Purpose:To investigate expression of keratinocyte growth factor(KGF) messenger ribonuclunc acid in human NSCLC and to study the role of KGF in the development of NSCLC. Methods:The expression of KGF mRNA in 50 cases with NSCLC was detected by in situ hybridization (ISH). Results:On ISH slides, positive KGF mRNA was mainly shown as fusiform stain in plasma of fibroblast and blood vessel smooth muscle cell in mesenchymal of NSCLC, also, some parenchyma cell plasma was stained. 2.Research of the correlation with pathology and kinetics of TH1/TH2 cells and expression of iNOS in animals of tuberculosis

Objective To investigate the correlation of pathology to immunology in animals with tuberculosis by molecular pathology. Methods BALB/c mice were randomly divided into two groups: tuberculosis model (A) and control mice (N). Immunohistochemistry technology was used to investigate the correlation between the expression of IFN-γ, IL-4 and iNOS and the type as well as magnitude of tissue injury. Results Two phases were observed during the 2nd week characterized by inflammatory infiltration in the alveolar-capillary interstitium and the tissues surrounding blood vessels. A predominance of TH1 cells was shown by a higher percentage of IFN-γ positive cells and a lower percentage of IL-4 positive cells in the inflammatory infiltrate identified by immunohistology. From the 4th week after infection to the end of experiment, we observed a chronic advanced phase characterized by pneumonia and local necrosis with a TH0 balance due to an equivalent proportion of IFN-γ and IL-4 positive cells in the lung lesions. The iNOS expression was increased in acute phase and decreased in chronic phase. Conclusion There is a closely correlation on the kinetics of TH1/TH2 cells and iNOS expression with the type and magnitude of tissue injury during the infection of tuberculosis.
3.Effects of rapid decompression on ultrastructure of lung in rabbits

Objective To study the effect of rapid decompression on ultrastructure of the lung in rabbits. Methods Thirty healthy New-Zealand rabbits were randomly divided into low decompression group and rapid decompression group. The pulmonary ultrastructure of rabbits was observed with transmission electron microscope (TEM). Results In rabbits of the rapid decompression group, a number of pathological changes were found, such as disruption of cell membrane structure of types Ⅰ and Ⅱ alveolar cells, loosening of intercellular linkage, edematous mitochondria, expanded rough endoplasmic reticulum, engorged capillary of alveolar wall, edematous capillary endothelium cells, and increased capillary permeability. These changes became especially obvious when the decompression peak value was above 9.8 kPa. Conclusion Rapid decompression could inflict injury to the lung, which was aggravated with the increase of the decompression peak value.
4.Research Advance of Muir-Torre Syndrome

Objective To review recent studies on Muir Torre syndrome (MTS) and to improve the knowledge about MTS.Methods The literatures in recent years on clinic and gene research of MTS were reviewed.Results MTS was is a rare autosomal dominant disorder characterized by the predisposition to both sebaceous tumors (or multiple keratoacanthomas) and internal malignancies. Gastrointestinal cancers were the most common kind of internal malignancies in MTS patients(61%),followed by genitourinary cancers(22%). In most cases(56%),sebaceous tumors appeared after the emergence of internal maliganancy. Both hereditary nonpolyposis colorectal cancer(HNPCC) and MTS were caused by germline mutations in the DNA mismatch repair genes. MTS patients exhibit significantly more mutations in the hMSH2 than in the hMLH1. In these cases , both internal and skin tumors showed the characteristic of high microsatellite instability(MSI).Conclusion The presence of sebaceous tumors(or multiple keratoacanthomas) necessitates the search for internal malignancies. It is mandatory that patients with MTS, as patients with HNPCC, should be regularly followed up to search new malignancies. Evaluation and monitoring of the family members of patients are also necessary. The patients and their families should be counseled for genetic test. Sequencing the hMSH2 gene should be the prior selection of further examinations when clinical manifestations, history and laboratory tests suggest MTS.
5.Immunohistochemical evaluation of mutant p53 protein over-expression in non-mucinous adenocarcinoma in-situ and invasive adenocarcinoma, NOS of lung.

OBJECTIVETo study the over-expression of mutant p53 protein in non-mucinous adenocarcinoma in-situ (NMAIS) and invasive adenocarcinoma, NOS of lung.
METHODSImmunohistochemical study for p53 protein was performed on 17 cases of NMAIS and 70 cases of invasive adenocarcinoma, NOS of lung. The difference in p53 over-expression between the two tumor subtypes was analyzed.
RESULTSThe over-expression of mutant p53 protein was observed in 0 case (0%) of NMAIS and 37 cases (52.9%) of invasive adenocarcinoma, NOS of lung. The difference was of statistical significance (P = 0.000).
CONCLUSIONMutant p53 protein over-expression may play a role in the progression of NMAIS to invasive adenocarcinoma, NOS.

