1.The clinical application of magnetic induction hyperthermia and analysis of thermal field

Objective To study the laws of heating and thermal field of heating seed arrays in different magnetic field intensity during the magnetic induction hyperthermia.Methods 3.0 cm×3.0 cm ×2.6 cm and 5.0 cm ×5.0 cm×2.6 cm of magnetic heating seed was implanted respectively in 10.0 cm×10.0 cm ×10.0 cm pure agar phantom.The regions was divided into one file per 5 Gs in the range of 60-120 Gs magnetic induction.The temperature measurement experiments were implemented in each sub-file.Muscle-agar mixture model was used in the magnetic induction 60 Gs and 90 Gs to implement GTV-heating simulation experiments and thermal field analysis.Results The pure agar phantom heating experiments show that the heating is of basically the same rule when seeds with various diameters were implanted in the magnetic thermal array.Along with the increase of magnetic induction,the final temperature of the focal point heated for 40 min increased accordingly.and time required for heating to 50℃ was shortened correspondingly.The thermal field analysis of muscle-a mixture agar mixture model showed that the implantation density of magnetic heating seed is closely associated with the temperature of the target area and thermal field in difierent magnetic field intcnsity.Conclusions The magnetic induction has a significant impact on heating of the seeds,as the magnetic modium,and this impact is trending downwards when the magnetic field reaches a certain intensity.Within a certain size,adequate magnetic heating seed density is necessary to achieve the desired temperature required for the temperature value.The heating seeds should be put in a certain order to heat the venue evenly,not in a simply order,but in line with the principle of outer dense and inner sparse.
2.The effects of WeChat platform nursing intervention on complications of ear expanded skinflap among children

Objective Comparative effect of WeChat platform nursing intervention and routine nursing intervention on reducing the incidence of complications of children skin expansion in the process of external ear reconstruction. Methods 132 cases of congenital microtia received skin expander implantation at our institution from Jan 2014 to Aug 20 14 were included as control group. 122 cases of congenital microtia received skin expander implantation at our institution from Sep 2014 to Apr 2015 were included as the intervention group. Children and their families in the control group were treated with routine propaganda and education, and the intervention group were added with internet nursing intervention. Collection and analysis the family health knowledge degree and the occurrence of complications in children of the two groups. Results The score of health knowledge about skin expansion in the intervention group was (33.67 ± 2.44),which was significant higher than that of in the control group (25.07 ± 2.02), t=9.71, P < 0.01. The incidence rate of complication in the intervention group was 2.46%(3/122), which was significant lower than that of in the control group, 8.33%(11/132),χ2=4.20, P<0.05. The satisfaction rate of patients′family member in the intervention group was 95.08%(116/122), which was significant higher than that of in the control group, 84.09%(111/132 ),χ2=15.65, P<0.05. Conclusions WeChat platform nursing intervention can reduce expansion flap complications occurrence rate in the periods of expander. It is worthy of clinical application.
3.Preparation and characterization of polymethylmethacrylate-based magnetic bone cements containing micron carbonyl iron powder

BACKGROUND:Magnetic bone cements have been used to treat bone metastasis in Japan, which are made by adding Fe3O4 nanoparticles to bone cements. Magnetic bone cements containing micron carbonyl iron powder have not been reported.
OBJECTIVE:To prepare polymethylmethacrylate-based cements containing carbonyl iron powder, and to test the magnetic characterizations and the heat-generating abilities of al samples according to ISO 5833 standard in vitro. METHODS:The carbonyl iron powder was mixed with polymethylmethacrylate-based bone cement power to prepare magnetic bone cements containing 0%, 20%, 30%, 40%, and 50%carbonyl iron powder, respectively. The setting time, polymerization temperature, compressive strength, magnetic property and in vitro heat-generating ability were tested.
RESULTS AND CONCLUSION:The setting time and polymerization temperature were increased with the increased content of carbonyl iron powder. The highest polymerization temperature of each sample was 65-70 ℃. The increased content of carbonyl iron power could not change the highest polymerization temperature but delay its appearance. The compressive strength of each sample was higher than 60 MPa, and moreover, the compressive strength of the pure polymethylmethacrylate-based bone cement was higher than 60 MPa, which met the ISO 5833 standard. The saturation magnetic intensity was increased with the increasing of carbonyl iron power content. The heat-generating ability of magnetic bone cements had a positive correlation with the magnetic field strength and the content of carbonyl iron powder.
4.Emergency treatment of large amputated ear defect with auricular cartilage replantation.

