1.Pyroptosis and neonatal brain injury: a review

Pyroptosis is a way of programmed cell death which is newly discovered in recent years. Animal studies have shown that pyroptosis is involved in the occurrence and development of brain injury from various causes. Inhibition of pyroptosis plays a protective role in the nervous system in animal models by reducing the neurological symptoms. Pyroptosis may provide a target for clinical treatment of neonatal brain injury. This paper reviews pyroptosis's mechanism and its role in the pathogenesis in brain injury in various conditions for a better understanding of neonatal brain injury.
3.Clinical effect of Chinese herbal medicine decoction combined with mifepristone in the treatment of uterine fibroids and its influence on sex hormone levels

Objective To observe the curative effect of Chinese herbal medicine decoction combined with mifepristone in the treatment of uterine fibroids and the effects on hormone levels.Methods 100 patients with uterine fibroids were divided into the treatment group and control group according to random number table, 50 cases in each group.The treatment group was given the liquid form joint mifepristone, the control group was given mifepristone.The two groups were treated for a course of 3 months.The sex hormone levels before and after treatment, the symptoms and signs integral related indicators were compared.The clinical efficacy was evaluated.Results The total effective rate of the treatment group was 84%, which was obviously higher than 66% of the control group (x2=9.88,P<0.05).Compared with before treatment[the control group,LH:(57.2±9.6)U/L,E2:(17.9±6.9)pmol/L,FSF:(32.7±8.5)g/L,P:(17.5±5.2)nmol/L;the treatment group,LH:(56.7±10.2)U/L,E2:(17.3±7.9)pmol/L,FSF:(33.5±7.9)g/L,P:(18.4±3.2)nmol/L], after treatment the levels of sex hormones in the two groups significantly reduced[the control group,LH:(47.5±8.3)U/L,E2:(12.5±3.8)pmol/L,FSF:(18.3±6.9)g/L,P:(14.8±3.2)nmol/L;the treatment group,LH:(38.7±8.1)U/L,E2:(7.5±4.3)pmol/L,FSF:(10.2±5.2)g/L,P:(10.5±4.8)nmol/L],the differences were statistically significant(all P<0.05).Conclusion Chinese herbal medicine decoction combined with mifepristone in the treatment of uterine fibroids has good clinical curative effect, by increasing the body's immunity, lowering levels of sex hormones in the body, inhibition of smooth muscle tissue, thus to narrow the myoma, improve clinical symptoms, it is worthy of promoting.
4.The clinical significance of thrombomodulin in Kawasaki disease

Objective To explore the mechanisms of coronary artery lesions in patients with Kawasaki disease by detecting thrombomodulin level in both acute and convalescence stages. Methods Fifty-eight patients were recruited in which 34 were male and 24 were female. This group was further divided into coronary artery lesion group(25 cases) and non-coronary artery lesion group(33 cases). Normal control group was consisted of 30 healthy children in which 17 were male and 13 were female. Thrombomodulin was measured by enzyme linked immunosorbent assay. Results Compared with the control group, TM level was increased in the Kawasaki disease group. TM level in the acute stage group and convalescence group was higher than that of the control group, TM level in acute stage group was higher than that of the convalescence group (P<0.05). Compared with non-coronary artery lesion group, TM level of the coronary artery lesion group was increased and the difference was significant (P<0.01). Correlation analysis showed that the TM level was positively related with coronary complications of the Kawasaki disease (r=0.855, P<0.01 ). Conclusion TM increases significantly in Kawasaki disease. It is correlated with the development of coronary artery lesions. In addition, it is also associated with apparent hypereoagulation and thrombocytophilia. TM can predict the development of coronary artery lesions.
5.Observation of therapeutic effect of minimally invasive surgery combined with wintermine in treating severe hypertensive cerebral hemorrhage

Objective To study the effective treatment for severe hypertensive cerebral hemorrhage. Methods 102 cases of severe hypertensive cerebral hemorrhage were randomly divided into two groups: One group with minimally invasive surgery and intravenous drip of wintermine started 1 ~ 5 days after operation, another only with minimally invasive surgery. The 55 subjects with conservative treatment were selected as control group. Results The quality of life in the survival group was better than in group B and C (P < 0.05, or P < 0.01), the mortality of group A was also significantly lower than the other two groups (P < 0.05, or P < 0.01). Conclusion Minimally invasive surgery combined with intravenous drip of wintermine can be a safe and effective method for severe hypertensive cerebral hemorrhage.
6.Treatment effect of the coronal fractures of the femoral condyle with open reduction and internal fixation