6.Biomechanical properties of a magnesium alloy absorbable rib intramedullary nail

BACKGROUND: At present, the materials that are successful y applied in rib fracture internal fixation mainly include titanium, nitinol rib encircling bone bonding plate, imported poly-L-lactic acid absorbable rib nails and anatomical plate.
OBJECTIVE: To study the biomechanical characteristics of new magnesium al oy absorbable rib intramedul ary nail in the rib fracture fixation, and to compare with imported poly-L-lactic acid absorbable rib intramedul ary nail in order to provide scientific basis for clinical application.
METHODS: Thirty fresh adult fifth rib specimens were col ected, and the specimens were used to make models for the middle rib fractures. The specimens were fixed with AZ31B magnesium al oy absorbable rib intramedul ary nail (magnesium al oy group) and poly-L-lactic acid absorbable rib intramedul ary nail (poly-L-lactic acid group), and the normal rib specimen group was set as control. The biomechanical characteristics of the nails in each group were tested with stress analysis.
RESULTS AND CONCLUSION: Three-point bending strength measurement results showed that the bending strength in the magnesium alloy group was close to the normal value of the specimen (P > 0.05), and there was significant difference in the bending strength between magnesium alloy group and the poly-L-lactic acid group (P < 0.05). Torsional strength measurement showed that there was no significant difference in torsional strength between magnesium alloy group and the normal specimens, and the results showed that the magnesium alloy was better than poly-L-lactic acid in fixation (P < 0.05). Tensile tests showed that the tensile strength and anti-pulling force of the magnesium alloy fixation were better than those of poly-L-lactic acid fixation (P < 0.05). The results indicate that the magnesium alloy absorbable rib intramedullary nail is better than poly-L-lactic acid absorbable rib intramedullary nail in strength and tensile strength, which is the ideal fixation material for rib fixation.
7.Correlation study on virtual touch tissue quantification and the pathological stages of liver fibrosis in chronic hepatitis B

Objective To investigate the correlation between virtual touch tissue quantification (VTQ) and the pathological grading of liver fibrosis in chronic hepatitis B.Methods 64 chronic hepatitis B patients (the chronic hepatitis group) and 40 healthy volunteers (the controlled group) were collected.The patients in the chronic hepatitis group were underwent liver biopsy.According to the hepatic fibrosis degree,the patients in the test group were classified into stage 0,1,2,3 and 4.The liver shear wave velocities (SWV) of all the participant were measured by VTQ.The cut-off values were determined by an analysis of receiver operating characteristic (ROC) curve.Results The mean SWV was (1.04± 0.13)m/s in the controlled group.The SWV in stages 0,1,2,3,and 4 were (1.17 ± 0.08)m/s,(1.33 ± 0.32)m/s,(1.53 ±0.32) m/s,(2.09 ± 0.54) m/s,(2.18 ± 0.70) m/s,respectively.There was a significantly difference in SWV between the controlled group and the chronic hepatitis group (F =34.97,P =0.00).The SWV were significantly different not only between stages 0-2,and 3,but also between 0-2 and 4 (F =8.87,P =0.00).A positive correlation was observed between the liver fibrosis and the SWV in the chronic hepatitis group (r =0.67,P =0.00).When a cut off value was set at 1.43 m/s,area under ROC curve was 0.875.The sensitivity and specificity were 100 % and 62.5 %.Conclusions SWV has a better correlation with liver fibrosis.VTQ can make an accurate assessment for stage 3 and stage 4 of the chronic hepatitis B.Therefore,VTQ can be used as a noninvasive and reliable diagnostic indicator for chronic hepatitis B.
8.Influence of various volume fractions of platelet-rich plasma on dog bone marrow mesenchymal stem cell proliferation

