1.Efficacy of traditional Chinese medicine in the treatment of intrahepatic cholestasis of pregnancy

Objective To discuss the efficancy of traditional Chinese medicine in treatment of intrahepatic cholestasis of pregnancy .Methods A total of 60 patients who were admitted to our hospital for severe intrahepatic cholestasis of pregnancy(ICP) from January 2009 to December 2012 were randomly divided into two group using random number table .Patients in study group (n=30) took traditional Chinese medicine three times a day plusUDCA twice a day orally .Patients in control Group (n=30)were on‐ly given UDCA twice a day orally .Conventional treatment and care for ICP were given to both groups .Biochemical index (TBA , ALT ,and AST)level ,pregnancy outcome and Caspase‐3 of the two groups were recorded and compared and compared .Results The expressing of Caspase‐3 in placental cytotrophoblast ,syncytiotrophoblast .Decidual cells and interstitial cells of study group were (10 .58 ± 2 .62)% ,(15 .8 ± 4 .02)% ,(10 .17 ± 3 .58)% ,(14 .23 ± 4 .39)% ,respectively ,which were significantly lower than those in control group(P<0 .05) ,which were (20 .46 ± 8 .68)% ,(33 .89 ± 7 .05)% ,(20 .56 ± 4 .23)% ,(28 .29 ± 5 .58)% .TBA ,ALT、AST in two groups were declined ,and ctedy group was better than in control group(P<0 .05) .The index of liver function were all lower in study group than that of control group(P<0 .05) .Incidence rate of neonatal asphyxia was lower in study group than that of con‐trol group (P<0 .05) .Conclusion Traditonal Chinese medicine can reduce the expression of Caspase‐3 in study group .It is safe and eflfective in the treatment of ICP .
2.Expression of immune molecules in epileptogenic rats

BACKGROUND: The relation of epilepsy to immunity has been investigated at cellular and molecular levels in recent years, and the results show many immunological changes in epileptic patients such as immunocytes,immune molecules and immune functions.OBJECTIVE: To investigate the immunologic pathogenesis of epilepsy and expression of MHC- Ⅰ, MHC- Ⅱ and C3b receptor of microglia. DESIGN: Randomized and controlled trial. SETTING: Medical College of Dalian University; University of Sanchong,Japan.MATERIALS: The experiment was conducted in the Immunology Laboratory of Jilin Beihua University from 2001 to 2002. Forty adult Wistar rats were randomized into model group and control group with 20 in each group.METHODS: 12 mg/kg kainic acid was administered subcutaneously into rats in model group, while no intervention was given to rats in control group. Seizure was observed within 6 hours following kainic acid administration. Rats were killed 3 days after medication. Neuronal degeneration was observed with crystal violet staining and the expression of MHC molecules and C3b receptor in the hippocampus of epileptogenic rats was observed immunnhistochemically.administration, spasm occurred within 3 hours after kainic acid administration and continued from the first stage to the fifth stage. More severe served in cone-like cell layer of hippocampus of rats in model group, but molecule, MHC- Ⅱ molecule and C3b receptor was (201.6±6.43), (493.8±7.92) and (362.5±3.18) cells per visual field respectively in the hippocampus of epileptogenic rats inmodel group, but no obvious expression was observed in control group. The differences were significant between the two groups (P < 0.01).pocampal sclerosis, immunological inflammation is observed combined with the complement system, which indicates immunologic inflammatory mechanism of epilepsy.
3.Detection of anti-glutamic acid receptor 1 antibody in serum of epilepsy children

