2.Treatment of unstable pelvic fractures with complex fixation

[Objective]To evaluate the outcomes of treatment of unstable pelvic fractures with complex fixation.[Method]Twenty-eight cases of patients with unstable pelvic fracture from January 2000 to June 2007 were treated and followed-up.Eighteen cases were male,10 cases were female,aged from 22 to 68 years old,with the average of 38-years-old.The types of fracture were classified by Tile classification:Type B2 9 cases,Type B3 8 cases,Type C1 6 cases,Type C2 2cases,Type C3 3 cases.The 28 cases had different complicated injury,all the patients were treated with complex fixation.[Result]After being evaluated by the Matta scale and the functional outcome,the outcomes of reduction were excellent in 18 cases,good in 7,fair in 2 and poor in 1,with an excellent and good rate of 85%.[Conclusion]External fixation can win the time for the rescue,choose internal fixation after stable condition,the complex fixation technology is a good way to treat unstable pelvic fractures.
3.Clinical analysis of phacoemulsification and intraocular lenses implantation on 546 patients

Objective To analyze the therapeutic effects of phacoemulsification with intraocular lens in several kinds of cataract.Methods The surgery was performed under topical anesthesia on 672 eyes of 546 patients with senile ,complicated ,congenital and traumatic cataracts through different incision.Results Postoperatively, the visual acuities with spherical correction or without correction were 0.5 or better in 83.0%, 88.9% of the eyes three hours later and in one day; and 1.0 or better in 27.2%, 28.7% in one week and one month. The mean postoperative astigmatism was all under +1.0D which was not found preoperatively in only 35 patients. There was no significant difference of the visual acuities between every two kinds of different cataract.Conclusion The phacoemulcification with intraocular lens implantation can be widely used in treating different kinds of cataract.
4.Analysis of the correlation between osteoporosis and degeneration of intervertebral disc in the aging rat

Objective To study the correlation between osteoporosis and degeneration of intervertebral disc in the aging rat.Methods Fifty healthy SD rats were fed for twenty-two months in order to build the model of aging rats,the histology of femoral bone,tibial bone and lumbal vertebra was used to observe,then we evaluated the indexes which reflect the degeneration of intervertebral disc including the quantity of collagen type Ⅱ,Ⅹ of intervertebral disc by the immunohistochemistry methods,the relative area of vascular bud,the thickness of calcified layer and un-calcified layer on cartilage endplate and the indexes which reflect the osteoporosis including the thickness ratio of cortical bone,the relative area of bone trabecula,the bone density,the max stress and straining of bone.Results There was moderate or strong negative correlation among the indexes of the degeneration of intervertebral disc including the relative area of vascular bud,the ratio of the un-calcified and the calcified layer,the quantitation of collagen type Ⅱand the indexes of osteoporosis including the thickness ratio of cortical bone,the relative area of bone trabecula,the density of femoral bone and vertebral body bone,the max stress and straining of bone.However,there was moderate or strong positive correlation with the quantitation of collagen type Ⅹ,of which the bone density of vertebral body had the strongest dependablity with collagen type Ⅹ.And the indexes of bone biomechanics including the max stress and straining had no correlation with the indexes of degeneration of intervertebral disc.Conclusion Osteoporosis has negative correlation with the degeneration of intervertebral disc,which is not distributed to the environment factors;but the indexes of bone biomechanics including the max stress and straining have no correlation with the indexes of degeneration of intervertebral disc.

6.Prognostic risk factors and therapeutic effect of radiofrequency ablation for primary hepatic carcinoma:a report of 195 cases

Objective To investigate the effect and the prognostic factors of radiofrequency ablation(RFA) for primary liver cancer(PLC).Methods RFA was performed in 195 patients with PLC,and the efficency was evaluated with univariate analysis.Cox′s regression analysis model was used to analyze the factors affecting the prognosis.Results In the whole group of patients,the 1,2,3 and 5-year overall survival rate was 80.5%,67.4%,49.1% and 32.7% respectively,and in the patients with small PLC(the size≤3 cm),the 1,2,3 and 5-year overall survival rate was 91.7%,81.2%,60.5% and 40.4% respectively.The Cox regression models indicated that the independent factors in determining the prognosis were: Liver function of Child-Pugh stage,tumor size and occlusion of hepatic inflow during RFA.Conclusions RFA is an effective mini-invasive treatment for patients with PLC,and especially for the patients with small PLC.The efficency of RFA for small PLC has the same outcome as that of surgical resection of the tumor.Liver function of Child-Pugh stage,tumor size and occlusion of hepatic inflow during RFA are the independent prognostic factors.
7.Effect of Acute Cold or Heat Stress on p38 MAPK Activation in the Spinal Cord and Ganglia of Rats

Objective To investigate whether cold or heat stress affected signal transduction of p38 mitogen-activated protein kinase (MAPK) in the spinal cords and ganglia.Methods The rat models of acute cold and heat stresses were established, and the immunohistochemistry and Western blotting analysis were used.Results The phospho-p38 (p-p38) immuno-positive cells were observed in the Rexed Ⅱ layer of normal thoracic spinal cord, with low cell density and brown nucleus staining, and without staining in Rexed Ⅲ~Ⅹ layers. In the spinal ganglia, the p-p38 immunoreaction was mainly located in the nuclei of a few small-sized neurons. Acute heat stress (10 min & 20 min) reduced the number of positive cells in Rexed Ⅱ layer, with body-temperature (38℃) stress (20 min) having no effect. No immunostaining was changed in the spinal ganglia. Acute cold stress (10 min & 20 min) slightly reduced the positive cells in the Rexed Ⅱ layer, with unchanged immunostaining in spinal ganglia. Western immunoblot analysis showed that activation level of p-p38 was differently downregulated by heat and cold stresses compared to the normal groups and body temperature (38℃)-bathed groups.Conclusion Constitutive activation of p38 can be observed in the normal spinal cord and ganglia; heat stress and cold stress significantly and gentlely inhibite the phosphorylation level of p38, respectively.
8.Effects of rhTGF-?_1 and TGF-?_1 gene transfection on the proliferation of cultured rabbit corneal endothelial cells in vitro