BACKGROUND: Proliferation and differentiation of mesenchymal stem cells (MSCs) is associated with platelet concentration in platelet-rich plasma (PRP). Low enrich multiple cannot reach proper effects, but high level had inhibitory effects on osteoanagenesis.OBJECTIVE: To observe the effect of different volume fraction of PRP on dog BMSC proliferation.DESIGN: A cytological in vitro study.MATERIALS: Healthy 12-month male Beagle dogs were supplied by the Experimental Animal Center, Xiangya Medical College,Central South University.METHODS: Dog BMSCs of 5 passage were adjusted to 3×10~8/L, and incubated in a 96-well plate at 200 μL per well. Following 24 hours of routine culture, primary medium and non-adherent cells were discarded. Prepared PRP gel was mixed with serum-free low-glucose DMEM containing penicillin and streptomycin, and then diluted into 5%, 6.25%, 7.5%, 8.75%, 10% volume fraction. 200 μL above-described liquid was added into the 96-well plate, which was subsequently placed in a incubator.We set up a blank control.MAIN OUTCOME MEASURES: MTT was used to investigate effect of different volume fraction of PRP on dog BMSC proliferation.RESULTS: Compared with the blank control group, various volume fraction of PLP could promote dog BMSC proliferation in early stage. With prolonged time, proliferation speed began to increase at day 6 in the 8.75% and 10% PRP groups, entering platform stage. BMSC number was increased rapidly in the 5% and 6.25% PRP groups, especially in the 6.25% PRP group.CONCLUSION: PRP gel could promote BMSC proliferation markedly and proliferation strength of BMSCs was correlated to the density of PRP. BMSC proliferation would be accelerated by the low density of PRP.
9.Platelet-rich plasma induces osteogenetic activity of canine bone marrow mesenchymal stem cells in vitro

BACKGROUND: Platelet-rich plasma (PRP) contains abundant growth factors that were needed for osteanagenesis. Moreover,the proportion of each growth factor formed by an organism, with good synergism.OBJECTIVE: To explore the influence of PRP on osteogenetic activity of canine bone marrow mesenchymal stem cells (BMSCs) after induction in vitro.DESIGN, TIME AND SETTING: The in vitro cytological experiment was performed at the Central Laboratory of Xiangya Hospital of Central South University from June 2007 to February 2008.MATERIALS: Healthy 12-month male Beagle dogs were supplied by the Experimental Animal Center, Xiangya Medical College,Central South University.METHODS: The 3~(rd) generation BMSCs were collected and divided into 4 groups. BMSCs in the control group were incubated in standard medium. BMSCs in the osteogenetic induction medium group were incubated in high-glucose DMEM containing fetal calf serum, dexamethasone, beta-sodium glycerophosphate and vitamin C. BMSCs in the PRP group were incubated in low-glucose DMEM containing 6.25% PRP. BMSCs in the combination group were incubated in high-glucose DMEM containing 6.25% PRP, dexamethasone, beta-sodium glycerophosphate, and vitamin C.MAIN OUTCOME MEASURES: Alkaline phosphatase activities were measured. Expression of collagen type I was examined by immunocytochemical staining. Calcium tuberoses were labeled using modified Von Kossa staining. Expression of osteocalcin mRNA was examined by RT-PCR.RESULTS: Levels of alkaline phosphates of all groups became increased along with time. The alkaline phosphates level of combination group was strongest (P < 0.05). Following 7 and 14 days of induction, type I collagen expressed positively in the osteogenetic induction medium and combination groups, but negatively in the PRP and control groups. Following 14 days,formation of calcium nodules were found in the osteogenetic induction medium and combination groups. Following 7 and 14 days,expression of osteocalcin mRNA were similar between the control and PRP groups (P > 0.05), which was significantly lower than the osteogenetic induction medium and combination groups (P < 0.05). Expression of osteocalcin mRNA was significantly lower in the osteogenetic induction medium group compared with the combination group (P < 0.05).CONCLUSION: PRP gel can effectively promote osteoblastic effect of BMSCs after induction in vitro following induction in osteogenetic medium.
10.Outcomes of traumatic flail chest treated by operative fixation versus conservative approach

Objective To compare the outcomes of traumatic flail chest with multiple injuries treated by operative fixation versus conservative approach. Methods The clinical data of 60 patients with traumatic flail chest with multiple injuries were retrospectively analysed, and the outcomes between operation group (treated by operative fixation, n=32)and non-operation group (treated by conservative approach, n=28) were compared. Results The mean time of hospital stay, ICU stay and mechanical ventilation was significantly shorter, and the prevalences of chest wall deformity, pulmonary infection, pulmonary atelectasis and respiratory failure were significantly lower in operation group than those in non-operation group (P<0.05). Six months after discharge, the pulmonary function parameters such as inspiratory capacity, forced vital capacity, forced expiratory volume in one second, peak expiratory flow, total lung capacity and maximal midexpiratory flow were significantly higher in operation group than those in non-operation group (P<0.05). Conclusion Traumatic flail chest with multiple injuries treated by operative fixation may lead to less flail chest associated complications. Operative fixation has short- and long-term benefits to flail chest.
