OBJECTIVE: To study the pharmacological activities and toxicity of the crude alkaloids of Corydalis adunca maxim and to provide pharmacological data for further development of this herbal medicine. METHODS: The anti- inflammatory effects of the crude alkaloids of Corydalis adunca maxim were observed by using xylol and agra to induce the turgidness in the rats. The analgesic effects were observed by body distortion methods. The LD50 and 95% creditability were calculated with developed Karber Method. RESULTS: The administration of alkaloids of Corydalis adunca maxim had the function of inhibiting the auricle swelling caused by xylol and joint swelling caused by agar, and of decreasing the body- distortion of the rats. The LD50 of the crude alkaloids of Corydalis adunca maxim was 1. 833g. kg- 1 and the 95% creditability was 1. 18～ 2. 06g? kg- 1. CONCLUSIONS: The crude alkaloids of Corydalis adunca maxim has anti- inflammatory and analgesic effects in rats.

Objective To investigate the potential role of Lycium bararum polysaccharide (LBP) with or without interferon -inducible protein 10 ( CXCL10) in inducing dendritic cells ( DC) functional maturation by monitoring the alteration of cytokines for inducing DC maturation in peripheral blood and by detecting the expression of S-100 protein in tumor tissue, thus to reveal its mechanism of inhibiting experimental liver cancer. Methods H22 bearing mice model was established. The mice were randomized into model group, LBP group (50 mg/kg, ig), CXCL10 (right axillary subcutaneous injection of 15 μg/kg), LBP + CXCL10 group (LBP 50 mg/kg, ig, and right axillary subcutaneous injection of CXCL10 15 μg/kg), 5- fluorouracil (5FU) group ( intraperitoneal injection of 12mg/kg) , 12 mice in each group. The mice were administered the corresponding medicine once a day. After treatment for 2 continuous weeks, blood was sampled from infraorbital vein, and the tumor mass, spleen, thymus were extracted for the calculation of anti-tumor rate, thymus index and spleen index separately . The mRNA expression levels of interleukin 12 (IL-12) and tumor necrosis factor-α (TNF-α) in peripheral blood were detected by fluorescence quantitative PCR, the expression of S-100 protein in tumor tissues was detected by immunohistochemical assay. Results Compared with the model group, tumor growth in LBP group and LBP+CXCL10 group was obviously inhibited, and tumor-inhibitory rate was 55.90%, 50.91%, respectively. Meanwhile, the mRNA expression level of IL-12 was 2.94 folds higher in LBP group and 3.39 folds higher in LBP + CXCL10 group, and TNF-α mRNA expression level was 1.55 folds higher in LBP group and 4.74 folds higher in LBP+CXCL10 group than the model group, the differences being statistical significant ( P<0.05 or P<0.01). Results of immunohistochemical assay showed that S-100+DC number in LBP group and LBP+CXCL10 group was larger than that in the model group (P<0.05 ). Conclusion LBP and LBP+CXCL10 exert significant effect on inhibiting experimental liver cancer. The mechanism may be related with inducing the secretion of IL-12 and TNF-α, which plays a key role in inducing DC maturation, and with the increase of the number of DC in tumor microenvironment.

Objective To investigate the mechanism of recurrent laryngeal nerve injury during total thyroidectomy.Methods The clinical data of 36 patients suffering from recurrent laryngeal nerve injury in total thyroidectomy from 2003 to 2014 was analyzed retrospectively.Results 21 cases had temporary recurrent laryngeal nerve injury while 16 cases had permanent injury.The cases of injury included giant thyroid goitre,with variation of recurrent laryngeal nerve,undergoing secondary surgery,with tumor invasion and complicating thyroiditis.During the first surgery,the incidence of temporary recurrent laryngeal nerve injury was 0.43％,and the incidence of permanent damage was 0.27％.The temporary and permanent injury incidence of secondary surgery after initial subtotal thyroidectomy was 4.59％ and 4.59％,respectively.For patients with grade 3 thyroid gland,that was 1.81％ and 0.30％,respectively and 0.51％,0.72％ respectively in malignant cases.In cases complicating thyroiditis temporary recurrent laryngeal nerve injury occurred in 1.01％.Conclusions There is increased risk of recurrent laryngeal nerve injury in giant goiter cases undergoing secondary operations.Hence it is suggested that initial surgical procedure be hemithyroidectomy or total thyroidectomy

A 9-year-old boy presented with pruritic erythema and erosions of the scalp for 1 month, which spread all over the body for 1 week. Histopathological examination of the skin lesions showed blisters forming under the granular layer of the epidermis, a large number of acantholytic cells in the blisters, and perivascular infiltration of lymphocytes and eosinophils in the superficial dermis. Direct immunofluorescence study showed reticular deposition of IgG and complement C3 between epidermal cells, and negative staining for IgM and IgA. Enzyme-linked immunosorbent assay showed the presence of serum anti-Dsg1 antibody (157.00 U/ml) . The patient was diagnosed with pemphigus foliaceus. After admission, the patient showed poor response to the treatment with prednisone at a dose of 40 mg/d. After the treatment with low-dose rituximab (100 mg per week, 4 weeks) combined with prednisone (20 mg/d) , her condition was well controlled. The dosage of prednisone gradually decreased to 7.5 mg/d for maintenance treatment, and no recurrence was observed during 24-month follow-up.