[Objective] To discuss the method and clinical effects of coronal fractures of the femoral condyle treated with open reduction plus cancellous lag screw internal fixation.[Method]From May 1998 to June 2006,17 cases of coronal fractures of femoral condyle were treated with open reduction 'and cancellous lag screw internal fixation by using anterolateral for typ Ⅰ or typ Ⅲ lateral femoral condylar fracture(LFCF)/anteromedial for type Ⅰ or type Ⅲ medial femoral condylar fracture(MFCF)or posterolateral for type ⅡLFCF/posteromedial approach for type Ⅱ MFCF.There were 15 males and 2 females with the average age 39.5 years,ranging from 19 to 48 years.According to Letenneur classification,there were type Ⅰfracture 7 cases,type Ⅱ 4 cases,and type Ⅲ 6 cases.Among them,14 cases were closed fracture,3 eases were open fracture.Auto ilium transplant or homologous allograft bone transplant(5 cases)were also used.All patients were treated with plaster external fixation after operation.The interval between injury and surgery averaged 3.6 days(2.5 hours to 8 days).[Result]All patients were followed up for 8~26 months with an average of 18.2 months.All fractures got bone union with average union time 3.6 months.Knee joint function was rated based on Letenneur evaluation system and showed excellen and good in 14 cases,fine in 3 cases and no poor.There were no deep infection,implant failure or bone necrosis.[Conclusion]Open reduction plus cancellous lag screw internal fixation is an ideally safe and effective operation to treat coronal fractures of the femoral condyle.Accurate choice of approach,satisfactory reduction and rigid internal fixation have important influence on clinical outcomes.
7.Effect of Invigorating Spleen,Clearing Heat and Eliminating Stasis Drugs on EGF and EGFR Expression in Rat Model of Gastric Ulcer

Objective To study the effect and mechanism of invigorating spleen, clearing heat and eliminating stasis drugs on QOUH of gastric mucosa and expression of EGF and EGFR in rat model of gastric ulcer. Methods The rat model of gastric ulcer was constructed by injection of acetic acid. The rats were randomly divided into model group, ranitidine group, invigorating spleen group, clearing heat group, eliminating stasis group, complex prescription group and normal control group. The ulcer index (UI), regenerate mucosal thickness, lamina muscularis mucosal width were observed by HE, and the level of EGF was detected by ELISA. The expression of EGFR in the mucosa around the ulcer was detected by SABC kit immunohistochemical method. 9.Role of phosphatidylinositol 3-kinase/protein-serine-threonine kinases signal transduction pathway in EphB receptor-mediated neuropathic pain in rats

Objective To investigate the role of phosphatidylinositol 3-kinase (PI3K)/protein-serine-threonine kinases(Akt) signal transduction pathway in EphB receptor-mediated neuropathic pain in rats.Methods Forty-eight pathogen-free male SD rats aged 2-3 months weighing 150-180 g were randomly divided into 6 groups (n =8 each):groups sham operation (groups S1 and S2 ); groups chronic constrictive injury (CCI) (groups C1and C2 ) and groups EphBI-Fc (EphB receptor antagonist) + CCI (groups E1 and E2 ).Neuropathic pain was induced by placing 4 ligatures on left sciatic nerve at 1 mm intervals with 5-0 silk thread in groups C1,C2,E1 and E2.EphBI-Fc 0.5 μg in 5 μl normal saline was injected intrathecally 1 h before operation and at 1 and 2 d after operation (group E1 ) or on 5th day after operation (group E2).Normal saline 5 μl was injected intrathecally instead of EphBI-Fc 1 h before operation and at 1 and 2 days after operation (groups S1 and C1 ) or on 5th day after operation (groups S2 and C2 ).Pain withdrawal latency to noxious thermal stimulation (PWL) and pain withdrawal threshold to noxious mechanical stimulation (PWT) were measured before operation and at 1,3 and 5 d after operation.The animals were sacrificed at 5 d after operation after measurement of pain threshold.The lumbar segment of spinal cord (L4-6) was removed for determination of c-Fos,PI3K and phosphorylated Akt(p-Akt) expression.Results CCI significantly reduced PWL and PWT and up-regulated spinal c-Fos,PI3K and p-Akt expression in groups C1 and C2 as compared with groups S1 and S2.EphB1-Fc significantly decreased hyperalgesia and the upregulated spinal Fos,PI3K and p-Akt protein expression induced by CCI in groups E1 and E2 as compared with groups C1 and C2.Conclusion Spinal EphB receptor is involved in the development and maintenance of neuropathic pain through PI3K/Akt signal transduction pathway.
10.Demyelinating encephalopathy in adult onset Still's disease

Objective To report clinical features,diagnosis and treatment in a case of adult onset Still's disease (AOSD) accompanied by demyelinating encephalopathy.Methods We reported a case of Stills disease with signs of encephalopathy.We also reviewed and discussed the literature on the neurological manifestations in AOSD.Results The 35-year-old patient had recurrent fever and arthralgias for 3 years,headache for 1 month and transient loss of consciousness.Laboratory tests showed non-specific immunological activity.MRI showed tumor-like lesions at left parietal and occipital lobes surrounded by sleeve-like edema.The lesion had significant occupation effect.Biopsy proved the presence of demyelinating changes.The patient recovered favorably after administration of corticosteroids and immunoglobulin.The lesions had almost disappeared on follow-up MRI 4 months later.Conclusions Demyelinating encephalopathy may develop in patient with AOSD.MRI may show tumor-like damage,which is rarely reported in the literature.Diagnosis depends on history,clinical manifestation and neuroimaging.Biopsy provides important information in making diagnosis.Treatment with corticosteroids and intravenous immunoglobulin was found to achieve good recovery.
