Chinese Journal of Applied Clinical Pediatrics 2015;30(11):872-874
Legg-Calvé-Perthes disease is one of the most concerned hip diseases in the pediatric orthopaedic field.Although there are many treatments for Legg-Calvé-Perthes disease, new ideas for the therapy of Legg-Calvé-Perthes disease still exist. Moreover, the prognosis of the disease mostly focuses on the studies of the degree of involvement of the epiphyseal plate, age of onset and lateral pillar classification. This contribution summarizes the treatments and prognosis of Legg-Calvé-Perthes disease.

Objective To detect the apoptosis of femoral head cartilage cells and to observe the expression of apoptosis-related proteins in the tissues of the femoral head,as well as to explore the main pathway for regulating the apoptosis in steroid induced by juvenile rabbit models with avascular necrosis of femoral head. Methods The models with avascular necrosis of the femoral head and the control group model were made in New Zealand infancy albino rabbits induced by glucocorticoid(GC).The immunohistochemical method was used to detect the expressions of Caspase3,Caspase-8,apoptosis protease activating factor-1 (apaf-1),calpain-1 in the femoral heads. Results 1.The integrated optical density(IOD) values of Caspase-3 in GC-induced subgroup, the subgroup that was not induced by GC and control group were 25 142.72 ± 21 528.48,2 069.63 ± 1 096.96 and 301.80 ± 99.66,respectively.The IOD values of Caspase-8 in GC-induced subgroup, the subgroup respectively and the control group were 24 942.63 ± 18 942.99,2 016.31 ± 1 518.70,236.85 ±97.94,respectively.The IOD values of apaf-1 in GC-induced subgroup, the subgroup that was not induced by GC and the control group were 8 5 14.23 ± 6 384.20,1 118.49 ± 1 360.59,95.13 ± 38.05,respectively.The IOD values of calpain-1 in GC-induccd subgroup, the subgroup that was not induced by GC and control group were 9 636.71 ± 9 123.81,1 881.10 ± 3 277.86,126.71 ± 47.35,respectively.The IOD value differences of the Caspase-3 between GC-induced subgroup and the subgroup that was not induced by GC,the control group were extremely significant (H =l 1.470,23.996,P < 0.01).The IOD value differences of the Caspase-8,apaf-1,calpain-1 between GC-induced subgroup and the control group were extremely significant (H =22.178,22.808,13.553,P < 0.01).2.The linear regression analysis results showed that under the joint action of Caspase-8,apaf-1,calpain-1,only the Caspase-8 could significantly predict Caspase-3,and its regression equation regression got significant effect and could explain 40.3% of the variance;while the regression effects of the apaf-1 and calpain-1 to Caspase-3 were not significant. Conclusion Death receptor pathway might play a major regulation role in the apoptotic process of avascular necrosis.

Objective To discuss the clinical diagnosis and treatment of xanthogranulomatous pyelonephritis (XGP).Methods Clinical data of 41 XGP patients admitted from May 1981 to December 2017 were retrospectively analyzed. There were 17 male cases and 24 female cases, aged from 23 to 78 years, with an average age of 57 years. All were unilateral lesions, including 19 cases on the left and 22 cases on the right.Disease duration ranged from 1 week to 12 years, with an average of 3.5 years. The clinical manifestations showed that 7 cases with gross hematuria (17.1%),16 cases with palpable flank or abdominal mass (39.0%),36 cases with flank pain (87.8%),26 cases with acute or intermittent fever (63.4%) and 17 cases with weight loss (41.5%).Laboratory findings showed that 28 cases with leukocytosis (68.3%),24 cases with anemia (58.5%),27 cases with pyuria (65.9%),positive urine culture 19/34 (55.9%),positive urine lipid-laden macrophages 3/14(21.4%),and 20 cases with rapid erythrocyte sedimentation rate (48.8%).Imaging examination:among 35 patients who received CT examination,25 were diagnosed as XGP,mainly presenting diffuse changes in the kidney with enlarged renal shadow, accompanied by diffusive distribution of multiple low-density lesions. There was no obvious enhancement in the low-density lesions on enhanced scan, accompanied by thickening of renal fascia, adhesion to psoas major muscle or involvement of surrounding tissues and organs.According to the imaging results, there were 33 cases of diffuse type and 8 cases of localized type in this group.Renal biopsy was performed 11 cases and 3 cases were diagnosed as XGP.Results All patients were treated with surgical procedure.33 patients with diffuse type were performed nephrectomy.6 of the 8 patients with localized type underwent partial nephrectomy successfully, and the other 2 patients underwent nephrectomy due to severe renal adhesion and unclear lesion boundaries. All postoperative pathologies suggested xanthogranulomatous pyelonephritis.Patients were followed up for 9 months to 10 years postoperatively, with an average of 4.5 years. All patients had no recurrence. Conclusions The clinical manifestations of XGP are lack of specificity.Diagnosis is difficult and depends on pathology.According to imaging examination, lesions can be divided into diffuse type and localized type.Nephrectomy and partial nephrectomy were performed respectively.

Objective:To assess the changes of selenium nutrition of school-age children in Kashin-Beck disease (KBD) areas of Linzhou County, Lhasa City and Xietongmen County, Shigatse City in Tibet Autonomous Region (referred to as Tibet), and provide a scientific basis for evaluating the effectiveness of prevention and control measures. Methods:According to the historical condition of KBD, a total of 344 children's hair samples were collected to determine the content of selenium in Kazi (KBD area) and Jiangxia townships (non-KBD area) of Linzhou County in 2013 and 2021, Renqinze (KBD area) and Tongmen townships (non-KBD area) of Xietongmen County in 2015 and 2021.Results:Compared to 2013/2015, in 2021, the hair selenium level of children in the four townships increased ( P < 0.001). The selenium nutritional level of more than 90% of the children reached medium or above (hair selenium > 0.25 μg/g) in 2021. The hair selenium levels of girls in the two KBD areas (Kazi and Renqinze townships) were lower than those of boys ( Z = - 2.83, - 2.83, P < 0.05). Conclusions:The selenium nutrition level of school-age children in KBD areas in Linzhou and Xietongmen counties has increased rapidly in recent years. However, the selenium nutrition level of girls is significantly lower than that of boys. It is necessary to strengthen prevention, controlling and monitoring, and to further improve the dietary structure of school-age children through the joint efforts of families and schools, to increase the proportion of exogenous high selenium food intake.
