomized Study.

PURPOSE: We evaluated the effect of tamsulosin for the short-term treatment of urinary stones. MATERIALS AND METHODS: Two hundred forty-seven patients who were diagnosed with urinary stones were enrolled in this prospective, randomized multicenter study. The treatment and extracorporeal shock wave lithotripsy (ESWL) group (Group 1, n=115) was given diclofenac sodium 100 mg plus tamsulosin 0.2 mg for 1 week. The control and SWL group (Group 2, n=92) was given diclofenac sodium for 1 week. The treatment and no SWL group (Group 3, n=19) was treated the same as Group 1. The control and no SWL group (Group 4, n=21) was given diclofenac sodium only. Patients in Groups 3 and 4 did not want to take SWL treatment. The size of the stone, expulsion rate according to stone location and ESWL machines, changes in pain score, and distance of stone migration when expulsion of the stone failed were compared among the groups. RESULTS: There were no differences in the 4 groups in sex or stone size. The stone expulsion rate of lower ureter stones in group 1 (59.6%) was significantly higher than in group 2 (30.8%) (p=0.01). The distance of stone migration in group 3 was longer than that in group 4 (5.63+/-5.48 cm compared with 0.33+/-0.68 cm; p=0.002). Although the difference was not significant in the SWL groups, the distance of stone migration in group 1 was longer than that in group 2 (7.08+/-6.9 cm compared with 5.46+/-7.4 cm; p>0.05). There were no significant differences in changes on the pain scale among the groups. CONCLUSIONS: These results suggest that adjunctive medical therapy for 1 week with tamsulosin after SWL increases the stone expulsion rate of lower ureteral stones and increases the distance of stone migration in case of failure of stone expulsion.

9.Two Cases of Epiphora Following Le Fort I Osteotomy.

The lacrimal drainage system begins at the puncta and empties through the valve of Hasner into the inferior meatus. The varieties of the anatomic location, size and morphology of this valve can predispose susceptible individuals to an increased incidence of iatrogenic injury during nasal surgical procedures, thus necessitating a comprehensive understanding of the system. We report two cases of epiphora following two-jaw surgeries for cosmetic purposes. Nasolacrimal duct obstructions were diagnosed, and the symptoms were improved with dacryocystorhinostomies.

10.A Case of Adenosquamous Carcinoma Ex Pleomorphic Adenoma of Submandibular Gland.

We experienced a case of carcinoma ex pleomorphic adenoma arising in the submandibular gland of a 47-year-old male patient. The patient underwent submandibular gland ressection and supraomohyoid neck dissection. Histologic examination revealed that the malignant component of carcinoma ex pleomorphic adenoma was adenosquamous carcinoma. The patient refused postoperative radiation therapy and tumor recurred at the neck and lung about 18 month later. Modified radical neck dissection was carried out, and additional postoperative radiotherapy and palliative chemotherapy were initiated. To the best of our knowledge, this is the first case of carcinoma ex pleomorphic adenoma, of which the malignant component is adenosquamous carcinoma in the submandibular gland. Therefore the authors report this rare case with a literature review.
