2.Spontaneous Hemorrhagic Transformation After Cerebral Infarction and Its Imaging Assessment

The research of hemorrhagic transformation (HT) after cerebral infarction is now focus on HT after thrombolytic therapy. However, attention should also be aroused to the occurrence of spontaneous HT in most patients who cannot receive thrombolysis promptly because of various reasons, for it has important influence on the treatment of cerebral infarction and prognosis. This article reviews the concept, typing, pathogenesis, correlation factors, imaging diagnosis and prediction of HT, as well as the effects of non-thrombolytic therapies on HT, such as antiplatelet, defibrase, anticoagulant, anti-free radical, and the relations between HT typing and prognosis.
3.Donor cytokine gene polymorphisms affect the development of acute rejection after kidney transplantation

Objective To investigate the influence of renal allograft donor cytokine and cytokine receptor gene polymorphisms on acute rejection after renal transplantation.Methods (1) 126 cases of cadaveric renal allograft recipients were divided into two groups according to the presence or absence of acute graft rejection.The distribution of 22 polymorphisms in 13 cytokine genes and production types of some cytokines were compared between donors of two groups as well as latent factors affecting acute rejection.(2) Based on the result of HLA-DR matching,all recipients were stratified into two conditions:0~1 locus HLA-DR mismatched and HLA-DR completely mismatched.Previous positive gene polymorphisms were compared between rejection group and no rejection group under two conditions.Results (1) Compared with no rejection group,the number of HLA-DR mismatched was significantly higher in rejection group.In the donors of rejection group,the genotype frequency of IL-1α-889 C/C,IL-1Rα msp I 11100 T/T,IL-4Rα+ 1902 A/A,TGF-β1 codon 10 C/C,IL-10-1082 A/A and lower production type frequency of IL-10 were significantly higher,whereas the genotype frequency of IL-12-1188 A/A,IL-2-330 G/G and IL-10 GCC/ATA was significantly lower.(2) With 0~1 locus HLA-DR mismatched,the genotype frequency of IL-1Rα msp I 11100 T/T,IL-4Raα+1902 A/A,IL-2-330 G/G,TGF-β1 codon 10 C/C and lower production type frequency of IL-10 showed significant difference between two groups,whereas with HLA-DR completely mismatched,the genotype frequency of IL-12-1188 A/A had significant difference.Conclusions This study verifies renal allograft donor genotype of IL-1α-889 C/C,IL-4Rα+ 1902 A/A,IL-1Ra msp I 11100 T/T,TGF-β1 codon 10 C/C,IL-10-1082 A/A and lower production type of IL-10 gene as the genetic safe factors for the development of acute allograft rejection and genotype of IL-12-1188 A/A and IL-2-330G/G as the major genetic risk factors for acute rejection after kidney transplantation Furthermore,the condition of HLA-DR mismatching might interfere with the action of these cytokine and cytokine receptor polymorphisms.
4.Effects of nifedipine combined with Magnesium Sulfate on serum creatinine, uric acid and cystatin C in patients with pregnancy induced hypertension

Objective To explore the effects of nifedipine combined with Magnesium Sulfate on the serum of patients with hypertension of pregnancy blood creatinine, uric acid and serum cystatin C level.Methods68 cases of patients with pregnancy induced hypertension from December 2013 to December 2015 in our hospital were selected, according to the different treatment methods were divided into control group and experimental group, 34 cases in each group, the two groups were treated with conventional therapy and symptomatic treatment, the control group was given 25% Magnesium Sulfate Injection 20mL+5%, Glucose Injection 20mL, 8~10min intravenous injection, after 25% Magnesium Sulfate Injection 60mL+5% Glucose Injection 500mL intravenous drip, 1~2g/L,1 times a day, Phentolamine Mesilate Injection 20mg+5%, Glucose Injection 250mL intravenous drip, 30min drop, 1 times a day;The experimental group was given the Extended Release Nifedipine Tablets 30mg sublingual in the control group on the basis of 3 times a day.Two group of gestational age<38 weeks treatment time was seven days,, the end of pregnancy over 38 weeks of treatment time was three days.After treatment the clinical efficacy, serum creatinine, uric acid, cystatin C, alpha-fetoprotein and adverse pregnancy outcome were compared between the two groups.ResultsAfter treatment,compared with the control group, the total effective rate of treatment group was higher (P<0.05), 2 groups of the blood serum uric acid, creatinine, Cystatin C and AFP levels were decreased (P<0.05), compared with the control group, the experimental group of serum uric acid, serum creatinine, Cystatin C and alpha fetoprotein level was lower (P<0.05).The experimental group, the complication rate was 5.88%, incidence rate of complications in the control group 14.71%, 2 groups of adverse pregnancy outcome rate comparison, the difference was not statistically significant.ConclusionNifedipine combined with Magnesium Sulfate significantly on clinical efficacy in patients with pregnancy induced hypertension, decrease the level of serum creatinine, uric acid and serum cystatin C levels, improve the quality of pregnancy, drug safety is high.
5.Curved Planar Reformation of Body Canals with Multislice Spiral CT

