1.Effect of propofol on perioperative inflammatory response in patients with lung cancer

Objective To investigate effect of propofol total intravenous anesthesia and sevoflurane inhalation anesthesia on perioperative inflammatory response in patients with lung cancer.Methods Forty patients with simple resection of lung cancer were selected in our hospital.They were divided into observation group and control groups according to random sampling method (n=20).Observation group was given propofol total intravenous anesthesia while control group received sevoflurane inhalation anesthesia.The serum concentrations of MMP-9,TNF-α,IL-6 were compared at the T0 (immediately before the induction of anesthesia) , T1 (1 hour after surgery) , T2 (end of the surgery immediately) , T3 (24 hour after surgery) . Results Compared with the T0 period,the serum concentrations of MMP-9,TNF-αand IL-6 at the T1,T2,T3 periods were increased significantly in the both groups(P<0.05);the concentrations of TNF-α, IL-6 were lower significantly in observation group compared with control group (P<0.05) . Conclusion Propofol anesthesia for resection of lung cancer patients can significantly reduce the inflammatory response.
2.The correlation of serum YKL-40 level and albuminuria in type 2 diabetes

A total of 175 patients with type 2 diabetes mellitus (T2DM) were composed of 69 with normoalbuminuria,60 with microalbuminuria,and 46 with macroalbuminuria.The control group consisted of 64 healthy individuals.Serum YKL-40 levels were determined with ELISA method and related metabolic data were collected.Serum YKL-40 levels were significantly higher in T2DM group than in control group(P<0.01).Significant correlations of YKL-40 were found with the ratio of microalbuminuria to uric creatinine(r=0.677,P<0.01),HbA1C (r =0.562,P<0.01),systolic blood pressure (r =0.372,P =0.001),HOMA-IR (r =0.460,P =0.001),low density lipoprotein-cholesterol (r=0.304,P=0.012),age(r=0.260,P=0.015),blood uric acid (r=0.329,P=0.018),and high density lipoprotein-cholesterol (r=-0.247,P=0.032).YKL-40 may play a role in the occurrence and development of diabetic nephropathy.

Object To study the ultrastructure destruction of plants in the process of microwave assisted solvent extracion and its mechanism.Methods Cell structures of fresh leaves of Ginkgo biloba L. were observed by transmission electron microscpoe.Results The cell structure changed when the leaves were under microwave radiation or soaked by solvents or heated, such as plasmoysis, destruction of cell organellaes, disappearance of starch grain and so on, while the cell wall was not broken both in microwave assisted solvent extraction and in traditional heating extraction.Conclusion Microwave radiation could lead to the relaxation of the cell structures, but not be enough to make the cell wall broken.
5.Comparison of the SLIPA mask airway and endotracheal intubation in the cesarean section operation under general anesthesia

Objective To observe the safety and efficacy of the SLIPA mask airway in the cesarean section operation under general anesthesia.Methods Fifty single birth pregnant women who underwent cesarean operation under general anesthesia were randomized into 2 groups.SLIPA mask group(group S,25 cases)and endotracheal intubation(ETT)group(group T,25 cases).Mean arterial pressure(MAP),heart rate(HR),partial pressure of carbon dioxide in end expired gas(P_(ET)CO_2),peak airway pressure(Paw)were measured before induction of anesthesia,just before intubation,2min after intubation,just before extubation,2 min after extubation.Bucking,style,reflow,vomiting,aspiration were observed at inmbation or SLIPA mask airway insertion,before and after extubation and intraoperative.Umbilical arteries and veins blood were collected at delivery for the blood gas analysis.All delivery times,operation time and Apgar scores at 1,5 min were recorded.Results The successful rate ofthe first time intubation was 100% in group S,there was 2 cases by the second time intubation in group T.There Was 3 cases with gently air leak,but no influence on respiratory management.After intubation and extubation,MAP and HR increased significantly in group T (P<0.05),while group S had no significant change.There was no occurrence of bucking,style,reflow,vomiting,aspiration intmoperative,but 17 cages occurred at extubation in group T.The incidence and degree of sore throat in group T(10 cases after 2 hours and 6 cases after 24 hours) were significantly higher than those in group S(P<0.05).The Apgar scores at 1,5 min and delivery times were not significant between the two groups(P>0.05).Conclusion There is less adverse effects of SLIPA mask airway than ETT under general anesthesia in the cesarean section operation,SLIPA mask airway is safe and efficacious.