OBJECTIVETo investigate the emergency treatment for large amputated ear defect.
METHODSFrom Feb. 2010 to Oct. 2013, 5 cases with large unilateral amputated ear defects were treated. The amputated auricular cartilage was replanted subcutaneously in mastoid area at the first stage. Cranioauricular sulcus was reconstructed and skin grafting was performed at the second stage.
RESULTSAll cases were followed up for 3-6 months with satisfactory result. The reconstructed helix, scapha and cavity of auricular concha had similar color and elasticity as the ear at healthy side. Scar at donor sites, such as inguinal region and armpit, was inconspicuous.
CONCLUSIONSThe method of auricular cartilage replantation is an effective way for emergency treatment of large amputated auricular defect with less morbidity at donor site.

5.Preliminary study on the application of videonystagmograph in Chinese patients with multiple sclerosis and neuromyelitis optica

Objective To investigate the characteristics of ocular movement disorders in patients with multiple sclerosis (MS) and neuromyelitis optica (NMO), and explore the clinical application of videonystagmograph (VNG) exami?nation in the diagnosis and differential diagnosis of MS. Methods Sixteen MS ,10 NMO and 30 control ( sudden deafness ) patients were enrolled prospectively. Ocular movement disorders including saccades, gaze fixation, smooth pursuits, opto?kinetic nystagmus and spontaneous nystagmus were evaluated by using VNG. Results The positive rate of ocular motility disorders in MS patients detected by VGN was 68.75%. The incidences of abnormalities in saccades, smooth pursuits and optokinetic nystagmus were significantly higher in MS than in control groups (P= 0.000, 0.001 and 0.001, respectively). The positive rate of ocular motility disorders in NMO patients detected by VGN was 80.00%. The incidences of abnormal?ities in saccades, gaze fixation, smooth pursuits and optokinetic nystagmus were significantly higher in NMO than control groups (P=0.000, 0.012, 0.000 and 0.002, respectively). The positive rate of ocular motility disorders was not significant? ly different in MS and MS patients (68.5%vs. 80%,P>0.05). Compared with bedside physical examination, VNG showed a notable higher sensitivity in the detection of ocular motility disorders(68.75% vs. 37.50%). Furthermore, VNG disor?ders might indicate brain lesions undetected by MRI. Conclusion This small sample research indicates that VNG is a valuable tool in the detection of ocular motility disorders as well as brain lesions in MS and NMO patients. However, its role in the differential diagnosis between MS and NMO is not confirmed.
6. The Application of a 2D Printing of Rib Cartilage in Personalized Ear Framework Fabrication

Objective:
To explore a 1∶1 2D printing method of rib cartilage based on CT scanning in assisting ear framework fabrication for auricular reconstruction.
Methods:
From January 2016 to January 2017, 73 cases of microtia aging from 5 to 7 referred to our department. A 1∶1 2D printing of rib cartilage based on CT scanning was applied in preoperative assessment and measurments of bilateral sixth, seventh and eighth costal cartilage were completed before the surgery. Personalized rib cartilage ear framework fabrication was determined by the measurements.
Results:
Among the 73 cases, 58 cases received auricular reconstruction successfully. There were 15 patients whose rib cartilages did not meet the requirements and postponed the surgery. The patients who underwent auricular reconstruction were followed up for 6 to 12 months (average, 9 months). During the follow-up, 52 cases had satisfactory outcomes in size, shape, symmetry and details. No deformation of reconstructed ears or chest wall was found during the follow-up.
Conclusions
The 1∶1 2D printing based on CT scanning is a simple and timesaving way to evaluate the rib cartilage. It can be beneficial to save the amount of cartilage and improve the personalized ear framework fabrication.
7.Application of WeChat platform on health education of patients with external ear reconstruction