AIM:To investigate the effects of rhTGF-?1 and TGF-?1 gene transfection on the proliferation of cultured rabbit corneal endothelial cells in vitro.METHODS:Cell growth induced by various concentrations of rhTGF-?1 was determined by MTT proliferation assay.Under the induction of liposomes,recombinant pSecTag2-TGF-?1MP vectors were transferred into the corneal endothelial cells.Morphological changes of transfected cells were observed by HE staining.The expression levels of TGF-?1 were assessed by ELISA.Cell cycle analysis was assessed by flow cytometry.DNA fragment analysis was used to confirm the presence of apoptosis.RESULTS:rhTGF-?1 in concentrations of 5-20 ?g/L showed a significant suppressive effect on the proliferation of corneal endothelial cells,0.5-1 ?g/L had no effect,0.05-0.1 ?g/L facilitated cell growth,as compared with negative controls.The morphous of transfected corneal cells had no significant abnormality compared with normal cells.According to the result of ELISA,the concentration of TGF-?1 in the supernatant was calculated to be(98?3)ng/L.Flow cytometry assay showed that S and G2/M phase of transfected cells decreased significantly compared with that of control group,but the cell cycle recovered normally after adding 10 ?g/L EGF into the culture medium.Agarose electrophoresis didn't show marked ladders in transfected group.CONCLUSION:Effects of rhTGF-?1 on the proliferation of corneal endothelial cells are different with various concentrations.TGF-?1 gene transfection shows suppressive effect on the proliferation of cultured corneal endothelial cells,but does not induce cell apoptosis.EGF is the antagonist of this suppressive effect.
9.Analysis of causative gene mutations in one child with primary gout

Objective To improve the knowledge of primary gout in children. Methods Clinical data of a 12-year-old girl with primary gout was collected. Analysis of UMOD gene, REN gene and HNF-1βgene was performed using PCR and di-rect sequencing. Results The girl was admitted for 1-month history of left hallux pain accompanied with elevations of serum uric acid concentration and serum creatinine concentration. Several examinations showed serum uric acid/creatinine ratio was greater than 2.5. The fractional excretion of uric acid was 3.4%-6.6%. The X-ray showed that the proximal phalanxes of halluces were erosion. The diagnosis of renal biopsy was ischemic renal injury and chronic tubulointerstitial nephropathy. Blood uric acid concentrations of parents were normal, and the family history of gout was negative. Two single nucleotide polymorphisms (c.264C>T heterozygous and c.866-71 G>A heterozygous) in UMOD gene, 1 single nucleotide polymorphism (c.373+44C>G heterozygous) in REN gene, and 2 single nucleotide polymorphisms (c.100-50-49ins TCTG heterozygous and c.781-22T>C homozygous) in HNF-1βgene were detected. No pathological mutation was detected in these 3 genes. Conclusions This child is highly suspected to have primary gout caused by familial juvenile hyperuricemic nephropathy.
10.Clinical and imaging analysis of white matter lesions combined with congenital sensorineural hearing loss in children

Objective To analyze the clinical and imaging characteristics of congenital sensorineural hearing loss (CSNHL)children combined with white matter (WM)lesions in order to provide evidence for clinical practice. Methods With referral to the Department of Pediatrics,Peking University First Hospital from November 201 1 to De-cember 201 5,documents of 78 patients of CSNHL combined with WMlesions were collected and analyzed for the clini-cal and imaging characteristics.Results Bilateral severe -profound hearing loss existed in all 78 cases,48.1 %(25 /52 cases)of the patients exhibited gross motor development delay,98.1 %(51 /52 cases)of them had normal cognition development.One hundred percent (61 /61 cases)of patients had abnormal language development.Infection occurring during pregnancy existed in 21 .2%(1 1 /52 cases)of the patients,the premature and smaller for the gestational age in-fants accounted for 28.9% (1 5 /52 cases).The bilateral multiple WMlesions from the brain MRI were in dot to flake sizes with sharp boundary,the intensity of T1 -weighted imaging decreased,T2 -weighted imaging and fluid attenuated inversion recovery increased.Eighty -two point one percent (64 /78 cases)of the patients were found to have the periventricular and subcortical WM involvement.The most frequently affected periventricular region was the posterior horn (91 .9%,68 /74 cases),followed by the anterior horn and temporal horn,and the least with the body involvement. The former three had a combined lesion tendency (55.4% -68.9%).There was an extensive involvement in the sub-cortical WMof parietal,frontal,temporal and occipital lobes respectively(73.5% -88.2%).Subcortical WM involve-ment of multiple lobes was common (accounted for 67.6% -77.9%).The enlargement of bilateral ventricles existed in 37.2%(29 /78 cases)of the patients and cystic changes in the subcortical WM of anterior temporal lobe could be found in 9.0% (7 /78 cases)patients.Calcification in 2 CT cases was reported.Corpus callosum and basal ganglia of all cases were normal.For cases with MRI scans more than once,WMlesions of 96.0%(24 /25 cases)patients became silent or self -restored.Conclusions The clinical presentations of CSNHL combined with WM lesions are mild,not paralleled with their multiple foci.It is considered as demyelination or a delay of myelination.Due to its benign course, it is probably not the contraindication for the cochlear implantation.