Objective To study the method and diagnostic value of curved planar reformation(CPR)of body canals with multislice spiral CT.Methods 50 patients with various disease of chest and abdomen scanned by multislice spiral CT.CPR of canals of chest and abdomen were performed.Results The images of CPR were acquired satisfactory in all cases.3 cases were normal.There were vascular diseases of chest in 6 cases,lung cancer in 9 cases,disease of abdominal vessels in 10 cases,disease of biliary tract in 10 cases.Carcinoma of the head of pancrease in 4 cases,ureter cancer in 4 cases,ureter stone in 2 cases,tuberculosis of ureter in 1 case,malformation of ureter in 1 case.Conclusion CPR of body canal with multislice spiral CT is an effective method in the diagnosis of various diseases of chest and abdomen.It is an useful supplement of routine CT scan.
6.Interleukin-10 promoter single nucleotide polymorphism and acute rejection in renal transplantation: a donor-recipient paired study

Objective:To study the influence of 3 single nucleotide polymorphisms (SNP) of IL-10 promoter (-1 082, -819 and -594) on acute rejection after renal transplantation in a donor-recipient paired mode. Methods: Three IL-10 promoter SNPs of 52 donor-recipient pairs were genotyped and phenotyped by PCR using sequence-specific primers (SSP). Recipients were followed up for 1 year after transplantation, and acute rejection episodes were observed. Incidences of rejection were compared against pair patterns and differences were analyzed using Fisher's exact test. Results: No genotype of high producer was found in any donor and recipient in this study. Forty-nine recipients were followed up for over 12 months. Of them, 18 recipients (36.7%) had acute rejection and 7 recipients (14.3%) had multiple rejection(MR). The relationship between donors with intermediate producer and acute rejection and MR tended to be statistically significant (P=0.076, P=0.051).The MR rate increased significantly in the pair of donor intermediate producer/recipient low producer (P=0.036). Conclusion: It is preferable to take donor’s IL-10 SNP into consideration in study of IL-10 SNP and renal transplantation. Donor intermediate producer can increase the risk of acute rejection. The combination of donor intermediate/recipient low producer can increase the risk of MR.
7.The diagnosis and treatment of cytomegalovirus (CMV) pneumonia after renal transplantation

Objective To explore the diagnosis and treatment of cytomegalovirus (CMV) pneumonia in patients who received immunosuppressive therapy after renal transplantation. Methods 15 patients out of the 110 recipients of renal transplantation were diagnosed as to have CMV pneumonia, during a period of the 2nd to 4th month after renal transplantation. All the 15 patients were treated with a systematic strategy, including rational use of antibiotics, withdrawal of immunosuppressant drugs and rebuilding the body′s immunity function, timely application of mechanic ventilator, and continuous high volume hemofiltration (CVVHF). The clinical responses of the 15 patients were reviewed and retrospectively analyzed. Results 15 recipients contracted CMV pneumonia out of 110 cases (13.6%), and it occurred on a mean of 93.3 days after renal transplantation. All of them were febrile with cough, and showed manifestations of interstitial pneumonia on chest films. CMV-IgM were positive in 13 cases, CMV DNA positive in 9 cases, and 4 patients presented signs of acute respiratory distress syndrome. The condition developed quickly in all the patients. 9 patients recovered and 5 patients died, including one who gave up treatment for financial reason. Conclusion CMV infection is a serious complication after renal transplantation. Early diagnosis with detection of CMV DNA and serum antibody of CMV-IgM should be emphasized. The comprehensive protocol principally composed of anti-virus therapy with cymevan or ganciclovir, monitoring of peripheral lymphocyte subgoups, and adjustment of dosage of immunosuppressive agent may improve recovery rate of CMV pneumonia.
8.Curved Planar Reformation of the Ossicular Chain in Multislice Spiral CT and Its Clinical Application