Objective To explore the effects of health education based on WeChat platform to reduce external ear reconstruction complications through skin quantitative expansion method.Methods A number of 254 congenital microtia patients and their families admitted from January to August 2014 were treated as control group who received routine health education.A total of 238 cases patients with the same disease and their families admitted from September 2014 to April 2015 were chosen as observation group who received routine and WeChat platform for health education.Water injection nurses collected the data of patients' health knowledge mastering degree,comparing two groups of patients and families disease knowledge mastering degree,complication rates and nursing satisfaction.Results Patients and families in observation group grasped health knowledge better than that of control group,with a lower complication incidence than that of control group(P <0.01).After the interventions of health education,the satisfaction was 94.96% in the observation group comparing with 83.46% in the control group (P < 0.01).Conclusions Nursing intervention based on WeChat platform can reduce the incidence of expanded skin flap complications,improve patient and families' nursing knowledge and elevate nursing satisfaction.
8.The methods of total ear reconstruction for microtia:selection and application

Objective Based on the different physiological characteristics of the mastoid skin and soft tissue in congenital microtia malformation ,the different ear reconstructivemethod were respectively applied for the individuals with microtia .And the feasibility of personalized treatment to microtia patients was explored in this study .Method Considering different thickness and tightness in mastoid hairless skin and soft tissue,2129 microtia patients were received the skin expansionmethod of ear reconstruction surgery , while 1321 cases were treated with Nagata ' s technique, and 330 cases with skin and fascia expansionmethod . Result With average 10-month follow-up,1944 ( 91.31%) cases using the mastoid skin expansionmethod were satisfied with the outcome of the reconstructed ear including three dimensional position and subunit appearance.305(92.42%)cases using the skin and fascia expansionmethod were satisfied with theresult .1264 (95.69%) cases using Nagata ' s two—stagemethod received satisfactory outcomes .49 ( 3.71%) cases showed partial exposure of the cartilage framework .There were no postoperative complications , such as chest wall deformity or affected normal physical function .Conclusion Based on the different physiological characteristics of the mastoid skin and soft tissue ,it's beneficial to select individualized operationalmethod in ear reconstruction for congenital microtia among Chinese patients .
9.5-fluorouracil combined with triamcinolone acetonide injection for the treatment of anaphylaxis caused by stem cell exosomes: a case report

Stem cell exosome therapy has shown significant potential in treating hair loss, but its safety profile has not been thoroughly studied. This paper reported a case of a 32-year-old female patient who experienced an allergic reaction following adipose-derived stem cell exosomes injection for hair loss treatment at the Plastic Surgery Hospital, Chinese Academy of Medical Sciences in June 2023. Within 24 hours post-injection, the patient developed swelling of the forehead and right postauricular lymph nodes, accompanied by mild pain, and left posterior ear lymph nodes 48 hours later. Ultrasound examination revealed thickening of the local soft tissue and enlarged lymph nodes. The condition was diagnosed as an injection-induced allergic reaction. The symptoms gradually subsided after combined treatment with lidocaine, 5-fluorouracil, and triamcinolone acetonide injections. During a 6-month follow-up period, the patient did not experience any recurrence of allergic reactions, and the therapeutic effects remained stable. This case report highlights the potential risk of allergic reactions associated with stem cell exosome therapy for hair loss, underscoring the need for close monitoring and timely intervention in clinical applications to ensure both safety and efficacy.