BACKGROUND: The recent researches show that there are various kinds of autoantibodies in epilepsy patients such as anti-nuclear antibodies, anticardiolipin antibodies,anti-?2 glucoprotein antibodies and anti-glutamic acid decarboxylase antibodies,which indicate that some types of epilepsy are mediated by autoimmune mechanism.OBJECTIVE: To detect the level of anti-glutamic acid receptor (GluR1)antibody in epilepsy children.DESIGN: A controlled trial.SETTING: Medical College of Dalian University, General Hospital of Jilin Chemistry Group and Sanchong University, Japan.PARTICIPANTS: Totally 56 cases of epilepsy children were from Maternal and Child Health in Qingdao City including 30 males and 26 females and aged from 3 months to 13 years.While 48 health examinees and 12 patients with brain tumor from General Hospital of Jilin Chemistry Group were taken as controls, including 38 males and 12 females and aged from 6months to 17 years.Their guardians were all informed of the detection index and consented to join the study.METHODS: Polystyrene plates were coated with GluR1 polypeptide antigen,and optimal conditions of enzyme linked immunosorbent assay (ELISA) for GluRI antibody was ascertained with chessboard titration.MAIN OUTCOME MEASURES: The GluR1 antibody level was detected with ELISA.RESULTS:The concentration of coating GluR1 polypeptide antigen was20 mg/L; The dilution of serum and horseradish peroxidase sheep anti-hu-man (HRP-SaH) IgG were 1:50 and 1:1 500 respectively. Intra-assay andinter-assay coefficientof variability were 4.48%-8.80% and 11.18%-16.60% respectively; The specific absorption test demonstrated that the ab-sorbance value of positive serum decreased 2.4 times after absorption ofGluR1 polypeptide compared with that before absorption.There was obvi-ous higher positive rate of GluR1 antibody in serum of epilepsy childrenthan control group (40%, 5%, P<0.01).CONCLUSION:①ELISA is specific and stable for GluR1 antibody in serum. ②There is the possible autoimmune response of GluR1 antibody in serum of epilepsy children.
4.Effect of Buzhong Yiqi Wan on Thyroid Hormones in Rat Model of Spleen-asthenia

【Objective】To observe the effect of Buzhong Yiqi Wan(BYW)on thyroid hormones in rat model of spleen-asthenia.【Methods】Thirty-four SD male rats were randomized into 3 groups:normal control group(N=10),spleen-asthenia model group(N=12),BYW group(N=12).Except the normal control group,the rats in other groups were given the decoction of Radix et Rhizoma Rhei(4 g?d-1 for each rat)to induce spleen asthenia,and meanwhile,BYW group reveived BYW(3.9 g?kg-1?d-1).The treatment lasted 20 days.Radioimmunoassay was used to detect serum levels of triiodothyronine(T3),thyroxine(T4),reverse triiodothyronine(rT3)and thyroid stimulating hormone(TSH).Meanwhile,the body weight,spleen and thymus weight as well as their ratio with the body weight were also observed.【Results】Serum T3 and T4 levels,spleen and thymus weight as well as their ratio with the body weight were decreased in the model group as compared with those in the normal control group(P
5.Effect of Therapy of Tonifying Kidney and Replenishing Essence on Protein Expression Profile in Testes of Aged Rats

Objective To observe the effects of therapy of tonifying kidney and replenishing essence on protein expression profile in testes of aged rats.Methods Twenty male SD rats with 22 months old were equally randomized into the blank control group and the treatment group.The treatment group received gastric gavage of herbs at the dose of 0.05g?kg-1.d-1,which has the actions of tonifying kidney and replenishing essence.The treatment lasted 60 days.After treatment,the right testes were taken out from the rats,and the differentially expressed proteins in the rats testes of two groups were analyzed by two-dimensional gel electrophoresis(2-DE).Results Compared with the model group,obvious changes were found in 7 kinds of differentially expressed proteins in rats testes of the treatment group,of which 4 kinds were up-regulated and 3 down-regulated.After digestion and identification by the matrix-assisted laser desorption/ionization-time of flight (MALDI-TOF) mass spectrum,5 kinds from the 7 kinds of proteins were identified as phosphatidyl ethanolamine-binding protein(PEBP),heat shock 70 kDa protein 8,triose-phosphate isomerase,glyceraldehyde-3-phosphate dehydrogenase(GAPDH),and glutathione S-transferase Mu 1(GSTM1-1).Conclusion The anti-aging mechanism of therapy of tonifying kidney and replenishing essence is probably related with the regulation of testis protein expression in aged rats.
6.Advances in drug resistance mechanism of immunological checkpoint inhibitors and post-drug resistant therapeutic strategy