BACKGROUND: Previous experiments have shown that transforming growth factor-?1 (TGF-?1),matrix metalloproteinase-2 (MMP-2) and tissue inhibitor of metalloproteinase-1 (TIMP-1) play an important role in chronic allograft nephropathy,but the mechanisms remains unclear. OBJECTIVE: To investigate the expression and significance of TGF-?1,MMP-2 and TIMP-1 in the renal tissues of chronic allograft nephropathy and normal renal tissues. DESIGN,TIME AND SETTING: A histological and morphological control observational experiment of cytology was performed at Pathological Laboratory of the 175th hospital from June to October 2008. PARTICIPANTS: The transplanted renal tissues extracted from 24 patients with chronic allograft nephropathy were used in the experiment,and the normal renal tissues in 10 patients were assigned as controls. METHODS: Immunohistochemical staining was performed in all the renal tissues. All the renal tissues were fixed with formaldehyde and embedded by paraffin wax. Then the sections were prepared with a thickness of 3 ?m,and were then closed with normal non-immune serum. Subsequently,the sections were incubated as follows: primary antibody,secondary antibody,streptavidin peroxidase complex,and diaminobenzidine (DAB),followed by HE counterstaining. The sections were cleared and coverslipped. MAIN OUTCOME MEASURES: The expressions of TGF-?1,MMP-2 and TIMP-1 in the renal tissues were observed by immunohistochemical staining. The relationships among TGF-?1,MMP-2 and TIMP-1,as well as between them and the grade of chronic allograft nephropathy were analyzed. RESULTS: Qualitative analysis: The TGF-?1,MMP-2 and TIMP-1 showed strongly positive expressions in the cytoplasm and membrane of renal tubular epithelial cells in transplanted renal tissues. Objective To investigate the efficacy of Saccharomyces boulardii combined with ur-sodeoxycholic acid in the treating rectal ulcerative colitis(UC)and its influence on intestinal mucosal barrier function.Methods A total of 88 patients with rectal UC were randomly divided into observa-tion group and control group,with 44 cases in each group.The scores of main symptoms(abdominal pain,diarrhea,purulent stool),score of the Inflammatory Bowel Disease Questionnaire(IBDQ),lev-els of inflammatory factor indicators[interleukin-6(IL-6),interleukin-1β(IL-1β),tumor necrosis factor-α(TNF-α)]and levels of intestinal mucosal barrier function indicators[the ratio of urinary lac-tolic acid to mannitol(L/M),serum D-lactic acid(D-LA),serum diamine oxidase(DAO),and ser-um lipopolysaccharide(LPS)]were compared between the two groups before and after treatment.The incidence of adverse reactions were compared between two groups.Results After treatment,the scores of abdominal pain,diarrhea and purulent stool in both groups were significantly lower than those before treatment,while the IBDQ score was significantly higher than that before treatment,and the scores of abdominal pain,diarrhea and purulent stool in the observation group were significantly lower than those in the control group,while the IBDQ score was significantly higher than that in the control group(P<0.05).After treatment,the levels of serum D-LA,DAO,LPS and urinary L/M in both groups reduced significantly,and the levels of these indicators in the observation group were signifi-cantly lower than those in the control group(P<0.05).After treatment,the levels of serum IL-6,TNF-α and IL-1β in both groups reduced significantly,and the levels of these indicators in the observation group were significantly lower than those in the control group(P<0.05).The total ef-fective rate inthe observation group was 86.36%,which was significantly higher than 68.18%in the control group(P<0.05).There were no significant differences in incidence rates of adverse re-actions such as lower abdominal pain,allergies and refractory constipation between two groups(P>0.05).Conclusion Saccharomyces boulardii combined with ursodeoxycholic acid is effective in the treatment of patients with rectal UC,which can effectively improve symptoms such as abdominal pain,diarrhea and purulent stool,inhibit the inflammatory response of the body,and alleviate dam-age to the intestinal mucosal barrier.