Objective Using curved planar reformation(CPR)to delineate the ossicular chain(OC)in one image and evaluate its clinical application. Methods HRCT of temporal bone was performed in 122 cases(165 ears).The CPR of OC was then performed.The main parameters were as follow:collimation 0.5mm,pitch 0.875,reformation interval 0.2~0.3mm,reformation matrix 1024?1024.Results (1)CPR of normal OC were performed on 80 ears,the three ossicles and its joints could be showed on one image clearly.(2)Clincal application(72cases):in 39 cases of temporal bone trauma,OCs were normal in 21 cases and abnormal in 18 cases.Subluxation or dislocation of malleoincudal joint(MIJ)(5,5) and incudostapedial joint(ISJ)(5,6),dislocation of MIJ and ISJ with rotation of incus(1 case),fracture of manubrium of malleus(1 case) were found respectively.In 16 cases of external auditory canal dysplasia(EACD),the most commonly associated type of OC deformity was absence of manubrium of malleus(10 cases)。Absence of OC associated with coarctation of middle ear cavity(3 cases) and fusion of the malleus and incus associated with absence of long process of incus represented in 1 case.Congenital ossicular anomalies without associated EACD were found in 2 cases.The long processes of the incus of both side were absent in 1 case.Incudostapedial disconnection was demonstrated in 1 case due to improper development of the long process of the incus.In 15 cases of chronic otitis media(COM),6 cases were complicated cholesteatoma.Destruction of OC was demonstrated in 6 cases.Conclusion CPR of OC with multislice spiral CT is an effective method in the diagnosis of conductive hearing loss.
9.The Study of the Facial Nerve Canal Abnormalities in the Congenital External Auditory Canal Atresia by MSCT CPR

Objective To study the facial nerve canal (FNC) abnormalities of the congenital external auditory canal (EAC) atresia by MSCT curved planar reformation (CPR).Methods 15 cases(17 ears) of congenital EAC atresia were scanned by MSCT. Post-processing images were completed by Philips Mx8000 workstation.Results In 17 ears, there were 2 ears with stenosis of EAC, 2 ears with membranous atresia of EAC and 13 ears with bony atresia of EAC.There were 1 ear with congenital cholesteatoma, 3 ears with malformation of tympanum, 7 ears with malformation of the ossicular chain and 1ear with malformation of inner ear. In 17 ears with abnormalities of FNC, included 11 ears with the shortness of FNC general length, 5 ears with the anomalous course of FNC, 9 ears with the displacement of FNC, 1 ear with the narrowing of FNC and 1 ear with the branch of FNC.Conclusion MSCT CPR of FNC, combined with HRCT axial scan, can clearly show the FNC abnormalities of the congenital EAC atresia and provide valuable information for surgery.
10.Cognition and behavior of sun-protection and facial photoageing in a Nanjing population

Objective To investigate the cognition and behaviors of sun-protection and the facial photoaging in a Nanjing population, and to analyze the relationships between them. Methods The objects being investigated in Nanjing (n=974) were divided into 10 groups according to age. The ordinary information, knowledge and behavior of sun-protection and Glogau photoageing type of face were studied by questionnaires. The results were analyzed by a logistic regression model to select the related factors to photoageing. Results The risk of skin photoageing increased with age. Most of the 46-65 years old crowds were type Ⅲ photoageing. Most of the objects being investigated had some knowledge and active awareness of sun-protection and could use some ways to protect themselves from sun. However, most people did not use the sunscreen correctly. Those who had higher level awareness and knowledge of sunprotection suffered less risk from skin photoageing. Sunbath without sunscreen for a long time outside activity was a high risk factor of development to skin photoageing. Shade, broad-brimmed hat and sun-protection in autumn were the protective factors. Conclusion Active awareness and correct methods could help prevent skin photoageing. In order to avoid the damages to skin from ultraviolet efficiently, the accurate ways of using sunscreens should be well understood.