Immune-checkpoint blockers(ICBs)have been well received in a variety of tumors,and the quality of patient life has improved significantly.However,the reasons why not all patients treated with ICBs benefit from lesion control,symptom improvement,and survival time.Many patients are resistant to the first time when they have been using ICBs for a period of time.This is a clinical challenge.This review lists possible causes of primary drug resistance and acquired resistance to ICBs.The primary resistance is associated with several mechanisms,including tumor microenvironment,cancer cells themselves and other related factors.The acquired resistance includes nonclassical immunoprecipitation molecules secondary overexpression,abnormalities of antigen presenting signal pathway and dysfunction of T cell activation killer.Finally,we have described a variety of possible new combination of treatment,including combined radiotherapy and chemotherapy,and combined targeted therapy with other measures.
7.Effect of different puncture distances in internal arteriovenous fistula on dialysis adequacy in maintenance hemodialysis patient

Objective To investigate the effect of different puncture methods on dialysis adequacy in maintenance hemodialysis patients. Methods One hundred and twenty patients receiving maintenance hemodialysis were divided into the observation group and the control group according to the sequence of receiving the hemodialysis with 60 cases in each group. Patients in the observation group were treated with increasing distance arteriovenous fistula of more than 10 cm , while patients in the control group received normal dialysis care without changing the way of puncture. The difference of dialysis adequacy before and after intervention were compared between the two groups. Result The dialysis adequacy after intervention in the observation group was improved significantly compared with the control group (P<0.05). Conclusion The dialysis adequacy in patients with maintenance dialysis can be improved by increasing the puncture distance of internal arteriovenous fistula.
8.Incidence of metabolic syndrome in systemic lupus erythematosus and its influence by glucocorticoids

Objective To investigate the incidence and correlative factors of metabolic syndrome (MS) in patients with systemic lupus erythematosus (SLE).Methods A total of 116 SLE patients and 115 controls were enrolled into the study.The incidence of MS,SLE disease activity index(SLEDAI) of patients with SLE combined with MS (MS-SLE) and patients without MS (n-MS-SLE),lupus characteristics,cumulative glucocorticoids,administration dose of glucocorticoids and hydroxychloroquine were compared between SLE group and the control group.Results The incidence of MS of SLE group was obviously higher than that of the control ( 34.48% vs 14.78%,P < 0.05 ).The ratios of patients with lower HDL-C,higher TG and higher blood pressure in SLE group ( 50.86%,56.03%,46.55% ) were higher than those in the controls ( 34.78%,16.52%,20.00%,all P < 0.05 ).MS-SLE group had significantly higher mean waist circumference,BMI,systolic blood pressure and diastolic blood pressure and lower HDL-C than n-MS-SLE group (all P <0.05 ).No significant difference was found regarding duration of disease,renal involvement,ESR,C-reactive protein,high-sensitivity C-reactive protein,SLEDAI,cumulative and current glucocorticoids use in MS-SLE group and n-MS-SLE group.The ratio of patients taking hydroxychloroquine in n-MS-SLE group was higher than that of MS-SLE group (46.05% vs 15.00%,P<0.05).Conclusions Patients with SLE has a higher incidence rate of MS.Hydroxychloroquine may reduce their MS incidence.
9.Study on enterovirus infection in family close contacts of patients with hand, foot and mouth disease and the contamination of environment