Objective:To assess the value of spiral CT in diagnosis and treatment of chronic otitis media.Methods:The spiral CT findings of 74 cases including 93 ears proved by operation and pathology were studied.Results:The lesions such as the disruption of the ossicular chain showed in spiral CT or three-dimensional image were in accord with those seen in the operation,the accuracy was 95.7%,the disruption of the ossicular chain and bony erosion in the tympanic cavity and antrum were severe in the typeⅢ chronic otitis media.Conclusion:Spiral CT is helpful to diagnose and definite the chronic otitis media,three-dimensional image can provide valuable information for surgery.

With the introduction and development of the concept of precision surgery,3D printing individualized guide template has been gradually applied in the field of spinal surgery because of its advantages of increasing surgical precision,reducing the difficulty and risk of surgery,reducing the operation time,and reducing the intraoperative transmission dose,etc..Individualized guide plates designed and manufactured according to the patient~ surgical site,self-conditions and individual differences before surgery can accurately restore the preoperative design during surgery to meet the needs of modernized spine surgery for individualization and precision.Individualized guide template has great guiding significance for complex spinal surgery.This article reviews the current clinical application of 3D printing individualized guide template in spine surgery,summarizes its produce principles and procedures,advantages and limits,and make prospect for it's future applications.

Objective:To explore the clinical features, treatment and prognosis of focal nodular hyperplasia (FNH) of the liver.Methods:A retrospective analysis of the clinical data of 36 patients with FNH who had undergone surgery and were pathologically confirmed from May 2013 to Aug 2021 was made at the General Surgery Department of the First Affiliated Hospital of Nanchang University.Results:There were 13 males and 23 females, with an average age of (35.9±15.3) years. 50% patients were asymptotic,21 cases (58.3%) were diagnosed as FNH by imaging before operation. All patients underwent surgical resection. There was no postoperative mortality.Pathology confirmed preoperative tentative diagnosis of FNH in all cases. The average follow-up time was 21.1 months, all patients were alive, and 1 patient had a relapse.Conclusions:FNH is a benign liver tumor like lesion. Surgery is suggested for cases suspected of tumor and the prognosis is good.

OBJECTIVETo investigate the effect of utilizing octreotide during perioperative period on pancreatic fistula after pancreaticoduodenectomy (PD).

METHODSThree hundreds and six patients admitted from January 2010 to October 2014, who prepared to undergo pancreaticoduodenectomy (PD) were randomly divided into octreotide group (147 cases) and control group (159 cases). In octreotide group, octreotide was used in subcutaneous injection instantly after PD, each 8 hours until postoperative 10(th) day, and patients in control group were injected with the same volume of saline. Differences of pancreatic fistula (Grade A, Grade B, Grade C), hospitalization days and treatment cost were compared. χ(2) test, t-test and Fisher exact test were used to analyzed to the data, respectively.

RESULTSNo statistical significance (P>0.05) between two groups in the incidence of pancreatic fistula after PD (Grade A: 8.8% vs. 10.2%, Grade B: 2.7% vs. 4.4%, Grade C: 0.7% vs. 1.3%; χ(2)=0.197, 0.700, 0.288; P=0.657, 0.403, 0.591), the length of hospitalization((12.1±1.2)days vs. (13.0±1.2)days)(t=1.711, P=0.104) and treatment cost (79 700±6 700 vs. 77 600±5 200)(t=1.378, P=0.185). When accompanied with high risk factors, such as soft texture of pancreas, pancreatic duct size less than 3 mm, BMI≥25 kg/m(2) and diabetes, compared with control group, octreotide group had the lower incidence rate of pancreatic fistula and clinical correlative pancreatic fistula(all P<0.05) after PD.

CONCLUSIONSGenerally, octreotide makes no contribution to reduce the incidence of pancreatic fistula after PD. However, for patients who is accompanied with high risk factors, such as soft texture of pancreas, pancreatic duct size less than 3 mm, BMI≥25 kg/m(2) and diabetes, octreotide can effectively prevent pancreatic fistula after PD.

Pancreatic cancer is a digestive system malignant tumor with extremely poor prognosis.With the rapid development of science and technology,although breakthrough results has been achieved in basic research of pancreatic cancer and some achievement in surgical techniques and adjuvant treatment approach,the survival of patients with pancreatic cancer has not been significantly improved.Among them,the serious disconnection between basic research and clinical practice has led to a slow improvement in the diagnosis and therapy.As a bridge between basic research and clinical work,translational medicine can promote progress of the early detection and precision treatment of pancreatic cancer,thus providing new hope for patients with pancreatic cancer.