Objective To understand the enterovirus infection in family close contacts of patients with hand,foot and mouth disease (HFMD) and the contamination of environment.Methods Forty-one HFMD cases confirmed by laboratory from web-based surveillance system during July to August 2010 in Guangdong Province were selected.All members of the cases′ family were investigated by collecting their information on demography,habit of domestic hygiene and hygiene status in household.The stool samples of all members and the smear samples from the surface of family belongings from 16 families were collected and the enterovirus was detected by real time quantitative polymerase chain reaction.The data were analyzed by chi square teat and t test.ResultsForty-one HFMD cases′ families and 135 close contacts were included in this survey.The infection rate of the enterovirus was 39.2% (53/135) in all close contacts.Of all the investigated families,the infectionrate was 58.5% (24/41) in family with one or more close contacts and 9.8% (4/41) in family with all close contacts.The differences of infection rates of enterovirus among the members of parents (32.5%,25/77),grandparents/aunts/ uncles (43.3%,13/30) and cousins (53.6%,15/28) didn′t show statistical significance (χ2 =4.07,P=0.131).The infection rate of enterovirus in close contacts from family with more than 5 members was higher than that from family with 4 or less members (OR=1.4,95%CI 1.1-1.9).Among 135 close contacts,27.4% (37/135) were infected with the same types of entervirus as that of HFMD case in the family and 11.9% (16/135) were infected with the different virus types.In 33 family belongings samples from 16 families,the positive rate of enterovirus detection was 6.1% (2/33).Between 17 families with enterovirus testing negative and 23 families with enterovirus testing positive in close contacts,there were no statistical differences of the family hygiene status,hand-washing of babysitter,disinfection of tableware and drinking,sharing towels,airing bedding articles and toy cleaning (P>0.05).ConclusionsThe infection rate of enterovirus in close contacts of HFMD cases is high and the enterovirus contamination exists in case family environment.Management of close contacts of HFMD cases and disinfection of the family environment are important in HFMD controls.
10.Effect of Acupuncture Combined with Antidepressant on Behavioral Changes, and mRNA Expression of 5-Hydroxytryptamine Receptors in Nucleus Raphes Dorsalis of Sleep Deprivation Depression Rats

Objective To investigate the effect of acupuncture combined with antidepressant on behavioral changes and 5-hydroxytryptamine 1A and 2A receptor ( 5-HT1AR and 5-HT2AR) mRNA expression in nucleus raphes dorsalis (NRD) of sleep deprivation depression (SDD) rats. Methods Eight normal Sprague Dawley (SD) rats were caged together without any stimulus or treatment, serving as the normal group. Thirty-two SD rats were given solitary raise for 21 days, together with chronic unpredictable mild stress ( CUMS) and rapid eye movement ( REM) sleep deprivation to establish the model. Twenty-eight rats completed the modeling successfully, and were divided into model group, acupuncture group, medicine group, and combination group, 7 rats in each group. Rats in the model group were given modeling treatment but without any intervention. Rats in the acupuncture group were given acupuncture on acupoint Yintang, Shenting and bilateral Taichong, acupuncture stimulation lasting 15 s every 10 min and acupuncture retention for 20 min in each day. Rats in the medicine group were given gastric gavage of Zoloft solution (0.83 mg·kg-1·d-1) and Alprazolam solution (0.067 mg· kg-1·d-1) , and rats in the combination group were given both acupuncture and medicine intervention. The intervention for the three intervention groups lasted for 7 days. Results Compared with the normal group, body weight, Open-field scores and sucrose preference of the model rats were significantly decreased on modeling day 7, 14, 21 (P<0.05). On modeling day 28, body weight and Open-field scores of each intervention group were significantly increased compared with those of the model group (P<0.05), and Open-field scores of combination group differed from those of acupuncture group and medicine group ( P<0.05). Only sucrose preference of combination group was improved significantly compared with the model group (P<0.05). Compared with the model group, NRD 5-HT1AR mRNA expression level was increased in the combination group (P<0.05), and NRD 5-HT2AR mRNA expression level was down-regulated in all of the three intervention groups ( P<0.05). Conclusion Acupuncture and antidepressant treatment covering 7 days can evoke rapid effect on SDD rats, which can improve the slow effect of antidepressant.
